En la presente contribución se contextualiza, analiza e interpreta una efigie femenina antropomorfa galo-romana de plomo conservada en el Musée Dobrée de Nantes (Francia), atendiendo a las manipulaciones rituales de intensidad variada que presenta a lo largo de su fisonomía. La documentación de estrategias coercitivas tales como la agresión craneal, la doble transfixión ocular, la mutilación de parte de las extremidades superiores e inferiores, y una incisión en los genitales, que han sido constatadas en un amplio número de figurillas mágicas greco-romanas, permiten proponer una nueva interpretación sobre la funcionalidad de esta efigie, que no sería una pieza votiva sino que representaría a la víctima de un ritual mágico de atracción erótica (agogai), quien, en virtud de dicha execración, experimentaría un deseo y pasión irrefrenable por el defigens.

5. Bilateral Paracentral Corneal Melting and Left-Eye Perforation under Tobemstomig Novel Treatment

Abstract

Introduction: We present a rare occurrence of bilateral corneal melting and a left-eye corneal perforation in an oncologic patient undergoing a new biological therapy. Case Presentation: A 63-year-old male with a two-day history of a painful left red eye and bilateral visual impairment was enrolled in a multicenter phase-II study comparing tobemstomig/RO7247669, a PD1-LAG3 bispecific antibody, with nivolumab. Clinical examination revealed a bilateral central corneal thinning, and corneal OCT imaging indicated a significant stromal thinning of 124 μm in the right eye and a central corneal perforation of 286 μm in the left eye. Subsequently, the patient underwent surgical intervention involving an autologous partial scleral patch with a Gundersen conjunctival flap in the left eye, alongside a comprehensive topical and systemic treatment regimen. Due to this immune-related adverse event, the patient was excluded from the clinical trial subsequently later revealing he had been on the bispecific treatment. Conclusion: While immune checkpoint inhibitors hold promise in oncology, they can lead to ocular surface issues, including dry-eye keratitis and, in severe cases, anterior segment thinning culminating in corneal perforation. Timely withdrawal of immunotherapy, coupled with multi-level treatment involving anti-inflammatory and corneal healing approaches, is crucial. In cases of corneal perforation, surgical intervention such as cyanoacrylate application or tectonic surgery becomes imperative.

The direct and simple fluorination of representative organic laser dyes with emission covering the entire visible spectrum, from blue to red, including Coumarin 460, Pyrromethene 546, Rhodamine 6G and Perylene Red, enhances laser efficiencies by a factor up to 1.8 with respect to the corresponding non-fluorinated parent dyes. 14. Chlorinated BODIPYs: Surprisingly Efficient and Highly Photostable Laser Dyes

Abstract

A series of mono‐ to hexachlorinated BODIPY dyes have been prepared in good to excellent yields through the use of chlorosuccinimide as an inexpensive halogenating reagent. This library of chlorinated dyes allowed analysis in detail, from the experimental and theoretical points of view, of the dependency of the photophysical and optical properties of the dyes on the number and positions of the chlorine substituents on their BODIPY cores. Quantum mechanical calculations predict the regioselectivity of the halogenation reaction and explain why some positions are less prone to chlorination. The new chlorinated BODIPYs exhibit enhanced laser action with respect to their non‐halogenated analogues, both in liquid solution and in the solid phase. Abstract

Background and aimsDespite enteral nutrition (EN) is the preferred route of nutrition in patients with critical illness, EN is not always able to provide optimal nutrient provision and parenteral nutrition (PN) is needed. This is strongly associated with gastrointestinal (GI) complications, a feature of gastrointestinal dysfunction and disease severity. The aim of the present study was to investigate factors associated with the need of PN after start of EN, together with the use and complications associated with EN.MethodsAdult patients admitted to 38 Spanish intensive care units (ICUs) between April and July 2018, who needed EN therapy were included in a prospective observational study. The characteristics of EN-treated patients and those who required PN after start EN were analyzed (i.e., clinical, laboratory and scores).ResultsOf a total of 443 patients, 43 (9.7%) received PN. One-third (29.3%) of patients presented GI complications, which were more frequent among those needing PN (26% vs. 60%, p = 0.001). No differences regarding mean energy and protein delivery were found between patients treated only with EN (n = 400) and those needing supplementary or total PN (n = 43). Abnormalities in lipid profile, blood proteins, and inflammatory markers, such as C-Reactive Protein, were shown in those patients needing PN. Sequential Organ Failure Assessment (SOFA) on ICU admission (Hazard ratio [HR]:1.161, 95% confidence interval [CI]:1.053–1.281, p = 0.003) and modified Nutrition Risk in Critically Ill (mNUTRIC) score (HR:1.311, 95% CI:1.098–1.565, p = 0.003) were higher among those who needed PN. In the multivariate analysis, higher SOFA score (HR:1.221, 95% CI:1.057–1.410, p = 0.007) and higher triglyceride levels on ICU admission (HR:1.004, 95% CI:1.001–1.007, p = 0.003) were associated with an increased risk for the need of PN, whereas higher albumin levels on ICU admission (HR:0.424, 95% CI:0.210–0.687, p = 0.016) was associated with lower need of PN.ConclusionA higher SOFA and nutrition-related laboratory parameters on ICU admission may be associated with the need of PN after starting EN therapy. This may be related with a higher occurrence of GI complications, a feature of GI dysfunction.Clinical trial registrationClinicalTrials.gov: NCT03634943.

Abstract

A new library of E- and C-4,4-difluoro-4-bora-3a,4adiaza-s-indacene (BODIPY) derivatives has been synthesized through a straightforward protocol from commercially available BODIPY complexes, and a systematic study of the photophysical properties and laser behavior related to the electronic properties of the B-substituent group (alkynyl, cyano, vinyl, aryl, and alkyl) has been carried out. The replacement of fluorine atoms by electron-withdrawing groups enhances the fluorescence response of the dye, whereas electrondonor groups diminish the fluorescence efficiency. As a consequence, these compounds exhibit enhanced laser action with respect to their parent dyes, both in liquid solution and in the solid phase, with lasing efficiencies under transversal pumping up to 73% in liquid solution and 53% in a solid matrix. The new dyes also showed enhanced photostability. In a solid matrix, the derivative of commercial dye PM597 that incorporated cyano groups at the boron center exhibited a very high lasing stability, with the laser emission remaining at the initial level after 100 000 pump pulses in the same position of the sample at a 10 Hz repetition rate. Distributed feedback laser emission was demonstrated with organic films that incorporated parent dye PM597 and its cyano derivative. The films were deposited onto quartz substrates engraved with appropriate periodical structures. Abstract

We report the synthesis of novel multichromophoric organic architectures, where perylene red is decorated with BODIPY and/or hydroxycoumarin dyes acting as light harvesters and energy donors. The computationally-aided photophysical study of these molecular assemblies reveals a broadband absorption which, regardless of the excitation wavelength, leads solely to a bright red-edge emission from perylene bisimide after efficient intramolecular energy transfer hops. The increase of the absorbance of these molecular antennas at key pumping wavelengths enhances the laser action of the commercial perylene red. Abstract

Linking amino and hydroxycoumarins to BODIPYs through the amino or hydroxyl group lets the easy construction of unprecedented photostable coumarin–BODIPY hybrids with broadened and enhanced absorption in the UV spectral region, and outstanding wavelength-tunable laser action within the green-to-red spectral region (∼520–680 nm). These laser dyes allow the generation of a valuable tunable UV (∼260–350 nm) laser source by frequency doubling, which is essential to study accurately the photochemistry of biological molecules under solar irradiation. The tunability is achieved by selecting the substitution pattern of the hybrid. Rootstock micropropagation has been extensively used as an alternative to propagation by cuttings. Although studies have recently been conducted on other species, no conclusive reports have been published on the effect of rootstock micropropagation on the field performance of fruit trees. Here, we present the results of a five-year study of peach varieties grafted on two rootstocks (Adesoto 101 and Adafuel), either micropropagated or propagated by cuttings, to ascertain the effect of the rootstock propagation method on field performance. Fruit trees are woody plants with a long life cycle; so, to reduce the influence of environmental or cultural factors on the agronomical results, studies need to last for several years, in which data are obtained from the same individuals over time (longitudinal data). This hinders the analysis because these data lack independence. In contrast with a more traditional approach with data aggregation and repeated-measures ANOVA analysis, in this study, we used linear mixed-effects models to control the variance associated with random factors without data aggregation. The growth of the fruit trees did not appreciably differ between the rootstock propagation methods, neither in the flowering period nor in the yield. The models constructed for different parameters of the field performance (trunk cross-sectional area (TCSA), cumulative yield, cumulative yield efficiency, and cumulative crop load) showed a very good fit (R2 > 0.97), allowing the conclusion that the rootstock propagation method did not affect the field performance of fruit trees in this study.

Abstract

Background: Brief interventions (BI) for risky drinkers in primary healthcare have been demonstrated to be cost-effective but they are still poorly implemented. Digital BI seems to be a complementary strategy to overcome some barriers to implementation but there is a scarcity of studies in clinical environments. We present the results of a randomized controlled non-inferiority trial which tests the non-inferiority of facilitated access to a digital intervention (experimental condition) for risky drinkers against a face-to-face BI (control condition) provided by primary healthcare professionals. Method: In a non-inferiority randomized controlled trial, unselected primary healthcare patients (≥ 18 years old) were given a brief introduction and asked to log on to the study website to fill in the 3-item version of the Alcohol Use Disorders Identification Test. Positively screened patients (4+ for women and 5+ for men) received further online assessment (AUDIT, socio-demographic characteristics and EQ-5D-5L) and were automatically randomized to either face-to-face or digital BI (1:1). The primary outcome was the proportion of patients classified as risky drinkers by the digitally administered AUDIT at month 3. A multiple imputation approach for the missing data was performed. Results: Of the 4499 patients approached by 115 healthcare professionals, 1521 completed the AUDIT-C. Of the 368 positively screened patients, 320 agreed to participate and were randomized to either intervention. At month 3, there were more risky drinkers in the experimental group (59.8%) than in the control group (52%), which was similar to the distribution at baseline and less than the pre-specified margin of 10%. The difference was not significant when accounting for possible confounders. Conclusion: Digital BI was not inferior to face-to-face BI, in line with previous findings and the a priori hypothesis. However, the low power of the final sample, due to the low recruitment and loss to follow-up, limits the interpretation of the findings. New approaches in this field are required to ensure the effective implementation of digital interventions in actual practice.

Abstract

[ES] En la última década hemos presenciado un cambio muy acelerado en el cultivo de pistacho en España: superficie, material vegetal, cultivo ecológico, aparición de inversiones por grandes capitales, etc. España, con más de un 1% de la producción mundial, puede tener un nicho: producir un pistacho de calidad y ecológico. En este trabajo repasamos los avances realizados en el cultivo y lo aportado desde nuestro grupo de investigación respecto a dos temas importantes: el injerto y la micropropagación de portainjertos clonales selectos. Por último, analizamos algunos de los retos a los que se enfrenta el cultivo en la actualidad: la aparición de plagas y enfermedades por la mayor superficie plantada, la falta de una certificación de la planta producida, la distorsión del mercado local por la aparición de macroplantaciones de 1.000 ha o más o la desaparición de equipos de investigación que ya no darán apoyo a los agricultores., [EN] In the last decade we have witnessed a very rapid change in the cultivation of pistachio in Spain: area, plant material, organic cultivation, the appearance of investments by large capitals, etc. Spain, with more than 1% of world production, may have a niche: producing a quality and organic pistachio. In this paper we review the advances made in cultivation and the contributions from our research group regarding two important topics: grafting and micropropagation of select clonal rootstocks. Finally, we analyze some of the challenges that the crop currently faces: the appearance of pests and diseases due to the greater planted area, the lack of certification of the plant produced, the distortion of the local market due to the appearance of macro–plantations of 1,000 ha or more or the disappearance of research teams that will no longer support farmers.
