Abstract

In Italy, according to the International Study on Asthma and Allergies in Child- hood study, the prevalence of current asthma, allergic rhinoconjunctivitis, and atopic eczema in 2006 was 7.9%, 6.5%, and 10.1% among children aged 6–7 and 8.4%, 15.5%, and 7.75% among children aged 13–14 yr. University education in this field is provided by the Postgraduate Schools of Pediatrics and those of Aller- gology and Clinical Immunology, as well as several annual Master courses. The Ital- ian Society of Pediatric Allergology and Immunology (SIAIP) was founded in 1996 and counts about 1000 members. SIAIP promotes evidence-based management of allergic children and disseminates information to patients and their families through a quite innovative website and the National Journal ‘Rivista Italiana di Allergologia Pediatrica’. In the last decade, four major regional, inter-regional, and national web-based networks have been created to link pediatric allergy centers and to share their clinical protocols and epidemiologic data. In addition, National Registers of Primary Immune-deficiencies and on Pediatric HIV link all clinical excellence cen- ters. Research projects in the field of pediatric allergy and immunology are founded by the Italian Ministry of Education, University and Research (MIUR) and by the National Research Council (CNR), but the overall investments in this research area are quite low. Only a handful Italian excellence centers participate in European Pro- jects on Pediatric Allergy and Immunology within the 7th Framework Program. The European Academy of Allergy and Clinical Immunology currently hosts two Italians in its Executive Committee (EC) and one in the EC of the Pediatric Section; moreover, major European Academy of Allergy and Clinical Immunology meetings and courses in the area of pediatrics (e.g., PAAM, Venice, 2009) have been held in Italy in the last 3 yr. Italian hallmarks in the management of allergic diseases in childhood are a quite alive and spread interest in Molecular Allergology and a remarkable predominance of sublingual (SLIT) compared to the subcutaneous (SCIT) immunotherapy.

Abstract

Introduction: The health-related quality of life in X-linked agammaglobulinemia was investigated in 25 children and adolescents patients through the Italian version of Pediatric Quality of Life Inventory 4.0 Generic Core Scale for patients aged less then 18 years, comparing child perception to that of the parents and the physician's evaluation. The data were compared with the ones of 80 healthy controls and the literature data of a group of patients with rheumatic diseases. Discussion: The agammaglobulinemia subjects perceived a lower global quality of life than the healthy subjects, but significantly higher than the rheumatic diseases controls. The clinical relevance of health-related quality of life assessment in X-linked agammaglobulinemia pediatric patients is discussed. © 2008 Springer Science+Business Media, LLC.

Abstract

1. Premessa.Oggetto della presente decisione è l’indennizzo a favore di soggetti danneggiati da trattamenti sanitari obbligatori riguardanti danni irreversibili derivanti da epatite contratta a seguito di somministrazione di derivati del sangue.Con tale sentenza la Corte costituzionale, amplia la platea dei beneficiari della disciplina prevista dalla l. n. 210/1992, riconoscendo che la vigente limitazione dell’indennizzo disposta per il contagio dell’epatite contratta a seguito delle sole trasfusioni comporta un irragionevole disparità di trattamento.Viene, pertanto, dichiarata l’illegittimità costituzionale dell’art 1, co. 3, della l. n. 210/1992 che non prevede la concessione del beneficio a quei soggetti che hanno contratto l’epatite a seguito della somministrazione di emoderivati. 2. La questione.Il Tribunale di Palermo, sezione lavoro, con ordinanza del 15 novembre 2007, nel corso di un giudizio avente ad oggetto una richiesta di indennizzo per i danni subiti a seguito di «somministrazione di siero antitetanico per via intramuscolare», dubita della legittimità costituzionale dell’art. 1, co. 2, della legge 25 febbraio 1992, n. 210 “Indennizzo a favore dei soggetti danneggiati da complicanze di tipo irreversibile a causa di vaccinazioni obbligatorie, trasfusioni e somministrazioni”, nella parte in cui non prevede che i benefici riconosciuti dalla legge citata ‹‹spettino anche ai soggetti che presentino danni irreversibili derivanti da epatiti contratte a seguito di somministrazione di derivati del sangue››.Tale disposizione, secondo il rimettente, contrasterebbe con l’art. 3 della Costituzione per l’irragionevole disparità di trattamento che essa determina tra i soggetti che abbiano contratto l’epatite a seguito di somministrazione di emoderivati, ai quali non è riconosciuto alcun indennizzo, e coloro che abbiano contratto l’infezione da HIV per la medesima ragione, ai quali la legge, invece, accorda il beneficio.Il rimettente denuncia, altresì, la violazione degli artt. 2, 32 e 38 della Costituzione dal momento che non vi sarebbero ragioni per cui la tutela della salute e l’assistenza sociale correlata siano escluse per i soggetti che subiscano danni irreversibili derivanti da epatiti contratte a seguito di somministrazione di derivati del sangue. 3. La decisione.A fronte della questione sollevata, la Corte precisa l’oggetto del giudizio rilevando, che benché il rimettente abbia specificamente denunciato il co. 2 dell’art. 1 della l. n. 210/1992, le censure debbano più propriamente intendersi riferite al co. 3 del medesimo articolo, il quale disciplina l’indennizzo per il caso di epatite laddove, invece, il co. 2 si occupa dei danni derivanti da infezioni da HIV. L’analisi della disciplina dettata dalla l. n. 210/1992 evidenzia che, mentre per l’HIV l’indennizzo è sempre riconosciuto senza distinguere tra le diverse modalità di contagio, perl’epatite il beneficio è concesso solo nel caso in cui la malattia sia conseguita a trasfusione, ovvero, se si tratta di operatori sanitari, nelle ipotesi di contatto con il sangue o suoi derivati.Ne consegue che nel primo caso l’indennizzo è riconosciuto per un gamma di situazioni causali più ampia comprendenti la trasfusione e la somministrazione degli emoderivati, nel secondo caso la tutela viene riconosciuta solo a fronte di un evento lesivo trasfusionale. In altri termini, resta priva di tutela l’ipotesi oggetto del giudizio a quo, in cui l’infezione da epatite sia conseguita dalla somministrazione di emoderivati.La Corte individua la ratio del sostegno economico previsto dall’art. 1, co. 2 e 3, della l. n. 210/1992, nel principio di solidarietà collettiva sancito dagli artt. 2 e 38 della Costituzione.La prestazione è corrisposta per far fronte ad eventi generanti una situazione di bisogno.In tal senso è, pertanto, irragionevole trattare in modo diverso situazioni che dal punto di vista sostanziale e formale si presentano identiche.Il mancato riconoscimento dell’indennizzo a favore di coloro che abbiano contratto l’epatite a seguito di somministrazione di emoderivati non trova, quindi, alcuna ragionevole giustificazione che possa giustificarne la mancanza di tutela.Accertata l’evidente disparità di trattamento, la Corte costituzionale ha quindi dichiaratol’illegittimità costituzionale dell’art. 1, co. 3, l. n. 210/1992 nella parte in cui non prevede che i benefici riconosciuti dalla legge citata spettino anche ai soggetti che presentino danni irreversibili derivanti da epatite contratta a seguito di somministrazione di derivati del sangue.

Abstract

Background: No study has compared allergic sensitization patterns in infants with atopic eczema from different countries. The aim of this study was to investigate the patterns of allergic sensitization in a cohort of infants with atopic eczema participating in a multicentre, international study. Methods: Two thousand one hundred and eighty-four infants (mean age 17.6 months) with atopic eczema from allergic families were screened in 94 centres in 12 countries to participate in a randomized trial for the early prevention of asthma. Clinical history, Severity Scoring of Atopic Dermatitis Index, measurements for total serum IgE and specific IgE antibodies to eight food and inhalant allergens were entered into a database before randomization to treatment. A history of type of feeding in the first weeks of life and exposure to animals was recorded. Results: A total of 52.9% of the infants had raised total IgE, and 55.5% were sensitized to at least one allergen. There was a wide difference in the total IgE values and in the sensitization rates to foods and aeroallergens among infants from different countries. The highest prevalence rates of allergen-sensitized infants were found in Australia (83%), the UK (79%) and Italy (76%). Infants from Belgium and Poland consistently had the lowest sensitization rates. In each country, a characteristic pattern of sensitization was found for aeroallergens (house dust mite > cat > grass pollen > Alternaria), but not for food allergens. Conclusions: In infants with atopic eczema, there is a wide variation in the pattern of allergic sensitization between countries, and data from one country are not necessarily generalizable to other countries.

Abstract

Contrary to common believing, the prevalence of the intolerance to food additives in the general population is rather low. Nowadays many doubts persist with regard both to the pathogenetic mechanisms and to the clinical and diagnostic aspects in this field. Symptoms due to, or exacerbated from, food additives usually involve non-IgE-mediate mechanisms (pseudo-allergic reactions, PAR) and are usually less severe of those induced by food allergy. The most frequent clinical feature of the intolerance to food additives still remains the urticaria-angioedema syndrome, although these substances are really involved only in a minority of patients. Other possible clinical features include anaphylaxis, atopic eczema, behaviour disturbances, asthma and non-allergic rhinitis. The diagnostic approach consists in diary cards, reporting symptoms and food habits, elimination diet and double blinded placebo-controlled oral challenge with suspected additives. However, such procedure still remains poorly standardized and numerous uncertainties persist with regard to optimal conditions for performing and interpret the challenge results. The therapeutic approach consists in the exclusion of foods and products containing the additive involved, and, in patients not compliant to the diet, in treatment with symptomatic drugs.

Abstract

There is increasing interest in the potential influence of fetal and early life conditions on childhood wheezing.To investigate the associations between maternal complications and procedures in pregnancy and at birth and the risk of various wheezing phenotypes in young children.We studied 15,609 children, aged 6-7 yr, enrolled in a population-based study. Standardized questionnaires were completed by the children's mothers.Of the children, 9.5% (1,478) had transient early wheezing, 5.4% (884) had persistent wheezing, and 6.1% (948) had late-onset wheezing. Maternal hypertension or preeclampsia was associated with an increased risk of all three wheezing phenotypes (for transient early wheezing: odds ratio [OR], 1.40; 95% confidence interval [95% CI], 1.08-1.82; for persistent wheezing: OR, 1.59; 95% CI, 1.15-2.19; and for late-onset wheezing: OR, 1.47; 95% CI, 1.06-2.01). Use of antibiotics for urinary tract infections was associated with transient early wheezing (OR, 1.52; 95% CI, 1.16-2.00), whereas antibiotic administration at delivery was associated with both transient early wheezing (OR, 1.21; 95% CI, 1.01-1.46) and persistent wheezing (OR, 1.39; 95% CI, 1.10-1.75). Children who had a mother with diabetes were also more likely to have persistent wheezing (OR, 1.72; 95% CI, 0.99-3.00). Neither amniocentesis/chorionic villus sampling, nor weight gain in pregnancy, nor cesarean section was associated with the subsequent development of wheezing. Maternal asthma or atopy was not an effect modifier of the associations found.Some maternal complications during pregnancy and at delivery may increase the risk of developing different phenotypes of wheezing in childhood.

Abstract

Epidemiological studies have provided evidence that exposure to vehicular traffic increases the prevalence of respiratory symptoms and may exacerbate pre-existing asthma in children. Self-reported exposure to road traffic has been questioned as a reliable measurement of exposure to air pollutants. The aim of this study was to investigate whether there were specific effects of cars and trucks traffic on current asthma symptoms (i.e. wheezing) and cough or phlegm, and to examine the validity of self-reported traffic exposure.

Abstract

486 children born to HIV-positive mothers, 57 children infected by blood products, and 1 child for whom the personal history was not available were studied. Perinatal infection had a more varied clinical picture and a worse outcome compared with infection acquired later in childhood. Severe secondary infections, neurological disorders, and hepatitis (but not lymphoid interstitial pneumonia) were linked to a high mortality rate in perinatally infected children, in whom an early onset of symptoms was also a bad prognostic factor. Perinatal HIV infection occurred in 32·6% of children born to seropositive mothers, with a higher transmission rate in children born by vaginal delivery and then breast-fed. Preterm delivery and low birthweight seemed to be related to drug abuse during pregnancy, not to intrauterine HIV infection. Girls had a higher rate of perinatal infection and, of those infected, had an increased mortality.
