10. Globalisation and language policy in Tunisia : shifts in domains of use and linguistic attitudes

Abstract

This thesis builds on previous studies on the linguistic situation, attitudes and domains of language use in the Maghreb countries undertaken by Marley (2000, 2004, 2005, 2008) in Morocco, Benrabah (2007a, 2007b), in Algeria, and by Jabeur (1999) Daoud (2001, 2011a, 2011b) and Bahloul (2001) in Tunisia. It explores the shifts in the domains of language use, the differences in linguistic attitudes between generations in Tunisia and the role played by globalisation in this process. In order to investigate the ways in which domains of use and attitudes to English and French are shifting, a questionnaire was administered to a group of 100 teachers aged 40 to 60 and a group of 200 students aged 17 to 19. SPSS data analyses showed some statistically significant differences between the two groups. With respect to domains of use, the younger group uses significantly more English as a lingua franca, in chatting online, reading for pleasure, watching TV programmes and listening to songs. Teachers, by contrast, use significantly more French in activities such as reading and watching TV programmes. French remains the preferred foreign language of the older generation, but they believe it is threatened by English in Tunisia, whereas the younger generation preferred English. One main reason for these differences could be what the older generation consider to be the negative impact of globalisation on French and its positive impact on English. Interesting qualitative data were also extracted from the responses to a vox pop questionnaire submitted to 100 lay people in the street and from essays written by two groups of 25 students. These confirmed that the majority of Tunisians consider English to be the most useful foreign language in Tunisia and that it should be given more importance in academic settings due to its world status as an international lingua franca. This thesis also investigates language policy in Tunisia by analysing all relevant extracts from the speeches of Ben Ali, Tunisia’s ex-president, and interviews conducted with the three senior inspectors of the three main languages. Policy has promoted English over French in two ways: first, Arabic rather than French is now the vehicle for the teaching of the human and natural sciences in the Basic Education and, second, new measures in favour of English have simultaneously been taken in and outside academia. To conclude, the findings of this study contribute to knowledge in three ways: Firstly, by identifying differences in domains of language use and attitudes between generations in contemporary Tunisia, secondly, by scrutinizing the way students, teachers and lay people feel about French and English, the two main rival foreign languages in the country and, thirdly, by exploring the political discourse which influences the language situation both directly, through language policy, and indirectly, through hearts and minds.

Background: Hormonal changes in women, especially at menopausal transition, may have significant consequences on respiratory function. This issue in asthmatic patients is more frequent, more severe, and less controlled after menopause. Tunisian data regarding this issue are limited; therefore, we assessed the clinical and functional particularities of asthma at menopausal transition. Materials and Methods: This descriptive-analytical study was performed for two years (2016–2017) on 82 asthmatic women followed up in the pulmonology department of Charles Nicolle hospital of Tunis. According to hormonal status, two groups were defined: G1 (menopausal patients) and G2 (non-menopausal patients). Asthma control and severity of asthma as well as other variables including gender, age, body mass index (BMI), comorbidities, allergenic status, spirometry results, health care use in the past 12 months, and prescribed medications were evaluated. Results: The mean age of patients was 50 years and half of them (60%) were menopausal women. Allergy was the most common cause of asthma (82%). A mean of 2.3 exacerbations per patient was recorded. Abstract

Tracheobronchopathia osteochondroplastica is a rare idiopathic disease of the trachea and the main bronchi, characterized by multiple submucosal osteocartilaginous nodules. Although the etiology of tracheobronchopathia osteochondroplastica remains unknown, several theories have been proposed. We report a case of a 47-year-old non-smoker woman with wheezing dyspnea over the past two years, which was treated as asthma without improvement. Investigations, including chest computed tomography scan, fiberoptic bronchoscopy, and endobronchial biopsy, indicated tracheobronchial amyloid light-chain (AL) amyloidosis. Thirteen years later, she was admitted for cough and wheezing. The bronchoscopic examination demonstrated nodular lesions distributed along the cartilaginous rings of the lower portion of the trachea and the main bronchi. Endobronchial biopsy confirmed tracheobronchopathia osteochondroplastica. We found tracheobronchopathia osteochondroplastica to be the end stage of amyloidosis.

26. Caractéristiques de la tuberculose pulmonaire chez les sujets tabagiques

Introduction Le tabagisme et la tuberculose (TB) sont deux enjeux majeurs de sante publique au niveau mondial, en particulier dans les pays en voie de developpement. Le but de cette etude etait de determiner les particularites cliniques, radiologiques et evolutives de la tuberculose pulmonaire chez les sujets tabagiques. Methodes Il s’agit d’une etude retrospective et comparative portant sur des patients hospitalises pour une TB evolutive au service de pneumologie de l’hopital Charles Nicolle de Tunis entre janvier 2018 et aout 2020. Resultats Nous avons inclus 71 patients dont 84 % etaient de sexe masculin. Cinquante-cinq patients (77 %) etaient tabagiques avec une consommation moyenne de 33 ± 27 PA [5–120 PA]. Sept patients ont deja arrete de fumer. L’âge moyen etait de 40 ans chez les fumeurs vs 42 ans chez les non-fumeurs (p ˃ 0,05). Le delai moyen de consultation etait plus long chez les tabagiques (60 jours vs 39 jours, p = 0,02). les signes fonctionnels les plus frequents etaient l’alteration de l’etat general (94 % chez les tabagiques vs 87,5 % chez les non tabagiques), fievre (61 % vs 68 %), sueurs nocturnes (38 % vs 43 %), toux (25 % vs 43 %) et l’hemoptysie (23,6 % vs 12,5 %) p > 0,05. Les lesions radiologiques etaient plus etendues chez les tabagiques : 56 % vs 12 % (p = 0,03). La radiographie pulmonaire montrait plusieurs anomalies (infiltration, excavations, opacites, micronodules) et seules les excavations etaient significativement plus presentes chez les tabagiques : 53 % vs 20 % (p = 0,01). La confirmation de TB etait bacteriologique chez tous les patients. A deux mois du traitement, la bacilloscopie etait toujours positive chez 30 % des tabagiques vs 13 % des non tabagiques (p = 0,03) avec un retard de guerison. Les patients tabagiques etaient plus susceptible de garder des sequelles pulmonaires : 61 % vs 18 % (p = 0,01). Conclusion Le tabagisme influence la presentation et l’evolution de la tuberculose pulmonaire. De ce fait, le sevrage tabagique doit faire partie integrante de la prise en charge des patients atteints de TB pulmonaire.

Abstract

Pulmonary embolism (PE) is a frequent and serious disease. In the absence of pathognomonic clinical signs or symptoms diagnostic management lies in the evaluation of clinical pre-test probability followed by a laboratory or an imaging test. Methods: We report a series of one hundred fifty three patients admitted to the Pneumo-Phthisiology Department of Charles Nicolle hospital for confirmed PE. The Wells and revised Geneva scores were applied to evaluate the clinical probability of PE, and the computed tomography pulmonary arteriography was a gold standard. Results: The PE was confirmed in all our patients. The average age was 54±17, 25 years. Eighty nine men and sixty four women. the average total delay was 3,2 days. Deep vein thrombosis was associated with PE in 14 patients. Risk factors for thromboembolic disease were found in 70% of patients. Hypoxia was found in 40,7% of cases, hypocapnia in 56,7% and tachycardia in 53,3%. The average of D-Dimer was 2101. One patient with negative result had PE. The probability of PE according to Geneva score was low in 28,6% of cases, moderate in 60% and high in 11,4%. The probability of PE according to wells score was low, moderate and high in respectively 22,9%, 69% and 7,3% of cases. There was no significant difference between the Wells score and Geneva score. Pulmonary arterial hypertension was found in 6 cases. Conclusion: The assessment of clinical probability by simplified scores remains an essential prerequisite for the diagnostic strategy; although, these scores may sometimes underestimate the disease.
