Amaç: Benign anorektal hastalıklar sık görülen hastaların yaşam konforunu etkileyen hastalıklardır. Bu çalışmada benign anorektal hastalıkların beslenme ve kabızlık durumunu ile ilişkisinin araştırılması amaçlanmıştır. Materyal ve Metot: Çalışmaya Haziran 2018 ile Aralık 2018 tarihleri arasında polikliniğimize başvuran benign anorektal hastalığı bulunan 16 hasta (Grup 1) ile sağlıklı 16 gönüllü (Grup 2) dâhil edilip bir anketle beslenme ve tuvalet alışkanlıkları değerlendirilmiştir. Bulgular: İlk grupta defekasyon sırasında tuvalette geçirilen süre 14,06 (±11,07) dakika iken ikinci grupta 6,68 (±4,39) dk olup aradaki fark istatistiksel olarak anlamlı bulunmuştur (p=0,02). Günlük karbonhidrat alımı ise ilk grupta 223,94±38,15 gram, ikinci grupta 186,39±45,54 gram olup aradaki fark istatistiksel olarak anlamlıdır (p=0,02). Sonuç: Yaşam tarzı değişikliği ve buna yönelik verilecek eğitimler benign anorektal hastalıkların gelişimini azaltıp hasta bireylerin yaşam kalitesini arttırabilir.

Amaç: Güvenli ve ucuz bir yöntem olarak, su yardımlı kolonoskopi yöntemi, kolonoskop ile eişimi zor kolona giderken karşılaşılan güçlükleri gidermek için endoskopi yapan cerraha benzersiz bir yöntem sunmaktadır. Su yardımlı uygulamanın bir diğer yararı da sedasyon için kullanılan ilaçların gereksinimini azaltmasıdır. Su yardımlı yöntemlerin gerçek yararı hakkında hala çok sayıda tartışma olmasına karşın az sayıda klinik yayın mevcuttur. Bu çalışmada, zorlu geçmesi öngörülen vakalarda, konvansiyonel hava insuflasyonu ile yapılan kolonoskopileri, hava insuflasyonu yerine su zerk edilerek yapılan kolonoskopilerle çekum entübasyon başarısı ve hızı, ağrı skoru ve işlem sonrası analjezik ihtiyacı açısından karşılaştımayı amaçladık.Gereç ve Yöntemler: Zorlu kolonoskopi öngörülen hastalar ve/veya geçirilmiş abdomino-pelvik cerrahi öyküsü olan toplam 40 hasta çalışmaya dahil edildi. İşlem öncesi veya esnasında herhangi bir sedasyon uygulanmadı. Hastalar randomize olarak su ve hava grubu olarak iki gruba ayrıldı. One benefit of the water technique is the reduction of the requirement for medications used for sedation. Although there is still much contention among endoscopists regarding the true benefit of the water-assisted methods, a few clinical study was reported on that issue. In this study, we aimed to compare the colonoscopy with conventional air insufflation and the colonoscopes with water inserted instead of air insufflation in terms of the cecum intubation success and speed, the pain score and the post-procedural analgesic requirements in the arduous cases.Material and Method: A total of 40 patients with suspected colonoscopy and/or previous history of abdominal-pelvic surgery were included in the study. Treatment of thoracic aortic aneurysms constitutes high mortality and morbidity rates despite improvements in surgery, anesthesia, and technology. Endovascular stent grafting may be an alternative therapy with lower risks when compared with conventional techniques. However, sometimes the branches of the aortic arch may require transport to the proximal segments prior to successful thoracic aortic endovascular stent grafting. Atherosclerosis is accounted among the etiology of both aneurysms and occlusive diseases that can coexist in the same patient. In these situations stent grafting may even be more complicated. In this report, we present the treatment of a 92-year-old patient with aortic arch aneurysm and proximal descending aortic aneurysm. For successful thoracic endovascular stent grafting, the patient needed an alternative route other than the native femoral and iliac arteries for the deployment of the stent graft. In addition, debranching of left carotid and subclavian arteries from the aortic arch was also required for successful exclusion of the thoracic aneurysm.

Background and objectives: Lowering the blood cholesterol and low density lipoprotein levels may reduce the risk of coronary heart disease. This study was designed to evaluate the effect of lipid-lowering therapy by simvastatin 20 mg versus atorvastatin 20mg on patients suffering from hyperlipidemia. Method: This study is 16-weeks duration included 75 patients with hyperlipidemia. Patients were assigned randomly to receive either simvastatin 20 mg/day group (1), or atorvastatin 20 mg/day group (2). After 12 hours fasting, lipid profile, atherogenic index and alanine aminotransferase were assessed for the patients at baseline, 8-weeks and at the end of 16 -weeks of treatment. Results: After therapy for both groups of patients, as compared to the levels at the baseline, the serum total cholesterol (TC), triglycerides (TG), low density lipoprotein cholesterol (LDL-C) and very low density lipoprotein cholesterol (VLDL-C) were significantly reduced while high density lipoprotein cholesterol (HDL-C) was significantly increased. Serum alanine aminotransferase (ALT) increased by both groups of treatment with no significant differences between the two modes of treatment. Conclusion: After treatment with simvastatin 20 mg and atorvastatin 20 mg, there were a reduction in total cholesterol, triglyceride, LDL-C, and VLDL-C, and an increase in HDL-C in both groups. Comparing the two types of treatment, atorvastatin 20 mg was more effective in lowering triglyceride and VLDL-C than simvastatin 20 mg while Simvastatin led to greater reduction in LDL-C. Both modes of treatment were well tolerated by the patients.

Son görüşlere göre H.pylori infeksiyonu mide kanser riskini artırmaktadır. Bu ilişkiyi incelemek amacı ile 60 mide kanserli ve 15 asemptomatik kişi çalışmaya alındı. H.pylori infeksiyonu tanısı için histoloji kullanıldı. Histolojik inceleme asemptomatiklerde antral biyopsilerle, rezeksiyon uygulananlarda tümörsüz dokudan alınan örneklerde yapıldı. Tümör tiplemesinde Lauren klasifikasyonuna uyuldu.H.pylori infeksiyonu diffüz tip mide kanserlerine % 77.8, intestinal tip mide kanserlerine % 55.6 oranında eşlik etmekte idi. H.pylori infeksiyonu mide kanserli hastalarda % 66.7 (40/60) asemptomatiklerde % 73.3(11/15) saptandı.H.pylori infeksiyonu ile mide kanseri ve tipleri arasında anlamlı bir ilişki kurulamadı. According to the recent opinions Helicobacter pylori infection increases the risk of gastric carcinoma. To detect this relationship we studied 60 gastric cancer patients and 15 asymptomathics. For the diagnosis of H.pylori infection, histology was used. Histologic examination for H.pylori infection was done with antral biopsies in asymptomathics, and tumor free tissues who under-gone resection. Lauren classification was used for typing cancer. H.pylori positivity was detected in diffuse type gastric carcinoma with 77.8 %, in intestinal type gastric carcinoma with 55.6 %. H.pylori infection was 66.7 % (40 of 60) in gastric cancer patients, 73.3 % (11 of 15) in asymptomathics. No relation was found between H.pylori infection and gastric carcinoma and types of gastric carcinoma 60
