Abstract

Epidermolysis bullosa (EB) is a rare disorder that presents with urological complications. We present a 6-year-old boy admitted with urological symptoms that revealed an inherited EB misdiagnosed. We also review the literature on this disorder and management of the common urological complications.

Abstract

Thyroid hormones (THs) operate numerous physiological processes through modulation of the nuclear thyroid hormone receptors and several other proteins. We report direct activation of the nuclear peroxisome proliferator-activated receptor gamma (PPARg) and retinoid X receptor (RXR) by classical and nonclassical THs as another molecular activity of THs. The T4 metabolite TETRAC was the most active TH on PPARg with nanomolar potency and binding affinity. We demonstrate that TETRAC promotes PPARg/RXR signaling in cell-free, cellular, and in vivo settings. Simultaneous activation of the heterodimer partners PPARg and RXR resulted in high dimer activation efficacy. Compared to fatty acids as known natural ligands of PPARg and RXR, TETRAC differs markedly in its molecular structure and the PPARg-TETRAC complex revealed a distinctive binding mode of the TH. Our observations suggest a potential connection of TH and PPAR signaling through overlapping ligand recognition and may hold implications for TH and PPAR pharmacology.

Abstract

Many cancers resist to chemotherapy drugs during treatment and it have been proposed various mechanisms in connection with drug resistance. High expression of ATP-dependent membrane proteins as a family of ATP binding cassette (ABC) are one of the main reasons for drug resistance so that P-glycoprotein (a member of this family) plays an important role in drug resistance. ATP-binding cassette (ABC) transporters are an example of ATP-dependent pumps. ABC transporters are ubiquitous membrane-bound proteins, present in all prokaryotes, as well as plants, fungi, yeast and animals. These pumps can move substrates in (influx) or out (efflux) of cells. In mammals, ABC transporters are expressed predominantly in the liver, intestine, blood-brain barrier, blood-testis barrier, placenta and kidney. In addition, multidrug resistance associated protein as other membrane of this family is involved in drug resistance. These proteins have property of endogenous substrates transferring. Over-expression of these proteins in cancer cells is most important obstacle to treat cancer. However, drug resistance can occur through other ways such as anti-cancer drugs associated with metabolism purines and pyrimidines or microtubules dysfunction. In this review, mechanisms of drug resistance were described.

Abstract

Pilocytic astrocytoma (PA) is a slow growing brain tumor that tend to occur in young patients. Although PA is often a benign tumor, dissemination to leptomeninges has been described in the literature. We describe a sixteen-yearold boy who initially presented with paraplegia and progressed to severe neurological features. An intradural intramedullary spinal cord tumor was identified in medullar MRI, and diagnosis of PCA by a surgical biopsy of the lesion was performed. Subsequently, patient developed an intracranial hypertension syndrome and Craniospinal MRI revealed an intracranial and spinal leptomeningeal dissemination. The patient was treated with courses of cytotoxic consisting of carboplatin - vincristine-based chemotherapy. He died of disease two months later. Our case emphasizes the lack of benefit of chemotherapeutic agents in PA with diffuse leptomeningeal dissemination and review existing literature. [ABSTRACT FROM AUTHOR]
