Introduction: Nurses play a crucial role in providing healthcare services in prisons and psychiatric hospitals in Kosovo. In Kosovo, there are ten prison facilities with a capacity of 2500 inmates. At those facilities there are ten Prison Health Units with six Medical Clinics and three medical stationeries. At every PHU, basic healthcare services are offered round-the-clock. Purpose: The purpose of this study is to evaluate the role of nurses in providing healthcare services in prisons and psychiatric hospitals in Kosovo. The study aims to assess the workload and responsibilities of nurses and the quality of healthcare services they provide. Additionally, the study analyzes the management of health services in these institutions. Methods: This study is based on statistical data from the Prison Health Service in Kosovo from 2013 to 2022. The study calculated statistical coefficients related to the number of nurses and healthcare staff per 1,000 prisoners. Results: The study found that there were 10.09 nurses per 1,000 inmates in the 10 prison facilities in Kosovo, and the total number of medical staff per 1,000 inmates was 15.49. The ratio of physicians per 1,000 inmates was 3.05, indicating a relatively low number of physicians compared to nurses. The study also found a higher ratio of nursing staff to inmates, indicating more individualized attention and care. Conclusions: The study concludes that nurses play a vital role in providing healthcare services in prisons and psychiatric hospitals in Kosovo. The study recommends hiring more physicians to improve the quality of healthcare services in prisons.

Background: The COVID-19 pandemic has led to an increase in drug use among inmates in prisons worldwide, including Kosovo, due to restrictions on visitation and addiction recovery programs. Objectives: This study analyzed drug use among 33,144 inmates in Kosovo prisons over nine years and aimed to identify potential solutions, foster collaboration between relevant institutions, and develop better programs and treatments. Methods and Results: The study is based on data from the medical files of inmates examined over nine years (2014-2022) in 10 Kosovo prison health units. From the total number of inmates, 8.22% were identified as drug users, and the percentage of drug use among inmates steadily increased from 5.8% in 2014 to 15.48% in 2019. In 2020, the first year of the pandemic, there was a decrease of 1.16% (from 15.48% to 14.28%), which was the first decrease in six years. The study also found a disproportional decrease in the number of inmates admitted during the same period. The Medico Psycho Therapeutic Center “Labyrinth” showed a similar trend. Suicide attempts, death under custody, and tobacco use increased linearly, with alcohol use remaining stable. Conclusion: The study highlights the need for effective prevention, treatment, and management strategies for drug use among inmates and the general population in Kosovo. Continuous collaboration between relevant institutions and stakeholders is essential to improve the health and well-being of those affected by drug use and create a brighter future for Kosovo.

Predictive maintenance is one of the most important topics within the Industry 4.0 paradigm. We present a prototype decision support system (DSS) that collects and processes data from many sensors and uses machine learning and artificial intelligence algorithms to report deviations from the optimal process in a timely manner and correct them to the correct parameters directly or indirectly through operator intervention or self-correction. We propose to develop the DSS using open-source R packages because using open-source software such as R for predictive maintenance is beneficial for small and medium enterprises (SMEs) as it provides an affordable, adaptable, flexible, and tunable solution. We validate the DSS through a case study to show its application to SMEs that need to maintain industrial equipment in real time by leveraging IoT technologies and predictive maintenance of industrial cooling systems. The dataset used was simulated based on the information on the indicators measured as well as their ranges collected by in-depth interviews. The results show that the software provides predictions and actionable insights using collaborative filtering. Feedback is collected from SMEs in the manufacturing sector as potential system users. Positive feedback emphasized the advantages of employing open-source predictive maintenance tools, such as R, for SMEs, including cost savings, increased accuracy, community assistance, and program customization. However, SMEs have overwhelmingly voiced comments and concerns regarding the use of open-source R in their infrastructure development and daily operations.

A limited number of individuals with non-obstructive azoospermia (NOA) may recover spermatozoa through traditional testicular sperm extraction (TESE) techniques. There is an ongoing debate over the effectiveness of microdissection TESE compared to standard TESE methods. Microdissection TESE (micro-TESE) techniques enable the identification of spermatogenesis foci in non-obstructive forms of azoospermia. Only histological examination can provide an objective and definitive assessment of the testicular phenotype. This study aimed to evaluate the correlation between histopathological findings after microdissection TESE (micro-TESE) and the predictive role of various factors in determining the success of sperm retrieval. We evaluated 24 patients with azoospermia who underwent micro-TESE and considered the patient's hormonal profile, testis ultrasound, genetic evaluation, histology, and immunohistology (PLAP antibody) of collected testis biopsies. The preoperative blood FSH level, in conjunction with other parameters, may aid in the prediction of micro-TESE success. Sensitivity increases, and specificity decreases with higher FSH levels. Furthermore, testicular volume and FSH levels are typically normal in patients with maturation arrest. In conclusion, hormones, ultrasound evaluation of the testicles, testis volume, and available genetic tests have a predictive value in differentiating obstructive azoospermia (OA) from NOA with various sensitivity and specificity rates. Erectile dysfunction is a multifactorial disease; it has been demonstrated that endothelial dysfunction plays an essential role in the pathogenesis of this disease, and Vitamin D deficiency is considered to favor endothelial lesions. Our study, based on a group of 58 patients who have erectile dysfunction and a control group of 26 healthy subjects, tends to confirm that low levels of vitamin D could potentiate the severity of erectile dysfunction, promoting endothelial dysfunction. Statistical analysis using the Pearson's correlation criteria showed a robust and significant correlation between vitamin D levels and erectile dysfunction severity (ρ=0.752, p<0.000) according to the SHIM (Sexual Health Inventory For Men) questionnaire. Also, in patients with erectile dysfunction, there is a strong association between vitamin D and testosterone levels (ρ=0.728, p<0.000). At the same time, a negative correlation between vitamin D and BMI (ρ=-0.517, p<0.000); cholesterol (ρ=-0.560, p<0.001), and triglycerides(ρ=-0.529, p<0.005) was observed. Also, a moderate correlation between erectile dysfunction severity degree and testosterone levels (ρ=0.544) was also detected, and the same severity parameter of erectile dysfunction correlates negatively with cholesterol levels (ρ=-0.534). In its turn, the testosterone level correlates negatively with other biochemical indices: cholesterol (ρ=-0.694) and triglycerides (ρ=-0.670). Vitamin D level reduction, concomitantly with decreased testosterone and increased cholesterol, contributes to the development and maintenance of erectile dysfunction, more probably through endothelial mechanisms. The assessment of vitamin D values can be used as an independent marker in erectile dysfunction assessment. Thus, one of the diagnostic tests recommended for erectile dysfunction should be the determination of the vitamin D serum level., (©Carol Davila University Press.)
