The last decade has seen a sharp increase in the number of scientific publications describing physiological and pathological functions of extracellular vesicles (EVs), a collective term covering various subtypes of cell-released, membranous structures, called exosomes, microvesicles, microparticles, ectosomes, oncosomes, apoptotic bodies, and many other names. However, specific issues arise when working with these entities, whose size and amount often make them difficult to obtain as relatively pure preparations, and to characterize properly. The International Society for Extracellular Vesicles (ISEV) proposed Minimal Information for Studies of Extracellular Vesicles ("MISEV") guidelines for the field in 2014. We now update these "MISEV2014" guidelines based on evolution of the collective knowledge in the last four years. An important point to consider is that ascribing a specific function to EVs in general, or to subtypes of EVs, requires reporting of specific information beyond mere description of function in a crude, potentially contaminated, and heterogeneous preparation. For example, claims that exosomes are endowed with exquisite and specific activities remain difficult to support experimentally, given our still limited knowledge of their specific molecular machineries of biogenesis and release, as compared with other biophysically similar EVs. The MISEV2018 guidelines include tables and outlines of suggested protocols and steps to follow to document specific EV-associated functional activities. Finally, a checklist is provided with summaries of key points.

- Abstract
The last decade has seen a sharp increase in the number of scientific publications describing physiological and pathological functions of extracellular vesicles (EVs), a collective term covering various subtypes of cell-released, membranous structures, called exosomes, microvesicles, microparticles, ectosomes, oncosomes, apoptotic bodies, and many other names. However, specific issues arise when working with these entities, whose size and amount often make them difficult to obtain as relatively pure preparations, and to characterize properly. The International Society for Extracellular Vesicles (ISEV) proposed Minimal Information for Studies of Extracellular Vesicles (“MISEV”) guidelines for the field in 2014. We now update these “MISEV2014” guidelines based on evolution of the collective knowledge in the last four years. An important point to consider is that ascribing a specific function to EVs in general, or to subtypes of EVs, requires reporting of specific information beyond mere description of function in a crude, potentially contaminated, and heterogeneous preparation. For example, claims that exosomes are endowed with exquisite and specific activities remain difficult to support experimentally, given our still limited knowledge of their specific molecular machineries of biogenesis and release, as compared with other biophysically similar EVs. The MISEV2018 guidelines include tables and outlines of suggested protocols and steps to follow to document specific EV-associated functional activities. The last decade has seen a sharp increase in the number of scientific publications describing physiological and pathological functions of extracellular vesicles (EVs), a collective term covering various subtypes of cell-released, membranous structures, called exosomes, microvesicles, microparticles, ectosomes, oncosomes, apoptotic bodies, and many other names. However, specific issues arise when working with these entities, whose size and amount often make them difficult to obtain as relatively pure preparations, and to characterize properly. The International Society for Extracellular Vesicles (ISEV) proposed Minimal Information for Studies of Extracellular Vesicles ("MISEV") guidelines for the field in 2014. We now update these "MISEV2014" guidelines based on evolution of the collective knowledge in the last four years. An important point to consider is that ascribing a specific function to EVs in general, or to subtypes of EVs, requires reporting of specific information beyond mere description of function in a crude, potentially contaminated, and heterogeneous preparation. For example, claims that exosomes are endowed with exquisite and specific activities remain difficult to support experimentally, given our still limited knowledge of their specific molecular machineries of biogenesis and release, as compared with other biophysically similar EVs. The MISEV2018 guidelines include tables and outlines of suggested protocols and steps to follow to document specific EV-associated functional activities. Finally, a checklist is provided with summaries of key points.

INTRODUCTION: Pulmonary lymphangioleiomyomatosis (LAM) is a rare disease affecting mainly young women. BACKGROUND: The respiratory manifestations are characterized by a progressive cystic destruction of the lung parenchyma. Extrapulmonary involvement includes benign renal tumours called angiomyolipomas and abdominal lymphatic masses called lymphangioleiomyomas. At the pathological level, the cellular proliferation found in LAM is in part due to the presence of mutations in the tumour suppressor genes TSC1 and TSC2 (Tuberous Sclerosis Complex). These mutations lead to the activation of the mTOR pathway, which is currently the main therapeutic target. mTOR inhibitors such as sirolimus or everolimus have shown a beneficial effect on the decline in pulmonary function and a reduction of angiomyolipoma size, but are necessary in only some patients. PERSPECTIVES: LAM cells have migratory properties mediated by the formation of new lymphatic vessels. They are also able to secrete metalloproteases, which enhance their invasiveness. Moreover, the expression of estrogen and progesterone receptors by LAM cells suggests a possible role for sex hormones in the pathogenesis of the disease. CONCLUSION: A better understanding of mTOR-independent mechanisms would allow the development of novel therapeutic approaches.

National audience; Lymphangioleiomyomatosis is a rare pulmonary disease encountered almost exclusively in women of reproductive age. Pulmonary involvement is characterized by multiple thin-walled cysts in the lungs, recurrent pneumothorax, obstructive lung disorders, and progression to chronic respiratory failure over a mean period of 10 years. Certainty of diagnosis requires a lung biopsy, but international criteria have been proposed for a diagnosis without such a biopsy. International recommendations were recently issued for the diagnosis and treatment of lymphangioleiomyomatosis. Treatment is principally symptomatic and relies on the management of bronchial obstruction by bronchodilators; of hypoxemia by oxygen therapy; of pleural complications by pleurodesis, most often surgical; and of renal angiomyolipomas by percutaneous embolization in cases of hemorrhagic risk. Hormone treatment is not recommended. Hopes are high for mTor inhibitors (sirolimus and everolimus) and treatment trials are currently underway. Lung transplantation must be considered when chronic respiratory failure occurs in patients younger than 60 years.; La lymphangioléiomyomatose est une maladie pulmonaire rare rencontrée quasi exclusivement chez la femme en période d’activité génitale. L’atteinte pulmonaire est caractérisée par des kystes pulmonaires multiples à parois fines, des pneumothorax récidivants, un trouble ventilatoire obstructif, et une évolution vers l’insuffisance respiratoire chronique dans un délai moyen de 10 ans. Le diagnostic de certitude repose sur la biopsie pulmonaire, mais des critères diagnostiques internationaux ont été proposés pour le diagnostic en l’absence de biopsie pulmonaire. Des recommandations internationales ont été récemment établies pour le diagnostic et la prise en charge thérapeutique de la lymphangioléiomyomatose. Le traitement est principalement symptomatique, et repose sur la prise en charge de l’obstruction bronchique par les bronchodilatateurs ; de l’hypoxémie par l’oxygénothérapie ; des complications pleurales par pleurodèse le plus souvent chirurgicale ; des angiomyolipomes rénaux par embolisation percutanée en cas de risque hémorragique. Le traitement hormonal n’est pas recommandé. Des espoirs reposent sur les inhibiteurs de mTOR (sirolimus et évérolimus) pour lesquels des essais thérapeutiques sont en cours. National audience; Oncogenes and their mechanisms of transformation have been known for more than 30 years. These genes have undergone,certain alterations and produce a protein, which has a deregulated function that is capable of transforming cells. These proteins can be found in numerous cell types that are closely involved in the life and death of cells including the induction and control of growth, differentiation or apoptosis. The most recognised specific treatment developed against an oncogene is Gleevec (R) (imatinib), which acts against the protein from oncogene Ab1 found in chronic myeloid leukaemia.; Les oncogènes et leurs mécanismes de transformation sont connus depuis plus de 30 ans. Ces gènes ont subi certaines altérations et produisent une protéine dont la fonction est dérégulée, capable de transformer les cellules. Ces protéines peuvent être répertoriées dans de nombreuses classes, qui sont intimement impliquées dans la vie et la mort des cellules : induction et régulation de la prolifération, différenciation ou apoptose. L’exemple le plus connu de traitement spécifique développé contre un oncogène est le Gleevec® (imatinib) qui agit contre la protéine issue de l’oncogène Abl dans les leucémies myéloïdes chroniques.

National audience; Jaagsiekte Sheep RetroVirus (JSRV) est un bétarétrovirus, de la famille des rétrovirus, infectant les petits ruminants. Il induit chez le mouton un adénocarcinome pulmonaire, adénomatose pulmonaire, lié à la transformation des cellules épithéliales du poumon profond que sont les pneumocytes de type II dans l’alvéole et les cellules de Clara dans la bronchiole. Cette tumeur est apparentée au cancer bronchioloalvéolaire, une forme particulière de cancer du poumon chez l’homme, pour laquelle une étiologie virale a été suggéré e depuis de nombreuses années, mais non formellement prouvée. Il est maintenant clairement établi que JSRV induit la transformation cellulaire, in vitro et in vivo, via son enveloppe. La transformation met en jeu l’activation des voies Pi3k/Akt et MAPk et une dérégulation de la sénescence réplicative.

JSRV (jaagsiekte sheep retrovirus) is a betaretrovirus, infecting small ruminants. This virus is responsible for the development of pulmonary adenocarcinoma, by the transformation of epithelial cells of the bronchioli and alveoli. This animal cancer is related to human bronchioloalveolar cancer (BAC), a specific form of human lung cancer for which a viral etiology has been proposed for several decades. In small ruminants JSRV interacts with the cells through the Hyal2 receptor. JSRV genome is simple and does not contain already known oncogene. It is now well established that the envelope protein is oncogenic by itself, via the cytoplasmic domain of the transmembrane glycoprotein and some domains of the surface glycoprotein. Activation of the PI3K/Akt and MAPK pathways participates to the envelope-induced transformation. The tumour development is associated with telomerase activation., Le jaagsiekte sheep retrovirus (JSRV) est un bêtarétrovirus, de la famille des rétrovirus, infectant les petits ruminants. Il induit un adénocarcinome pulmonaire, un cancer du poumon se développant par transformation des cellules épithéliales du poumon profond que sont les pneumocytes de type II et les cellules de Clara. Cette tumeur est apparentée au cancer bronchioloalvéolaire, une forme particulière de cancer du poumon chez l’homme, pour laquelle une étiologie virale a été suggérée depuis de nombreuses années. Le JSRV interagit avec la cellule par sa fixation sur le récepteur Hyal2. Son génome est simple et ne contient pas d’oncogène connu. Il est maintenant clairement établi qu’il induit la transformation cellulaire, in vitro et in vivo, via son enveloppe. Son pouvoir transformant implique le domaine intracytoplasmique de la transmembrane et certains domaines de la glycoprotéine de surface. La transformation met en jeu l’activation des voies PI3K/Akt et MAPK ; le développement de la tumeur est associé à l’activation de la télomérase.
