Many patients in psychiatric outpatient settings complain of insomnia. However, the causal relationship between insomnia and depressive symptoms in all mental illnesses remains unclear. Moreover, research on insomnia and the continuation of outpatient treatment is lacking. We hypothesize a high correlation between depression and insomnia among patients with diverse mental illnesses. Additionally, we posit that insomnia significantly influences the continuity of outpatient visits. To this end, we evaluated insomnia and depression symptoms in psychiatric patients both at their initial visit and one year later. We also examined factors related to insomnia at the outset and factors associated with the ongoing utilization of outpatient treatment.Patients and Methods: The participants of the study consisted of patients who made their first visit to the outpatient department of psychiatry and neurology at Showa University East Hospital between June 1, 2021, and March 31, 2023, and who continued attending the outpatient clinic for one year. Clinical characteristics were assessed using the Self-rating Depression Scale (SDS) and the Athens Insomnia Scale (AIS).Results: The study’s findings were collected from a cohort of 1106 patients and revealed that more than 70% experienced insomnia at the time of their initial visit. In total 137 patients continued to receive outpatient treatment for one year, and their AIS scores improved from 9 points to 5 points. A multivariate analysis revealed that the SDS items of depressed mood and insomnia were confounding factors influencing AIS improvement.Conclusion: Given that 70% of patients complained of insomnia at the time of their first visit and that sleep improved in many of the 12.4% of patients who continued to receive outpatient treatment for at least one year, the results suggest that sleep status is an important determinant of whether a patient continues to attend outpatient clinics.Keywords: insomnia, sleep disorder, depression, continuation rate of outpatient treatment

ObjectiveKnee osteoarthritis (OA) is predominantly characterized by pain with weight-bearing activities. Pain at rest also occurs but the mechanisms for this are not clear. We evaluated the relations of nociceptive signal alterations to weight-bearing and non-weight-bearing pain in knee OA.DesignWe used data from a NIH-funded longitudinal cohort of older adults with or at risk of knee OA. We evaluated quantitative sensory testing (QST) measures (pressure pain threshold (PPT) at patellae and the wrist; mechanical temporal summation (TS); conditioned pain modulation (CPM)). Each WOMAC pain question was dichotomized as having at least moderate pain, and we further categorized them as weight-bearing pain and non-weight-bearing pain. We evaluated the relation of QST measures to each pain outcome using logistic regression, adjusting for potential confounders.Results2,749 participants (5,479 knees) were included (mean age 64 ± 11, 57% female). Each SD unit decrease in patellar PPT was associated with greater odds of both weight-bearing pain (OR 1.51 (95% CI 1.27, 1.79)) and non-weight-bearing pain (OR 1.46 (1.20-1.77)), while wrist PPT was associated with greater odds of weight-bearing pain (OR 1.27 (1.15, 1.39)) but only with pain during sitting/lying (OR 1.20 (1.01, 1.43)). TS was significantly associated with greater odds of pain with walking and stairs (OR 1.11 (1.01, 1.23), 1.11 (1.03, 1.20), respectively). CPM was not associated with any pain outcomes.ConclusionsOur findings challenge the hypothesis that non-weight-bearing pain may reflect greater pain sensitization and/or inefficient CPM than weight-bearing pain in knee OA, suggesting other mechanisms are likely responsible.

A recent meeting focused on accelerated materials design and discovery examined user requirements for a general, collaborative, integrative, and on-demand materials research platform.

This study involves the development of the auxiliary stress approach for producing elastically-homogeneous lattice models of damage in geomaterials. The lattice models are based on random, three-dimensional assemblages of rigid-body-spring elements. Unlike conventional lattice or particle models, the elastic constants of a material (e.g., Young's modulus and Poisson's ratio) are represented properly in both global and local senses, without any need for calibration. The proposed approach is demonstrated and validated through analyses of homogeneous and heterogeneous systems under uni- and tri-axial loading conditions. Comparisons are made with analytical solutions and finite element results. Thereafter, the model is used to simulate a series of standard laboratory tests: (a) split-cylinder tests, and (b) uniaxial compressive tests of sedimentary rocks at the Horonobe Underground Research Laboratory in Hokkaido, Japan. Model inputs are based on physical quantities measured in the experiments. The simulation results agree well with the experimental results in terms of pre-peak stress-strain/displacement responses, strength measurements, and failure patterns.

The formation process of hexagonal boron nitride (hBN) monolayer sheets on single-crystalline ZrB2(0001) thin films grown on Si(111) wafers has been investigated by electron diffraction, scanning tunneling microscopy, and photoelectron spectroscopy. A two-step reaction was identified, resulting first in the formation of a silicon nitride layer by room temperature exposure of the silicene-terminated ZrB2 thin film surface to nitrogen radicals and then in the formation of an hBN monolayer replacing the silicon nitride layer through annealing at 900 °C. A large-scale moiré pattern and a clear dispersion of the π-electronic band provide evidence for the formation of an epitaxial hBN monolayer sheet directly on the diboride surface. The unique ability of the ZrB2(0001) surface, upon which both silicene and hBN monolayers can be formed, opens a way toward the integration of these two very different two-dimensional materials. This article provides research on age-related body composition and density in Japanese youth. The authors found that body density was lowest in 11-year-old males, fat percentage in males was highest at 11 and lowest at 14 years; highest body fat for females was at 16 years, and lowest at 11 years; only 6.8% of males and 3.1% of females were obese; variance of fat-free mass explained 60% of male variance and 26% of female variance.
