Aim: The aim of the study was to identify factors associated with nurses' pain management practices in older patients undergoing surgery. Design: A cross‐sectional study. Methods: The study sample was 85 Registered nurses working in inpatient surgical units in a tertiary care hospital, Thailand. Data were analysed using descriptive statistics, Pearson's and point biserial correlation coefficients, and stepwise multiple regression analysis. Results: A positive correlation was found between practices and (a) pain management training and (b) perceptions related to collaboration with physicians. However, practices were negatively related to (a) the nurses' perception of their workload and (b) the number of years of nursing experience. The recent pain management training, the nurses' perceptions of both collaboration with physicians and their workload explained 24.40% of the variance in practices. Appropriate nursing workloads, regular pain management training and promoting collaboration among the healthcare team could all improve the quality of nurses' postoperative pain management. Aims and Objectives: To identify and compare frailty instruments used with hospitalised orthopaedic patients aged over 65. Background: Frailty predicts clinical events in orthopaedic patients aged over 65. However, the strengths and limitations of different approaches to measuring frailty in this population are rarely discussed. As such, a comprehensive review to address the gap is needed. Design: Scoping review using Arksey and O'Malley framework. Methods: PubMed, CINAHL, PsycINFO, Scopus and EMBASE databases were searched to identify studies published from 2006 to 2020 regarding frailty instruments in older orthopaedic patients. The Preferred Reporting Items for Systematic Reviews and Meta‐analyses were followed. Results: The initial search resulted in 1,471 articles. After review against inclusion and exclusion criteria, a final set of 31 articles containing 15 unique frailty instruments were evaluated. Most of the articles were from Western countries. Fried's phenotype and Frailty Index were commonly used. The frailty index was mostly modified to measure frailty. In hip fracture, physical function items were frequently modified in the measurement of frailty. Trained physicians and nurses administered most frailty instruments. Frailty screening was commonly conducted at hospital admission and used to prognosticate both postoperative complications and hospital outcomes. Most instruments could be completed within 10 min. Reported psychometrics had acceptable reliability and validity. Conclusion: Many reliable frailty measures have been used in the inpatient orthopaedic settings; however, evidence is still lacking for a gold standard frailty instrument. More research is needed to identify the best‐performing measure. Frailty evaluation in patients with physical limitations is challenging with existing instruments. Clinical context, resources required and instrument quality are essential factors in selecting a frailty instrument. Relevance to clinical practice: Musculoskeletal symptoms in older patients may bias frailty assessment. Background: Kinesiophobia is described as fear of physical movement resulting in painful injury. Older adults with knee osteoarthritis usually suffer from joint pain. Assessment of kinesiophobia is beneficial for prevention of further deterioration in performing activity. Even though, the Tampa Scale of Kinesiophobia (TSK-11) has been developed to briefly examine pain related to fear of movement in patients with chronic pain, the TSK-11 Thai version has not been examined for its validity and reliability yet. Objective: To examine psychometric properties of the TSK-11 Thai version. Materials and Methods: A cross-sectional study with 200 older people with knee osteoarthritis living in the northeastern part of Thailand was used in the present study. Participants were asked to complete the demographic questionnaire, the TSK-11-Thai version, the numeric rating scale (NRS), and the Pain Catastrophizing Scale (PCS) Thai version. To confirm the TSK-11 Thai version validity, construct validity was examined using confirmatory factor analysis. Pearson correlation coefficients were used to confirm the TSK-11-Thai version's convergent validities. For internal consistency reliability, Cronbach's alpha coefficients were also assessed. Results: The results of confirmatory factor analysis indicated that a two-factor model, including somatic factor and activity avoidance, fitted with the data. The TSK11-Thai version was positively correlated with pain catastrophizing. Cronbach's alpha coefficients of the total TSK11-Thai version was at 0.77. For subscale, Cronbach's alpha coefficients of the TSK somatic factor and activity avoidance were 0.61 and 0.69, respectively. Conclusion: The Thai version of TSK-11 has acceptable validity and reliability. The TSK-11-Thai version is suitable to use to examine pain-related fear of movement in patients with knee osteoarthritis for clinical and research purposes. [ABSTRACT FROM AUTHOR]

Objective: To investigate the relationships among locomotive syndrome, depressive symptoms, and quality of life in older adults living in rural areas. Materials and Methods: The present research was a descriptive cross-sectional study. The sample was 160 community-dwelling older people living in sub-districts under the services of five health promoting hospitals located in Chiang Mai, Thailand. The prospective participants were recruited by multi-stage random sampling. They had completed instruments, including The Demographic Questionnaire, The 25-Question Geriatric Locomotive Function Scale, The 15-Item Geriatric Depression Scale, and The World Health Organization Quality of Life Questionnaire in Thai Elderly. The data were analyzed using descriptive statistics, Pearson's production-moment correlation. Results: The findings revealed that the locomotive syndrome was found in 50% of participants with the cut-point score of 16 (mean 30.98, SD 14.03), while 26.9% of the participants revealed depressive symptoms (mean 7.07, SD 1.98). The participants had a good quality of life 79.4% (mean 105.12, SD 9.03). There was a positive correlation between locomotive syndrome and depressive symptoms (r=0.47, p<0.01). An inverse correlation was found between the locomotive syndrome and quality of life (r=-0.56, p<0.01) and between depressive symptoms and quality of life (r=-0.46, p<0.01). Conclusion: Findings from the present study would be useful for the health care providers to design interventions to promote physical function along with psychological well-being. Aim: To examine a causal model of health status among older people with knee osteoarthritis. Methods: A cross‐sectional, correlational design was used with a convenience sample of 220 older Thai people with knee osteoarthritis (mean age 68.96 years; SD = 6.22). Participants were asked to complete a demographic questionnaire, the Pain Catastrophizing Scale, the Tampa Scale of Kinesiophobia, The Medical Outcomes Study Social Support Survey, and the Arthritis Impact Measurement Scales 2‐Short Form. Structural Equation Model was used to examine a hypothesized model. Results: Using Chi‐square, hypothesized model was statistically nonsignificant. A model of health status fitted with the empirical data and explained 19.2% of variance. Pain catastrophizing had negative direct effect on self‐efficacy and health status. Pain catastrophizing also had negative indirect effect on health status through self‐efficacy. Self‐efficacy and social support had positive direct effects on health status. However, pain‐related fear had no direct or indirect effect on health status. Conclusion: Lower level of pain catastrophizing can improve self‐efficacy. Increasing of self‐efficacy and social support can then lead to improvement on health status. This study highlighted the effect of psychosocial factors on health status. It is necessary to develop a program to minimize pain catastrophizing, improve social support, and promote self‐efficacy to improve the health status of older people with knee osteoarthritis.
