Onkoplastik meme cerrahisi, malignite tanısı bilinen hastalarda yaygın kullanımı olan rezeksiyon sonrası kalan meme dokusunun şeklini düzenlemeye yönelik bir uygulamadır. Birçok onkoplastik yöntem ile meme kanseri tedavisinde kozmetik etkinlik ve onkolojik başarının birlikte sağlanabildiği bildirilmiştir. Çalışmada bu yöntemler, meme kanserinin cerrahi tanısına uyarlanmakta; yöntemlerin estetik başarısıyla onkolojik sonuçları prospektif randomize araştırma biçimiyle sorgulanırken geleneksel eksizyonel biyopsi, onkoplastik yaklaşımla karşılaştırılmaktadır. Çalışmanın ön hazırlık döneminde, eksizyonel meme biyopsisi kararı alınan hastalarda meme yapısı ve lezyon özelliklerine göre hangi yöntemin kullanılacağı literatür derlenerek belirlenmiş ve kanıta dayalı onkoplastik yaklaşımlı eksizyonel meme biyopsisi protokolü belirlenmiştir; onkapeks protokolü. Meme büyüklüğü ve lezyon konumuna özel, seçilmiş prosedürler, çıkartılması planlanan piyesin meme hacmine oranıyla belirlenen iki düzeyli onkoplasti protokolüne uygun bir çizelgede toplanmaktadır. Farklı lokalizasyonlarda farklı kozmetik sonuçların bildirildiği bilindiğinden çalışmada meme dokusu 2 sagital plan ile 3 tabakaya ayrılarak incelenmiştir; medial, santral ve lateral. Santral ve lateral tabakaların kozmetik sonuçlarının onkoplastik grupta daha iyi olduğu gösterilmiştir (p0,05).Tüm radyoloji ve patoloji raporlarının uyumluluğu sorgulanarak cerrahi tanı süreci gereksiz biyopsileri belirlemek üzere hasta düzeyinde mercek altına alınmıştır. İleri non-invaziv biyopsi yöntemleri ile saptanabilecek olan lezyonların olası klinik seyri üzerine çıkarımlarda bulunulmuştur. Gelecekte hasta katılımının daha yüksek olduğu çalışmalar ve metaanalizler ile desteklenebilirse onkapeks protokolü meme kanserinin tanısında altın standart yöntem kabul edilen konvansiyonel eksizyonel biyopsinin yerini alabilir. Böyle bir durumda multidisipliner kurullarda hastaya özel tedavi stratejisinin geliştirilmesinin önemi daha da fazla olacaktır. Oncoplastic breast surgery is a common surgical procedure that relies on reshaping the remaining breast tissue after resection in patients with a known diagnosis of malignancy. Reportedly, cosmetic efficacy and oncologic success in breast cancer treatment are likely to be achieved simultaneously by many oncoplastic procedures. Here in this study, these procedures are adapted to the surgical diagnosis of breast cancer and oncologic outcomes with the aesthetic success of these methods were investigated, using a prospective randomized clinical trial design comparing conventional excisional biopsy with the oncoplastic approach.During the preliminary phase of the study, literature was reviewed to establish an evidence-based oncoplastic approach to excisional breast biopsies, oncapex protocol, for different breast and lesion characteristics of patients who are decided to be suitable for excisional breast biopsy. Specifically, selected procedures varying according to the breast size and lesion site, were tabulated in the bilevel oncoplastic protocol determined by calculating the anticipated volume ratio of the resection specimen. Since it is known that different cosmetic results reported for different segment lesions, breast tissue is divided into 3 blocks with 2 sagittal planes; medial, central and lateral blocks. Central and lateral blocks yielded better cosmetic results (p0,05).All the pathology reports and radiology were reviewed to identify discordance for all lesions and the complete diagnostic process is scrutinized to expose any unnecessary biopsies. Potential implications of advanced non-invasive biopsy methods on the clinical course of detectable lesions are discussed. If supported by future large-scale studies, this approach is likely to revolutionize breast cancer diagnosis and oncapex biopsy could be the new gold standart diagnostic method. 104

Tiroid cerrahisinin bugün için başlıca komplikasyonları inferior ve süperior laringeal sinirlerin yaralanması, hipokalsemi ve kanamadır. Tiroidektomi sonrası gelişen hi¬pokalseminin en önemli nedeni hi-poparatiroididir. Hipoparatiroidi; paratiroid bezlerin travması, paratiroidlerin devaskülarizasyonu, tiroid kapsülü¬nün üstünde veya nadiren altında bulunan paratiroid dokusunun yanlış¬lıkla çıkarılması sonucu gelişir. Semptomatik hipokalsemi %5-30 has¬tada geçici, %1-4 hastada kalıcı olabilir. Çalışmamızda geçici hipokalsemi oranı %16,5 iken, kalıcı hipokalsemi oranı %4,7 olarak bulunmuş olup, literatürdeki oranlara benzemektedir.Hastalarda postoperatif 48-72 saat içinde geli¬şebilecek hipokalsemi nedeniyle hastaların 48-96 saat süreyle hospi¬talizasyonu birçok merkezde yaygın olarak uygulanmaktadır. Total tiroidektomi uygulanan hastalarda erken postoperatif dönemde parathormon ölçümü etkili bir parametre olup has¬taların en geç 24 saat sonunda güvenli bir şekilde eve çıkarılabilmelerini sağlamaktadır. Çalışmamızda postoperativ 20. dakika, 4. saat ve 24. saatte ölçülen PTH değerleri hipokalseminin erken dönemde saptanması açısından istatistiksel olarak anlamlı bulunurken (p:0,0001), üç PTH değeri arasında istatistiksel açıdan anlamlı bir fark bulunamamıştır (p>0.05). Yapmış olduğumuz bu çalışmada hastalar 24. saat PTH değerlerine bakılarak taburcu edildi. Parathormon düzeyi (15-65 pg/ml) < 10 pg/ml olan hastalar bir gün daha hospitalize edilip intravenöz kalsiyum replasmanı yapıldıktan sonra 2-5 g/gün kalsiyum karbonat ve 0,5-1 µg/gün kalsitriol ile taburcu edildi. PTH düzeyi 10-20 pg/ml olanlar sadece kalsiyum karbonatla taburcu edildi. PTH düzeyi 20 pg/ml üzerinde olanlara ise herhangi bir tedavi verilmedi.Total tiroidektomi sonrası gelişebilecek hipokalseminin erken dönemde tespitinde postoperatif kalsiyum değerleri çalışmamızda anlamlı bulunmamıştır. Semptomatik hipokalseminin tanı¬sında kalsiyum ölçümünün, parathor¬mon ölçümüne göre daha ekonomik olmasına rağmen çalışmamızda PTH ölçümü ile takip, kalsiyum ölçümü ile takibe göre daha efektif olarak bulunmuştur. Postoperatif 20. dakikada ölçülecek PTH değeri ile hastaların 24. saatte güvenle taburcu edilebileceğini ve 10 pg/ml den büyük PTH değerleri varlığında postoperatif hipokalsemi semptomlarıyla karşılaşıl¬mayacağı kanaatindeyiz. The principal complications of thyroid surgery are the injury of inferior and superior laryngeal nerves, hypocalcemia and bleeding for today.The most important reason of hypocalcemia which developing after thyroidectomy is hypoparathyroidism. Hypoparathyroidism; develops as a result of accidentally removal of the parathyroid gland located at above or rarely at the bottom of thyroid capsule, trauma of theparathyroid glandsand devascularization of parathyroids.Symptomatic hypocalcemia can be temporary on 5-30% and permanent on 1-4% of the patients. In our study, the rate of permanent hypoparathyroidism was found 4.7% while the rate of temporary hypocalcemia is 16.5% and proportions similar to those in the literature.The hospitilization of the patients for 48-96 hours is widely practiced in many centers due to hypocalcemia on patients which can postoperatively develop within 48-72 hours. Parathyroid hormone measurement is an effective parameter for the patients who applied total thyroidectomy in the early postoperative period and thus provides safe discharges to their home after 24 hours at most.In our study, While the PTH values which are the postoperatively measured on the 20th minute, 4th hour and 24th hour are statistically being founded significant in terms of early detection of hypocalcemia (p:0,0001), a significant difference couldn't be found between the three PTH values as statically (p>0.05). The patients were discharged by looking at the values of PTH within 24 hours.Patients have parathormone level (15-65 ph/ml) < 10 ph/ml have been hospitalizied one more day, treated with intravenous calcium replacement and discharged with 2-5 g/day calcium carbonat and 0,5-1 µg/day calsitriol treatment. Patients have PTH level 10-20 pg/ml were discharged only with calcium carbonat. Patients have PTH level above 20 pg/ml weren't given any treatment.In our study, postoperative calcium values were not significant for early detection of hypocalcemia after total thyroidectomy.Although Calcium measurement in the diagnosis of symptomatic hypocalcemia was founded more economical than parathyroid hormone measurement, the follow-up study by the measurement of PTH were found more effective compared with the follow-up by the measurement of calcium.With postoperative PTH values measured on the 20th minute, patients will be safely discharged on the 24th hour and we believe that patients will not come across with the postoperative symptoms of hypocalcemia in the presence of PTH values more than 10 pg/ml.Key words: Total thyroidectomy, hypocalcemia, parathyroid hormone. 70
