Background: Evidence suggests marginal superiority of static aortic systolic blood pressure (aSBP) compared with brachial SBP (bSBP) regarding the association with organ damage and prognosis of cardiovascular disease (CVD). The noninvasive 24-hour aSBP assessment is feasible and associates better with presence of left ventricular hypertrophy compared with 24-hour bSBP. We aimed at comparing the association of 24-hour aSBP and 24-hour bSBP with indices of arterial damage and examining the role of 24-hour SBP amplification variability (within-subjects’ SD) in this association. Methods: Consecutive subjects referred for CVD risk assessment underwent 24-hour aortic and brachial ambulatory BP monitoring using a validated oscillometric device (Mobil-O-Graph). Arterial damage was assessed by carotid intima-media thickness (IMT) and detection of carotid and femoral atheromatosis (plaque presence). Results: Cross-sectionally 501 individuals (aged 54 +/- 13 years, 57% men, 80% hypertensives) were examined. Multivariable analysis revealed superiority of 24-hour aSBP regarding the association with IMT, carotid hypertrophy and carotid-but not femoral-atheromatosis. In receiver operator characteristics analysis, 24-hour aSBP displayed a higher discriminatory ability-compared to 24-hour bSBP-for the detection of both carotid hypertrophy (area under the curve, 0.662 versus 0.624, P

Background: Recent epidemiological evidence suggests a J-shaped, rather than the classical linear, association between dietary sodium (Na) intake and cardiovascular (CV) disease. Numerous animal studies have shown the acceleration of atheromatosis in a low-salt diet but data in humans are scarce. Our aim was to test the hypothesis that in a cohort of patients who are CV-free, yet at increased CV risk, moderate Na intake is associated with lower prevalence of atheromatosis and arterial stiffening than those at very low Na intake. Methods: Two 24-h dietary recalls were conducted to estimate Na intake. Atheromatosis (carotid and femoral plaques) was assessed by B-mode ultrasonography and arterial stiffness through tonometry (carotid-to-femoral pulse wave velocity, cf-PWV). Results: In 901 individuals (age: 52.4 +/- 13.8 years, 45.2% males), only females at 3rd and 4th quartile of total Na intake (derived from food and discretionary salt) had significantly lower probability to present femoral plaques than those at 1st quartile (751.0 +/- 215.5 mg/day), even in the full-adjusted model [0.462(0.229-0.935) and p = 0.032 3rd quartile; 0.274(0.118-0.638) and p = 0.003 4th quartile]. On the contrary, male and female individuals at 3rd quartile had significantly higher probability to present arterial stiffness (PWV >10 m/s) than those at 1st quartile [1.991(1.047-3.785) and p = 0.036]. Conclusions: Overall, the present data suggest that very low Na intake is associated with: a) accelerated atheromatosis, verifying findings from animal models, and providing a possible explanation of the modern epidemiology and b) lower arterial stiffness, which is in line with previous human findings, therefore suggesting a diverging effect of Na in the two major arterial pathologies. The last decade, a growing number of evidence support J-shape or inverse - instead of positive linear -- associations between dietary sodium intake and cardiovascular morbidity/mortality. A careful evaluation of these studies leads to the following observations: less accurate methods for dietary sodium assessment are usually used; most studies included high-risk participants, enhancing the possibility of a `reverse causality' phenomenon. However, these limitations do not explain all the findings. Few carefully designed randomized clinical trials comparing different levels of sodium intake that address the issue of the optimal and safe range exist; therefore, current guidelines recommend a higher cut-off instead of a safe range of intake. Given the demonstrated harmful effects of very low sodium diets leading to subclinical vascular damage in animal studies, the `J-shape hypothesis' cannot yet be either neglected or verified. There is a great need of well-designed general population-based prospective randomized clinical trials to address the issue.

Εισαγωγή – Σκοπός: Αρκετές μελέτες υποστηρίζουν την υπεροχή της αορτική πίεσης ιατρείου έναντι της βραχιόνιας ως προς την αξιολόγηση της - σχετιζόμενης με την αρτηριακή πίεση (ΑΠ) – καρδιαγγειακής βλάβης και την πρόγνωση μελλοντικών καρδιαγγειακών επεισοδίων. Η 24ωρη περιπατητική καταγραφή της αρτηριακής πίεσης (ΠΚΑΠ) θεωρείται ιδανική μέθοδος αξιολόγησης του προφίλ της αρτηριακής πίεσης. Πλέον είναι εφικτή η μη επεμβατική αξιολόγηση της 24ωρης αορτικής πίεσης και μιας πληθώρας άλλων βιοδεικτών αρτηριακής σκληρίας (ταχύτητα μετάδοσης σφυγμικού κύματος – ΤΜΣΚ) και ανακλωμένων κυμάτων πίεσης (δείκτης επαύξησης – ΔΕ) με μία ταλαντωσιμετρική συσκευή με χρήση βραχιόνιας περιχειρίδας (Mobil-O-Graph, IEM, Stolberg, Germany). Σκοπός της μελέτης μας ήταν: α) η διερεύνηση της επαναληψιμότητας της 24ωρης αορτικής πίεσης και των κεντρικών αιμοδυναμικών παραμέτρων που αξιολογεί η συσκευή Mobil-O-Graph, β) η διερεύνηση του κιρκάδιου προφίλ της αορτικής πίεσης και των λοιπών αιμοδυναμικών παραμέτρων καθώς και η αξιολόγηση των παραγόντων που επηρεάζουν την ενίσχυση της πίεσης παλμού (ΕΠΠ), γ) η αξιολόγηση της συσχέτισης (και πιθανής υπεροχής) της 24ωρης αορτικής πίεσης (έναντι τόσο της 24ωρης βραχιόνιας όσο και της βραχιόνιας και αορτικής πίεσης ιατρείου) με την υπερτροφία και τη διαστολική δυσλειτουργία της αριστερής κοιλίας καθώς και την παρουσία υπερτροφίας του καρωτιδικού τοιχώματος, δ) η διερεύνηση της συσχέτισης της μεταβλητότητας της αορτικής πίεσης και της πιθανής υπεροχής της έναντι της μεταβλητότητας της βραχιόνιας πίεσης ως προς την αναγνώριση υπερτροφίας και διαστολικής δυσλειτουργίας της αριστερής κοιλίας καθώς και παρουσίας υπερτροφίας του καρωτιδικού τοιχώματος. Μέθοδοι: Διαδοχικά άτομα που παραπέμφθηκαν στη μονάδα μας για αξιολόγηση του συνολικού καρδιαγγειακού κινδύνου υποβλήθηκαν σε καταγραφή της 24ωρης βραχιόνιας και αορτικής ΑΠ, καθώς και των λοιπών 24ωρων αιμοδυναμικών παραμέτρων με χρήση της συσκευής Mobil-O-Graph. Η βραχιόνιος και αορτική ΑΠ ιατρείου αξιολογήθηκαν με πιστοποιημένη ταλαντωσιμετρική συσκευή και εφαρμογή ανάλυσης σφυγμικού κύματος αντίστοιχα. Η υπερτροφία και η διαστολική δυσλειτουργία της αριστερής κοιλίας καθώς και η υπερτροφία των κοινών καρωτίδων αξιολογήθηκαν με υπερηχογράφημα υψηλής ευκρίνειας. Για την ανάλυση επαναληψιμότητας επανατοποθετήθηκε η συσκευή σε 30 ασθενείς μετά από διάστημα 1-4 εβδομάδων. Αποτελέσματα: Ο δείκτης συμφωνίας μεταξύ 2 επαναλαμβανόμενων μετρήσεων intraclass correlation coefficient υπέδειξε πολύ καλή επαναληψιμότητα όλων των καταγραφόμενων παραμέτρων, ενώ τα γραφήματα Bland-Altman δεν ανέδειξαν την ύπαρξη κάποιου συστηματικού σφάλματος. Σε 497 διαδοχικά άτομα (ηλικία 54,1± 12,8 έτη, 56,7% άνδρες, 79,7% υπερτασικοί) αξιολογήθηκε το κιρκάδιο πρότυπο των περιφερικών και κεντρικών αρτηριακών αιμοδυναμικών παραμέτρων. Όλες οι παράμετροι παρουσίασαν κιρκάδια κατανομή. Συγκεκριμένα, πτώση κατά τη διάρκεια του ύπνου εμφάνισε η ΕΠΠ, η βραχιόνια και αορτική ΑΠ, η μέση ΑΠ και η ΤΜΣΚ, ενώ αντίθετα αυξημένες τιμές παρατηρήθηκαν για τη βραχιόνια και αορτική πίεση παλμού καθώς και το ΔΕ κατά τη διάρκεια του ύπνου. Επιπλέον σημειώθηκε μια δυσανάλογα αυξημένη πτώση της βραχιόνιας σε σύγκριση με την αορτική ΑΠ κατά τη διάρκεια του ύπνου. Οι παράγοντες που σχετίστηκαν με τη νυκτερινή εμβύθιση της ΕΠΠ ήταν η ηλικία, το φύλο, το ύψος, η παρουσία υπέρτασης, η λήψη αντιυπερτασικής αγωγής και η νυκτερινή μεταβολή της ΤΜΣΚ, του ΔΕ, της μέσης ΑΠ και των σφύξεων. Η ανάλυση παλινδρόμησης που πραγματοποιήθηκε σε 229 άτομα (ηλικία: 54,3 ± 14,6 έτη, 56% άνδρες, 75% υπερτασικοί) έδειξε ότι η 24ωρη αορτική ΑΠ σχετίζεται στατιστικά πιο σημαντικά με την υπερτροφία και τη διαστολική δυσλειτουργία της αριστερής κοιλίας σε σχέση με την 24ωρη βραχιόνια ΑΠ καθώς και την ΑΠ ιατρείου (βραχιόνιο και αορτική), ανεξάρτητα από την ηλικία, το φύλο ή τη λήψη αντιυπερτασικής αγωγής. Η ανάλυση καμπύλης λειτουργικού χαρακτηριστικού δέκτη (Receiver Operator Characteristic-ROC) ανέδειξε στατιστικά υψηλότερη διακριτική ικανότητα της 24ωρης αορτικής ΑΠ ως προς την ανίχνευση υπερτροφίας και διαστολικής δυσλειτουργίας της αριστερής κοιλίας σε σχέση με την 24ωρη βραχιόνια ΑΠ. Διαπιστώθηκε επίσης υψηλός βαθμός επικάλυψης της 24ωρης αορτικής συστολικής ΑΠ μεταξύ ατόμων στα οποία η 24ωρη βραχιόνια ΑΠ υποδήλωνε διαφορετικά στάδια αρτηριακής υπέρτασης. Η 24ωρη αορτική ΑΠ υπερτερεί της βραχίονας ως προς τη συσχέτιση με το πάχος του έσω-μέσου χιτώνα των καρωτίδων (IMT) σε 497 άτομα (ηλικία 54±13 έτη, 57% άνδρες, 80% υπερτασικοί). Από τα πολυπαραγοντικά μοντέλα (διορθωμένα για πιθανούς συγχυτικούς παράγοντες) ανεδείχθη υπεροχή της 24ωρης αορτικής ΑΠ τόσο ως προς τη συσχέτιση με το IMT όσο και με την παρουσία καρωτιδικής υπερτροφίας. Από την ανάλυση ROC ανεδείχθη αυξημένη διακριτική ικανότητα της 24ωρης αορτικής ΑΠ ως προς την ανίχνευση καρωτιδικής υπερτροφίας συγκριτικά με την 24ωρη βραχιόνιο ΑΠ. Σε πολυπαραγοντικά μοντέλα διορθωμένα για πιθανούς συγχυτικούς παράγοντες, μόνο η μεταβλητότητα της αορτικής πίεσης (και όχι η μεταβλητότητα της βραχιόνιας πίεσης) συσχετιζόταν με την υπερτροφία της αριστερής κοιλίας και φάνηκε να συσχετίζεται ισχυρά με τη διαστολική δυσλειτουργία της αριστερής κοιλίας. Αντίστοιχα, μόνο η μεταβλητότητα της αορτικής πίεσης συσχετιζόταν με την παρουσία καρωτιδικής υπερτροφίας, ενώ παρείχε μεγαλύτερη διακριτική ικανότητα στην αναγνώριση της τελευταίας συγκριτικά με τη μεταβλητότητα της βραχιόνιας πίεσης. Συμπεράσματα: H μη επεμβατική 24ωρη περιπατητική καταγραφή της αορτικής πίεσης και των αιμοδυναμικών παραμέτρων με τη συσκευή Mobil-O-Graph παρέχει επαναλήψιμες μετρήσεις. Διερευνήθηκαν πιθανοί παθοφυσιολογικοί μηχανισμοί που οδηγούν στην εμβύθιση της ΕΠΠ κατά τη διάρκεια του ύπνου. Βρέθηκε ότι η 24ωρη αορτική πίεση, συγκριτικά με την 24ωρη βραχιόνια ΑΠ, βελτιώνει την εξατομικευμένη αξιολόγηση της - σχετιζόμενης με την ΑΠ - καρδιαγγειακής βλάβης και συγκεκριμένα της υπερτροφίας και διαστολικής δυσλειτουργίας της αριστερής κοιλίας καθώς και της παρουσίας καρωτιδικής υπερτροφίας. Τέλος, φάνηκε πως η μεταβλητότητα της αορτικής πίεσης υπερέχει της μεταβλητότητας της βραχιόνιας πίεσης ως προς την αναγνώριση υπερτροφίας και διαστολικής δυσλειτουργίας της αριστερής κοιλίας, καθώς και της υπερτροφίας των καρωτίδων. Introduction – aim: Evidence suggests the superiority of office aortic pressure compared to brachial pressure on the evaluation of – the blood pressure (BP) associated - cardiovascular damage and prognosis of cardiovascular disease (CVD); 24-hour ambulatory blood pressure monitoring (ABPM) is regarded as the optimal method for assessing BP profile. The non-invasive assessment of 24 hour aortic BP, as well as indices of aortic stiffness (pulse wave velocity – PWV) and wave reflections (augmentation index – AI) is now feasible with the use of a brachial-cuff based oscillometric device (Mobil-O-Graph, IEM, Stolberg, Germany). The aim of our study was a) to investigate the reproducibility of 24hour aortic BP and central hemodynamic indices as assessed by the aforementioned device, b) to investigate the circadian pattern of aortic BP and central hemodynamics, as well as the factors associated with the circadian variability of pulse pressure amplification (PPA), c) to examine the association (and possible superiority) of 24-hour aortic over 24-hour brachial ABPM (as well as office brachial and aortic BP) with left ventricular hypertrophy (LVH), left ventricular diastolic dysfunction (LVDD) and common carotid artery (CCA) hypertrophy, d) to investigate the association of aortic blood pressure variability (BPV) and possible superiority over brachial BPV in terms of LVH, LVDD and CCA hypertrophy recognition. Methods: Consecutive subjects referred to our unit for global CVD risk assessment underwent 24hour brachial and aortic ABPM and central hemodynamics evaluation using Mobil-O-Graph. Office brachial and aortic BP were assessed by a validated oscillometric device and implementation of pulse wave analysis respectively. High resolution ultrasound was used for assessing LVH, LDD and CCA hypertrophy. Thirty consecutive subjects had a test-retest ABPM with 1-4 weeks interval to test the reproducibility of the recorded parameters. Results: Intraclass correlation coefficients of 24hour aortic BP and central hemodynamics indicated very good reproducibility, while Bland–Altman plots indicated no evidence of systematic bias. In 497 consecutive subjects (aged 54.1± 12.8 years, 56.7% male, 79.7% hypertensives) we assessed the circadian pattern of peripheral and central hemodynamics. All parameters exhibited a circadian variation. Sleep dipping (i.e. decrease) pattern of PPA, brachial and aortic systolic BP, mean BP and PWV was noted, while on the contrary higher sleep than wake values were observed for brachial PP, aortic PP and AI. A disproportionately higher sleep fall was noted for brachial as compared to aortic systolic BP. The factors independently associated with the sleep-dipping pattern of PPA were age, gender, height, the presence of hypertension, the use of antihypertensive medication, sleep decrease in arterial stiffness (PWV), sleep increase of pressure wave reflections (AI), sleep decrease in heart rate and mean BP. Regression analysis performed in 229 individuals (aged 54.3±14.6 years; 56% men; 75% hypertensives) showed that 24-hour aortic systolic BP was significantly better associated with LVH and LVDD than 24-hour brachial, as well as office (brachial or aortic) BP, independently of age, gender, obesity or treatment. Receiver operator characteristics curve analysis showed a higher discriminatory ability of 24-hour aortic than brachial BP to detect the presence of LVH and LVDD. A high degree of interindividual overlap regarding aortic 24-hour average systolic BP level was found in individuals in whom the corresponding brachial measurements denoted different hypertension levels. It was shown that 24-hour aortic systolic BP was significantly better correlated with intima media thickness (IMT) as compared with brachial systolic BP in 497 subjects (aged 54±13 years, 57% men, 80% hypertensives). Multivariate analysis (adjusted for possible confounders) revealed superiority of 24-hour aortic systolic BP regarding the association with IMT, as well as with carotid hypertrophy. In ROC analysis, aortic BP had a higher discriminatory ability compared to brachial for the detection of carotid hypertrophy. In multivariate models adjusted for possible confounders, it was shown that aortic BPV (and not brachial BPV) was significantly associated with LVH and LVDD. Moreover, only aortic BPV was associated with the presence of carotid hypertrophy, while providing higher discriminatory ability for the detection of carotid hypertrophy as compared to brachial BPV. Conclusions: Non invasive 24hour aortic BP and central hemodynamics assessment by Mobil-O-Graph device provides reproducible values. Possible pathophysiological mechanisms were identified, leading to the sleep dipping of PPA. It was shown that 24hour aortic ABPM, when compared to brachial ABPM, improves the individualized assessment of the BP-associated cardiovascular damage, i.e. LVH, LVDD and CCA hypertrophy. Last, aortic BPV was found to be superior to brachial BPV in terms of association with LVH, LVDD, as well as CCA hypertrophy.
