Uninterrupted and overlapping satellite instrument measurements of Earth’s radiation budget from space are required to sufficiently monitor the planet’s changing climate, detect trends in key climate variables, constrain climate models, and quantify climate feedbacks. The Clouds and Earth’s Radiant Energy System (CERES) instruments are currently making these vital measurements for the scientific community and society, but with modern technologies, there are more efficient and cost-effective alternatives to the CERES implementation. We present a compact radiometer concept, Trutinor (meaning “balance” in Latin), with two broadband channels, shortwave (0.2–3 μm) and longwave (5–50 μm), capable of continuing the CERES record by flying in formation with an existing imager on another satellite platform. The instrument uses a three-mirror off-axis anastigmat telescope as the front optics to image these broadband radiances onto a microbolometer array coated with gold black, providing the required performance across the full spectral range. Each pixel of the sensor has a field of view of 0.6°, which was chosen so the shortwave band can be efficiently calibrated using the Moon as an on-orbit light source with the same angular extent, thereby reducing mass and improving measurement accuracy, towards the goal of a gap-tolerant observing system. The longwave band will utilize compact blackbodies with phase-change cells for an absolute calibration reference, establishing a clear path for SI-traceability. Trutinor’s instrument breadboard has been designed and is currently being built and tested.

The Johns Hopkins University Applied Physics Laboratory (JHU/APL) is developing a compact, light-weight, and low power midwave-infrared (MWIR) imager called the Compact Midwave Imaging Sensor (CMIS), under the support of the NASA Earth Science Technology Office Instrument Incubator Program. The goal of this CMIS instrument development and demonstration project is to increase the technical readiness of CMIS, a multi-spectral sensor capable of retrieving 3D winds and cloud heights 24/7, for a space mission. The CMIS instrument employs an advanced MWIR detector that requires less cooling than traditional technologies and thus permits a compact, low-power design, which enables accommodation on small spacecraft such as CubeSats. CMIS provides the critical midwave component of a multi-spectral sensor suite that includes a high-resolution Day-Night Band and a longwave infrared (LWIR) imager to provide global cloud characterization and theater weather imagery. In this presentation, an overview of the CMIS project, including the high-level sensor design, the concept of operations, and measurement capability will be presented. System performance for a variety of different scenes generated by a cloud resolving model (CRM) will also be discussed.

One of the major challenges in metal additive manufacturing is developing in-situ sensing and feedback control capabilities to eliminate build errors and allow qualified part creation without the need for costly and destructive external testing. Previously, many groups have focused on high fidelity numerical modeling and true temperature thermal imaging systems. These approaches require large computational resources or costly hardware that requires complex calibration and are difficult to integrate into commercial systems. In addition, due to the rapid change in the state of the material as well as its surface properties, getting true temperature is complicated and difficult. Here, we describe a different approach where we implement a low cost thermal imaging solution allowing for relative temperature measurements sufficient for detecting unwanted process variability. We match this with a faster than real time qualitative model that allows the process to be rapidly modeled during the build. The hope is to combine these two, allowing for the detection of anomalies in real time, enabling corrective action to potentially be taken, or parts to be stopped immediately after the error, saving material and time. Here we describe our sensor setup, its costs and abilities. We also show the ability to detect in real time unwanted process deviations. We also show that the output of our high speed model agrees qualitatively with experimental results. These results lay the groundwork for our vision of an integrated feedback and control scheme that combines low cost, easy to use sensors and fast modeling for process deviation monitoring.

Psychoanalytic understanding in its status as a hermeneutic activity is different from other forms of understanding. It is distinct, for instance, from psychodynamic psychotherapy, which may be able to establish itself as an empirical science. Empirical science deals with rules and establishes facts, while hermeneutic science deals with meanings, which cover a wider area than facts. Meanings offer a different kind of explanation than facts offer. Psychodynamic psychotherapy can be differentiated from psychoanalysis on the basis of their being different forms of scientific activity. Psychodynamic psychotherapy is not a diminished or lesser form of psychoanalysis, inasmuch as it employs both interpretation and other therapeutic activities, while psychoanalysis is best seen as restricted to interpretation. The understanding that results from psychoanalysis is unique to psychoanalysis.

The CDC reports that among older adults, falls are the leading cause of injury-related death and rates of fall-related fractures among older women are twice those of men. We set out to 1) determine patient perceptions (analyzed by gender) about their perceived fall risk compared to their actual risk for functional decline and death and 2) to report their comfort level in discussing their fall history or a home safety plan with their provider. Elders who presented to the Emergency Department (ED†) were surveyed. The survey included demographics, the Falls Efficacy Scale (FES) and the Vulnerable Elders Survey (VES); both validated surveys measuring fall concern and functional decline. Females had higher FES scores (mean 12.3, SD 5.9) than males (mean 9.7, SD 5.9 p = .007) in the 146 surveys analyzed. Females were more likely to report an increased fear of falling, and almost three times more likely to have a VES score of 3 or greater than males (OR = 2.86, 95% CI: 1.17-7.00, p = .02). A strong correlation was observed between FES and VES scores (r = 0.80, p < .001). No difference in correlation was observed between males and females, p = .26. Participants (77 percent) reported they would be comfortable discussing their fall risk with a provider; there was no difference between genders (p = .57). In this study, irrespective of gender, there appears to be a high association between subjects’ perceived fall risk and risk for functional decline and death. The majority of patients are likely willing to discuss their fall risk with their provider. These findings may suggest a meaningful opportunity for fall risk mitigation in this setting.

This paper expands upon and further develops the centrality of empathy in psychoanalysis as offered by Heinz Kohut. Using clinical examples, it differentiates sustained empathy as the distinctive component of psychoanalysis, and it demonstrates some of the difficulties in determining the boundaries of empathy in the practice of psychoanalysis. A further distinction is from mind reading, a purely cognitive exercise, as is intuition (Carruthers 2009). To pursue a psychoanalytic perception of empathy one must confront its limitations and go beyond the somewhat simplistic claim of its unquestioned therapeutic effect. Empathy is more than a cognitive act, and as sustained over time it can be difficult to achieve, can be misunderstood, and can at times have no therapeutic effect.

Primary Care practices in the United States are undergoing rapid transformation into Patient Centered Medical Homes (PCMHs), prompting a need to train resident physicians in this new model of primary care. However, few PCMH curricula are described or evaluated in the literature. We describe the development and implementation of an innovative, month-long, team-based, block rotation, integrated into the Brown Family Medicine Residency Program, within the context of statewide PCMH practice transformation in Rhode Island. The PCMH resident team (first-, second- and third-year residents) gain PCMH skills, with progressive levels of responsibility through residency. In addition to traditional supervised direct outpatient care, learning activities include: active participation in PCMH transformation projects, population health level patient management, quality improvement activities, interdisciplinary teamwork, chronic disease management (including leading group medical visits), and PCMH specific didactics paired with weekly projects. This new clinical block rotation and team holds promise as a model to train residents for future PCMH primary care practices.

In contrast to the viewpoint that sees conflict as deriving from defense against instinctual drives and thus as an intrapsychic phenomenon, this essay presents conflict as sometimes being an external opposition between disparate configurations of the self. These parallel sectors with different goals and ambitions can be seen in a continuum from the dramatic narcissistic behavior disorders to more subtle instances of moral dilemmas. A clinical illustration is offered to demonstrate its occurrence and management in psychoanalysis.

The concept of confidentiality is examined from its absolute position espoused by some psychoanalysts to the many exceptions to this position allowed by others. The suggestion made in this paper is offered as a psychoanalytic one which urges us to see confidentiality as posing risks in both positions, that is, the absolute and the necessary exceptions. Confidentiality is representative of many, if not all, of the rules and methods that are implicit in the conduct of an analysis and thus may well be taken for granted. The suggestion offered is for a periodic re-examination of the implicit background of psychoanalysis.

The author considers the concept of enactment as a ubiquitous event that is best seen as part of a sequence in the process of understanding a patient. As such, enactments are not unusual or special save as they are often subject to disavowal or to being singled out by the analyst as especially subject to scrutiny. Once recognized, enactments need to be interpreted: not so much in terms of their unconscious origins, but more with regard to the need to include them in the analytic dialogue.

The author compares American pragmatism with American psychoanalysis in an effort to place the existence of the many diverse schools and theories of psychoanalysis in a historical context. Pragmatism is seen as a theory of instrumentation or a collection of tools for accomplishing goals; it claims that many of our efforts to know and seek truth are based upon myths. Psychoanalysis, too, can be seen to pursue certain theoretical claims based on myths. The present climate of pluralism in psychoanalysis is not a phase, but an indication of our diverse ways of achieving in-depth understanding of another person.
