Objective: Biochemical recurrence (BCR) after prostate cancer (PCa) treatment is undesirable. It is important to inform a patient about BCR in preoperative evaluation. We aimed to demonstrate the effectiveness of the (The Prostate Cancer Risk Assessment) CAPRA score used to predict this situation in our study. Material and Methods: The study included 348 patients who underwent Radical Prostatectony (RP) for localized PCa. Demographic, preoperative and postoperative data were collected. CAPRA score based on preoperative total PSA value, Gleason Score, clinical T stage, percentage of positive biopsy cores and age was calculated using these data. BCR was defined as a total PSA value >0.2 ng/dL for two consecutive times after RP. Follow-up periods, recurrence status and time of recurrence were recorded. Results: BCR positivity was detected in 60 (17.2%) of 348 patients. In univariate analyses, PSA level, lesion volume on MRI, ISUP grade, D'Amico risk classification, Seminal vesicule invasion (SVI) and CAPRA score were statistically significant in the groups. In multivariate analyses, PSA level, Neutrophile Lymphocyte Ratio, lesion dimension, intermediate risk according to D'amico classification, Extraprostatic extension (EPE) showed differences between both groups. The probability of biochemical progression-free in CAPRA risk groups shows a significant decrease in the probability of biochemical progression-free in the long term as risk increases in CAPRA risk groups: 91.4% in the low-risk group, 77.8% in the intermediate-risk group and only 61.7% in the high-risk group at 80-month follow-up. Conclusion: CAPRA scoring system should be supported by MpMRI findings and a new nomogram should be developed with these findings. [ABSTRACT FROM AUTHOR]

Introduction: Extracorporeal shock wave lithotripsy (ESWL) is one of the methods applied in urinary system stone disease. ESWL has a very high success rate, and there are many factors that affect its success. In our study, we aimed to present our shock wave lithotripsy (SWL) results in proximal ureter stones, the reasons for re-admission to the hospital after SWL, and the final success achieved after additional interventions. Methods: Between March 2017 and October 2019, 142 patients aged 18 years and over who underwent SWL for proximal ureteral stones were retrospectively evaluated. Age, sex, body mass index, stone laterality, stone size and volume (p×1/6×length×width×height), stone-skin distance, stone density, hydronephrosis degrees, number of SWL sessions, reasons for re-admission, and final success results were evaluated. The stone size determined on CT<4 mm was defined as successful SWL treatment. Other interventional procedures were performed to unsuccessful SWL patients, and final success rate was determined. Complications were classified according to Clavien-Dindo classification. Results: Age, BMI, laterality of the stone, degree of renal ectasia, number of SWL sessions, and stone-skin distance did not contribute to SWL success (p>0.05). Factors affecting ESWL success include male gender, stone size, stone volume, and stone density; stone-skin distance and degree of renal ectasia were found to affect final success (p<0.05). Complications were more frequent in the unsuccessful group (p<0.05). Discussion and Conclusion: Stone volume, size and density of stone in proximal ureter stones are among the factors affecting the SWL success. The final success rate can be increased after additional interventions performed in the necessary situations of SWL, which is a minimally invasive procedure. [ABSTRACT FROM AUTHOR]

Objective: Vesicourethral anastomotic stenosis (VUAS) is frequently seen after prostate surgery because of various operative and postoperative factors. In this study, we aimed to present our results of perineoscopic bladder neck reconstruction, which is a new technique of the perineal approach in the treatment of patients with VUAS after prostate cancer surgery. Material and methods: Sixteen consecutive patients who underwent perineoscopic bladder neck reconstruction in our clinic between July 2017 and March 2019 were included in the study. Demographic characteristics, surgical history, postoperative continence status, and additional treatment requirements were recorded. Perineoscopic surgery is defined as the visualization of the surgical site with instruments used in laparoscopy and the surgeon performing the entire operative procedure through the screen. Results: The mean number of preoperative endoscopic bladder neck resections of the patients was 7±5.1, with a history of suprapubic cystostomy in 7 (43.7%) and radiotherapy in 5 (31.2%) patients before surgery. The mean surgical time was 126.2±13.1 min. The mean follow-up period was 13.2±6.8 months, and the success rate was 81.25%. During follow-up, two (12.5%) patients received perineoscopic re-do reconstruction because of stricture recurrence, and one (6.2%) patient was included in a urethral dilatation program. Conclusion: Improving visualization and ergonomics with the perineoscopic approach can increase the success rate of bladder neck reconstruction in comparison with the standard approach. In addition, the lack of need for expanded dissection (corporal separation, inferior pubectomy) reduces postoperative complication rates. [ABSTRACT FROM AUTHOR]

Objective: Evaluation of the results of dorsolateral buccal mucosal augmentation urethroplasty in patients with a long- segment urethral strictures. Material and methods: Twenty m ale patients who u nderwent u rethroplasty i n our clinic b etween November 2015 and January 2017 were evaluated. The outcomes of single-stage dorsolateral buccal mucosal augmentation urethroplasty were retrospectively evaluated. Patients were followed-up at 2nd-3rd weeks, 3rd and 6th months after the operation. Results: Mean age of the patients was 59.45±13.6 years. Mean length of the strictures was 4.59±1.99 cm (3-11 cm) and mean length of buccal mucosal graft was 6.8±1.98 cm (5-13 cm). Mean duration of operation was 149.25±47.39 minutes (95-270 min) and mean blood loss was calculated as 165.5±63.05 mL (75-280 mL). The success rate of dorsolateral buccal mucosal augmentation urethroplasty was calculated as 85% after a mean follow-up of 7.38±2.6 months. There were no perioperative or postoperative complications in the urethroplasty region or the mouth except one patient. Three patients who were found to have a decline in the maximum voiding rate in the postoperative 3rd month were included in the dilation program. Conclusion: Single-stage dorsolateral buccal mucosal augmentation urethroplasty is a surgical option to be used in the treatment of long segment urethral strictures with high success and low complication rates in experienced hands. Introduction: Priapism is a penis erection that lasts longer than 4 hours without sexual stimulation and is a urological emergency that requires immediate treatment. Priapism can be seen rarely after hemodialysis. We aimed to present the patient with resistant priapism who underwent hemodialysis. Case Report: A 47-year-old man who underwent hemodialysis for two months due to end-stage renal disease admitted to the emergency department due to priapism. The penis was painful and rigid. Despite the drainaged of approximately 150 cc venous blood from corpus cavernosum with in 30 minutes, no detumescence was achieved. We performed a T-shunt combined with corporal tunneling. In the follow-up visits, the patient who was not able to have detumescence was informed and planned to have an early implantation of penile prosthesis due to necrosis, fibrosis, permanent loss of erection. Conclusion: Mechanism of priapism in hemodialysis patient is accused of inadequate heparinization and dialysis-induced hypoxemia and acidosis. First line treatment for ischemic priapism in emergency department is very important. The patient should be intervened quickly and directed to the urologist. [ABSTRACT FROM AUTHOR]

Background: By examining patients with testicular torsion (TT) that caused problems in medicolegal terms, the present study aims to define markers causing medical neglect or malpractice in similar conditions and perform a retrospective examination to characterize the medical aspects of patients with TT., Methods: In this study, 53 patients who underwent orchiectomy for TT following interventions made between 2004 and 2019 in different hospitals of Turkey and had satisfactory clinical findings in their files based on medicolegal inspections were included., Results: The median age of the patients was nine years. Twenty-three (43.4%) of the patients had TT on the left side, 29 (54.7%) had TT on the right side, and one (1,9%) patient had bilateral TT. It was noticed that 31 (58.5%) patients had epididymo-orchitis (EO), seven patients had (13.2%) urinary infection, five (9.4%) patients had a hydrocele, and four (7.5%) patients had renal colic, and the others had testicle contusion, gastroenteritis, inguinal hernia, and acute appendicitis as misdiagnoses. The mean time that passed between admission and TT diagnosis was detected as 59±11.2 hours. A statistically significant relation was detected between the branch of the physician who first evaluated the patients and the presence of performing scrotal examination and imaging during admission. The ratio of physical scrotal examinations by emergency service physicians was lower than with the urologists. Among the preliminary examiner allowed to be an advanced evaluation for the possibility of missing diagnosis by an independent specialist physicians, 25 (47.2%) were urologists, 22 (41.5%) were emergency service physicians, four (7.5%) were pediatricians, and two (3.8%) were radiologists., Conclusion: Physicians should perform the required evaluations for a suitable diagnosis and treatment by putting aside their medicolegal concerns and prevent the problems by giving priority to patient health. For the correct diagnosis and proper management of TT, it is necessary to increase the information levels of physicians, and patients should be explored urgently in the event of any clinical suspicion.
