Cutaneous polyarteritis nodosa (CPAN) is a comparatively rare form of vasculitis that affects small arteries and arterioles in the panniculus and dermo-subcutaneous junction. Limited information is available regarding its course in the European population. The aim of this study is to characterize the manifestations and prognostic markers of recurrence in CPAN.

Background: The management of HIV-positive patients with psoriasis is controversial and limited to individual cases or short series of patients. Objectives: To evaluate the safety and effectiveness of conventional and biologic immunosuppressive drugs in the treatment of patients with psoriasis and concomitant HIV infection. Methods: A retrospective multicenter study was conducted. The study included data from 2008 to 2016. Inclusion criteria were: HIV adult patients with moderate-to-severe psoriasis, HIV viral load determinations at baseline and at least after 6 months of treatment, and systemic immunosuppressive treatment for at least 6 months. A descriptive analysis was performed. Results: Twenty-three patients with plaque-type psoriasis and HIV infection (five with AIDS) were included. Median follow-up time was 3.2 years. The main drugs used were etanercept, methotrexate, and ustekinumab. In most cases, viral load and CD4 cell count not only remained stable but also improved throughout the follow-up. Six patients presented severe adverse events during the follow-up, four of them in the AIDS stage. At the end of the follow-up period, 76.5% of the patients had achieved a PASI 75. Conclusion: Biologic drugs, both anti-TNF alpha agents and ustekinumab, seem to have an acceptable safety profile and high effectiveness in HIV-positive patients.

In several areas like Global Optimization using branch-and-bound methods, the unit n-simplex is refined by bisecting the longest edge such that a binary search tree appears. This process generates simplices belonging to different shape classes. Having less simplex shapes facilitates the prediction of the further workload from a node in the binary tree, because the same shape leads to the same sub-tree. Irregular sub-simplices generated in the refinement process may have more than one longest edge when n\geqslant 3. The question is how to choose the longest edge to be bisected such that the number of shape classes is as small as possible. We develop a Branch-and-Bound (B&B) algorithm to find the minimum number of classes in the refinement process. The developed B&B algorithm provides a minimum number of eight classes for a regular 3-simplex. Due to the high computational cost of solving this combinatorial problem, future research focuses on using high performance computing to derive the minimum number of shapes in higher dimensions.

OleB' is the first half of an oleandomycin ABC transporter of Streptomyces antibioticus. This protein contains a conserved Lys in the well known 'Walker A' motif. For several ABC transporters it has been shown that mutation of this amino acid alters the transport process indicating that this amino acid has a specific role in at least one of the different phenomena regulating transport (ATP binding, hydrolysis, structural modification or substrate binding). In this work we observed that substitution of Lys 71 of Walker A motif by a Gly does not change drastically the ability of ATP to bind to OleB' as observed by the extrinsic fluorescence of TNP-ATP and by competition between TNP-ATP and ATP for the binding site.

Three series of tests on a total number of 24 reinforced concrete columns were performed by the authors at Instituto Técnico de Materiales y Construcciones (INTEMAC) with the purpose of studying the influence of different kinds of bore fills from drilled core test-pieces made in the columns. The conclusions obtained make it possible to claim that column strength will be completely recovered either by coating bore surfaces with resin followed by concrete filling or by filling of bore with concrete and expansion agent. The increase in column load while concrete filling is still young does not affect its final load bearing capacity. Core drilling and filling of columns under an applied load of 50% of ultimate load does not affect final strength either. Los autores han realizado en el Instituto Técnico de Materiales y Construcciones CINTEMAC) tres series de ensayos con un total de 24 pilares de hormigón armado, estudiando la influencia de distintos tipos de relleno de los taladros de probetas-testigo efectuados en ellos. Las conclusiones obtenidas permiten establecer que la resistencia de los pilares se recupera íntegramente al rellenar, tanto con tratamiento de resina en las paredes del taladro y relleno de hormigón, como si el relleno se realiza con hormigón y un agente expansivo. El hecho de que se incremente la carga del pilar, cuando el hormigón de relleno es joven, no altera fa capacidad final de la pieza. Tampoco el que el taladro y posterior relleno se realice en pilares sometidos al 50% de su carga de rotura afecta a la resistencia final.

The vast amount in complexity of data generated in Genomic Research implies that new dedicated and powerful computational tools need to be developed to meet their analysis requirements. Blast2GO (B2G) is a bioinformatics tool for Gene Ontology-based DNA or protein sequence annotation and function-based data mining. The application has been developed with the aim of affering an easy-to-use tool for functional genomics research. Typical B2G users are middle size genomics labs carrying out sequencing, ETS and microarray projects, handling datasets up to several thousand sequences. In the current version of B2G. The power and analytical potential of both annotation and function data-mining is somehow restricted to the computational power behind each particular installation. In order to be able to offer the possibility of an enhanced computational capacity within this bioinformatics application, a Grid component is being developed. A prototype has been conceived for the particular problem of speeding up the Blast searches to obtain fast results for large datasets. Many efforts have been done in the literature concerning the speeding up of Blast searches, but few of them deal with the use of large heterogeneous production Grid Infrastructures. These are the infrastructures that could reach the largest number of resources and the best load balancing for data access. The Grid Service under development will analyse requests based on the number of sequences, splitting them accordingly to the available resources. Lower-level computation will be performed through MPIBLAST. The software architecture is based on the WSRF standard.

INTRODUÇÃO Em Portugal, nas décadas de 40 a 60, o parto era considerado como um “ato doméstico” realizado com ajuda de mulheres experientes na área. A falta de assistência especializada à gravidez e ao parto revelava-se nas elevadas taxas de mortalidade materna e neonatal, verificadas na época. O parto era um acontecimento que envolvia vários procedimentos e utensílios rudimentares e a ação de mulheres leigas designadas de parteiras. A assistência no parto sofreu melhorias até aos dias de hoje, melhorando também a prestação de cuidados na área de saúde materna, obstétrica e neonatal. Em parceria com a evolução científica, atualmente o parto no domicílio está novamente a ser equacionada sobretudo por mulheres e profissionais defensores do humanismo no parto. OBJETIVO Explorar a evidência científica e as vivências do parto no domicílio nas décadas de 40 a 60 do século XX em Portugal. MÉTODOS Revisão integrativa da literatura segundo o método PICOD e estudo exploratório realizado com 55 mulheres portuguesas com idades compreendidas entre 72 e 97 anos que viveram a experiência do parto no domicílio nas décadas de 40 a 60 do século XX em Portugal. RESULTADOS Os resultados revelaram que na década de 40 a 60, as mulheres eram assistidas maioritariamente por parteiras que utilizavam procedimentos muito rudimentares. As posições de preferência foram a ginecológica e a lateral, e a maioria ficou com sequelas devido às condições de assistência precárias, destacando-se a deformação da vagina e a incontinência urinária. CONCLUSÕES A evolução da arte de partejar foi uma mais-valia para os dias de hoje, contribuindo para uma diminuição significativa das taxas de mortalidade materna e perinatal e para um aumento da qualidade dos cuidados prestados à parturiente e ao recémnascido. É imprescindível que os profissionais de saúde reflitam sobre as vantagens e desvantagens de cada tipo de parto e sobre as condições mais humanas e seguras para o nascimento. As políticas de saúde devem ser centradas na Mulher e devem garantir a possibilidade de escolha no acesso ao tipo de cuidados na assistência pré e pós-natal e ao local de parto. As mulheres devem ter a possibilidade de optar pelo tipo de parto e local de parto que desejam, por isso, o parto domiciliar planeado não deve ser descurado, mas considerado como um direito e uma possibilidade de escolha pela mulher e sua família. É, no entanto, necessário que as políticas de saúde invistam nesta dimensão da prestação de cuidados obstétricos, garantindo também no domicílio assistência perinatal segura e humanizada, baseada na melhor evidência científica / recomendações. info:eu-repo/semantics/publishedVersion

Background Leishmaniasis is a neglected disease caused by different species of the protozoa Leishmania spp. Cutaneous lesions are the most common clinical manifestation. This disease is prevalent in tropical and subtropical areas, including the Mediterranean basin. In Spain, Leishmania (L.) infantum is the only endemic species, but imported cases are often diagnosed. Different classical parasitological methods can be performed for cutaneous leishmaniasis (CL) diagnosis; but currently molecular techniques serve as a relevant tool for the detection and characterization of Leishmania parasites. We aimed to evaluate clinical and epidemiological characteristics of CL diagnosed patients by real-time PCR in a tertiary hospital over a six-year period. Methodology/Principal findings Clinical, epidemiological and microbiological data were retrospectively collected and analyzed. In our study, CL was confirmed in 59 (31.4%) out of 188 patients by real-time PCR, showing an increase over recent years: 11 cases of CL between 2014 and 2016 and 48 between 2017 and 2019. Real-time PCR was performed on skin swabs and/or biopsies samples, with a positivity of 38.5% and 26.5%, respectively. Results were 100% concordant when biopsy and skin swab were performed simultaneously. L. (L.) infantum was the most frequent species detected (50%), followed by L. (L.) major (45%) and Viannia subgenus (5%), which were detected only in imported cases. L. (L.) major was almost entirely detected in travelers/migrants from Morocco. Multiple and atypical skin lesions were more common in imported cases than in autochthonous cases (44.4% vs. 21.8%). Conclusions/Significance An increase in both autochthonous and imported CL cases has been observed in past years in our hospital. Molecular techniques assist in improving CL diagnosis and characterization of the Leishmania species, mainly in imported cases., Author summary Leishmaniasis is a neglected disease caused by different species of the genus Leishmania. (L) infantum, although there are cases of imported leishmaniasis due to travel and migration of people from other endemic areas. Cutaneous lesions are the most common clinical form, but L. (L) infantum can also cause mucosal or visceral involvement. Although microscopic diagnosis and skin biopsy specimens are still used in routine diagnosis for cutaneous leishmaniasis, they are time-consuming and invasive, respectively. Currently, molecular techniques, such as real-time PCR, are very sensitive tools for the detection and identification of Leishmania parasites. In addition, the skin swab sample seems to be a non-invasive alternative for the diagnosis of cutaneous leishmaniasis. In the present work, we describe the clinical characteristics of 59 patients diagnosed by real-time PCR of cutaneous leishmaniasis, as well as the molecular epidemiology of the causative species. We show that there has been an increase in cutaneous leishmaniasis cases, both autochthonous and imported cases, over the study period. The presence of Leishmania species other than L. (L) infantum is also highlighted.
