Background: Laparoscopic skills are unlikely to be achieved exclusively in the operating theatre, so simulation training has become mandatory to acquire specific psychomotor skills to be merged in a more complex procedure., Objective: To compare 3-day vs. 1-day laparoscopic suturing courses and to better address participants' needs according to their level of experience., Methods: Observational cohort study conducted between January 2017 and December 2018 including 107 participants amongst which 61 attended a 3-day and 46 the 1-day suturing course., Results: Data analysis showed no significant difference in the pre-test suturing scores between the two groups. On each course, when comparing the pre- and post-tests results, the participants reached a statistically significant improvement in both precision and knotting score (p< 0.01). However, when comparing the two types of courses, the data showed a better performance in the post-session test for those attending the 3-day course (p<0.05), as well as a higher mean score improvement (4.7 vs. 2.8; p<0.05) and time needed to complete exercises (-270s vs. -150s; p<0.05). Furthermore, grouping the participants according to their experience, the experts achieved a significantly better improvement attending the 3-day course, when compared to the beginners., Conclusions: Both 3 and 1-day course are successful in improving laparoscopic suturing skills regardless of the participant's experience. However experienced participants benefit more from a longer course while the 1-day one should be dedicated to pre-surgical competences acquisition., (Copyright © 2020 Facts, Views & Vision.)

Study Objective: To point out the relevant anatomy of the ureter and to demonstrate its rules of dissection., Design: An educational video to explain how to use ureteral relevant anatomy and the principle of dissection to perform safe ureterolysis during laparoscopic procedures., Setting: A tertiary care university hospital and endometriosis referential center., Interventions: Anatomic keynotes of the ureter and examples of ureterolysis., Conclusion: This video shows the feasibility of laparoscopic ureteral dissection and provides safety rules to perform ureterolysis. Identification and dissection of the ureter should be part of all gynecologic surgeons' background to reduce the risk of complications [1]. Knowledge of anatomy plays a pivotal role, allowing the surgeon to keep the ureter at a distance and minimizing the need for ureterolysis. Unfortunately, the need for ureteral dissection is not always predictable preoperatively, and gynecologic surgeons need to master this technique, especially when approaching more complex procedures such as endometriosis [2]. An implicit risk of damage cannot be denied when performing ureterolysis; therefore, the ureter should be dissected only when strictly necessary and handled with care to minimize the use of energy [3]., (Copyright © 2018. Background: Laparoscopic promontofixation is the gold standard to treat apical defects but the dissection of the anterior compartment is variable since based on surgical judgment only. We therefore evaluate the placement of the anterior mesh using an ultrasonographic measurement after promontofixation., Design: A prospective cohort study (Canadian Task Force Classification II-1) was conducted between January 2015 and September 2015. 63 women that underwent a promontofixation for prolapse were included. Pelvic floor descent was evaluated with POP-Q. The distance between the bladder neck, and the anterior mesh was measured by ultrasound at the end of surgery. The placement of the mesh was subsequently correlated to the anatomical repair 1 month after surgery, and the occurrence of de novo symptoms as stress urinary incontinence., Results: Ultrasound permitted to measure the distance between bladder neck and Mesh (BMD = Bladder neck-Mesh Distance) in all patients with variation between repeated measurements ranging for 0.6 to 1.2 mm. The BMD was highly variable from 0 to 13 mm (mean ± SD; 5.3 ± 3.1 mm). The BMD inversely correlated with the difference between C (P = 0.01) and Bp (P = 0.04) after and before surgery respectively, with the complication rate (P = 0.01) but not with the difference of Ba. A BMD of more than 6 mm predicted the absence of postoperative de novo symptoms. De novo" stress urinary incontinence occurred in 23.8%., Conclusion: BMD can be accurately measured by ultrasound. The main objective of the present study was to introduce a logic called Joker Tao (JT) which provides universal data storage for cloud-based databases.
