Objective: To analyze the way of life of the unhoused people to enhance health care in the pandemic. Methods: A qualitative, interdisciplinary research, with participant observation and 24 interviews with the unhoused people. Empirical categories and bibliographic search on this population and COVID-19 guided simple actions aimed at care. Results: The group at greatest risk for COVID-19 use drugs compulsively; starves constantly; discontinues drug treatment for tuberculosis, HIV, and diabetes; has underdiagnosis of Depression; has difficulty sheltering and uses inhaled drugs. This way of life increases the risk of worsening COVID-19 and brings great challenges to health services. Several proposals to guide care considered these results and the new routine caused by the pandemic. Final considerations: The way of life of the studied population increased their vulnerability in the pandemic, as well as the perception of risk of disease transmission by the population in general. RESUMEN Objetivo: Analizar el modo de vida de la población en situación de calle (PSC) para potenciar el cuidado en salud en la pandemia. Métodos: Investigación cualitativa, interdisciplinaria, con observación participante, y 24 entrevistas con población en situación de calle. Categorías empíricas y búsqueda bibliográfica sobre dicha población y COVID-19 determinaron propuestas simples orientadas al cuidado. Resultados: El grupo más vulnerable a COVID-19 usa drogas compulsivamente, pasa hambre, interrumpe tratamiento médico para tuberculosis, VIH y diabetes; tiene subdiagnóstico de depresión, dificultad para hallar refugio y usa drogas inhalantes. Dicho modo de vida incrementa el riesgo de agravamiento del COVID-19, imponiendo mayores desafíos a los servicios de salud. Diversas propuestas de orientación del cuidado consideraron estos resultados y las nuevas rutinas impuestas por la pandemia. Consideraciones finales: El modo de vida de la población estudiada incrementó su vulnerabilidad en pandemia, así como la percepción de riesgo de transmisión de la enfermedad a la población en general. Resultados: O grupo de maior risco para COVID-19 faz uso compulsivo de drogas; passa fome constantemente; interrompe tratamento medicamentoso para Tuberculose, HIV e Diabetes; tem subdiagnóstico de Depressão; tem dificuldade de abrigamento; usa drogas inalatórias. Esse modo de vida aumenta risco de agravamento da COVID-19 e impõe maiores desafios aos serviços de saúde. Diversas propostas para orientar o cuidado levaram em conta esses resultados e a nova rotina impressa pela pandemia. 6. Desafios para a implementação do Programa de Planejamento Familiar em uma comunidade de baixa renda em Aracaju (Sergipe), Brasil

A study on the reproductive characteristics and the meaning of family planning for childbearing age women and for the health team providing care to them was conducted in a low-income community in Aracaju - SE. In the descriptive stage of the study structured questionnaires were applied to 90 users of a Primary Care Unit random-selected from the family files containing personal data and information about the reproductive life of the women. This stage was followed by a qualitative approach in focal groups with six users and the health team applying semi-structured questionnaires for assessing the practice of family planning. The study showed that knowledge and offer of contraceptive methods are no guarantee of proper family planning. More than 97% of the participants related knowing about the condom, the pill and the contraceptive injection. 73,6% of the sexually active women practiced contraception but 76% of those who already had had a pregnancy referred to one or more unintended gestations. The focal groups revealed the need for other approaches to family planning such as preconception care, access to the partners and education in health to be developed by the team in order to help the users to better plan their offspring. Um estudo das características reprodutivas e do significado do planejamento familiar para mulheres em idade fértil e para a equipe de saúde que as assiste foi realizado em uma comunidade de baixa renda em Aracaju (SE). Na etapa descritiva, foram aplicados questionários estruturados a 90 usuárias de uma Unidade Básica de Saúde, por sorteio aleatório das pastas-família, contendo dados pessoais e da vida reprodutiva. Esta etapa foi seguida de uma abordagem qualitativa, na qual foram realizados grupos focais com seis usuárias e com a equipe de saúde, utilizando questionários semi-estruturados para avaliar a prática de planejamento familiar. As mulheres mostraram que conhecimento e oferta de métodos anticoncepcionais não garantem um planejamento familiar adequado. Mais de 97% das participantes referiram conhecer a camisinha, a pílula e o injetável, e 73,6% das sexualmente ativas praticam anticoncepção, mas 76% daquelas que já engravidaram referiram uma ou mais gestações não-planejadas. Nos grupos focais, outras abordagens do planejamento familiar, como assistência à pré-concepção, acesso ao homem e educação em saúde, mostraram que precisam ser trabalhadas pela equipe para que as usuárias programem melhor sua prole. more...

- Abstract
Drugs have been consumed since ancient times, and their taboo has generated segregation and hindered the proper confrontation of harmful use. This study investigated the real and symbolic values of drug use for ill-housed people beyond pathologization, seeking to understand factors that hinder giving them up, even with intense losses and suffering. It also aimed to understand its implications for health care. This is a phenomenological qualitative research based on participant observation of ill-housed people care provided by the Street Clinic, interviews with ill-housed people and focus group with professionals from the Health Care Network in Rio de Janeiro city, Brazil, between March 2017 and July 2019. Results showed that "drug use" interacted with the research categories as follows: drugs are the main reason for living and staying on the streets; drugs motivate having financial activity; drugs cause abandonment of health treatment; drugs soothe and cause depression, suffering and hunger; drugs make them seek shelter and religion to "escape" using, but without adequate treatment hope leads to relapse and low self-esteem - which one want to get rid of, but few successes undermine hope. As expected, it is the reason for divergences among professionals regarding the type of care and support for ill-housed people. Given the results, how can we face abusive use that relieves hunger, loneliness, suffering and depression? How to take care of a use that generates hunger, depression, affective, health and self-esteem losses? A cyclical story told by people in situations of extreme vulnerability. Unveiling the symbolic plurality of drug use enables reflections and new ways of caring for ill-housed people. more...

8. Working conditions in primary health care in the COVID-19 pandemic: an overview of Brazil and Portugal.

- Abstract
The COVID-19 pandemic has put pressure on public health systems worldwide since 2020. This article aims to discuss working conditions in Primary Health Care (PHC) in Brazil, while dialoguing with primary health care (CSP, Cuidados de Saúde Primários) in Portugal in the pandemic scenario. For that purpose, data from the study "Working conditions of health professionals in the context of COVID-19 in Brazil" are presented, for further discussion with the report produced by "Family Health Units - National Association (USF-AN, Unidades de Saúde Familiar - Associação Nacional)" on the CSP in Portugal. In Brazil, regarding the sample of 3,895 PHC health professionals, it was observed: multiple employment relationships, lack of institutional support and specific training, living with fake news and the lack of political cohesion between health authorities, with significant changes in these workers' mental health. In Portugal, the increase in the workload and the presence of Burnout Syndrome among FHU professionals was emphasized. The pandemic had a significant impact on the health and daily work of health professionals in PHC and CSP. However, the Brazilian context was more adverse due to fake news and divergences regarding conducts related to the fight against the pandemic, due to the actions and denialism by the federal government. more...
