Coronary artery aneurysm or ectasia (CAAE) is a term that includes both coronary artery ectasia (CAE) and coronary artery aneurysm (CAA), despite distinct phenotypes and definitions. This anomaly can be found in 0.15–5.3% of coronary angiography. CAE is a diffuse dilatation of the coronary artery at least 1.5 times wider than the diameter of the normal coronary artery in a patient with a length of over 20 mm or greater than one-third of the vessel. CAE can be further subdivided into diffuse and focal dilations by the number and the length of the dilated vessels. Histologically, it presents with extensive destruction of musculoelastic elements, marked degradation of collagen and elastic fibers, and disruption of the elastic lamina. Conversely, CAA is a focal lesion manifesting as focal dilatation, which can be fusiform (if the longitudinal diameter is greater than the transverse) or saccular (if the longitudinal diameter is smaller than the transverse). Giant CAA is defined as a 4-fold enlargement of the vessel diameter and is observed in only 0.02% of patients after coronary. An aneurysmal lesion can be either single or multiple. It can be either a congenital or acquired phenomenon. The pathophysiological mechanisms responsible for the formation of CAAE are not well understood. Atherosclerosis is the most common etiology of CAAE in adults, while Kawasaki disease is the most common in children. Other etiological factors include systemic connective tissue diseases, infectious diseases, vasculitis, congenital anomalies, genetic factors, and idiopathic CAA. Invasive assessment of CAAE is based on coronary angiography. Coronary computed tomography (CT) is a noninvasive method that enables accurate evaluation of aneurysm size and location. The most common complications are coronary spasm, local thrombosis, distal embolization, coronary artery rupture, and compression of adjacent structures by giant coronary aneurysms. The approach to each patient with CAAE should depend on the severity of symptoms, anatomical structure, size, and location of the aneurysm. Treatment methods should be carefully considered to avoid possible complications of CAAE. Simultaneously, we should not unnecessarily expose the patient to the risk of intervention or surgical treatment. Patients can be offered conservative or invasive treatment. However, there are still numerous controversies and ambiguities regarding the etiology, prognosis, and treatment of patients with coronary artery aneurysms. This study summarizes the current knowledge about this disease’s etiology, pathogenesis, and management.

Conclusions: In adults with a long history of T1D, olfactory function and structures are impaired. The presence of diabetic neuropathy and ongoing smoking addiction might be considered predictors of the degradation of rhinencephalon structures in people with T1D.

Myocardial infarction with non-obstructive coronary arteries (MINOCA) is a working diagnosis and requires identification of the underlying causes to optimize treatment, improve prognosis, and prevent the recurrence of myocardial infarction. According to the literature, the prognosis of patients diagnosed with MINOCA is comparable to the group of patients with myocardial infarction (MI) and significant stenosis of the coronary arteries. Intracoronary imaging is a crucial diagnostic tool used in identifying epicardial causes of MINOCA that are not visible in coronary angiography. Optical coherence tomography (OCT) provides the highest spatial resolution, simultaneously allowing detailed visualization of plaque pathology in individuals with MINOCA and identifying the cause of MI in up to 80% of patients. Common causes of a MINOCA may include plaque disruption, spontaneous coronary artery dissection (SCAD), coronary artery spasm, and coronary thromboembolism. The optimization of pharmacological treatment in this group of patients, especially dual antiplatelet therapy and statins, improves the prognosis. Data on the indications for invasive treatment of patients with MINOCA based on OCT findings are insufficient. There is a strong need for research comparing treatment strategies, especially in high-risk lesions visualized in OCT. The main aim of this review is to demonstrate the usefulness of OCT in determining the mechanism of MINOCA.

Diabetic peripheral neuropathy (DPN) is a heterogenic disorder prevalent amongst patients suffering from diabetes mellitus (DM), with symptoms comprising neuropathic pain, paresthesia, and numbness in distal lower limbs. Alpha-lipoic acid (ALA) is proposed as a pathogenesis-oriented treatment option, targeting underlying causes of neural lesions such as hyperglycemia, metabolic and microvascular dysfunctions, and cellular oxidative stress. We performed a comprehensive review of controlled clinical trials demonstrating the clinical usefulness of ALA in the treatment of DPN, published in the last 5 years to determine the benefits of ALA monotherapy and combined treatments with other known antioxidants. We also investigated the differential efficacy of oral versus intravenous ALA administration. Clinical trials show the efficacy of ALA treatment, attributed to its anti-inflammatory, anti-hyperglycemic, and antioxidant properties, as well as its function in the endothelial activation and lipid metabolism parameters. ALA supplementation is associated with amelioration in nerve conduction velocity scores, clinically significant reduction of reported neuropathic pain, burning and paresthesia, as well as a decrease in serum triglycerides, improved insulin sensitivity, and quality of life.

As we have previously reported, the ORPKI electronic data capture is monitored continuously and modified according to the needs of the rapidly changing everyday practice of interventional cardiology not only in Poland but also worldwide as required by ESC guidelines [1, 2]. The database is endorsed by the Association of Cardiovascular Interventions of the Polish Cardiac Society (AISN PTK) and is daily operated by the Jagiellonian University Medical College, with currently 154 interventional cardiology centers in Poland reporting. ORPKI database analysis of various trends and patterns is published each year [3–5]. On 31st of December 2020 there were 575 PCI operators certified by the AISN PTK in Poland.

The solar power industry is a rapidly growing sector of renewable energy, and it is crucial that the available energy is accurately forecast. Using numerical weather prediction models, we can forecast the global horizontal irradiance on which the amount of energy produced by photovoltaic systems depends. This study presents the forecast effects for one of the most challenging weather conditions in modelling, occurring in central and eastern Europe. The dates of the synoptic situations were selected from 2021 and 2022. Simulations were carried out for 18 days with a cold front and, in order to verify the model configuration, for 2 days with a warm front, 2 days with an occlusion front and 2 days with a high pressure situation. Overall, 24 forecasts were made for each of the three parameterizations of the Weather Research and Forecasting model. The data were compared with the values measured in situ at the station performing the actinometric measurements belonging to Germany’s National Meteorological Service. This paper presents the spatial distribution of the global horizontal irradiance parameters for several terms to explain the differences between the results of the different simulations.

Aims: We aimed to assess neurodegenerative changes in the rhinencephalon via magnetic resonance imaging (MRI) and relate it to olfactory function and diabetic peripheral neuropathy (DPN) in adults with type 1 diabetes (T1D). Materials and Methods: Individuals aged 18–65 with T1D duration over 10 years and control healthy subjects underwent olfactory assessment using Sniffin'Sticks and brain MRI to assess volumetric measurements of the olfactory bulbs and piriform cortex thickness. Results: 32 T1D (24 males) aged 43.5 years (IQR: 37.0–48), diabetes duration 24.5 years (IQR: 20.5–27.0), and A1C 7.95% (IQR: 7.4–8.4) were assessed. The control group consisted of 6 healthy adults (4 males) aged 41.0 years (IQR: 36.0–48.0). Significantly lower olfactory test results in TDI (threshold‐differentiation‐identification) (31.5 (IQR: 28.7–33.6) vs. 34.1 (IQR: 33.2–37.2), p = 0.02) were obtained in the T1D as compared to the controls. Summarized olfactory bulb (OB) volumes and thickness of the left pyriform cortex were significantly smaller in T1D than in controls (65.8 mm3 (IQR: 57.9–71.7) vs. 75.8 mm3 (IQR: 74.8–76.7); p = 0.0005 and 3.1 mm (IQR: 2.7–3.4) vs. 3.6 mm (IQR: 3.5–4.1); p =0.02). Patients with DPN had significantly smaller OB volumes than patients without DPN (58.1 mm3 (IQR: 54.0–70.9) vs. 69.8 mm3 (IQR: 65.0–72.2); p = 0.02). Tobacco smoking (β: −7.89; p = 0.013) and DPN (β:−7.02; p = 0.015) proved to be independent predictors of OB volume. Conclusions: In adults with a long history of T1D, olfactory function and structures are impaired. Introduction Although breast implant-associated anaplastic large cell lymphoma (BIA-ALCL) is infrequent, with less than 1000 noted cases worldwide, patients consenting for breast implant surgery should be aware of its risk. We describe the first Polish multicenter case-series data on BIA-ALCL patients and present diagnostic and treatment recommendation for breast surgeons. Material and methods In cooperation with the Polish Society of Surgical Oncology and Polish Lymphoma Research Group, we collected BIA-ALCL cases in Poland. Results We retrospectively reviewed clinical data of seven BIA-ALCL patients, diagnosed between July 2013 and November 2019. The median time from implant placement to the first BIA-ALCL symptoms was 65 months (range: 33–96 months). All the patients were exposed to textured implants at presentation. Capsulectomy with implant removal was performed in all the patients with immediate reimplantation in 2 cases. In a median follow-up of 19 months (range 5-81 months), there was no recurrence and all the patients stayed alive. Between 2013 and 2019, the incidence of BIA-ALCL in Polish female population age 30 and above ranged from 0 to 0.021/100 000/year. Conclusions BIA-ALCL is scarce in the Polish population. In a short-term follow-up, patients’ prognosis remains excellent. Due to the withdrawal of roughly textured implants from the market and the exclusion of likely the most potent etiologic factor, it might be expected that the incidence of BIA-ALCL will become even rarer.
