2. Magnetic interaction and anisotropy axes arrangement in nanoparticle aggregates can enhance or reduce the effective magnetic anisotropy

Abstract

The magnetic response of nanostructures plays an important role on biomedical applications being strongly influenced by the magnetic anisotropy. In this work we investigate the role of temperature, particle concentration and nanoparticle arrangement forming aggregates in the effective magnetic anisotropy of Mn-Zn ferrite-based nanoparticles. Electron magnetic resonance and coercivity temperature dependence analyses, were critically compared for the estimation of the anisotropy. We found that the temperature dependence of the anisotropy follows the Callen-Callen model, while the symmetry depends on the particle concentration. At low concentration one observes only an uniaxial term, while increasing a cubic contribution has to be added. The effective anisotropy was found to increase the higher the particle concentration on magnetic colloids, as long as the easy axis was at the same direction of the nanoparticle chain. Increasing even further the concentration up to a highly packed condition (powder sample) one observes a decrease of the anisotropy, that was attributed to the random anisotropy axes configuration., Comment: 12 pages, 3 figures, 2 tables. Abstract

Background: Extracorporeal membrane oxygenation (ECMO) has been widely used in patients with COVID-19, but uncertainty remains about the determinants of in-hospital mortality and data on post-discharge outcomes are scarce. The aims of this study were to investigate the variables associated with in-hospital outcomes in patients who received ECMO during the first wave of COVID-19 and to describe the status of patients 6 months after ECMO initiation. Methods: EuroECMO-COVID is a prospective, multicentre, observational study developed by the European Extracorporeal Life Support Organization. This study was based on data from patients aged 16 years or older who received ECMO support for refractory COVID-19 during the first wave of the pandemic—from March 1 to Sept 13, 2020—at 133 centres in 21 countries. In-hospital mortality and mortality 6 months after ECMO initiation were the primary outcomes. Mixed-Cox proportional hazards models were used to investigate associations between patient and management-related variables (eg, patient demographics, comorbidities, pre-ECMO status, and ECMO characteristics and complications) and in-hospital deaths. Survival status at 6 months was established through patient contact or institutional charts review. This study is registered with ClinicalTrials.gov, NCT04366921, and is ongoing. Findings: Between March 1 and Sept 13, 2020, 1215 patients (942 [78%] men and 267 [22%] women; median age 53 years [IQR 46–60]) were included in the study. Median ECMO duration was 15 days (IQR 8–27). 602 (50%) of 1215 patients died in hospital, and 852 (74%) patients had at least one complication. Multiorgan failure was the leading cause of death (192 [36%] of 528 patients who died with available data). In mixed-Cox analyses, age of 60 years or older, use of inotropes and vasopressors before ECMO initiation, chronic renal failure, and time from intubation to ECMO initiation of 4 days or more were associated with higher in-hospital mortality. 613 patients

Abstract

The role of climate change in the development and demise of Classic Maya civilization (300 to 1000 C.E.) remains controversial because of the absence of well-dated climate and archaeological sequences. We present a precisely dated subannual climate record for the past 2000 years from Yok Balum Cave, Belize. From comparison of this record with historical events compiled from well-dated stone monuments, we propose that anomalously high rainfall favored unprecedented population expansion and the proliferation of political centers between 440 and 660 C.E. This was followed by a drying trend between 660 and 1000 C.E. that triggered the balkanization of polities, increased warfare, and the asynchronous disintegration of polities, followed by population collapse in the context of an extended drought between 1020 and 1100 C.E.

Abstract

We find exact analytical expressions for mixing angles in matter in the context of three generation neutrino oscillations in matter to discuss the role of resonances in this phenomenon. We show that some knowledge from conventional two neutrino MSW effect, which has been extended to approximated solutions to three neutrino oscillations, has to be abandoned in this exact approach. We observe that maximal values for the mixing angles in matter are found in nonresonant regions and stationary phases do not coincide anymore with resonances in this simple extension of the MSW effect. Abstract

Introdução: A pandemia do covid-19 surgiu como uma emergência pública mundial trazendo para a promoção à saúde novos desafios na construção da integralidade no atendimento dos indivíduos e dos serviços de saúde. Objetivo: Traçar as perspectivas diante dos desafios da pandemia e sua contenção, em um mapa conceitual da promoção à saúde na Atenção Primária. Metodologia: Foi estruturada em um artigo de opnião com reflexões por meio de experiências obtidas durante a pandemia do COVID-19 na atenção primária e os seus desafios no fortalecimento dos eixos para a promoção da saúde. Resultados: Foram evidentes os desafios relacionados as perdas multifatoriais e multidimensionais de natureza social, física, psicológica e econômica adquiridos na pandemia. Conclusão: Para lidar com as consequências negativas desta pandemia, é imperioso um planejamento de saúde estratégico e multidisciplinar que deve estar sempre articulado e pautado de forma permanente para visar uma promoção efetiva da saúde.

Abstract

Understanding high-field amplitude electromagnetic heat loss phenomena is of great importance, in particular, in the biomedical field, because the heat-delivery treatment plans might rely on analytical models that are only valid at low field amplitudes. Here, we develop a nonlinear response model valid for single-domain nanoparticles of larger particle sizes and higher field amplitudes in comparison to the linear response theory. A nonlinear magnetization expression and a generalized heat loss power equation are obtained and compared with the exact solution of the stochastic Landau-Lifshitz-Gilbert equation assuming the giant-spin hypothesis. The model is valid within the hyperthermia therapeutic window and predicts a shift of optimum particle size and distinct heat loss field amplitude exponents, which is often obtained experimentally using a phenomenological allometric function. Experimental hyperthermia data with distinct ferrite-based nanoparticles and third harmonic magnetization data support the nonlinear model, which also has implications for magnetic particle imaging and magnetic thermometry. Abstract

Background: CD40 ligand (CD40L) deficiency, an X-linked primary immunodeficiency, causes recurrent sinopulmonary, Pneumocystis and Cryptosporidium species infections. Long-term survival with supportive therapy is poor. Currently, the only curative treatment is hematopoietic stem cell transplantation (HSCT). Objective: We performed an international collaborative study to improve patients' management, aiming to individualize risk factors and determine optimal HSCT characteristics. Methods: We retrospectively collected data on 130 patients who underwent HSCT for CD40L deficiency between 1993-2015. We analyzed outcome and variables' relevance with respect to survival and cure. Results: Overall survival (OS), event-free survival (EFS), and disease-free survival (DFS) were 78.2%, 58.1%, and 72.3% 5 years after HSCT. Results were better in transplantations performed in 2000 or later and in children less than 10 years old at the time of HSCT. Pre-existing organ damage negatively influenced outcome. Sclerosing cholangitis was the most important risk factor. After 2000, superior OS was achieved with matched donors. Use of myeloablative regimens and HSCT at 2 years or less from diagnosis associated with higher OS and DFS. EFS was best with matched sibling donors, myeloablative conditioning (MAC), and bone marrow–derived stem cells. Most rejections occurred after reduced-intensity or nonmyeloablative conditioning, which associated with poor donor cell engraftment. Mortality occurred mainly early after HSCT, predominantly from infections. Among survivors who ceased immunoglobulin replacement, T-lymphocyte chimerism was 50% or greater donor in 85.2%. Conclusion: HSCT is curative in patients with CD40L deficiency, with improved outcome if performed before organ damage development. MAC is associated with better OS, EFS, and DFS. Prospective studies are required to compare the risks of HSCT with those of lifelong supportive therapy.

Abstract

We present anisotropic nonparabolic position-dependent effective-mass calculations of the bound energy levels of electrons confined in lens-shaped InxGa1-xAs quantum dots embedded in a GaAs matrix. The strain and In gradient inside the quantum dots and their wetting layer (due to the strong In segregation effect present in the InxGa1-xAs/GaAs system) were taken into account. The bound eigenstates and eigenenergies of electrons in a finite 3D confinement potential were determined by the full numerical diagonalization of the Hamiltonian. The quantum dots and their wetting layer were sliced into a finite number of monolayers parallel to the substrate surface, each one with a specific In concentration, in order to be able to reproduce any composition profile along the growth direction. A comparison between the eigenenergies of the 'pure' InAs quantum dots and the quantum dots with an inhomogeneous In content indicates that In segregation dramatically affects their electronic structure and must be taken into account if one wishes to accurately simulate the real optoelectronic properties of such nanostructures. Abstract

La atención odontológica frecuente juega un papel importante en la prevención, diagnóstico y tratamiento precoz de enfermedades. Las personas viviendo con el virus del SIDA (PVVS) constituyen un grupo de riesgo a adquirir cualquier tipo de enfermedad, por lo cual deberían estar bajo un estricto régimen de control médico. Existen ciertos factores, como la discriminación por parte de miembros del personal de la salud (médicos, odontólogos, etc), que dificultan esta práctica. El objetivo del estudio fue describir la experiencia y opinión de las Personas Viviendo con el Virus del Sida sobre la atención odontológica recibida. Fue aplicado un cuestionario de 10 preguntas a 113 PVVS que consultaron en el Servicio de Atención Integral del Instituto de Medicina Tropical. El 71,7% de los encuestados calificó la atención odontológica recibida como excelente. El 59,3% de las Personas Viviendo con el Virus del Sida refirió no informar al odontólogo de su condición. 41,3% de los pacientes refirieron haber sido víctimas de discriminación por parte del odontólogo. Al relacionar la discriminación y tipo de servicio utilizado, se encontró que no existe evidencia estadística suficiente para afirmar que ser víctima de discriminación depende del tipo de servicio utilizado (p=0,09). Con este trabajo se concluye que el índice de discriminación es todavía muy alto en los consultorios odontológicos, y que las Personas Viviendo con el Virus del Sida no realizan suficientes esfuerzos por mantener su salud bucal, lo cual refleja la necesidad de informar y concienciar, al odontólogo en cuanto a la ilegalidad de discriminar a pacientes por su condición médica, y a las PVVS sobre los múltiples beneficios que trae acudir al odontólogo regularmente. Frequent dental care plays an important role in the prevention, early diagnosis and treatment of diseases. People living with HIV AIDS (PLWHA) are a risk group that can acquire any type of disease, which should be under strict medical supervision regime. Certain factors, such as discrimination by members of staff (doctors, dentists, etc.) hinder the practice. The aim of the study was to determine the experience and opinion of People Living with HIV/AIDS virus on dental care provided. A questionnaire of 10 questions was applied to 113 People Living with HIV/AIDS virus registered on the Institute of Tropical Medicine. 71.7% of respondents rated the dental care received as excellent. 59.3% of People Living with HIV/AIDS virus reported that they do not inform the dentist of their condition. 41.3% of patients reported experiencing discrimination from a dentist. Relating discrimination and type of service used, it was found that there is not enough statistical evidence to say that being a victim of discrimination depends on the type of service used (p=0.09). With the applied methodology we conclude that the discrimination index is still very high in dental clinics, and People Living with HIV/AIDS virus do not make sufficient efforts to maintain their oral health, reflecting the need to inform and educate the dentist regarding the unlawfulness of discriminating patients by their medical condition, and People Living with HIV/AIDS virus on the multiple benefits that comes with seeing a dentist regularly.

Abstract

The present paper discusses the insertion of Social Assistants in Public Health Policy under a perspective the expansion of social rights, since these profesionals work in prevention, health's promotion and regular improvements in public Health System in relation to access and extension rights health desk. In this way, the general objective is to analyze as the insertion of social assistants in health in the framework of the Health Reform Movement even an arrival of Neoliberalism in 90s. In the literature review have been studied authors who discuss the theme Social and Health Service as: Bravo (2006), Code of Ethics/a Social Worker (1993), Minayo (1994) and Parameters paragraph Practice of Social Workers in Health Policy (2013), besides documentary revision of the laws relating to health such as the Constitution of the Federative Republic of Brazil (1988) and Law 8.080. A qualitative approach was used from the method of the narrative analysis, which will address the major landmarks that customize the inclusion of social workers in health, focusing mainly on health reform, federal council of social assistant resolution and the arrival of Neoliberalism. The resolution legitimized the performance of these professionals in the field of health, being currently the area that employs more social workers in Brazil. Finally, the study found that the social worker has his activities often considered as irrelevant, this fact is given even greater importance to the curative practices, minimizing educational activities, mobilization and communication in health together with the various professionals in the field of health. Abstract

Biometric-based techniques have recently emerged as a trustworthy and effective approach of user authentication; however, unlike conventional authentication methods such as passwords and tokens, if an enrolled biometric template is compromised, usually it cannot be revoked or re-issued. In this paper, four naive cancelable techniques, namely, shifting, password-dependent shifting, XOR and adding, for a bimodal biometric cryptosystem are presented. The proposed cancelable techniques are designed to be embedded into any bimodal biometric cryptosystem. The bimodal biometric cryptosystem uses speech and electrocardiogram signals as biometric information. The biometric cryptosystem implements an error-correction layer using the Hadamard code. The performance of the four cancelable techniques is assessed using ECG signals from MIT-BIH database and speech signals from a speech database created for testing purposes. The results show that the best performance in terms of FAR and FRR metrics is achieved with XOR and adding techniques
