Abstract Alzheimer disease (AD) and Alzheimer Disease and Related Dementias (AD/ADRD) are growing public health challenges globally affecting millions of older adults, necessitating concerted efforts to advance our understanding and management of these conditions. AD is a progressive neurodegenerative disorder characterized pathologically by amyloid plaques and tau neurofibrillary tangles that are the primary cause of dementia in older individuals. Early and accurate diagnosis of AD dementia is crucial for effective intervention and treatment but has proven challenging to accomplish. Although testing for AD brain pathology with cerebrospinal fluid (CSF) or positron emission tomography (PET) has been available for over 2 decades, most patients never underwent this testing because of inaccessibility, high out‐of‐pocket costs, perceived risks, and the lack of AD‐specific treatments. However, in recent years, rapid progress has been made in developing blood biomarkers for AD/ADRD. Consequently, blood biomarkers have emerged as promising tools for non‐invasive and cost‐effective diagnosis, prognosis, and monitoring of AD progression. This review presents the evolving landscape of blood biomarkers in AD/ADRD and explores their potential applications in clinical practice for early detection, prognosis, and therapeutic interventions. It covers recent advances in blood biomarkers, including amyloid beta (Aβ) peptides, tau protein, neurofilament light chain (NfL), and glial fibrillary acidic protein (GFAP). It also discusses their diagnostic and prognostic utility while addressing associated challenges and limitations. Future research directions in this rapidly evolving field are also proposed.

We consider the detection and characterisation of brain state transitions, based on ongoing Magneto and Electroencephalography (M/EEG). Here a brain state represents a specific brain dynamical regime or mode of operation, which produces a characteristic quasi-stable pattern of activity at topography, sources or network levels. These states and their transitions over time can reflect fundamental computational properties of the brain, shaping human behaviour and brain function. The Hidden Markov Model (HMM) has emerged as a useful model-based approach for uncovering the hidden dynamics of brain state transitions based on observed data. However, the Geometric distribution of state duration (dwell time) implicit in HMM places highest probability on very short durations, which makes it inappropriate for the accurate modelling of brain states in M/EEG. We propose using Hidden Semi Markov Models (HSMM), a generalisation of HMM that models the brain state duration distribution explicitly. We present a Bayesian formulation of HSMM and use the Variational Bayes framework to efficiently estimate the HSMM parameters, the number of brain states and select among alternative brain state duration distributions. We assess HSMM performance against HMM on simulated data and demonstrate that the accurate modelling of state duration is paramount for accurately and robustly modelling non-Markovian EEG brain state features. Finally, we used actual resting-state EEG data to demonstrate the approach in practice and conclude that it provides a flexible parameterised framework that permits closer interrogation of possible generative mechanisms.

A cross-sectional study was done to determine the prevalence of fasciolosis in cattle slaughtered in abattoirs of Costa Rica and to compare 4 different diagnostic techniques (macroscopic examination of the livers, standard sedimentation of stool, standard sedimentation of the fluid of the gallbladder, and detection of copro-antigen in fecal samples. The sample was divided into two strata of probability of infection (high and low). Considering all the techniques, 11 positive samples from the stratum of high probability were found [prevalence 4.3% (confidence interval CI 95%: 1.8-6.7)] and 1 of the stratum of low probability [prevalence 0.5% (CI 95%: 0.0-1.4)]. When comparing all diagnostic methods, considering the liver condemnation as the reference method, values of sensitivity and specificity respectively of 40 and 100% for the standard sedimentation of stool, 70 and 99.8% for the standard sedimentation of the fluid of the gallbladder and 50 and 100% for the ELISA that detects copro-antigen in feces were obtained. It is concluded that the macroscopic examination of livers in slaughterhouses is a good tool for the monitoring of the disease and that this parasite is distributed heterogeneously in the country. Se realizó un estudio transversal con el objetivo de determinar la prevalencia de fasciolosis en bovinos sacrificados en mataderos de Costa Rica y comparar cuatro técnicas diagnósticas disponibles (inspección macroscópica de hígados, sedimentación estándar de heces, detección de coproantígeno en heces y sedimentación estándar del líquido de la vesí-cula biliar). Se trabajó con dos estratos de probabilidad de infección (alto y bajo). Tomando en cuenta todas las técnicas diagnósticas, se encontraron 11 muestras positivas del estrato de alta probabilidad [prevalencia 4,3% (intervalo de confianza IC del 95%:1,8-6,7)] y 1 del estrato de baja probabilidad [prevalencia 0,5% (IC 95%: 0,0-1,4)]. Al comparar los métodos diagnósticos, tomando el decomiso de hígados como método de referencia, se obtuvieron valores de sensibilidad y especificidad de 40 y 100% para la sedimentación estándar de heces, 50 y 100% para la detección de coproantígeno en heces (ELISA), y 70 y 99,8% para la sedimentación estándar del líquido de la vesícula biliar, respectivamente. Se concluye que la inspección macroscópica de hígados en los mataderos es una buena herramienta de vigilancia de la enfermedad y que esta parasitosis se distribuye de manera heterogénea en el país. El rash es un efecto secundario común asociado al uso de fármacos antiepilépticos. La frecuencia de rash con fenitoína se ha estimado en un 5,9% y asciende a un 25% en pacientes que han presentado rash con otro fármaco antiepiléptico. La anemia aplásica es una anomalía adquirida de las células madre hematopoyéticas caracterizada por pancitopenia de la sangre periférica y médula ósea hipocelular. Los pacientes tratados con fenitoína presentan un riesgo 3,5 veces mayor de desarrollar anemia aplásica. Presentamos el caso de una mujer de 70 años que desarrolló dos reacciones adversas severas y simultáneas a la fenitoína: un exantema maculopapular pruriginoso con compromiso de mucosas y una anemia aplásica. Ambas condiciones se resolvieron completamente con la suspensión del fármaco. Rash is a common side effect associated with antiepileptic drugs. The rate of a phenytoin rash is 5.9% and increases to 25% in those with another antiepileptic drug rash. Aplastic anemia is an adquired hematopoietic stem-cell disorder characterized by pancytopenia of the peripheral blood and hypocellular bone marrow. The use of phenytoin is associated with a 3.5 fold increased risk of aplastic anemia. We report a case of a 70-year-old woman who developed two severe adverse reactions simultaneously with phenytoin: a maculopapular pruritic rash with involvement of mucous and an aplastic anemia. Both conditions normalized after phenytoin withdrawal.

8. New mobilities for accessible cities: Toward scenarios for seamless journeys

- Abstract
While cities increasingly attest to plans to make their resources accessible for people with disabilities, the realities of achieving the travel considered integral to urban life continue to be frustrating and prohibitive for this group. Accessing the basic opportunities of contemporary urban life now presupposes the supports and resources afforded by new mobilities, combining virtual and actual travel and communication in negotiating our work, leisure, connections with families and culture. For the researchers applying the new mobilities paradigm, this requires a focus which is suited to capturing movement and its spatial and temporal coordinates and should also turn to illuminate the darker side of these relationships: coerced immobility experienced by people with disabilities. This chapter discusses an approach to research and the development of design scenarios – concepts emerging from research that may inform design - that take seriously the role of movement, time and space in the achievement of valued connections by individuals with disabilities with particular reference to the journey to work. In particular we apply, in a case study, concepts of time and space that are relevant to the in situ experience of getting to work; raising questions regarding the way getting ready and travelling are experienced in the context of risk and contingency and the actual and potential role of the technical, material and social environment. We then respond to the analysis of this case with a discussion about the way emergent scenarios can imagine “possible or preferable futures” for the mobile citizenship of people with disabilities.

9. Surviving the electronic panopticon: new lessons in democracy, surveillance, and community in young adult fiction

The ability of new information and communication technologies to pierce previously impenetrable physical, personal, and social boundaries has particular relevance to contemporary society and young people as there is now more information that can be collected, accessed, and distributed about individuals and groups. The ability to know about each other has become a central feature of many young people’s lives. The need to know is further complicated by other questions – Who knows? What do they know? What are the implications of this knowledge?. These questions are a consequence of society having become more mobile and networked enabling increased surveillance, tracking, and spreading of dis/information. With the acceleration of new pervasive and immersive technologies, these questions have taken on a new urgency and significance that go beyond an Orwellian Big Brother scenario. This chapter extends Foucault’s notion of the panopticon to take account of the challenges of an AmI environment of smart networked devices. By drawing on examples of recent young adult fiction, I examine some of the ways in which these texts invite their readers to reflect and speculate on the uneasy relationship between surveillance and democracy and what this means for individual rights and freedom, and a sense of place and belonging.

- Abstract
BACKGROUND: Cystic echinococcosis (CE) is an important zoonotic disease caused by the cestode parasite Echinococcus granulosus. It occurs in many parts of the world where pastoral activities predominate, including the Rio Negro province of Argentina. Although CE control activities have been undertaken in the western regions of Rio Negro for more than two decades, the disease continues to remain prevalent in both the human and livestock animal populations. Vaccination of animal intermediate hosts of CE with the EG95 vaccine may provide a new opportunity to improve the effectiveness of CE control measures, although data are lacking about field application of the vaccine. AIMS: Evaluate the impact of EG95 vaccination in sheep on the transmission of Echinococcus granulosus in a field environment. METHODOLOGY: Two trial sites were established in western Rio Negro province within indigenous communities. Vaccination of lambs born into one trial site was introduced and continued for 6 years. Prior to initiation of the trial, and at the end of the trial, the prevalence of CE in sheep was determined by necropsy. Weaned lambs received two injections of EG95 vaccine, approximately one month apart, and a single booster injection one year later. Vaccination was not implemented at the second trial site. A total of 2725 animals were vaccinated in the first year. Animals from this cohort as well as age-matched sheep from the control area were evaluated by necropsy. KEY RESULTS: Introduction of the vaccine led to a statistically significant in the number and size of hydatid cysts in comparison to the situation prior to the introduction of the vaccine, or compared to CE prevalence in the control area where the vaccine was not applied. The prevalence of infection in the vaccinated area was also significantly reduced by 62% compared to the re-intervention level, being lower than the prevalence seen in the control area, although the difference from the control area after the intervention

Tuberculosis (TB) is a worldwide infectious disease, caused by Mycobacterium tuberculosis (Koch bacilli), that has re-emerged since the decade of the 80's in relation to the pandemic of HIV infection. Chile has one of the lowest TB prevalence rates in Latin America. In children, TB exhibits some differences from adult disease in terms of pathogenesis, clinical manifestations and probability of progression from the infected state to disease, making it more difficult to diagnose and increasing the likelihood of developing the disease once the infection is acquired. There is a National Program for the Prevention and Control of TB that allows us to develop prevention and chemoprophylaxis strategies. This article summarizes these strategies to guide the study and management of children in contact with TB patients.

La tuberculosis (TBC) es una enfermedad de distribución mundial, producida por Mycobacterium tuberculosis (bacilo de Koch), que ha re-emergido desde la década de los 80 en relación a la pandemia de VIH. Chile es uno de los países latinoamericanos con menores tasas de prevalencia de TBC. En los niños, la TBC presenta algunas características distintas al adulto en cuanto a patogenia, clínica y probabilidad de progresión desde una infección latente a enfermedad, siendo más difícil realizar el diagnóstico y mayor la probabilidad de desarrollar enfermedad una vez infectados. Existe un Programa Nacional de Prevención y Control de la TBC que nos permite plantear estrategias de prevención y quimioprofilaxis. Este artículo esquematiza estas estrategias para orientar el estudio y manejo de niños que entran en contacto con pacientes con TBC.

Introducción: El SAHOS presenta una alta prevalencia en estudios internacionales, con importantes repercusiones en los sistemas cardio, cerebrovascular y en la calidad de vida de las personas. Método: Estudiamos prospectivamente a los pacientes enviados para Polisomnografia (PSG) a nuestro laboratorio de sueño, en un período de 6 meses. Previo consentimiento informado, se les aplicó la Escala de Somnolencia de Epworth (EE) y un Cuestionario de Sueño. Fueron entrevistados 100 pacientes, excluyéndose 3 por no haberse realizado PSG y 2 que no completaron la EE. Resultados: Se analizaron 95 pacientes, 89,5% hombres, edad promedio 47,4± 12,5 años, obesidad en 49,5%, EE mayor de 11 en 56,8%, ronquido en 93,7%, pausas respiratorias en 68,4%, somnolencia diurna excesiva en 57,9%, cansancio al despertar en 86,3%o. La PSG no demostró SAHOS en 14,7%, hubo 27,4% con SAHOS leve, 21 %> moderado y 36,8%> severo. Las variables presencia de pausas respiratorias observadas por la pareja e índice de masa corporal (IMC) predicen la ausencia de SAHOS severo, con una sensibilidad de 87% y especificidad de 50%. Conclusión: En la evaluación de pacientes con sospecha de SAHOS, la no observación de pausas respiratorias durante el sueño por parte de la pareja y el IMC menor de 30 Kg/m² hacen menos probable que exista un SAHOS severo. Introduction: Obstructive sleep apnea syndrome (OSAS) is a common disease associated with significant morbidity, including excessive daytime sleepiness, cardiovascular disease and stroke. Method: We studied prospectively the patients sent to our sleep laboratory for polysomnography (PSG) during 6 months. One-hundred patients were interviewed with a sleep questionnaire, 3 of them were ruled out because the lacking of PSG and 2 who no completed the Epworth sleep scale (ESS). Results: Out of the 95 patients, there were 85 men and 10 women, with a mean age of 47.4 ± 12.5 years, obesity was found in 42.5%, an ESS greater than 11 points was found in 56.8%, loud snoring in 93.7%, breathing cessation in 68.4%, excessive daytime somnolence in 57.9%. PSG revealed no OSAS in 14.7%, slight OSAS in 27.4%, moderate OSAS in 21% y severe OSAS in 36.8%. Body mass index and breathing cessation reported by the couple had the highest discriminative power with a sensibility of 87% and specificity of 50% for ruled out severe OSAS. Conclusion: A severe OSAS is less probable when there is absence of breathing cessation during sleep reported by the couple and an IMC

Método: Durante un período de 6 meses, se ingresó progresivamente una muestra randomizada de 180 pacientes mayores de 18 años (135 mujeres, 45 hombres) que consultaron en un Centro de Salud Familiar, para evaluar la frecuencia de trastornos de sueño con la Escala de Somnolencia de Epworth y un Cuestionario de Sueño especialmente diseñado que incluyó: estado nutricional, enfermedades médicas, uso de medicamentos, tabaco y alcohol, hábitos de sueño, ronquido, pausas respiratorias, insomnio, somnolencia diurna excesiva y síntomas depresivos. Resultados: Se encontró hipertensión arterial en 37,2%, diabetes en 11,1%, dislipidemia en 19,4% y tabaquismo en 34,4%, IMC > de 30 kg/m² en 38,3%, Epworth > 15 en 13,9%, ronquido en 56,7%, pausas respiratorias en 21,7%, somnolencia diurna excesiva en 29,4%, dificultad en la iniciación (21,7%) y mantención del sueño (21,7%). Tristeza persistente y/o pérdida de interés en sus actividades o falta de disfrute en el último mes fue referida por 43,8% de los pacientes. Conclusión: La alta frecuencia de trastornos del sueño encontrados, hacen necesario incluirlos en la planificación de programas de salud en nuestro país, para su adecuado diagnóstico y tratamiento, especialmente considerando el que algunos de estos trastornos constituyen factores de riesgo para enfermedades vasculares cardíacas y cerebrales. Introduction: Sleep disorders, especially sleep breathing disorders, have not been well studied in the patients attending a family health center in Chile. Sleep breathing disorders have been linked to cardiovascular and cerebrovascular diseases, which are important causes ofmorbidity and mortality. Method: We studied a randomized sample of 180 adult patients (135 women and 45 men, with a mean age of 45.6 years) consulting a public family health center with the Epworth Sleepiness Scale and a Sleep Questionnaire which include assessment ofobesity medical conditions, medications and smoking habit, blood pressure, sleep habits, snoring, breathing cessation, insomnia, daytime sleepiness and depressive symptomatology Results: Hypertension wasfound in 37.2%, diabetes in 11.1%, dislipidemia in 19.4%, smoking in 34.4%. Body mass index > of30 Kg/m² wasfound in 38.3%). Twentyfivepatients (13.9%>) had an Epworth score >ofl5. Loud snoring was reported in 56.7%>. Breathing cessation was reported by the spouses in 21.7%>. Difficulty for sleep initiation was present in 21.7% and to maintain sleep in 21.7%. Daytime sleepiness was present in 29.4%. Twelve patients (6.7%) had the highest score in a visual analog scale for daytime somnolence. An affirmative answer for persistent sadness or low mood and loss ofinterest or pleasure in the last month was reported by 43.8% ofthe patients. Conclusión: It is noteworthy the high frequency of undiagnosed sleep disorders found in a population consulting by others pathologies. These results would be useful for the planning of public health programs which should include sleep disorders.

Antibiotics are widely used in poultry production for the treatment of bacterial diseases. However, residues may remain in products and by-products destined for human consumption or animal feeding. The claws of chickens, which are a by-product of the poultry industry, can directly or indirectly enter the food chain as meals destined to feed other productive animals. Thus, it becomes necessary to determine and quantify antimicrobial residues present in this matrix. The objective of the study was to assess the depletion of oxytetracycline (OTC) and its metabolite 4-epi-OTC in broiler chicken's claws. Claws of 32 broilers treated with a therapeutic dosage of 10% OTC during 7 days were analysed. Samples were taken at days 3, 9, 15 and 19 post-treatment. As for the control group, eight broiler chickens were raised under the same conditions. Extraction was carried out through EDTA-McIlvaine buffer, and clean-up employed a SPE C-18 Sep-Pak®. Instrumental analysis was performed through LC-MS/MS. The concentrations of both analytes were determined in claw samples until day 19 post-treatment. Average concentrations were within the LOD (20 μg kg
– 1 ) and LOQ (22 µg kg– 1 ) for OTC and 84 μg kg– 1 for 4-epi-OTC. Withdrawal times (WDTs) of 39 days for OTC and 54 days for 4-epi-OTC were established in claws based on 95% confidence. These findings demonstrate that claws can be a source of antimicrobial residue entry into the food chain, since the results showed that OTC and its metabolite can be found in chicken's claws for long periods, even exceeding the average lifespan of a broiler chicken. - Abstract
In 1972, the United Nations (UN) Conference on the Human Environment expressed a growing realization that economic and social progress needed to be balanced with a concern for the environment and the stewardship of natural resources. The hard-to-grasp concept of "sustainable development" was first defined as "development that meets the needs of the present without compromising the ability of future generations to meet their needs" (World Commission on Environment and Development [WESDJ, 1987, p. 43). This definition contains two concepts: first, "human needs," with priority given to the world's poor, and, second, the environment's limits for meeting the state of technological and social organization (WESD, 1987, p. 43). At the 2002 World Summit on Sustainable Development (UN, 2002a), the focus on environmental protection broadened to encompass social justice and the fight against poverty as key principles of development that is sustainable. Three interdependent and mutually reinforcing "pillars" were recognized: economic development, social development, and environmental protection. These pillars must be established at local, national, and global levels. The complexity and interrelationship of critical issues such as poverty, wasteful consumption, urban decay, population growth, gender inequality, health, conflict, and the violation of human rights are addressed in all three pillars (Pigozzi, 2003, p. 3). Following the concept of sustainable development, we argue that the challenge for developing countries in contemporary society is to meet the very real need for economic development and opportunities for income generation, while avoiding the unintended and unwanted consequences of economic development and globalization. These consequences include social exclusion, loss of cultural heritage, and environmental and ecological problems.

Though popular, concepts such as Toffler's 'prosumer' (1970; 1980; 1990) are inherently limited in their ability to accurately describe the makeup and dynamics of current co-creative environments, from fundamentally non-profit initiatives like the Wikipedia to user-industry partnerships that engage in crowdsourcing and the development of collective intelligence. Instead, the success or failure of such projects can be understood best if the traditional producer/consumer divide is dissolved, allowing for the emergence of the produser (Bruns, 2008). A close investigation of leading spaces for produsage makes it possible to extract the key principles which underpin and guide such content co-creation, and to identify how innovative pro-am partnerships between commercial entities and user communities might be structured in order to maximise the benefits that both sides will be able to draw from such collaboration. This chapter will outline these principles, and point to successes and failures in applying them to pro- am initiatives.

Cystic echinococcosis is endemic in the Rio Negro province of Argentina. After 30 years of control using praziquantel in dogs the transmission rate to humans and sheep has decreased significantly, however transmission persists. The objective of the study is to assess the impact of the inclusion of the EG95 vaccine for sheep in the control programme, including analysis of the vaccine's operative feasibility in field conditions. The vaccine was applied in an area comprising four communities of native people including 79 farms with 3146 lambs and 311 dogs in total. Seventy one farms were designated as control areas where no vaccinations were undertaken while vaccinations of lambs undertaken on 91 farms. Lambs received two vaccinations with the EG95 vaccine followed by a single booster injection when the animals were 1-1.5 years of age. Farm locations were defined using GPS coordinates for the houses. Evidence for Echinococcus granulosus transmission was monitored by coproantigen ELISA on samples of dog faeces, by E. granulosus-specific PCR using soil samples, and anti-E. granulosus antibody assessments in sera from 2 to 4 teeth lambs, purgation of dogs to detect E. granulosus worms and necropsy on adult sheep. Before the vaccine was introduced, 26.2% of sheep with 2-4 teeth were positive using ELISA/WB, the prevalence decreased to 7.8% at the third year following use of the vaccine. Necropsy of animals older than 6 years (not vaccinated) showed that 66.1% of animals were infected with E. granulosus. In dogs, 4% was found positive for E. granulosus using arecoline purgation and 24.7% of the farms were infected using coproELISA/WB. During the first year of vaccination 2721 lambs received the first vaccine dose and 2448 received a booster. In the second year 2138 lambs were initially vaccinated and 1745 received a booster, and 1308 animals received the third dose. The authors provide a theoretically generative definition of cyberinfrastructure (CI) by drawing from existing definitions and literature in social sciences, law, and policy studies. They propose two models of domestic and international influencers on CI emergence, development, and implementation in the early 21st century. Based on its historical emergence and computational power, they argue that cyberinfrastructure is built on, and yet distinct from the current notion of the internet. The authors seek to answer two research questions: firstly, what is cyberinfrastructure? And secondly, what national and international influencers shape its emergence, development and implementation (in e-science) in the early 21st century? Additionally, consideration will be given to the implications of the proposed definition and models, and future directions on CI research in Internet studies will be suggested.

El rash es un efecto secundario común asociado al uso de fármacos antiepilépticos. La frecuencia de rash con fenitoína se ha estimado en un 5,9% y asciende a un 25% en pacientes que han presentado rash con otro fármaco antiepiléptico. La anemia aplásica es una anomalía adquirida de las células madre hematopoyéticas caracterizada por pancitopenia de la sangre periférica y médula ósea hipocelular. Los pacientes tratados con fenitoína presentan un riesgo 3,5 veces mayor de desarrollar anemia aplásica. Presentamos el caso de una mujer de 70 años que desarrolló dos reacciones adversas severas y simultáneas a la fenitoína: un exantema maculopapular pruriginoso con compromiso de mucosas y una anemia aplásica. Ambas condiciones se resolvieron completamente con la suspensión del fármaco.

The question posed in this chapter is: To what extent does current education theory and practice prepare graduates for the creative economy? We first define what we mean by the term creative economy, explain why we think it is a significant point of focus, derive its key features, describe the human capital requirements of these features, and then discuss whether current education theory and practice are producing these human capital requirements. The term creative economy can be critiqued as a shibboleth, but as a high level metaphor, it nevertheless has value in directing us away from certain sorts of economic activity and toward other kinds. Much economic activity is in no way creative. If I have a monopoly on some valued resource, I do not need to be creative. Other forms of economic activity are intensely creative. If I have no valued resources, I must create something that is valued. At its simplest and yet most profound, the idea of a creative economy suggests a capacity to compete based on engaging in a gainful activity that is different from everyone else’s, rather than pursuing the same endeavor more competitively than everyone else. The ability to differentiate on novelty is key to the concept of creative economy and key to our analysis of education for this economy. Therefore, we follow Potts and Cunningham (2008, p. 18) and Potts, Cunningham, Hartley, and Ormerod (2008) in their discussion of the economic significance of the creative industries and see the creative economy not as a sector but as a set of economic processes that act on the economy as a whole to invigorate innovation based growth. We see the creative economy as suffused with all industry rather than as a sector in its own right. These economic processes are essentially concerned with the production of new ideas that ultimately become new products, service, industry sectors, or, in some cases, process or product innovations in older sectors. Introducción: El SAHOS presenta una alta prevalencia en estudios internacionales, con importantes repercusiones en los sistemas cardio, cerebrovascular y en la calidad de vida de las personas. Método: Estudiamos prospectivamente a los pacientes enviados para Polisomnografia (PSG) a nuestro laboratorio de sueño, en un período de 6 meses. Previo consentimiento informado, se les aplicó la Escala de Somnolencia de Epworth (EE) y un Cuestionario de Sueño. Fueron entrevistados 100 pacientes, excluyéndose 3 por no haberse realizado PSG y 2 que no completaron la EE. Resultados: Se analizaron 95 pacientes, 89,5% hombres, edad promedio 47,4± 12,5 años, obesidad en 49,5%, EE mayor de 11 en 56,8%, ronquido en 93,7%, pausas respiratorias en 68,4%, somnolencia diurna excesiva en 57,9%, cansancio al despertar en 86,3%o. La PSG no demostró SAHOS en 14,7%, hubo 27,4% con SAHOS leve, 21 %> moderado y 36,8%> severo. Las variables presencia de pausas respiratorias observadas por la pareja e índice de masa corporal (IMC) predicen la ausencia de SAHOS severo, con una sensibilidad de 87% y especificidad de 50%. Conclusión: En la evaluación de pacientes con sospecha de SAHOS, la no observación de pausas respiratorias durante el sueño por parte de la pareja y el IMC menor de 30 Kg/m² hacen menos probable que exista un SAHOS severo.

The aim of the study is to analyze the composition and structure of personal networks of young Serbs in relation to the massive demonstration that took place in Belgrade on October 5th, 2000 and whose main outcome was the fall of Milosevic’s regime. The theoretical assumption is that at the micro level it is possible to find prints and patterns of the macro phenomena. The revolution is analyzed from the concept of ‘cooperative cascade’, where individual actions are combined to produce an emergent phenomenon. The cooperative cascade is analyzed from the personal networks perspective, where three main factors are combined: decrease of individual thresholds, characteristics of alters (composition) and centrality measures (structure).

Cystic echinococcosis is endemic in the Rio Negro province of Argentina. After 30 years of control using praziquantel in dogs, the transmission rate to humans and sheep has decreased significantly, however transmission persists. The objective of the study is to assess the impact of the inclusion of the EG95 vaccine in sheep in the control program. The vaccine was applied to lambs from communities of native peoples which included 70 producers (Anecón Grande, Mamuel Choique, Nahuel Pan y Río Chico Abajo). From 2009 on, lambs received two doses of the EG95 vaccine when they were 30 and 60 days of age, followed by a single booster injection when the animals were 1 year of age. The transmission of Echinococcus granulosus was assessed through necropsy in mature sheep, Arecoline test and coproantigen test in dogs and register of new clinical cases in inhabitants of the area. A total of 29.323 doses of EG95 vaccine were applied to lambs in the 2009-2017 period, working with 4 groups of 2 veterinaries each, covering 83.5% with one doses, 80.1% with two doses and 85.7% with three doses. Before the vaccine was introduced, in necropsy of sheep of 6 years old showed that 45.7% were infected with E. granulosus. The rate decreased to 21.1% in 2006. The number of cysts per animal decreased from 1.4% to 0.3. The number of producers with infected animals decreased from 84.2% to 22.2%. The humoral response to the vaccination, in field conditions, showed an increased in antibodies with the second dose, reaching its maximum after the booster at one year of age for many years and keeping constant. In dogs, 4.7% was found positive using arecoline purgation and 20.3% of the farms were infected using coproantigen test. The impact assessment showed a 5.6% of dogs positive to the arecoline test. Only a new case of echinococcosis was detected in kids with a 10 cm cyst, and its origin was probably before the vaccination. EG95 vaccine has been effective to prevent the infection in ovines up to six years of age, in spite of the difficulties of achieving better coverage. El inmunodiagnóstico en el ovino presenta problemas de sensibilidad y especificidad que limitan su aplicación en sistemas de vigilancia de la enfermedad. El Objetivo del trabajo fue desarrollar una técnica sensible, específica y económica para el diagnóstico de la echinococcosis quística en ovinos naturalmente infectados y evaluar la validez de la necropsia como prueba de referencia. 247 ovinos fueron estudiados en sala de faena confirmándose los diagnósticos parasitológicos mediante histología. Los sueros obtenidos fueron procesados mediante enzimo inmuno ensayo con tres preparaciones antigénicas: LHT (líquido hidatídico total), S2B (fracción purificada de LHT) y B y confirmados mediante western blot. EIE con las tres preparaciones antigénicas fue eficaz para discriminar E. granulosus de Cisticercos tenuícolis y Taenia spp. Sueros de ovinos negativos macroscópicamente resultaron reactivos a EIE y positivos a WB. La sensibilidad fue de 89.2% para LHT, 80.0% para S2B y 86.4% para B. La especificidad fue del 89.5% para LHT, 93.9% para S2B y 92.8% para B en el total de la majada. La sensibilidad en corderos resultó de 78.6 con LHT, 75.0 con S2B y 64.3 con B. El diagnóstico macroscópico en sala de faena demostró tener limitaciones como prueba de referencia para el inmunodiagnóstico de la equinococcosis quística en el ovino, resultando necesario incluir histología y WB como pruebas de referencia. EIE resultó una técnica con sensibilidad y especificidad adecuadas para su uso en sistemas de vigilancia y evaluación de programas de control, resultando LHT la preparación antigénica de mayor valor. An exhaustive bibliographic search was carried out on epidemiology, surveillance and control strategies for Rabies variant 4 transmitted by insectivorous bats. Likewise, as a case study, a retrospective study of insectivorous bats captured by passive surveillance and analyzed to determine the presence of rabies virus was carried out in the Province of Río Negro, Argentina. The objective was to update the information on the epidemiology of rabies variant 4 and to analyze the effectiveness of the control measures usually recommended by health services. The review carried out shows only 5 notifications in South America corresponding to canines and felines between 2009 and 2019 and with regard to people, three cases were reported: in Chile in 1963 and 2013 and in Colombia in 2012. The identification of insectivorous bats carrying rabies ranges from 4% to 15%. For its part, the veterinary public health service of the Health Ministry of Rio Negro collected insectivorous bats from passive surveillance activities. The bats were sent to the Laboratory of the Luis Pasteur Institute in the city of Buenos Aires for identification of rabies virus. A total of 294 insectivorous bats were analyzed in 26 locations. The proportion of the infected ones was 14.6% (95% CI 9.5-17.8) although there were no cases in dogs, cats or people. Insectivorous bats can be a risk factor for canines, felines, and humans, although notifications of variant 4 cases in these species are extremely low in South America, so an evaluation of the recommended and applied control strategy seems to be required, so far, as it is not cost-effective, and involves increasing operating costs in financial and human resources, proposing more efficient risk control alternatives. Recently a number of techniques have been developed that stimulate people to act towards specific objectives. These include persuasive technologies, gamification, user experiences, and various methods and tools used in open-source and other communities to encourage and organize participation. After surveying various examples of such applications, we generalize these approaches by proposing a new theoretical framework. The basic concept is a mobilization system, defined as an ICT environment that motivates and coordinates the actions of people, so as to increase their focus and commitment, while minimizing distraction, hesitation and procrastination. We then analyze the fundamental mechanisms of (individual) motivation and (collective) coordination, starting from Csikszentmihalyi's concept of flow and the mechanism of stigmergy. The basic requirements are clear goals in line with real needs, immediate and rich feedback, challenges in balance with the user's skill levels, and a shared workspace in which all tasks and results are registered for every contributor to see. We conclude our review of mobilization systems by pointing out their potential dangers, such as addiction and political or commercial exploitation, as well as their potential benefits, in domains such as work, health, education and research.

Cook and Garduno Freeman argue that social networks such as Flickr function as public repositories of personal photography that challenge and democratize the authority of traditional institutions of memory, like libraries and museums. Vernacular photography on these systems increases the visibility of individual's self-representation. In contrast to institutional collections, members on Flickr self-select the subject matter in their photographic contributions, the methods of representation, and the modes of participation.

The opportunistic pathogen Enterococcus faecium colonizes humans and a wide range of animals, endures numerous stresses, resists antibiotic treatment and stubbornly persists in clinical environments. The widespread application of antibiotics in hospitals and agriculture has contributed to the emergence of vancomycin-resistant E. faecium, which causes many hospital-acquired infections. In this Review, we explore recent discoveries about the evolutionary history, the environmental adaptation and the colonization and dissemination mechanisms of E. faecium and vancomycin-resistant E. faecium. These studies provide critical insights necessary for developing novel preventive and therapeutic approaches against vancomycin-resistant E. faecium and also reveal the intricate interrelationships between the environment, the microorganism and the host, providing knowledge that is broadly relevant to how antibiotic-resistant pathogens emerge and endure., (© 2024.
