Abstract

Background: Patients with chronic diseases such as chronic kidney disease (CKD) have been reported to have more adverse outcomes during the coronavirus disease 2019 (COVID-19) pandemic. There are insuficient data on the outcomes of patients with primary glomerular diseases (PGD) afer COVID-19. Methods: We designed a national multicenter observational study that included adult patients with biopsy-proven PGD who survived COVID-19. A control group was created from the same centers, including PGD patients without COVID-19. The clinical and laboratory data of the patients at baseline, frst, and third months afer COVID-19 were recorded. Results: A total of 129 patients from 21 centers were included (COVID-19 group, n = 77). Baseline characteristics were almost similar except the ratio of active disease in the non-COVID-19 group was signifcantly higher than in the COVID-19 group. No patients died during the frst and third months. Respiratory symptoms were signifcantly higher in the COVID-19 group than in the non-COVID-19 group in the frst month (7.8% vs. 0%, P = .039). All other follow-up outcomes, including initiation of chronic kidney replacement therapy and initiation of new immunosuppressive treatment, and the laboratory data were not diferent between the groups in the frst and third months. Conclusion: Primary glomerular disease patients in the post-COVID-19 period had more respiratory symptoms than non-COVID-19 PGD patients, but outcomes, including death and initiation of kidney replacement therapy, were not diferent in the frst and third months post COVID-19. Abstract

There is growing evidence that chronic kidney disease (CKD) is an independent risk factor for cognitive impairment, especially due to vascular damage, blood–brain barrier disruption and uremic toxins. Given the presence of multiple comorbidities, the medication regimen of CKD patients often becomes very complex. Several medications such as psychotropic agents, drugs with anticholinergic properties, GABAergic drugs, opioids, corticosteroids, antibiotics and others have been linked to negative effects on cognition. These drugs are frequently included in the treatment regimen of CKD patients. The first review of this series described how CKD could represent a risk factor for adverse drug reactions affecting the central nervous system. Abstract

Objective: Variations in care at national or global level may have an impact on the prognosis of patients on chronic hemodialysis. We aimed to describe regional differences in all-cause mortality or cardiovascular morbidity in chronic hemodialysis patients in Turkey. Methods: We enrolled 2461 patients who were initiated chronic hemodialysis in 93 centers in Turkey between January 27, 2017, and February 09, 2018. We included 2-year follow-up data of 1877 patients in this prospective study. The primary outcome, the rate of composite endpoint of all-cause mortality or cardiovascular morbidity, was compared between geographical regions. Secondary outcomes were the rates of hospitalization and infections. Results: In total, 552 patients (29.4%) developed the primary outcome. The highest and lowest rates of primary outcome occurred in the Mediterranean (34.5%) and Southeastern (26.5%) & Central Anatolian regions (26.5%), respectively, with no significant differences across regions (P = .82). Hospitalization events were detected in 377 patients (20.1%). The highest rate of hospitalization was detected in the Black Sea region (33.8%), and the lowest (7.6%) in the Southeastern region. The regions did not differ in hospitalization rates (P = .88). Infections occurred in 11.3% (n = 212) of the patients. The highest and lowest rates of infections occurred in the Aegean (18.2%) and the Southeastern (2.9%) regions, respectively. We detected significant difference between geographic regions (P = .02). Conclusions: Our study showed that almost 3 in every 10 chronic hemodialysis patients reached the primary endpoint of all-cause mortality/cardiovascular morbidity during the 2 years of follow-up. The occurrence of this outcome does not seem to exhibit geographical variation across the country.

23. COVID-19 Infection in Peritoneal Dialysis Patients: A Comparative Outcome Study with Patients on Hemodialysis and Patients without Kidney Disease

Abstract

Objectives: There is limited data about coronavirus disease-19 (COVID-19) characteristics and results in peritoneal dialysis (PD) patients. This study aimed to investigate the characteristics and outcomes among PD patients and compare them with matched hemodialysis (HD) patients and a control group without kidney disease. Methods: We included 18 PD patients and consecutive age- and gender-matched 18 HD and 18 patients without kidney disease (control group) registered into the Turkish Society of Nephrology database including 1301 COVID-19 patients. We compared demographic, clinical, radiological, laboratory data, and outcomes namely intensive care unit (ICU) admission, mechanical ventilation, mortality, and composite outcome (death and/or ICU admission). Results: ICU admission, mechanical ventilation, and mortality rates in PD patients (27.8%, 22.2%, and 22.2%, respectively) and the HD group (16.7%, 11.1%, and 16.7%, respectively) were higher than the control group (11.1%, 11.1%, and 5.6%, respectively), but intergroup comparison did not reveal difference. A total of 11 (20.3%) patients had composite outcome (6 PD patients, 3 HD patients, and 2 patients in the control group). In Cox regression analysis, higher age and higher CRP level were related to increased risk of composite outcome. Adjusted rate of composite outcome in PD group was significantly higher than the control group (P =.050). This rate was similar in HD and control groups (P =.30). Conclusions: Combined in-hospital mortality and/or ICU admission of PD patients with COVID-19 was significantly higher than the control patients. There is a need for careful surveillance of PD patients for infection signs and prompt treatment of COVID-19.

Abstract

Research conducted on positive and negative reciprocities mainly aims to determine the variables that impact this phenomenon. To this end, this study sought to ascertain the influence of participation in trekking and kickboxing as serious leisure activities along with several other variables on the positive and negative reciprocity attitudes. The study utilized a descriptive survey design and a random sampling method. The findings revealed a significant difference between the choice of serious leisure activities and demographic variables such as gender, age, and education level. Additionally, the mean scores of positive reciprocity attitudes increased proportionately to age and education level. Furthermore, the mean scores of positive reciprocity attitudes of the trekking participants were significantly higher than those of kickboxing participants. [ABSTRACT FROM AUTHOR]

Abstract

Background: we aimed to present the demographic characteristics, clinical presentation, and outcomes of our multicenter cohort of adult KTx recipients with COVID-19.Methods: we conducted a multicenter, retrospective study using data of patients hospitalized for COVID-19 collected from 34 centers in Turkey. Demographic characteristics, clinical findings, laboratory parameters (hemogram, CRP, AST, ALT, LDH, and ferritin) at admission and follow-up, and treatment strategies were reviewed. Predictors of poor clinical outcomes were analyzed. The primary outcomes were in-hospital mortality and the need for ICU admission. The secondary outcome was composite in-hospital mortality and/or ICU admission.Results: one hundred nine patients (male/female: 63/46, mean age: 48.412.4years) were included in the study. Acute kidney injury (AKI) developed in 46 (42.2%) patients, and 4 (3.7%) of the patients required renal replacement therapy (RRT). A total of 22 (20.2%) patients were admitted in the ICU, and 19 (17.4%) patients required invasive mechanical ventilation. 14 (12.8%) of the patients died. Patients who were admitted in the ICU were significantly older (age over 60 years) (38.1% vs 14.9%, p=0.016). 23 (21.1%) patients reached to composite outcome and these patients were significantly older (age over 60years) (39.1% vs. 13.9%; p=0.004), and had lower serum albumin (3.4g/dl [2.9-3.8] vs. 3.8g/dl [3.5-4.1], p=0.002), higher serum ferritin (679 mu g/L [184-2260] vs. 331 mu g/L [128-839], p=0.048), and lower lymphocyte counts (700/mu l [460-950] vs. 860 /mu l [545-1385], p=0.018). Multivariable analysis identified presence of ischemic heart disease and initial serum creatinine levels as independent risk factors for mortality, whereas age over 60 years and initial serum creatinine levels were independently associated with ICU admission. Abstract

Background: Chronic kidney disease (CKD) and immunosuppression, such as in renal transplantation (RT), stand as one of the established potential risk factors for severe coronavirus disease 2019 (COVID-19). Case morbidity and mortality rates for any type of infection have always been much higher in CKD, haemodialysis (HD) and RT patients than in the general population. A large study comparing COVID-19 outcome in moderate to advanced CKD (Stages 3-5), HD and RT patients with a control group of patients is still lacking. Methods: We conducted a multicentre, retrospective, observational study, involving hospitalized adult patients with COVID-19 from 47 centres in Turkey. Patients with CKD Stages 3-5, chronic HD and RT were compared with patients who had COVID-19 but no kidney disease. Demographics, comorbidities, medications, laboratory tests, COVID-19 treatments and outcome [in-hospital mortality and combined in-hospital outcome mortality or admission to the intensive care unit (ICU)] were compared. Results: A total of 1210 patients were included [median age, 61 (quartile 1-quartile 3 48-71) years, female 551 (45.5%)] composed of four groups: control (n = 450), HD (n = 390), RT (n = 81) and CKD (n = 289). The ICU admission rate was 266/1210 (22.0%). A total of 172/1210 (14.2%) patients died. The ICU admission and in-hospital mortality rates in the CKD group [114/289 (39.4%); 95% confidence interval (CI) 33.9-45.2; and 82/289 (28.4%); 95% CI 23.9-34.5)] were significantly higher than the other groups: HD = 99/390 (25.4%; 95% CI 21.3-29.9; P < 0.001) and 63/390 (16.2%; 95% CI 13.0-20.4; P < 0.001); RT = 17/81 (21.0%; 95% CI 13.2-30.8; P = 0.002) and 9/81 (11.1%; 95% CI 5.7-19.5; P = 0.001); and control = 36/450 (8.0%; 95% CI 5.8-10.8; P < 0.001) and 18/450 (4%; 95% CI 2.5-6.2; P < 0.001). 39. Effects of Interdialytic Weight Gain and Salt Intake on Serum Vascular Endothelial Growth Factor-C (VEGF-C) Levels in Hemodialysis Patients.

Abstract

Objective: Sodium may accumulate in subcutaneous tissue without any osmotic (non-osmotic) effect. There are insufficient data on non-osmotic sodium mechanisms in hemodialysis patients. We aimed to investigate the relationship between serum vascular endothelial growth factor-C levels, interdialytic weight gain, and salt intake in hemodialysis patients. Methods: The study included 20 anuric hemodialysis patients (11/9; female/male) with stable interdialytic weight gain in the last 3 months. Pre- and post-dialysis sodium and vascular endothelial growth factor-C levels were studied in 2 consecutive hemodialysis sessions. Fluid body components were determined by body composition monitor. The formulas derived from serum sodium and body fluids were used to calculate the interdialytic salt intake. The determinants of serum vascular endothelial growth factor-C levels were analyzed. Results: Salt intake in the interdialytic period was 10.36 ± 5.16 g/day and interdialytic weight gain 3.4% (0.53-6.98). While no correlation was found between salt intake and vascular endothelial growth factor-C levels in the correlation analysis, a moderate correlation was found between interdialytic weight gain and pre-and post-dialysis vascular endothelial growth factor-C levels (r: 0.453; P = .045 and r: 0.454; P = .044, respectively). In the partial correlation analysis performed by controlling the salt intake effect, an increase in this correlation was detected (r: 0.489; P = .34 and r: .525; P = .018, respectively). There was no relationship between age, gender, hemodialysis vintage, and vascular endothelial growth factor-C levels. Conclusion: A moderate correlation was detected between interdialytic weight gain and serum vascular endothelial growth factor-C levels and it may be a clinical reflection of long-term volume and salt loading. However, more extensive studies are needed to clarify this relationship and determine the factors affecting serum vascular endothelial growth factorC levels in hemodialysis patients. Abstract

BACKGROUND Blood pressure variability (BPV) is associated with end organ damage and cardiovascular outcomes in hypertensive patients. Prehypertensive patients frequently develop hypertension (HT). The purpose of the present study was to evaluate the effect of BPV on the development of HT. METHODS Two hundred and seven prehypertensive patients from the Cappadocia cohort were monitored over 2 years, and 24-hour ambulatory blood pressure monitoring (ABPM), office BP, and home BP measurements were subsequently performed at 4- to 6-month intervals. BPV was calculated as average real variability (ARV) from 24-h ABPM data, home BP, and office BP measurements at first visit. The relationship was evaluated between baseline ARV and the development of HT. RESULTS HT was diagnosed in 25.60% of subjects. Baseline 24-hour ABPM systolic blood pressure (SBP)ARV and diastolic blood pressure (DBP)ARV and home SBPARV were significantly higher in patients who developed HT than the other patients (P 0.006, 0.001 and 0.006, respectively). Baseline 24-hour ABPM SBPARV and home SBPARV exceeding the 90th percentile were identified as parameters affecting development of HT at logistic regression analysis. CONCLUSION In conclusion, our prospective observational cohort study showed that short-term BPV in particular can predict the development of HT in the prehypertensive population. Araştırmada okul yöneticilerinin kişilik özellikleri algı düzeylerini belirlemek amacıyla "Sıfatlara Dayalı Kişilik Testi" ölçeğinin Çeliköz ve Şeker Sır'ın (2016) Doğrulayıcı Faktör Analizi (DFA) ile test ederek geçerlik, güvenirlik çalışmasını yaptıkları ve 40 maddeden 27 maddeye indirdikleri Beş Faktörlü Kişilik Ölçeği kullanılmıştır. Okul yöneticilerinin duygusal zekâ algı düzeylerini ölçmek için Tatar, Tok ve Saltukoğlu (2011) tarafından gözden geçirilerek üç faktörlü bir yapıda Türkçe'ye uyarlaması yapılan Schutte Duygusal Zekâ Ölçeği (1998) kullanılmıştır. Öz-yeterlik algılarını ölçmek amacıyla Tschannen-Moran ve Gareis (2004) tarafından geliştirilen ve Türkçe'ye uyarlaması Negiş-Işık ve Derinbay (2015) tarafından yapılan 18 madde ifadesi bulunan tek faktörlü Yönetici Öz-yeterlik Ölçeğinden yararlanılmıştır. Araştırmada okul yöneticilerinin kişilik özellikleri, duygusal zekaları ve öz-yeterlikleri arasındaki ilişki pearson momentler çarpımı korelasyon katsayısı tekniği ile test edilmiştir. Okul yöneticilerinin kişilik özelliklerini öz-yeterlikleri ile duygusal zekanın yordama düzeyi regresyon analizi ile sınanmıştır. Okul yöneticilerinin cinsiyet ve öğrenim düzeyi değişkenlerine göre kişilik özelliklerine ilişkin puan ortalamaları arasında farklılaşmanın olup olmadığına ilişkin t-testi sonuçları karşılaştırılmıştır. Araştırma sonuçlarına göre; okul yöneticilerinin kişilik özelliklerinin alt boyut puanları ile duygusal zekanın alt boyut puanları arasında ve her iki değişkenin öz-yeterlik puanları arasında pozitif yönlü anlamlı ilişkiler olduğu görülmüştür., The purpose of this study is to explore the personality traits of school administrators in terms of emotional intelligence and self-efficacy. The study group consists of 384 school administrators working in the center of Konya in the 2017-2018 academic year. 17,7% of the school administrators in the study group were female and 82,3% were male, 86,7% of school administrators have a bachelor's degree, 13,3% have master degree and doctorate degree. Five Factor Personality Scale, its Confirmatory Factor Analysis (CFA) was tested to validity and reliability by Çeliköz and Şeker Sır (2016) as "Personality Test Based Adjectives" and reduced the scale from 40 to 27 items, used to determine the perception levels of the personality traits of school administrators in the study. The Schutte Emotional Intelligence Scale (2002), reviewed and adapted to Turkish in a three-factor structure by Tatar, Tok and Saltukoğlu (2011), was used to determine the emotional intelligence of school administrators. With the intention of measuring the perceptions of self-efficacy, it was utilised from the scale, which was developed by Tschannen-Moran and Gareis (2004) and adapted to Turkish and consisted of 18 items as a single-factor Administrator Self-Efficacy Scale by Negiş-Işık and Derinbay (2015). In the study, the relationship between the personality traits, emotional intelligence and self-efficacy of school administrators was analyzed with the pearson moment product correlation coefficient technique. The predictive level of emotional intelligence, self-efficacy and the personality traits of school administrators was tested by regression analysis. According to the variables of gender and education levels of school administrators, the t-test results were compared to whether differences exist between the mean scores in terms of the personality traits. According to the research findings; there was found a significant positive relationship between the scores of sub-dimensions of the personality traits and emotional intelligence of school administrators as well as between both of variables and self-efficacy scores.

Abstract

Coronavirus disease 2019 (COVID-19) has been accepted as a global pandemic, and poses a greater risk to the elderly and those with comorbidities. Comorbid diseases (particularly end-stage kidney disease with hemodialysis) and impaired immunity place patients in the high-risk group for COVID-19. In recent studies, it was also mentioned that exaggerated inflammation and a cytokine storm were the underlying causes related to the high mortality in COVID-19 patients. Currently, treatment modalities to balance the immune system of such vulnerable patient groups are essential, to protect them from the disease. Several vitamins (like vitamins C, D, and E), trace elements like zinc, and probiotics have been proposed as immune boosters to protect and combat infectious conditions. It is well known that these vitamins and elements are insufficient in hemodialysis patients. In this review, we aimed to evaluate the immune-boosting mechanisms of vitamins C, D, E, zinc, and probiotics, the studies related to their beneficial effects against infections, and their possible benefits for hemodialysis patients during the COVID-19 pandemic. 46. Evaluation of retinal nerve fiber layer thickness and macular thickness in amblyopic children.

Abstract

Objective: To compare central macular and retinal nerve fiber layer (RNFL) thickness using Spectral Domain Optical Coherence Tomography (SD-OCT) among patients with hyperopic anisometropic, strabismic and mixed amblyopia and healthy controls. Method: This prospective, cross-sectional study included 39 amblyopic children, their fellow eyes and 20 healthy individuals. Central macular and RNFL (superior, nasal, inferior and temporal quadrants and mean RNFL) thicknesses were measured with SD-OCT. Results: Difference was not statistically significant in the central macular thickness when the anisometropic, strabismic and mixed amblyopia groups were compared among themselves, fellow eyes and healthy control group. In the anisometropic amblyopic group, the inferior RNFL and nasal RNFL values were significantly thicker compared to the fellow eyes (p=0.009, p=0.028, respectively). Mean RNFL measurements were significantly thicker in anisometropic and mixed groups in the amblyopic eyes compared to the fellow eyes (p=0.009, p=0.031, respectively). In the temporal quadrant, the RNFL was found to be statistically significantly thinner in the anisometropic and mixed amblyopic groups compared to the fellow eyes (p=0.049, p=0.010, respectively). Conclusions: There was no considerably difference in the central macular thickness among the amblyopia groups, fellow eyes and healthy controls. However, significant changes were found in the mean RNFL and certain quadrants of the RNFL thicknesses in the anisometropic and mixed amblyopic groups. [ABSTRACT FROM AUTHOR]
