Abstract

This paper presents the results of a broad, transversal, observational survey on the social cost of female urinary incontinence in the general female population, resident in the ASL3-Monza area. In this population, in an 18-65 year old age group, the prevalence of U.I. observed is 28.5% (LIFE cases) and 18.6% (YEAR cases). The total number of incontinent women in the ASL3-Monza area and in the Lombardy Region can be estimated as follows: - ASL3-Monza 125,288 "LIFE" cases, 81,767 (18.6% of 439,609) "YEAR" cases. - Lombard Region 1,119,480 "LIFE" cases, 730,608 (18.6% of 3,928, 709) "YEAR" cases. In the light of the data collected, we can estimate that in the Lombardy Region and in the ASL3-Monza area, at least 379, 600 (52% of 730,608) and 42,518 (52% of 81, 767) incontinent women use protection and 175,200 (24% of 730,608) and 19,624 (24% of 81,767) consider using a pad to be the only useful solution to the problem. The "estimated" mean/annual cost per woman in an incontinent population with characteristics similar to those of the general female population of the ASL3-Monza is € 358,03, for a mean/life cost per woman of € 11.099,20. When there is no effective treatment of the symptom, we can estimate the following total cost/life in the ASL3-Monza area to be: 65,41 × 31 × € 358,03 = € 726.025.407. If the symptom disappears and the treatment is effective, this is also an advantage, apart from the obvious requirement for public health related to it, from a strictly pharmacological-economic point of view. The estimated total/life cost for the treatment (with the most effective medical, rehabilitative and/or surgical therapy available today) of a sample of 1,000 incontinent women of the ASL3-Monza area is significatively lower than the total cost/life expected in the case of persistence and "assistance" of the symptom: € 4.502.812, 10 vs. € 23.451.270,02. Despite the methodological limits of a pilot study (single centre collection of data, limited samples, analysis of "theoretical" populations), from the data collected it appears clear that the more incisive and determined the strategy of information and education, and the fuller and earlier the diagnosis, the more significative the social-economic, as well as the psycho-social saving

Abstract

One hundred and ten consecutive patients who had come to the Urogynaecological outpatients surgery of Desio Hospital with stress urinary incontinence were studied and treated with uro-rehabilitation at the Neuro-rehabilitation Division of Seregno Hospital between 1998 and 2000. The study only included the patients studied and treated in that three year period of research who suffered from stress urinary incontinence (genuine or mixed) with a 0/1 degree cystocele according to the Baden-Walker HWS classification with type I/II incontinence according to Blassas, with no suspect intrinsic clinical instrumental urethral defect and of whom all the subjective and objective parameters of evaluation of the gravity of the IUS adopted were available (micturition diary, VAS, Pad test according to the ICS, stress test according to Ferrari, I-S score, PC test) before and after treatment. All the patients voluntarily accepted to follow the combined therapeutic rehabilitation protocol at the Outpatients Surgery: One cycle of physiochinesitherapy associated to a three weekly cycle as outpatients of electro myographic biofeedback and FES with vaginal probe. The comparison between the initial and final numerical values studied after treatment show how the degree of the disorder, subjective and/or objective of the patients had significantly improved: the mean value of VAS passed from 6.8+/-2.1 to 3.5+/-2.6; the values of the PAD and stress tests went from 9.8±16.8 2.8±1.3 to 5.9±13.7 and 1.8±1.6 respectively an inverse trend being shown by the PC-test which rise from 1.1±1 to 2.2±0.9. In particular, 55% of the patients during their first visit mentioned a VAS equal to or above 7; while after the rehabilitation treatment in 51% of the population a VAS equal to or less than 3 was registered. At the last follow-up 35.5% remained unvaried, 25.4% improved and 39.1% completely recovered. Some anamnestic clinical instrumental parameters modify the response to treatment (in particular the objective gravity of IUS). It is however difficult to state, on the basis of the sample studied, that these parameters can represent significant risk factors of failure of the treatment, as has been pointed out several times in international literature. The results of this study confirm that uro-rehabilitation techniques can represent the first possible and effective therapeutic choice in the treatment of stress urinary incontinence (genuine or mixed).

Abstract

This paper presents the results of a broad, transversal, observational survey on the social cost of female urinary incontinence in the general female population, resident in the ASL3-Monza area. In this population, in an 18-65 year old age group, the prevalence of U.I. observed is 28.5% (LIFE cases) and 18.6% (YEAR cases). The total number of incontinent women in the ASL3-Monza area and in the Lombardy Region can be estimated as follows: - ASL3-Monza 125,288 "LIFE" cases, 81,767 (18.6% of 439,609) "YEAR" cases. - Lombard Region 1,119,480 "LIFE" cases, 730,608 (18.6% of 3,928, 709) "YEAR" cases. In the light of the data collected, we can estimate that in the Lombardy Region and in the ASL3-Monza area, at least 379, 600 (52% of 730,608) and 42,518 (52% of 81, 767) incontinent women use protection and 175,200 (24% of 730,608) and 19,624 (24% of 81,767) consider using a pad to be the only useful solution to the problem. The "estimated" mean/annual cost per woman in an incontinent population with characteristics similar to those of the general female population of the ASL3-Monza is € 358,03, for a mean/life cost per woman of € 11.099,20. When there is no effective treatment of the symptom, we can estimate the following total cost/life in the ASL3-Monza area to be: 65,41 × 31 × € 358,03 = € 726.025.407. If the symptom disappears and the treatment is effective, this is also an advantage, apart from the obvious requirement for public health related to it, from a strictly pharmacological-economic point of view. The estimated total/life cost for the treatment (with the most effective medical, rehabilitative and/or surgical therapy available today) of a sample of 1,000 incontinent women of the ASL3-Monza area is significatively lower than the total cost/life expected in the case of persistence and "assistance" of the symptom: € 4.502.812, 10 vs. € 23.451.270,02. Abstract

One hundred and ten consecutive patients who had come to the Urogynaecological outpatients surgery of Desio Hospital with stress urinary incontinence were studied and treated with uro-rehabilitation at the Neuro-rehabilitation Division of Seregno Hospital between 1998 and 2000. The study only included the patients studied and treated in that three year period of research who suffered from stress urinary incontinence (genuine or mixed) with a 0/1 degree cystocele according to the Baden-Walker HWS classification with type I/II incontinence according to Blassas, with no suspect intrinsic clinical instrumental urethral defect and of whom all the subjective and objective parameters of evaluation of the gravity of the IUS adopted were available (micturition diary, VAS, Pad test according to the ICS, stress test according to Ferrari, I-S score, PC test) before and after treatment. All the patients voluntarily accepted to follow the combined therapeutic rehabilitation protocol at the Outpatients Surgery: One cycle of physiochinesitherapy associated to a three weekly cycle as outpatients of electro myographic biofeedback and FES with vaginal probe. The comparison between the initial and final numerical values studied after treatment show how the degree of the disorder, subjective and/or objective of the patients had significantly improved: the mean value of VAS passed from 6.8+/-2.1 to 3.5+/-2.6; the values of the PAD and stress tests went from 9.8±16.8 2.8±1.3 to 5.9±13.7 and 1.8±1.6 respectively an inverse trend being shown by the PC-test which rise from 1.1±1 to 2.2±0.9. In particular, 55% of the patients during their first visit mentioned a VAS equal to or above 7; while after the rehabilitation treatment in 51% of the population a VAS equal to or less than 3 was registered. At the last follow-up 35.5% remained unvaried, 25.4% improved and 39.1% completely recovered. Abstract

Background: Neutrophils are essential in the early innate immune response to pathogens. Harnessing their antimicrobial powers, without driving excessive and damaging inflammatory responses, represents an attractive therapeutic possibility. The neutrophil population is increasingly recognised to be more diverse and malleable than was previously appreciated. Hypoxic signalling pathways are known to regulate important neutrophil behaviours and, as such, are potential therapeutic targets for regulating neutrophil antimicrobial and inflammatory responses., Methods: We used a combination of in vivo and ex vivo models, utilising neutrophil and myeloid specific PHD1 or PHD3 deficient mouse lines to investigate the roles of oxygen sensing prolyl hydroxylase enzymes in the regulation of neutrophilic inflammation and immunity. Mass spectrometry and Seahorse metabolic flux assays were used to analyse the role of metabolic shifts in driving the downstream phenotypes., Results: We found that PHD1 deficiency drives alterations in neutrophil metabolism and recruitment, in an oxygen dependent fashion. Despite this, PHD1 deficiency did not significantly alter ex vivo neutrophil phenotypes or in vivo outcomes in mouse models of inflammation. Conversely, PHD3 deficiency was found to enhance neutrophil antibacterial properties without excessive inflammatory responses. Abstract

Hypoxemia is a defining feature of acute respiratory distress syndrome (ARDS), an often-fatal complication of pulmonary or systemic inflammation, yet the resulting tissue hypoxia, and its impact on immune responses, is often neglected. In the present study, we have shown that ARDS patients were hypoxemic and monocytopenic within the first 48 h of ventilation. Monocytopenia was also observed in mouse models of hypoxic acute lung injury, in which hypoxemia drove the suppression of type I interferon signaling in the bone marrow. This impaired monopoiesis resulted in reduced accumulation of monocyte-derived macrophages and enhanced neutrophil-mediated inflammation in the lung. Administration of colony-stimulating factor 1 in mice with hypoxic lung injury rescued the monocytopenia, altered the phenotype of circulating monocytes, increased monocyte-derived macrophages in the lung and limited injury. Thus, tissue hypoxia altered the dynamics of the immune response to the detriment of the host and interventions to address the aberrant response offer new therapeutic strategies for ARDS., (© 2022. Abstract

Neutrophils are predominantly glycolytic cells that derive little ATP from oxidative phosphorylation; however, they possess an extensive mitochondrial network and maintain a mitochondrial membrane potential. Although studies have shown neutrophils need their mitochondria to undergo apoptosis and regulate NETosis, the metabolic role of the respiratory chain in these highly glycolytic cells is still unclear. Recent studies have expanded on the role of reactive oxygen species (ROS) released from the mitochondria as intracellular signaling molecules. Our study shows that neutrophils can use their mitochondria to generate ROS and that mitochondrial ROS release is increased in hypoxic conditions. This is needed for the stabilization of a high level of the critical hypoxic response factor and pro-survival protein HIF-1α in hypoxia. Further, we demonstrate that neutrophils use the glycerol 3-phosphate pathway as a way of directly regulating mitochondrial function through glycolysis, specifically to maintain polarized mitochondria and produce ROS. This illustrates an additional pathway by which neutrophils can regulate HIF-1α stability and will therefore be an important consideration when looking for treatments of inflammatory conditions in which HIF-1α activation and neutrophil persistence at the site of inflammation are linked to disease severity., (© 2022 by The American Society of Hematology.)

Abstract

Background: Acute respiratory distress syndrome (ARDS) is a severe critical condition with a high mortality that is currently in focus given that it is associated with mortality caused by coronavirus disease 2019 (COVID-19). Neutrophils play a key role in the lung injury characteristic of non-COVID-19 ARDS and there is also accumulating evidence of neutrophil mediated lung injury in patients who succumb to infection with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). Methods: We undertook a functional proteomic and metabolomic survey of circulating neutrophil populations, comparing patients with COVID-19 ARDS and non-COVID-19 ARDS to understand the molecular basis of neutrophil dysregulation. Results: Expansion of the circulating neutrophil compartment and the presence of activated low and normal density mature and immature neutrophil populations occurs in ARDS, irrespective of cause. Release of neutrophil granule proteins, neutrophil activation of the clotting cascade and upregulation of the Mac-1 platelet binding complex with formation of neutrophil platelet aggregates is exaggerated in COVID-19 ARDS. Importantly, activation of components of the neutrophil type I interferon responses is seen in ARDS following infection with SARS-CoV-2, with associated rewiring of neutrophil metabolism, and the upregulation of antigen processing and presentation. Whilst dexamethasone treatment constricts the immature low density neutrophil population, it does not impact upon prothrombotic hyperinflammatory neutrophil signatures.
