Abstract Monocyte chemoattractant protein-1 (MCP-1) or C-C Motif Chemokine Ligand 2 (CCL-2) is a key chemokine implicated in the inflammatory response to malaria. The objective of the study was to synthesize the evidence on variations in MCP-1/CCL-2 levels in relation to Plasmodium spp. infections and the severity of malaria. The protocol for this systematic review and meta-analysis was registered at PROSPERO (CRD42024565867). To identify relevant studies, a comprehensive search strategy was conducted using major databases, including PubMed, Scopus, Embase, Medline, Journals@Ovid, and Nursing & Allied Health Premium. The Joanna Briggs Institute (JBI) critical appraisal checklists were used to determine the risk of bias in selected studies. The pooled standardized mean difference (SMD, Hedge’s g) and their 95% confidence interval (CI) were estimated using the random-effects model. Thirty-three studies, with a total of 6,804 participants enrolled, were included in the systematic review. Most studies (60.6%) were published between 2010 and 2019; the majority (57.6%) were conducted in Africa. The predominant Plasmodium species studied was P. falciparum (66.7%). The meta-analysis revealed no significant difference in MCP-1/CCL-2 levels between Plasmodium-infected and uninfected individuals (P: 0.16, SMD: 0.99, 95% CI: -0.39–2.37, I 2 : 97.2%, number of participants: 2140). Subgroup analysis found an increase in MCP-1/CCL-2 levels in children with Plasmodium infections, with no heterogeneity observed (I 2 : 0%). Subgroup analysis found no difference in MCP-1/CCL-2 levels between P. falciparum-infected participants and uninfected participants, as well as between P. falciparum or non-P. falciparum-infected participants and uninfected participants. The meta-analysis revealed significantly higher MCP-1/CCL-2 levels in participants with severe Plasmodium infections compared to those with non-severe malaria (P: 0.04, SMD: 1.51, 95% CI: 0.06–2.95, I 2 : 98.5%, number of participants: 1371). The systematic review and meta-analysis suggest no statistically significant difference in MCP-1/CCL-2 levels in participants with Plasmodium infections overall. However, there was a significant increase in MCP-1/CCL-2 levels in patients with severe malaria. These findings suggest that MCP-1/CCL-2 may have potential as a prognostic biomarker for severe malaria. Future research should focus on large-scale, well-designed studies to validate the role of MCP-1/CCL-2 in malaria and further explore its prognostic potential.

Abstract Objective To identify the most commonly reviewed behaviour change techniques (BCTs) and their effectiveness based on consistency across reviews for lifestyle interventions of non-communicable diseases. Design Umbrella review of systematic reviews. Data sources PubMed, Embase, PsycINFO, Cochrane CENTRAL, Global Health. Data extraction and synthesis A narrative synthesis of extracted findings was conducted. The Behaviour Change Technique v1 Taxonomy was used to identify and code behaviour change techniques (e.g., goal setting) in a standardised manner, which were independently assessed by two reviewers. Study quality was independently assessed by two reviewers using the assessment of multiple systematic review tools. Results 26 reviews were included with a total of 72 BCT labels evaluated across the different lifestyle interventions and non-communicable diseases. A total of 13 BCT clusters were identified to be reported as effective. The most commonly reviewed BCTs and their effectiveness/ineffectiveness were as follows: ‘Goals and Planning’ (12 effective/1 ineffective), ‘Feedback and monitoring’ (9 effective/3 ineffective), ‘Social support’ (9 effective/1 ineffective), ‘Shaping knowledge’ (11 effective/1 ineffective), and ‘Natural consequences’ (6 effectiveness/ 2 ineffective). The vast majority of the studies were conducted in high-income and a few in upper middle-income countries, with hardly any studies from lower middle-income and lower income studies. Conclusion The most common BCTs were ‘Goals and Planning’, ‘Feedback and Monitoring’, ‘Shaping Knowledge’, ‘Social Support’, and ‘Natural Consequence’. Based on consistency across reviews, several BCTs such as ‘Goals and Planning’, Feedback and Monitoring’, ‘Shaping Knowledge’, and ‘Social Support’ have demonstrated effectiveness (Recommendation Grade A) in improving health behaviours across a limited range of NCDs. The evidence is less clear for other BCT techniques. It is also likely that not all BCTs will be transferable across different settings. There is a need for more research in this area, especially in low-middle-income countries. Background: Family medicine has been recognized as a medical specialty in Thailand. However, there is a lack of information about scholarly publications. Bibliometric analysis is a valuable methodology for identifying research trends and knowledge gaps in this field. This study aims to analyze scholarly publications related to family medicine in Thailand. Method: This bibliometric analysis was conducted based on the PubMed search in March 2023. All relevant literature related to 'family medicine' and 'Thailand' was retrieved. Performance analysis examined the publication trends from 1963 to 2023, the top 10 journals publishing relevant articles, and the number of publications by research areas. Science mapping depicted clusters of co-occurring author keywords, their relationships, and the research themes' trend over two decades. Results: A total of 1483 publications were retrieved. The number of publications showed an upward trend, increasing from 61 before 2000 to 359 in 2020. The Journal of the Medical Association of Thailand was the leading journal, publishing 267 articles (18.0 %). Research areas were categorized into Basic Knowledge (n = 211), Clinical Problem Solving (n = 714), Health Service (n = 256), Health System (n = 254), and Medical Education (n = 48). Science mapping identified six clusters based on 106 co-occurrence keywords, including public health, health services, medical issues in older adults, healthcare access, epidemiology, and others. Research themes have shifted from infectious diseases and public health to primary care and non-communicable diseases. Conclusion: Future research should focus more on implementation at a population level and healthcare system, with more investigation into geriatric care and child and maternal health.

Introduction Homelessness is a significant global challenge affecting people worldwide. In Thailand, the health-related issues of people experiencing homelessness have not been a major research focus. This scoping review aims to explore the scope of research on health-related issues among people experiencing homelessness in Thailand. Methods Eight databases (ACI, AMED, Embase, PsycINFO, PubMed, Scopus, TCI, and Web of Science) were searched from inception of each database to August 2022. The search terms consisted of terms related to people experiencing homelessness in Thailand. Research articles published in Thai or English were included. Results Of 186 articles, 167 were excluded during duplicate removal (n = 45), title and abstract screening (n = 106), seeking for full-text articles (n = 2), and full-text screening (n = 14). Nineteen articles were included for extraction and synthesis. Three topics (health status, causes of homelessness and effects of homelessness on health, healthcare and social services) were summarised. The included studies described mental health (e.g., depression, suicide, alcohol and drug dependence), physical health (e.g., poor self-hygiene, injuries, accidents), and health behaviours (e.g., alcohol drinking, substance use, unsafe sex). Social behaviours and health problems linked to homelessness, and several factors related to health and living conditions (e.g., stigma, discrimination) were reported. People experiencing homelessness had some barriers to access to healthcare and social services (e.g., health insurance, social welfare, financial difficulties). Conclusions The majority of studies on health-related issues in people experiencing homelessness in Thailand are descriptive studies. Future studies should focus on the interactions and mechanisms between homelessness and health.

Chronic abdominal pain is a challenging problem in clinical practice, with several pathophysiological mechanisms underlying its aetiologies. This case report presents a geriatric patient with multiple comorbidities who had experienced intermittent abdominal pain for over 10 years. Alarming symptoms were ruled out, and a functional gastrointestinal disorder was determined as the most likely cause. The patient’s medical history and previous treatments were thoroughly reviewed, revealing that long-term use of metformin and an oral iron supplement was the iatrogenic symptom triggers. The abdominal pain resolved upon discontinuation of these two medications. This case report highlights the significance of reviewing iatrogenic causes and periodically assessing chronic medical conditions to identify potential contributing factors of chronic abdominal pain.

Background: This systematic review aimed to summarize and evaluate the prevalence of physical activity (PA) counseling in primary care. Methods: Five databases (CINAHL Complete, Embase, Medline, PsycInfo, and Web of Science) were searched. Primary epidemiological studies on PA counseling in primary care were included. The Joanna Briggs Institute critical appraisal checklist for studies reporting prevalence data was used to assess the quality of studies. The review protocol was registered with PROSPERO (CRD42021284570). Results: After duplicate removal, 4990 articles were screened, and 120 full-text articles were then assessed. Forty studies were included, with quality assessment scores ranging from 5/9 to 9/9. The pooled prevalence of PA counseling based on 35 studies (199830 participants) was 37.9% (95% CI 31.2 to 44.6). The subgroup analyses showed that the prevalence of PA counseling was 33.1% (95% CI: 22.6 to 43.7) in females (10 studies), 32.1% (95% CI: 22.6 to 41.7) in males (10 studies), 65.5% (95% CI: 5.70 to 74.1) in people with diabetes mellitus (6 studies), 41.6% (95% CI: 34.9 to 48.3) in people with hypertension (5 studies), and 56.8% (95% CI: 31.7 to 82.0) in people with overweight or obesity (5 studies). All meta-analyses showed high levels of heterogeneity (I2=93% to 100%). Conclusion: The overall prevalence of PA counseling in primary care was low. The high levels of heterogeneity suggest variability in the perspectives and practices of PA counseling in primary care. PA counseling should be standardized to ensure its optimum effectiveness in primary care.

Introduction: Communication is a key element of palliative care. The concealment of advanced-stage diseases is a communication challenge. This study aimed to explore the patterns and difficulties in communication regarding the concealment of advanced-stage cancer between caregivers and patients. Methods: This qualitative study employed an interpretive phenomenological approach and was conducted at a district hospital in Thailand. Semi-structured in-depth interviews were performed to collect data from caregivers (i.e. family members) of patients regarding the concealment of advanced-stage cancer. The data analysis followed an inductive thematic approach. Results: Ten in-depth interviews were conducted among the caregivers of patients aged 57–97 years. Four themes emerged: (i) reasons for concealing the diagnosis and prognosis (personality of patients and concerns about negative effects), (ii) communication patterns between caregivers and patients (communicating symptoms/signs instead of the diagnosis/prognosis and distorting information), (iii) difficulties and challenges in maintaining concealment (feelings of guilt, hesitation in sharing the information and suspicion of patients’ awareness of their diagnosis/prognosis) and (iv) communication support from healthcare professionals (avoiding informing patients about their diagnosis/prognosis, supporting decision-making and disclosing the information). Conclusion: The concealment of advanced-stage cancer is perceived as an appropriate communication approach among some caregivers. Communicating information about advanced-stage cancer is dynamic. Some caregivers and families consider disclosing the information in the future. Healthcare professionals can support communication throughout care. Future studies should focus on decision-making and communication processes for better handling of information concealment or the conspiracy of silence in palliative care.

Physical activity (PA) effectively prevents and treats non-communicable diseases in clinical settings. PA promotion needs to be more consistent, especially in busy primary care. Sports scientists have the potential to support PA promotion in primary care. The Physical Activity with Sports Scientist (PASS) programme is created to personalise PA promotion led by a sports scientist in a primary care clinic. A pragmatic randomised controlled trial with two parallel groups will be conducted at a family medicine clinic. Physically inactive participants aged 35–70 years who have type 2 diabetes mellitus, hypertension or dyslipidaemia will be invited. The control group (n=60) will receive usual care. The intervention group (n=60) will receive the PASS programme and usual care. The PASS programme will consist of a tailored PA prescription after the physician’s consultation at the first visit and monthly phone follow-ups. The primary outcome is the proportion of participants who have achieved the PA goal defined as aerobic activity (≥150 min/week of moderate to vigorous-intensity PA), muscle-strengthening activity (≥2 days/week of moderate or greater intensity) and multicomponent PA (≥2 days/week of moderate or greater intensity). Secondary outcomes are body composition and physical fitness. The primary and secondary outcomes will be measured and compared between the control and intervention groups at visit 1 (month 0: baseline measurements), visit 2 (months 3–4: follow-up measurements), visit 3 (months 6–8: end-point measurements) and visit 4 (months 9–12: continuing measurements). The study protocol was registered with the Thai Clinical Trials Registry.Trial registration number: TCTR20240314001.

Generic medication is a product that contains the same active substance and pharmaceutical characteristics as brand-name medications. Generic medications are cost-effective and comparable to brand-name medications in terms of clinical endpoints. However, the use of generic medications instead of brand-name medications is a debatable issue among patients and healthcare providers. Two patients with essential hypertension experienced side effects after switching to different generic antihypertensives (one generic medication to another generic medication). Adverse drug reactions, including, hypersensitivity, side effects, and intolerance, should be identified through present and past medical history and clinical characteristics. The adverse drug reactions in both patients were more likely to be side effects of the medications after switching to different generic antihypertensives produced by different companies (patient 1: enalapril and patient 2: amlodipine). The side effects were possibly caused by the different inactive ingredients or excipients. These two case reports emphasise the importance of monitoring adverse drug reactions throughout the course of treatment and communicating with patients prior to switching to a new generic medication.

This study investigated physical activity (PA) levels in Thai university students and their associated factors at multiple levels. Data of 3,930 university students age ≥18 years analyzed in this study were retrieved from a cross-sectional online survey, namely the ASEAN University Network – Health Promotion Network (AUN–HPN) health behavioral survey. The hierarchical generalized linear model considering clustering effects was applied to examine factors associated with sufficient PA across multiple levels. At the individual level, being female, underweight, and overweight had significantly lower odds of having sufficient PA. Sophomores had 22 % greater odds compared with freshmen. Students engaging in 1–3 and 4–6 sports activities had 3 and almost 4 times respectively higher odds of having sufficient PA. Students engaged in >8 h of sedentary time, and consumed adequate amounts of fruit/vegetables had 70 % and 59 % respectively higher odds of having sufficient PA. At the environmental level, students who traveled inactively to/from university, attended a university with private recreational facilities, and a university that adopted the healthy university framework (HUF) had significantly greater odds of having sufficient PA. A majority of university students were sufficiently physically active, despite gender disparity. When promoting PA among them, focus may be targeted at females and sophomores, and consider addressing the number of sports activities engaged, fruit/vegetable consumption, availability of private university recreational facilities, and adoption of the HUF. The results support the implementation of the AUN–HPN HUF, and future initiatives within the country and the wider network for health and PA promotion.

Abstract Background Thailand has been one of the largest migration hubs in Southeast Asia for the past four decades and keeps attracting migrants from neighboring countries. Due to difficulties associated with their status, migration can place individuals at a heightened risk for sexually transmitted diseases. This study aimed to examine factors influencing HIV and syphilis preventive behaviors among Myanmar migrants in Samut Sakhon, Thailand. Methods A cross-sectional mixed-method study was conducted among Myanmar migrants aged 18 years and above in Samut Sakhon Province, Thailand. To fulfill this study's aims, four hundred seventy-three respondents completed a survey to provide quantitative data, and eight participants completed in-depth qualitative interviews. The factors associated with protective sexual behaviors were identified with multiple logistic regression analysis of the quantitative study data and thematic analysis of the qualitative data. Results The respondents showed good knowledge of HIV and syphilis (50.1%), but over half (55.6%) exhibited the negative perception of sexually transmitted disease prevention; about 81.4% of male respondents never used a condom when they had sexual intercourse in the past year. Based on multivariate analysis, income expenses-balance (adjusted odds ratio (AOR) = 2.379, 95% confidence interval (CI): 1.002–5.731, p = 0.049), number of sex partners (AOR = 3.044, 95% CI: 1.339–6.924, p = 0.008), and having sex with a prostitute (AOR = 6.085, 95% CI: 1.28–28.918, p = 0.023) were all statistically associated with unprotected sex. In the qualitative analysis, knowledge, understanding, beliefs; the influence of culture, community, and environment; and condom perceptions were also important factors. Conclusion Low use of condoms in sexual practice was identified, and the appropriate intervention or approach to improve the utilization of condoms in the community was provided.

Abstract Background This study investigated, through cluster analysis, the associations between behavioural characteristics, mental wellbeing, demographic characteristics, and health among university students in the Association of Southeast Asian Nations (ASEAN) University Network – Health Promotion Network (AUN-HPN) member universities. Methods Data were retrieved from a cross-sectional self-administered online survey among undergraduate students in seven ASEAN countries. A two-step cluster analysis was employed, with cluster labels based on the predominant characteristics identified within the clusters. The ‘healthy’ cluster was assigned as the reference group for comparisons using multinomial logistic regression analysis. Results The analytic sample size comprised 15,366 university students. Five clusters of student-types were identified: (i) ‘Healthy’ (n = 1957; 12.7%); (ii) ‘High sugary beverage consumption’ (n = 8482; 55.2%); (iii) ‘Poor mental wellbeing’ (n = 2009; 13.1%); (iv) ‘Smoker’ (n = 1364; 8.9%); and (v) ‘Alcohol drinker’ (n = 1554; 10.1%). Being female (OR 1.28, 95%CI 1.14, 1.45) and being physically inactive (OR 1.20, 95%CI 1.04, 1.39) increased the odds of belonging to the ‘High sugary beverage consumption’ cluster. Being female (OR 1.21, 95%CI 1.04, 1.41), non-membership in a sports club (OR 1.83, 95%CI 1.43, 2.34) were associated with ‘Poor mental wellbeing’. Obesity (OR 2.03, 95%CI 1.47, 2.80), inactively commuting to campus (OR 1.34, 95%CI 1.09, 1.66), and living in high-rise accommodation (OR 2.94, 95%CI 1.07, 8.07) were associated with membership in the ‘Smoker’ cluster. Students living in The Philippines, Singapore, Thailand, and Vietnam had a higher likelihood of being alcohol drinkers, compared with those who lived in Brunei. Conclusions ASEAN university students exhibited health-risk behaviours that typically clustered around a specific health behaviour and mental wellbeing. The results provided support for focusing interventions on one dominant health-risk behaviour, with associated health-risk behaviours within clusters being potential mediators for consideration.

Objective This study aims to assess the effect of home modification in preventing falls in older adults. Methods A systematic review and meta-analysis of randomized studies were performed. The review was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines and was registered prospectively. Five electronic databases were systematically searched for related articles. The titles and abstracts of the articles found using the key search phrases—home modification and falling—were screened using inclusion and exclusion criteria. The Cochrane risk of bias tool was used to evaluate the studies’ methodology. Results A total of 12 trials were included. A meta-analysis was conducted using 10 studies with n = 1, 960 participants showing a clinically meaningful 7% reduction in falls (risk ratio = 0.93; 0.87–1). Conclusions Falls can be significantly reduced with the use of home modification interventions that are thorough, well-focused, have an environmental-fit perspective, and have adequate follow-up.

Abstract Background This study explored the user experiences of paediatric postgraduate trainees in Malaysia and Thailand in using a 2 h and 15 min online module for breastfeeding developed for Southeast Asia, which was adapted from existing European online modules for European and German Continuing Medical Education (CME) credits. Methods A qualitative study using focus group discussions (FGDs) was conducted with paediatric postgraduate trainees who used an online English-language breastfeeding module in two Thai universities (May 2020, done online) and two Malaysian universities (Sept- Nov 2019, in-person). FGDs explored module usability and utility. Sessions were transcribed verbatim and analysed thematically. The process of coding was done collaboratively by Thai and Malaysian researchers. Results Twenty Six resident trainees participated (Thai, n = 13; Malaysian, n = 13). Ages ranged from 29–34 years old, with 21 females. Nineteen participants had never used online learning modules prior to this. Participants took between 1 to 5 sessions to complete the breastfeeding module. Four themes emerged from their experience. 1) The online learning module was more engaging and detailed than previous lectures, courses and/or books, but lacked hands-on training. 2) Using an online platform facilitated learning as eased navigation and resource searching, however, problems were encountered navigating the module on some devices. 3) Learners preferred less words and more graphics, as this helped them capture key messages. 4) Regionally tailored content elicited a mixed reaction from participants. Conclusions Users found that the adapted module compared favourably with previous learning experiences. However, online learning modules lack hands-on training, and implementation should ideally incorporate a mix of both. Consideration of device diversity and preferences for how content was adapted for local settings are needed for tailoring.

This research investigated the overall and domain-specific physical activity (PA) of university students in the coronavirus disease 2019 (COVID-19) pandemic. A cross-sectional study was applied to socioeconomic (SE) and PA online data collected from 15,366 students across 17 universities in seven Association of Southeast Asian Nations (ASEAN) countries. Statistical analyses using logistic regressions established SE–PA relationships. Over half (60.3%) of ASEAN university students met age-span specific PA guidelines. Students participated in recreational PA the most, followed by study-related activities and 44.1% of students engaged in >8 hrs/day of sedentary time (ST). Compared to students with a normal body mass index (BMI), students who were underweight (UW), overweight (OW), and obese (OB) respectively, had a 14% (UW odds ratio (OR) = 1.14, p = 0.005), 25% (OW OR = 1.25, p < 0.001), and 24% (OB OR = 1.24, p = 0.005) greater probability of meeting PA guidelines. Those who engaged in active transport and belonged to a sports club (SC) had 42% (SC OR = 0.58, p < 0.001, for both) less probability of meeting the PA guidelines, compared with those who travelled inactively and did not belong to a sports club, respectively. Students who participated in 4–6 sport or exercise activities had ten times more likelihood of meeting PA guidelines (OR = 10.15, p < 0.001), compared with those who did not play any sport or do any exercise. Students who spent >8 hrs/day of ST had 32% (ST OR = 0.68, p < 0.001) less probability of meeting PA guidelines, compared with those who spent

Health promotion and disease prevention (P&P) are essential components of primary health care. This study investigated the coverage of P&P services and barriers to services among primary care units in Thailand before and during the coronavirus disease 2019 (COVID-19) pandemic. A web-based cross-sectional survey was conducted to compare the data from primary care units across the 13 health regions in two fiscal years: October 2018 to September 2019 (before the pandemic) and October 2019 to September 2020 (during the pandemic). A total of 340 primary care units responded to the questionnaire. While most participating primary care units provided basic P&P services (n = 327, 96.2%) and community-based P&P services (n = 244, 71.8%), fewer offered area-based P&P services (n = 120, 35.3%) for all target populations. The high coverage of basic P&P services remained in place during the pandemic, while coverage of community-based P&P services for vulnerable and at-risk populations improved during the pandemic. Area-based P&P services improved for pregnant and postpartum women, preschoolers, children and adolescents, adults, and older people. Lack of human resources, materials and equipment, and financial support were cited as the primary challenges to offering P&P services. The higher coverage of P&P services in several target populations during the pandemic contributed to a heavy workload. Effective resource allocation, capacity building, and support from relevant parties, such as government and local agencies, are required to maintain high P&P service coverage.

The new World Health Organization (WHO) 2020 guidelines on physical activity (PA) and sedentary behavior include recommendations for adults with chronic conditions. The guidelines provide adaptable and general recommendations for people living with chronic medical conditions. This article summarizes the content and provides suggestions for the application of the guidelines for patients with essential hypertension in primary care. The WHO 2020 PA guidelines recommend broad advice for adults and older adults with chronic conditions. The key recommendations are consistent with other hypertension guidelines. A systemic approach to promote PA in primary care (i.e., PA assessment, safety considerations, PA prescription, behavioral counseling, and referral) along with applying the WHO guidelines is required. Health risk assessment and safety issues related to hypertension (e.g., current PA levels, level of blood pressure, treatment plans, comorbidities) should be concerned. The FITT Pro (frequency, intensity, time, type, and progression) can be adopted as a framework to break down the guidelines into specific PA prescription. The WHO 2020 PA guidelines address the importance of PA in clinical populations. The guidelines can be adapted for patients with hypertension in primary care settings.

Abstract Background Physical activity (PA) counselling is an effective approach to promote PA in primary health care (PHC). Barriers to PA counselling in PHC include time constraints, lack of knowledge and skills of providers, and systemic barriers. Using electronic health (eHealth) has the potential to promote PA. This scoping review aimed to identify usability and utility of eHealth for tailored PA counselling introduced in PHC settings. Methods A scoping review included primary research articles. The authors systematically searched six databases (Cochrane Library, CINAHL Complete, Embase, PubMed, Scopus and Web of Science) from the inception of the databases. The search terms consisted of three search components: intervention (PA counselling), platform (eHealth), and setting (PHC). Additional articles were included through reference lists. The inclusion criteria were research or original articles with any study designs in adult participants. Results Of 2501 articles after duplicate removal, 2471 articles were excluded based on the title and abstract screening and full text review. A total of 30 articles were included for synthesis. The eHealth tools had a wide range of counselling domains as a stand-alone PA domain and multiple health behaviours. The included articles presented mixed findings of usability and utility of eHealth for PA counselling among patients and providers in PHC settings. Technical problems and the complexity of the programmes were highlighted as barriers to usability. The majority of articles reported effective utility, however, several articles stated unfavourable outcomes. Conclusions eHealth has the potential to support PA counselling in PHC. Facilitators and barriers to eHealth usability should be considered and adapted to particular settings and contexts. The utility of eHealth for promoting PA among patients should be based on the pragmatic basis to optimise resources.

Abstract Background Mobile health applications (mHealth apps) are increasingly being used to perform tasks that are conventionally performed by general practitioners (GPs), such as those involved in promoting health, preventing disease, diagnosis, treatment, monitoring, and support for health services. This raises an important question: can mobile apps replace GPs? This study aimed to systematically search for and identify mobile apps that can perform GP tasks. Methods A scoping review was carried out. The Google Play Store and Apple App Store were searched for mobile apps, using search terms derived from the UK Royal College of General Practitioners (RCGP) guideline on GPs’ core capabilities and competencies. A manual search was also performed to identify additional apps. Results The final analysis included 17 apps from the Google Play Store and Apple App Store, and 21 apps identified by the manual search. mHealth apps were found to have the potential to replace GPs for tasks such as recording medical history and making diagnoses; performing some physical examinations; supporting clinical decision making and management; assisting in urgent, long-term, and disease-specific care; and health promotion. In contrast, mHealth apps were unable to perform medical procedures, appropriately utilise other professionals, and coordinate a team-based approach. Conclusions This scoping review highlights the functions of mHealth apps that can potentially replace GP tasks. Future research should focus on assessing the performance and quality of mHealth apps in comparison with that of real doctors.

Abstract Background Physical activity (PA) counselling is an intervention to promote PA among patients in primary care, yet it remains limited in this clinical setting. This study aimed to explore PA counselling practices among general practitioners (GPs), as well as barriers to and recommendations for improving PA counselling. Methods This qualitative study employed a descriptive approach. Data were collected by in-depth interviews and analysed by thematic analysis. The study was conducted in district hospitals in Nakhon Si Thammarat, Thailand, from February 2017 to February 2018. The study participants were GPs who worked at district hospitals. Results Seventeen GPs (6 males and 11 females, mean age 29.8 ± 3.4 years) from 6 district hospitals were interviewed. Their clinical experience ranged from 2 to 12 years (mean 4.7 ± 2.9). The GPs saw 30–80 outpatients/day (mean 56.2 ± 14.1) and spent 1–8 min (mean 3.8 ± 1.8) with each patient. They emphasised PA in patients with poorly controlled non-communicable diseases (NCDs) using the word ‘exercise’ in the Thai language as well as discussing time and frequency of exercise. PA was considered a non-pharmacological treatment in the management of NCDs, which improved patient health and quality of life. Barriers to PA counselling among GPs included time constraints, insufficient knowledge, and lack of opportunities to follow-up previously counselled patients. GPs suggested that training in PA counselling was required. Conclusions GPs concurred that PA counselling is essential in the treatment of NCDs. Physicians’ recommendations to improve the quality of PA counselling in primary care include 3 Ts: training in PA counselling, tools for prescribing PA, and teams of healthcare professionals. Implementing use of written PA prescriptions may encourage patients to increase PA. Multidisciplinary teams should be developed to support PA counselling in the clinical setting. Further studies should focus on appropriate techniques to implement PA counselling and overcome existing barriers.

Coronavirus disease 2019 (COVID-19) has been reported to affect malaria intervention strategies, the suspension of malaria elimination programs, and the publication of malaria research. We compared differences in authorship, affiliations, countries, funding sources, article types, keywords, languages, and citations between studies published before and during the COVID-19 pandemic. The searches were performed online using the Scopus database on 8 April 2022. The searches were limited to two periods: before the COVID-19 pandemic (2018–2019) and during the COVID-19 pandemic (2020–2021). The information of authorship, affiliations, countries, funding sources, article types, keywords, languages, and citations between studies published before and during the COVID-19 pandemic were compared using frequency and percentage. The relationships between the most productive authors, countries, affiliations, journals, and frequently used keywords were visualized using the VOSviewer (version 1.6.18) software. A total of 2965 articles were identified in two periods and, among those, 1291 relevant studies were included. There was no difference in malaria publications before and during the COVID-19 pandemic (679 articles, 52.6% vs. 612 articles, 47.4%). Compared between the two periods, the preliminary trend of malaria publications in terms of authorship, affiliations, countries, funding sources, article types, keywords, languages, and citations were different. In conclusion, the current study showed the preliminary trends in malaria publications before and during the COVID-19 pandemic. The findings of this study would encourage researchers to perform a scoping review or systematic review to better understand the direction of malaria publications during the COVID-19 pandemic.

Increased visceral fat is associated with an increased mortality rate. Bioelectrical impedance analysis (BIA) is a noninvasive method to assess visceral fat that is easily accessible and avoids exposure to radiation. It is unknown how a visceral fat rating from a noninvasive machine correlates with the serum lipid profile and fasting blood sugar (FBS). The aim of this research is to study the correlation of the visceral fat rating obtained by a noninvasive method with the serum lipid profile and FBS. This cross-sectional study involved 90 obese adults, ranging in age from 18 to 60 years old. The visceral fat rating was measured by BIA. The results demonstrated that significant positive correlations were found between the serum triglycerides and visceral fat rating (r = 0.287, P = 0.006) and between the FBS and visceral fat rating (r = 0.210, P = 0.047). There was a negative correlation between the serum high-density lipoprotein (HDL) cholesterol and visceral fat rating (r = -0.322, P = 0.002). In conclusion, statistically significant positive correlations were found between the serum triglycerides and visceral fat rating and between the FBS and visceral fat rating, and a negative correlation was found between the serum HDL-cholesterol and visceral fat rating.

Abstract Background Data systems for surveillance and monitoring are essential to develop understanding of the levels of physical activity (PA) occurring at the population levels. To comprehensively understand PA in medical schools, a suitable surveillance tool might be useful to present and monitor key PA-related metrics. This study investigated PA-related metrics in a Thai medical school and summarised the findings using a newly developed tool-the Medical School Physical Activity Report Card (MSPARC). Methods A mixed methods study was carried out at three campuses of a medical school in Southern Thailand. Data analysis included descriptive statistics and qualitative reviews. All 285 medical students from a medical school were the target population. The prevalence of PA (≥ 150 min/week of moderate- to vigorous-intensity PA) and sedentary behaviours (SB) (≥ 8 h/day of sedentary time) among medical students were analysed using data from a self-administered questionnaire. Usage patterns, quality, and accessibility of walkable neighbourhoods; bicycle facilities; and recreational areas were assessed. PA promotion programmes, education, and investment related to PA promotion were reviewed from the school documents. Results Of 279 participants, 138 (49.5%) met PA recommendation, but 71.7% met criteria for SB. Male students were more active (61.8 vs. 42.4%) and less sedentary (65.7 vs. 75.1%) than female students. Bicycle facilities were rated as having the lowest quality and accessibility among PA-related facilities. Most PA promotion programmes were sports clubs and sport competitions. A total of 25 h of PA education was taught throughout the entire curriculum, which provided minimal PA counselling training. The school invested 2136.14 Baht/student/year (US $64.34) in PA promotion, or 2.4% of the annual tuition. The MSPARC presented the summary of the findings by using simple symbols, infographics, and short texts. Conclusions To increase PA and decrease SB among medical students, there is a need to improve the quality and accessibility of the built environment as well as the natural environment, so as to establish health promoting policies. PA counselling training is required to develop the medical students’ essential skills and awareness for future practices. Monitoring and subsequent surveillance of PA in medical school are needed.

Abstract Background Physical inactivity is a global public health challenge. Physical activity (PA) promotion in healthcare delivery systems is effective to reduce physical inactivity. A primary care setting provides an appropriate environment for PA counseling since it is a primary contact with primary care or family physicians encounter the majority of the population. Lack of knowledge and inadequate training in PA counseling is one of the most important barriers to PA promotion. The purpose of this systematic review was to evaluate PA counseling training in primary care residency programs. Methods The authors systematically searched PubMed, Web of Science, Scopus and The Cochrane Library for articles published in English from 2000 to 2017. Articles regarding PA counseling in primary care residency training were extracted and outcomes assessed for this systematic review. Results Based on the initial review, 378 articles were excluded (362 articles excluded based on titles and abstracts and 16 articles excluded based on full texts). Four articles were included in this review, addressed PA counseling curricula in primary care residency training. All studies included PA counseling training as part of obesity and healthy lifestyle training. The training improved knowledge among primary care residents, but may not necessarily result in better attitudes or self-efficacy, which could be improved by elective rotations that focus on improved attitudes, self-efficacy, and professional norms for PA counseling. Brief training in counseling did not improve quality nor increase the rate of counseling. Conclusions This systematic review demonstrates a lack of evidence due to a small number of included studies. The heterogeneous outcomes from the minimal programs are needed to carefully interpret. However, this review sheds light on the importance of training in PA counseling in primary care residency programs. The development of training in PA counseling should focus on an approach that improves attitudes and the self-efficacy of primary care residents. Elective rotations, where residents voluntarily choose their subject, may provide the appropriate training period for PA counseling. Policymakers and academics should play an active role in the implementation of PA counseling as an essential competency for primary care physicians.

Objective: Medical Resources Integration and Optimization (MRIO) is a teaching method, for integration and optimization of the knowledge and experience of both learners and teachers to promote active learning by using a set of several learning activities, beneficial for institutions with limited instructors. This study aims to evaluate the effectiveness of this program. Methods: This is a mixed-method study design to determine medical student’s learning outcomes regarding three domains of knowledge, attitudes and skills including comments of students and teacher team staffing toward MRIO. It was operated by family medicine for fourth year medical students in January 2016 at Medical Education Center, Ratchaburi Hospital. Results: A total of 33 medical students participated in the MRIO. All students passed the minimum passing level of knowledge and skills with the scores of 74.5±8.6% and 94.1±1.9%, respectively. Attitudes toward understanding patients and care improvement of patients were identified. The strengths of this program were inter-professional education and effective learning. Conclusion: MRIO was an effective teaching program to integrate and optimize with limited resources in undergraduate family medicine practice. The implementation of this program in different settings may need collaborative teams to maximize the effectiveness of the program.

Abstract Background Reducing physical inactivity among the population is a challenge for many nations. Targeting leisure time physical activity (LTPA) may be useful in increasing overall physical activity as it is assumed it is associated with a higher degree of free choice and personal preference than physical activity at work and during travel. The study explored the prevalence of physical inactivity and focused on the overall level of energy expenditure and energy level spent during leisure time among those who were physically inactive and assessed the stages of change for LTPA among those who were physically inactive. Methods A population-based survey was conducted in 2014 in Chiang Mai, Thailand using a stratified two-stage cluster sampling technique. The Global Physical Activity Questionnaire (GPAQ) was used to collect the data on physical activity. Sufficient levels of physical activity (PA) were defined as ≥150 min/week of moderate-intensity PA or ≥75 min/week of vigorous-intensity PA or ≥600 metabolic equivalent of task (MET)-minutes/week. Weighted analyses were used to estimate the prevalence of physical inactivity, the total energy expenditure and expenditure during LTPA as well as stages of change among the physically inactive population. Results A total of 1744 people (808 men and 936 women), aged 15 to 64 years, participated in the study. We estimated that a quarter (26%) of the population were physically inactive. Physical inactivity was more commonly found among women than men in most age groups. LTPA contributed a small proportion of overall PA. On average, physically inactive men spent 132.8 MET-minutes/week and inactive women spent 208.2 MET-minutes/week in overall PA which is well below the 600 MET-minutes/week recommend by the World Health Organization. Around 75% of physically inactive people had no intention of engaging in regular LTPA. Conclusion About a quarter of the investigative population were physically inactive. Most physically inactive members of the population participate in low levels of LTPA, but the majority has no intention of increasing PA during leisure time. A large-scale health promotion program is needed, and it should focus on an approach for the pre-contemplated population.

Objectives To identify the essential content and approaches for developing a training programme in physical activity (PA) counselling for undergraduate medical curricula.Design A three-round Delphi survey was conducted to investigate four key topics: (1) contents of PA counselling in medical education; (2) teaching and learning methods; (3) medical school collaboration and (4) educational policy implementation. Round 1 collected opinions from the participants. Round 2 focused on scoring the opinions. Round 3 summarised the expert opinions. A mean score of 4 or above identified as an important item.Setting All 23 medical schools in Thailand.Participants Academic staff who were experts or in charge of medical schools in the fields of PA, health promotion or medical education.Results A total of 20 representatives from 18 of the 23 Thai medical schools participated in the study (for a response rate of 78.2%). The top three most important indicators of knowledge were (1) the definition and types of PA (4.75±0.55), (2) the FITT principle (frequency, intensity, time and type) (4.75±0.55) and (3) the benefits of PA (4.65±0.67). The most important component of the training involved general communication skills (4.55±0.60). An extracurricular module (4.05±0.76) was preferable to an intracurricular module (3.95±0.94). Collaborations with medical education centres and teaching hospitals (4.45±0.78) and supporting policies to increase medical students’ PA (4.40±0.73) were considered to be important.Conclusion Knowledge and counselling skills are important for PA counselling. Building collaborations between medical education and health institutions, as well as implementing effective educational policies, are key approaches to the integration of PA counselling into medical education. Future research should focus on investigating the effects of training in PA counselling on the learning outcomes of medical students and the clinical outcomes of patients.

Physical activity is one of the key factors for treatment and prevention of noncommunicable diseases. Recently, many physical activity guidelines have been established. This article includes seven current guidelines each one having some different detail. According to the guidelines, infants should be encouraged to do floor-based play. Toddlers and preschool children should do at least 180 minutes per day of physical activity at any intensity. Children (from 5 years old) and youths should do at least 60 minutes per day of moderate- and vigorous-intensity physical activity and at least 3 days in a week of bone and muscle strengthening activities. Adults should do at least 150 minutes per week of aerobic moderate-intensity physical activity and include at least 2 times per week of major muscle groups strengthening activities. Older adults should do the similar physical activity programme as adults, in terms of duration of aerobic and strengthening activities, and also combine at least 3 times a week of balance training for people with high risk of falling.

Pokémon GO becomes the most rapidly downloaded mobile application in history. This study aimed to determine the physical activity of medical students, who played Pokémon GO, and the change in their use of Pokémon GO and physical activity over time. An observational study was conducted. Physical activity was measured by using self-administered questionnaires at baseline (phase 0), 1 month (phase 1) and 3 months (phase 2) post-Pokémon GO download. The changes in physical activity (phase 0 to 1 and phase 1 to 2) were analysed using Wilcoxon Signed Ranked test. The trend (3-point analysis) of physical activity from phase 0, 1 to 2 were analysed using Friedman's test. The relationship between physical activity and time spent gaming was analysed by using Spearman's rank correlation. Twenty-six participants (mean age 22.04±1.70 years) participated in the study. There was no statistically significant change in physical activity during the three-month period (p = 0.45). Only 11 participants (42.3%) were still playing Pokémon GO 3 months after download. The key reasons for playing game were 'have fun' and 'pass time/boredom'. The most common commuting mode to play the game was walking; some drove a car or motorcycle while playing the game. There was no correlation between physical activity and time spent gaming. This study highlights how the lack of sustainability of the game and the motivation behind using Pokémon GO as a game rather than a physical activity app may have undermined the potential of using the game to improve physical activity. Further studies need to explore the reasons for the lack of sustainability and how to combine fun with behavioural change.

Cancer survivors have suffered from the diseases and are a unique medical population with a wide variety of treatment options that put them at risk for both short- and long-term side effects. Physical activity is one of the healthy behaviours which can decrease suffering and improve health quality. This article reviews the evidence-based data about the benefits of physical activity, a physical activity programme, and precautions and special considerations of physical activity among adult cancer survivors. The current evidence supports the view that cancer survivors get physiological and psychological benefits from physical activity. Aerobic and resistance training activities are main components of physical activity for cancer survivors during and after treatment. However, cancer survivors need cancer-specific approaches and individualised physical activity or exercise programmes to decrease the risk of injuries and complications. In summary, cancer survivors should do enough physical activity and avoid inactivity, depending on their health conditions. Most studies in this field have been done in breast, prostate, colon, haematologic, and gynaecologic cancer survivors. The knowledge in other types of cancers is still required.
