Introduction Mental disorders are common in adult patients with traumatic injuries. To limit the burden of poor psychological well-being in this population, recognised authorities have issued recommendations through clinical practice guidelines (CPGs). However, the uptake of evidence-based recommendations to improve the mental health of trauma patients has been low until recently. This may be explained by the complexity of optimising mental health practices and interpretating CGPs scope and quality. Our aim is to systematically review CPG mental health recommendations in the context of trauma care and appraise their quality. Methods and analysis We will identify CPG through a search strategy applied to Medline, Embase, CINAHL, PsycINFO and Web of Science databases, as well as guidelines repositories and websites of trauma associations. We will target CPGs on adult and acute trauma populations including at least one recommendation on any prevention, screening, assessment, intervention, patient and family engagement, referral or follow-up procedure related to mental health endorsed by recognised organisations in high-income countries. No language limitations will be applied, and we will limit the search to the last 15 years. Pairs of reviewers will independently screen titles, abstracts, full texts, and carry out data extraction and quality assessment of CPGs using the Appraisal of Guidelines Research and Evaluation (AGREE) II. We will synthesise the evidence on recommendations for CPGs rated as moderate or high quality using a matrix based on the Grading of Recommendations Assessment, Development and Evaluation quality of evidence, strength of recommendation, health and social determinants and whether recommendations were made using a population-based approach. Ethics and dissemination Ethics approval is not required, as we will conduct secondary analysis of published data. Introduction : Les personnes ayant un trouble lié à l’usage d’opioïdes (TUO) se retrouvent couramment en situation de vulnérabilité médicale et psychosociale. Au cours des dernières années, des études ont mis en lumière des changements dans les caractéristiques démographiques et biopsychosociales des personnes ayant un TUO. Afin de soutenir une offre de services tenant compte de ces caractéristiques, la présente étude vise à 1) identifier différents profils de personnes ayant un TUO; 2) décrire la perspective de personnes ayant un TUO en lien avec leurs besoins, leurs expériences de services et leur définition du rétablissement; et 3) distinguer les besoins en fonction des différents profils pour soutenir le développement d’une offre de services mieux adaptée. Méthode : Un devis mixte séquentiel explicatif a été employé. Au volet quantitatif, des variables démographiques et cliniques ont été colligées dans un échantillon de 296 dossiers de patients d’un centre de traitement spécialisé à Montréal. Des analyses descriptives ainsi qu’une analyse de classes latentes ont été effectuées afin d’identifier différents profils. Au volet qualitatif, 48 entrevues semi-dirigées ont été réalisées avec des personnes ayant une expérience de TUO. Les données ont été soumises à une analyse thématique visant à contraster les perspectives en fonction des profils cliniques identifiés, afin de soutenir une offre de services mieux adaptée. Résultats : L’analyse de classes latentes a fait émerger trois profils caractérisés par : 1) la précarité (37 %); 2) la stabilité (33 %); et 3) la douleur chronique (30 %). L’analyse thématique des entrevues met en relief des différences entre les profils, en ce qui concerne les préoccupations, les besoins et les expériences de traitement. Notamment, les personnes en situation d’itinérance mettent l’emphase sur l’importance de répondre à leurs besoins de base (c.-à-d. manger, dormir, s’habiller). Over the last years, studies have highlighted changes in demographic and biopsychosocial characteristics of people with OUD. In order to support a service offer that is tailored to distinct patient characteristics, this study aims to 1) identify different profiles of people with an OUD; 2) describe the perspective of people with an OUD in terms of needs, service experiences and recovery objectives; and 3) differentiate needs and experiences between profiles to support the development of a tailored service offer. Methods: A mixed methods explanatory sequential design was employed. In the quantitative strand of the study, demographic and clinical variables were collected in a sample of 296 patient charts in a Montréal specialized opioid agonist treatment (OAT) center. Descriptive analyses and a latent class analysis were conducted to identify different profiles. In the qualitative strand of the study, 48 semi-structured interviews were conducted with people presenting an OUD history. A thematic analysis was performed on the qualitative data to contrast perspectives between the identified clinical profiles, in order to support a tailored service offer. Results: The latent class analysis revealed three profiles characterized by: 1) precariousness (37%); 2) stability (33%); and 3) chronic pain (30%). Thematic analysis of the verbatim highlights differences between profiles with regards to OUD and OAT experiences. Notably, people experiencing homelessness accentuate the importance of fulfilling their basic needs (i.e., eating, sleeping, clothing). People presenting higher biopsychosocial stability insist on the importance of self-determination.
