1. Uniparental Disomy of Chromosome 4: A Case of Whole Chromosome UPD Presenting with LRBA Deficiency.

Abstract

Purpose: Lipopolysaccharide-responsive beige-like anchor protein (LRBA) encodes a widely expressed cytosolic protein that participates in polarized vesicle trafficking. Homozygous loss-of-function LRBA mutations can lead to immune deficiency due to the lack of immune regulation, classified as a part of Tregopathies. We present a case of a 49-year-old female, with polyarthralgia in metacarpophalangeal and proximal interphalangeal joints bilaterally, and morning stiffness, leading to the diagnosis of rheumatoid arthritis treated with pulse steroid therapy. She had experienced sepsis and in-depth scrutiny revealed panhypogammaglobulinemia. After being referred to the immunology clinic, she was followed under the diagnosis of common variable immunodeficiency (CVID)-like inborn errors of immunity (IEI). Methods and Results: Physical examination and diagnostic follow-up revealed massive splenomegaly accompanied by portal hypertension, and ulcerations in the colon. She also presented with periodic hematuria and dysuria. Cystoscopic biopsy revealed mast cell-derived interstitial cystitis which has not been previously reported in LRBA deficiency in the literature to our knowledge. A multi-gene next-generation sequencing panel performed for immune deficiencies (264 genes and 524 amplicons), resulted in the identification of an apparently homozygous LRBA mutation (p.Arg722His) in the Beige and Chediak-Higashi (BEACH) domain. The SNP array showed copy neutral absence of heterozygosity of the entire chromosome 4, which is consistent with uniparental isodisomy of chromosome 4. Conclusion: In conclusion, this case study underscores the critical role of LRBA in immune regulation and highlights the clinical heterogeneity associated with LRBA deficiency. The patient's presentation with severe immune dysregulation, including massive splenomegaly, portal hypertension, and the novel finding of mast cell-derived interstitial cystitis, expands the clinical spectrum of LRBA mutations. The identification of an apparently homozygous LRBA mutation via next-generation sequencing further emphasizes the importance of genetic analysis in diagnosing monogenic defects manifested as CVID-like phenotype. This is the first reported case of LRBA deficiency due to whole chromosome UPD to our knowledge. 6. Comparison of chest computed tomography and 3-T magnetic resonance imaging results in patients with common variable immunodeficiency.

Abstract

Background: Patients with common variable immunodeficiency (CVID) have an increased incidence of pulmonary infections and require frequent follow-up computed tomography (CT) scans. Purpose: To evaluate the diagnostic performance of 3-T magnetic resonance imaging (MRI) in patients with CVID. Material and Methods: In this prospective study, 3-T MRI was performed in 20 patients with CVID. The patients were imaged with CT and MRI scans on the same day. The MRI protocol included a T2-weighted HASTE sequence (TR=1400 ms, TE=95 ms, slice thickness (ST)=3 mm), T2-weighted BLADE sequence (TR=5379 ms, TE=100 ms, ST=3 mm), and 3D VIBE sequence (TR=3.9 ms, TE=1.32 ms, ST=3 mm). Mediastinal and parenchymal changes were compared. A modified Bhalla scoring system was used in the evaluation of CT and MRI scans. Results: A total of 17 (85%) patients had parenchymal abnormalities identified by CT or MRI. Similar findings were detected with CT and MRI in the assessment of the severity of bronchiectasis (P =0.083), bronchial wall thickening (P =0.157), and mucus plugging (P =0.250). Consolidations were detected with both modalities in all patients. There was excellent concordance between the two modalities in the evaluation of nodules >5 mm (nodule size 5–10 mm, P =0.317; nodule size >10 mm, P =1). However, MRI failed to detect most of the small nodules (<5 mm). Conclusion: 3-T MRI detected mediastinal and parenchymal alterations in patients with CVID and provided findings that correlated well with CT. Despite a few limitations, MRI is a well-suited radiation-free technique for patients requiring longitudinal imaging. [ABSTRACT FROM AUTHOR]

14. Giardia enfeksiyonuna bağlı duodenal lenfoid hiperplazi saptanarak yaygın değişken immün yetmezlik tanısı konulan olgu

Abstract

Common variable immunodeficiency disease is characterized by T and B cell dysfunction and low immunoglobulin levels. Disease is usually associated with repeated upper respiratory tract infections or giardia associated diarrhea. We presented a case who was evaluated for anemia. Upper gastrointestinal system endoscopy revealed multiple nodular lesions in the duodenum. Histopathology revealed the presence of lymphoid hyperplasia as well giardia infection in the duodenum. Hematological evaluation confirmed the diagnosis of common variable immunodeficiency with low immunoglobulin levels. Common variable immunodeficiency should be evaluated in patients with giardia infection and associated lymphoid hyperplasia of the duodenum., Yaygın değişken immün yetmezlik T ve B hücre disfonksiyonu ve düşük immunglobulin seviyeleri ile seyreden nadir görülen bir hastalıktır. Olgularda enfeksiyona yatkınlık söz konusu olup tekrarlayan üst solunum yolu enfeksiyonları veya giardia gibi enfeksiyonlara sekonder diyare sık izlenmektedir. Burada anemi nedeni ile tetkik edilen ve yaygın değişken immün yetmezlik tanısı konan bir olgu sunulmuştur. Üst gastrointestinal sistem endoskopisinde duodenumda çok sayıda nodüler lezyon izlenen olgunun histopatolojik incelemesi, duodenumda lenfoid hiperplazi ve giardia enfeksiyonu varlığını göstermiştir. Yaygın değişken immün yetmezlik şüphesi ile yapılan incelemelerde immunglobulinlerde düşüklük saptanmış ve yaygın değişken immün yetmezlik tanısı konmuştur. Giardia enfeksiyonu ve duodenumda lenfoid hiperplazi ile başvuran olgularda yaygın değişken immün yetmezlik tanısı akla gelmelidir.

Abstract

Diyabet birçok fizyolojik sistemi etkileyen bir hastalıktır. Bağışıklık sistemi de bunlardan biridir ve diyabet bağışıklık sisteminin çeşitli aşamalarını etkilemektedir. Bu yazıda kontrol edilmeyen diyabetin; ciddi ve sık tekrarlayan piyojenik infek- siyonlara ve nötrofil granülasyonunda azalmaya neden olduğu bir olgu sunulmuştur., Diabetes mellitus is a disease that affects many physiological systems. The immune system is one of these and diabetes affects various stages of the immune system. In this case, uncontrolled diabetes mellitus causing serious recurrent pyogenic infections and a reduction in neutrophil granulation is presented.

18. ATOPİK VE NON - ATOPİK HASTANE PERSONELİNDE LATEKS DUYARLILIĞI

Abstract

Natural rubber latex has become an important health problem particularly among hospital employees. In recent years the increase of latex glove use in health care workers for protection against life threatening infections has become the major reason of being sensitized. The investigators advocate that atopy is an important risk factor for latex sensitization. Based on these data we planned to determine latex prick reactivity frequency and it's relation to clinical symptoms in atopic or non-atopic hospital personnel working in departments in which latex glove use frequency is very high. We also enrolled the atopic and non-glove use patients being fallowed by our allergy department to investigate the effect of atopy on latex prick reactivity. We didn' t find significant difference between these groups related to latex sensitization but our latex sensitization ratios were higher than those reported for general population. This study shows that persistent contact with latex products is an important risk factor as being atopic in terms of developing latex sensitization., Doğal lateks duyarlılığı, önemi giderek artan ve özellikle sağlık sektöründe çalışanlarda belirgin morbidite nedeni olan bir sağlık sorunudur. Son yıllarda enfeksiyonla mücadelede lateks eldivenlerin kullanımının özendirilmesi, latekse duyarlanmada artışın temel nedenidir. Ayrıca atopik yapıya sahip olmanın da sensitivite geliştirme riskini arttırdığına yönelik veriler mevcuttur. Bu bilgiden yola çıkarak atopik ve non-atopik, sık eldiven kullanılan bölümlerde görev yapan hastanemiz personelinde, latekse deri testi reaktivitesi oranını ve bunun kliniğe yansımasını belirlemeyi hedefledik. Hastane personeli olmayan ve alerji nedeniyle kliniğimizce takip edilen olguları da çalışmaya dahil ederek, atopinin lateks duyarlanması üzerine etkilerini araştırdık. Çalışmaya dahi edilen üç grup arasında lateks duyarlılığ geliştirme açısından anlamlı fark saptamadık. Ancak tüm gruplarda lateks duyarlılık oranlarını genel populasyon verilerinin üzerinde bulduk. Bu çalışma sonuçlarına göre lateksle sürekli temas edilmesi, lateks duyarlılığ geliştirmede atopiden bağımsız bir risk faktörü olarak görülmektedir.

Abstract

ÖZET Kolelityazis, erişkin toplumun %10-20 gibi yüksek bir oranını ilgilendirmekte ve meydana getirdiği komplikasyonlar nedeniyle önemli bir mortalite ve morbidite nedeni olmaktadır. Her yıl 100-200 /1 milyon hastanın ölümüne neden olduğu tahmin edilen kronik böbrek yetmeliğinde, düşük proteinle beslenme-safra kolesterol saturasyon artışı-lipid profil değişiklikleri ve üremik otonom nöropati gibi faktörler, safra taşı gelişimine katkıda bulunabilir. Günümüze kadar yapılan ve hemodializ olgularının ağırlıklı olarak incelendiği çalışmalarda, kolesistektomi öyküsünü de dikkate alındığı ve safra taşı gelişimine etkisi olabilecek diğer parametrelerin (sex-BMl-kronik KC hastalığı-kan lipid profili-menapoz-gebelik ve çocuk sayısı-ilaç kullanımı-OKS kullanımı-kan Tx sayısı-diabet-sigara/alkol kullanımı-tam biyokimya profili-PTH seviyesi-diet) bağımsız olarak incelenmediği belirlenmiştir. Bu çalışmanın amacı : Kronik böbrek hastalarında ve çeşitli dializ tedavisi gören olgularda, safra taşı prevalansını araştırmaktır. Çalışmaya Ekim 97-Ekim 98 arasında, 5 ayrı dializ merkezinde düzenli hemodializ uygulanan 92 olgu, EÜTF dializ merkezince takip edilen 49 CAPD olgusu yanısıra, kontrol grubunu oluşturan, EÜTF nefroloji polikliniğince renal yetmezlik tanısıyla izlenmekte olan ve dialize girmeyen 88 hastadan oluşan toplam 229 vaka ultrasonografik olarak incelenmiştir. Dializ öncesinde veya dialize başlandıktan sonra kolesistektomi öyküsü verenler, çalışmadan dışlanmıştır. Ayrıca tüm olgularda anamnez kaydedilmiş, yaş, cins, BMİ, tanı, hastalık süresi, dializ süresi-tipi-sıklığı, diet protein içeriği, kan Tx sayısı, aile öyküsü, ilaç-OKS-sigara/alkol kullanım öyküsü, gebelik sayısı, menapoz süresi, diabet süresi değerlendirmeye alınmıştır. SGOT-SGPT-bilurubin-ALP-GGT- albumin-kalsiyum-fosfat-sodyum-potasyum-TG-kolesterol-üre-kreatinin'den oluşan biyokimya tetkikleri yapılmıştır. Olgularda Hct ve bakılabildiği kadarıyla PTH tayini yapılmış, HBV-HCV serolojisi değerlendirmeye alınmıştır. HD, CAPD ve poliklinik grupları arasında SGOT, SGPT, GGT, sodyum, potasyum, fosfat, Hct, PTH, TG düzeyleri ile cinsiyet, gebelik, menapoz, çocuk sayısı, BMİ, aile öyküsü, diabet sıklığı, sigara/alkol kullanımı açılarından anlamlı fark saptanmamıştır. En yüksek albumin ve ALP düzeyleri HD grubunda saptanırken, en yüksek Ca++ ve kolesterol ortalamasına CAPD grubunda rastlanmıştır. HD grubunda 7 kişide (%7.6), CAPD grubunda 4 kişide (%8.2) ve poliklinik grubunda 5 kişide (%5.7) olmak üzere toplam 16 olguda (%7.3) taş belirlenmiş, HD-CAPD-poliklinik grupları arasında taş açısından anlamlı fark saptanmamıştır. Taş saptanan olguların % 62.5'inin erkek ve % 37.5'inin kadın olduğu belirlenmiştir. Taş olan grupta yaş ve BMİ'i ve ALP düzeylerinin anlamlı oranda daha yüksek olduğu saptanmış yine bu grupta Ca++ ve TG düzeylerinin istatistiki anlama ulaşmamakla beraber, taş saptanmayan olgulara kıyasla daha yüksek olduğu, belirlenmiştir. Hastalık süresi, dializ süresi, dializ tipi, hepatit serolojisi, diabet süre ve sıklığı, Hct, kan Tx, PTH düzeyleri, ilaç kullanımı açılarından taş saptanan ve saptanmayan olgular arasında anlamlı fark saptanmamıştır. Ayrıca kolelityazis saptanan grupta, dializ süresinin diğer gruba kıyasla daha kısa olduğu ve farkın istatistiki açıdan anlamlı olduğu belirlenmiştir. Kolelityazis gelişimi üzerine 75etkili olabilecek parametreler 'forward stepwise' yöntemiyle incelendiğinde, önem sırasına göre yaş-BMİ-ALP düzeyi-GGT düzeyindeki artışların, taş gelişimi ile pozitif korelasyon gösterdiği saptanmıştır. Sonuç olarak böbrek yetmezliğinin, safra taşı prevalansına anlamlı etkide bulunmadığı, ancak yaş ve BMİ ile anlamlı korelasyon gösterdiği belirlenmiştir. Ayrıca ALP ve SGOT yüksekliği bulunanlarda, kolelityazis açısından ultrasonografik incelemenin uygun olacağı düşüncesine varılmıştır. 76 84

Abstract

Few reports have been notified regarding systemic allergic reactions in patients being put on angiotensin corverting enzyme (ACE) inhibitor during the course of venom immunotherapy. There have been conflicting suggestions with respect to ACE inhibitor use during the course of VI in the literature. There is no consensus report clarifying this issue in the clinical practice. Herein we reported a case experienced anaphylaxis after being put on angiotensin corverting enzyme (ACE) inhibitor and beta-blocker therapy by reason of heart attack while receiving venom immunotherapy injections with Apis mellifera in the maintenance phase and discussed the likely causitive factors. Abstract

Objective: Parasites might cause atypical and severe infections in immunocompromised hosts. The prevalence of diarrhea among common variable immune deficiency (CVID) syndrome patients varies between 20% and 94%, which indicates that diarrhea and gastrointestinal system (GIS) complaints could be the second leading cause of morbidity in CVID patients after respiratory tract infections. This study aimed to assess the prevalence of intestinal parasites in CVID patients with GIS complaints and diarrhea., Methods: In this study, all cases followed up in the Immunology and Allergy Clinic of Ege University School of Medicine from July 2008 to August 2015 with the diagnosis of CVID were reviewed retrospectively. The stool samples of patients with diarrhea were identified using direct microscopy of native (0.09% NaCl) and Lugol's iodine preparations followed by formol-ethyl acetate concentration to apply modified Kinyoun, trichrome, acid-fast trichrome, and modified trichrome stains for the presence of intestinal parasites., Results: Overall, 26 of 37 CVID patients had diarrhea; white and red blood cells (WBCs and RBCs, respectively) were identified in 11 and 10 of these 26 samples, respectively. Intestinal parasites were found to be present in 7 of the 11 patients with WBCs and 3 of the 10 patients with RBCs. With the addition of patients who neither had WBCs nor RBCs in their stool, a parasitic agent was detected in 13 (50%) of the 26 patients with diarrhea. There was no significant difference between the diarrheic patients with or without intestinal parasites with respect to cramps, fever, nausea and vomiting, tenesmus, bloody feces, and presence of mucus in the stool. Only one patient had malabsorption, which was not associated with intestinal parasites. The most common parasites detected in this study were Cryptosporidium spp. (n=9; 69.2%), Giardia spp. (n=7; 53.8%), and Blastocystis spp. (n=3; 23.1%). We also identified that parasitic diarrhea in CVID patients tended to last longer (M (mean): 16.2 days) than other causes of infectious diarrhea; this is in accordance with previous studies., Conclusion: Cryptosporidium spp. was found be the major cause of parasitic intestinal infection in this patient population. It was concluded that parasitic infections may cause chronic diarrhea, which are major causes of morbidity in CVID patients. Therefore, special attention is necessary for the identification of intestinal parasites in CVID patients with diarrhea.
