22. Adequação entre a terapia nutricional enteral prescrita e sua administração em pacientes críticos de um hospital geral

- Abstract
Introduction: Critically ill hospitalized patients tend to present high rates of weight loss, which, related to the clinical picture, is associated with increased occurrence of infections, morbidity and prolonged hospitalization. Objective: To identify the nutritional status and the caloric and protein adequacy of the enteral diet infused in patients in the intensive care unit. Materials and Methods: This is a cross-sectional, descriptive quantitative study, with participants of both genders, aged 18 years or older, using exclusive enteral nutritional therapy. Nutritional status was assessed by body mass index (BMI) and arm circumference (AC). To calculate energy and protein needs, the recommendations of the Brazilian enteral nutrition guidelines were considered. The adequacy was performed through daily collection of prescription and infusion information. For the analyses we applied tests considering statistical significance p<0.05. Results: 22 patients were evaluated, 68.18% were male. The main reasons for hospitalization were cardiovascular and pulmonary diseases, and the most predominant nutritional status according to the BMI was overweight in 55% of patients, while 50% were eutrophic. The calorie adequacy was adequate in 73% of patients, while protein adequacy was not adequate in 55%. The caloric and protein adequacy showed no association between the nutritional parameters and clinical outcome. Conclusion: The findings of this study allow us to affirm the importance of early nutritional assessment, as well as the adequate monitoring of nutritional support, aiming at the best clinical outcome of the patient., Introdução: Pacientes hospitalizado, em estado crítico tendem a apresentar altos índices de perda de peso, que relacionado ao quadro clínico, associa-se ao aumento de ocorrência de infecções, morbidade e tempo de internação prolongada. Objetivo: Identificar o estado nutricional e a adequação calórica e proteica da dieta enteral infundida em pacientes internados na unidade de tratamento intensivo. Materiais e Métodos: Trata-se de um estudo transversal, descritivo de natureza quantitativa, que contou com participantes de ambos os sexos, com idade igual ou superior a 18 anos, em uso de terapia nutricional enteral exclusiva. O estado nutricional foi avaliado pelo índice de massa corporal (IMC) e circunferência do braço (CB). Para cálculo das necessidades energéticas e proteicas, foram consideradas as recomendações das diretrizes brasileiras de nutrição enteral. A adequação foi realizada através de coleta diária das informações de prescrição e infusão. Para as análises foram aplicados testes considerando significância estatística p<0,05. Resultados: Foram avaliados 22 pacientes, destes 68,18% do sexo masculino. Os principais motivos de internação foram doenças cardiovasculares e pulmonares, e o estado nutricional segundo o IMC mais predominante foi excesso de peso em 55% dos pacientes, já quanto a CB 50% estavam eutroficos. A adequação calórica foi adequada em 73% dos pacientes, já adequação proteica não foi adequada em 55%. A adequação calórica e proteica não mostrou associação entre os parâmetros nutricionais e desfecho clínico. Conclusão: Os achados deste estudo permitem afirmar a importância da avaliação nutricional precoce, assim como o monitoramento adequado do suporte nutricional, visando o melhor desfecho clínico do paciente.

26. Engaging with destinations brands on social media: Self and social motivations and the mediating role of symbolic value

The competitiveness of the tourism industry has accelerated the need for branding tourist destinations, aiming to differentiate themselves. The symbolic nature and motivations (self and social) of travelling were already considered significant, but social media’s rise elevated them to a new level. This study investigates the mediating effect of perceived symbolic value of a destination in the relationship between self-congruity and social self-congruity with the destination and social media engagement. To analyse the results, Exploratory and Confirmatory Factor Analyses and the analysis of the mediation processes using AMOS software were conducted. Results confirmed that both self-congruity and social self-congruity with a destination impact its perceived symbolic value. While one of the dimensions of symbolic value (destination personality expression) totally impacts social media engagement, the other one (destination prestige) does not impact two of the three dimensions of social media engagement (contribution and creation). In an overall view, destination symbolic value mediates the impact of self-congruity and social self-congruity on destination social media engagement. The findings from this study suggest that marketers need to consider tourists’ need for expressing their identity and gaining acceptance of others in order to enhance social media engagement, by building a strong symbolic value. The effectiveness of a marketing strategy, that aims to highlight the symbolic value of the destination, can be assessed applying this model before and after its execution. A competitividade da indústria do turismo tem acelerado a necessidade de branding dos destinos turísticos, com o objetivo de se diferenciarem. A natureza simbólica e as motivações (pessoais e sociais) das viagens já eram consideradas significativas, mas a ascensão das redes sociais elevou-as a um novo patamar. Este estudo investiga o efeito mediador do valor simbólico percebido de um destino na relação entre autocongruência e autocongruência social com o destino e engajamento nas redes sociais. Para a análise dos resultados, foram realizadas Análises Fatoriais Exploratória e Confirmatória e análise dos processos de mediação utilizando software AMOS. Os resultados confirmaram que tanto a autocongruência como a autocongruência social com um destino impactam o seu valor simbólico percebido. Enquanto uma das dimensões do valor simbólico (expressão da personalidade do destino) impacta totalmente o engajamento nas redes sociais, a outra (prestígio do destino) não impacta duas das três dimensões do engajamento nas redes sociais (contribuição e criação). Numa visão geral, o valor simbólico do destino medeia o impacto da autocongruência e da autocongruência social no engajamento nas redes sociais do destino. Os resultados deste estudo sugerem que os profissionais de marketing precisam ter em consideração a necessidade dos turistas de exprimir a sua identidade e obter a aceitação de outros para aumentar o engajamento nas redes sociais, através da construção de um forte valor simbólico. A eficácia de uma estratégia de marketing, que vise destacar o valor simbólico de um destino, pode ser avaliada aplicando este modelo antes e depois da sua execução.

(Marcela) é uma planta nativa do Brasil e muito utilizada na medicina popular, sendo suas partes empregadas com várias finalidades terapêuticas, tais como analgésica, antioxidante, anti-inflamatória entre outras. O objetivo deste estudo foi buscar evidências de tradicionalidade do uso de plantas medicinais hepatoprotetoras nativas do Brasil e avaliar o efeito hepatoprotetor, em modelo in vitro, de A. satureioides contra danos induzidos por paracetamol. Em uma primeira parte deste estudo, foi feita uma revisão das espécies das plantas brasileiras utilizadas como agentes hepatoprotetores, citadas em 25 livros de diferentes regiões do país. Baseado nas 153 espécies encontradas no levantamento realizado na primeira parte deste estudo, foi realizada uma análise abrangente com estudos científicos mais relevantes entre as plantas utilizadas no uso tradicional, dentre eles A. satureioides. Em uma segunda parte, foi realizada experimentalmente a avaliação do efeito hepatoprotetor e citotóxico de A. satureioides em células HepG2 contra danos induzidos por paracetamol. Foram utilizadas as inflorescências de A. satureioides para o preparo dos extratos aquoso e etanólico para avaliar a toxicidade e o efeito protetor de ambos, nas concentrações de 6,25 a 100 μg/mL. A citotoxicidade foi avaliada pela redução do MTT e incorporação do corante vermelho neutro (VN). A atividade hepatoprotetora foi avaliada em células HepG2 após a incubação com paracetamol. Não houve alteração significativa na viabilidade celular nas diferentes concentrações testadas para os extratos aquosos de A. satureioides. Quanto ao extrato etanólico, não foi observada redução significativa na viabilidade celular no teste de redução do MTT. No entanto, no teste de VN observamos uma leve citotoxicidade em concentrações a partir de 12,5 μg/mL. Para a análise de hepatoproteção, os extratos foram co-incubados com 100 μg de paracetamol por 3h. Observamos que ambos os extratos de A. satureioides co-incubados com paracetamol protegem eficientemente as células da morte em todas as concentrações testadas e em ambos os ensaios. Da mesma forma, a silimarina (usada como controle positivo) também protegeu as células da morte. Os resultados contribuem para um possível uso seguro da planta, mas há a necessidade de realizar outros testes para validação do uso. Liver diseases have been one of the concerns and a challenge for public health. Despite advances in modern medicine, the search for therapeutic alternatives has increased, due to factors such as long-term adverse effects of the treatment of liver diseases. In view of this, natural products become a therapeutic alternative for the treatment of various disorders, including liver disorders. Achyrocline satureioides (Lam.) DC. (Marcela) is a plant native to Brazil and widely used in folk medicine, and its parts are used for various therapeutic purposes, such as analgesic, antioxidant, anti-inflammatory, among others. The objective of this study was to seek evidence of the traditional use of hepatoprotective medicinal plants native to Brazil and to evaluate the hepatoprotective effect, by in vitro model, of A. satureioides against paracetamol-induced damage. In the first part of this study, a review of the plant species used as hepatoprotective agents, cited in 25 books from different regions of the country, was carried out. Based on the 153 species studied, elaborated in the study carried out in the first part, a comprehensive analysis was carried out with the most relevant scientific studies among the plants used in the traditional use by them A. satureioides. In a second part, an evaluation of the hepatoprotective and cytotoxic effect of A. satureioides on HepG2 cells against paracetamol-induced damage was carried out experimentally. A. satureioides inflorescences were used for the preparation of aqueous and ethanolic extracts to evaluate the toxicity and protective effect of both, at concentrations of 6.25 to 100 μg/mL. Cytotoxicity was assessed by MTT reduction and neutral red dye (NR) incorporation. Hepatoprotective activity was evaluated in HepG2 cells after incubation with paracetamol. There was no significant change in cell viability at different concentrations tested for aqueous extracts of A. satureioides. As for the ethanol extract, no significant reduction in cell viability was observed in the MTT reduction test. However, in the VN test, we observed a slight cytotoxicity at concentrations from 12.5 μg/mL. For the analysis of hepatoprotection, the extracts were co-incubated with 100 μg of paracetamol for 3h. We observed that both A. satureioides extracts co-incubated with paracetamol efficiently protected cells from death at all concentrations tested and in both assays. Likewise, silymarin (used as a positive control) also protected the cells from death. The plant results contribute to a possible use, but there is a need to carry out tests to validate the use.

Dissertação de Mestrado Integrado em Arquitetura apresentada à Faculdade de Ciências e Tecnologia The classification of the ‘University of Coimbra - Alta and Sofia’ as a UNESCO World Heritage Site inscribed the city of Coimbra in a list of places recognized for their exceptional value, which has catapulted the city to the status of tourist attraction. The increasing number of tourists in Coimbra has led, in turn, to an increase in the infrastructure to support this activity. Of these, our work aims to focus on the accommodation, specifically on the typology of local accommodation (LA), and on the impacts of its proliferation in the historic center of Coimbra. Like other Portuguese cities, the historic center of Coimbra was relatively uninhabited at the time of the UNESCO classification, and this aspect is still visible today. This work aims to investigate the reasons behind this reduced urban vitality, specifically concerning housing, in order to try to understand whether the increase in LA in the area has had, or not, consequences on the offer and the type of accommodation available. It is intended, therefore, to assess how the functioning of the city changed due to the increase in tourist activity, the type of tourists it attracts, the attribution of the UNESCO brand, and the management of its classified spaces. A classificação da ‘Universidade de Coimbra – Alta e Sofia’ como Património Mundial da UNESCO inscreveu a cidade de Coimbra numa lista de locais reconhecidos pelo seu valor excecional, catapultando a cidade para o estatuto de atração turística. O aumento de turistas em Coimbra foi sendo cada vez mais expressivo, razão que levou , por sua vez , ao aumento das infraestruturas de apoio a essa atividade. Destas, o nosso trabalho visa forcar-se na acomodação, em concreto, na tipologia de alojamento local (AL), e nos impactos da sua proliferação no centro histórico de Coimbra. À semelhança de outras cidades portuguesas, o centro histórico de Coimbra apresentava-se, à data da classificação UNESCO, relativamente desabitado, sendo esse aspeto ainda visível nos dias de hoje. Este trabalho visa investigar as razões por detrás desta reduzida vitalidade urbana, em concreto no que à habitação diz respeito, para procurar compreender se o aumento do AL na área teve, ou não, consequências na oferta e no tipo de alojamento disponível. Pretende-se, assim, aferir de que forma o funcionamento da cidade se altera em função do aumento da atividade turística, do tipo de turista que atrai, da atribuição da marca UNESCO e da gestão dos seus espaços classificados.

The potato (Solanum tuberosum) is an annual herbaceous, growing up to 100 cm in height and produces a tuber that ranks as the fourth world food crop, after maize, wheat and rice. Although the United States is the fifth largest potato producer in the world, only a third of the potatoes is consumed fresh and 60% of annual output is processed into frozen products such as frozen French fries and wedges, crisps, dehydrated potato and starch. In Brazil, the annual consumption is around 14 kg per inhabitant, but this market is changing, particularly for processed potato snacks. The development of chronic non-communicable diseases is associated with several factors such as inadequate physical activity, excess intake of calories and fat, inadequate intake of vitamins and minerals, excessive alcohol consumption and genetics. The increase in the consumption of fried foods is critical, requiring alternatives to reduce the absorption of fats from fried foods. It is known that some edible coatings are able to reduce the oil absorption during the frying process (e.g., coatings based on hydroxypropyl methylcellulose and whey; guar gum; carboxymethylcellulose and pectin; Basil seed gum and salep; whey protein and pectin in the presence of transglutaminase; methylcellulose). One of the limitations of the implementation of this technological innovation in the industry is that few studies focusing on the effect of different drying parameters on covered foods may be found in literature and, in most studies, the foods are dried at room temperature, which would require longer processing time. In this context, this book chapter aimed to study the role of edible coatings based on natural polymers (soy protein-SPI and potato starch) on process variables such as drying temperature and time. TitleNeurosyphilis presenting with psychotic and dementia symptoms: When the patient refuses intravenous antibiotherapy(Introduction) Tertiary syphilis is not a frequent clinical condition in western countries. Nevertheless, this infection can originate complex clinical conditions difficult to diagnose and requiring a very specific treatment, that often is difficult to administrate.(Objectives) The aim of this poster presentation is to share the difficulties experienced in finding the diagnosis and mostly the numerous problems that we faced in the treatment approach because intravenous therapy is frequently painful and acutely decompensated patients were not able to accept this kind of side effects leading to a treatment refusal.(Methods) In this presentation, we describe two different situations that were committed to our psychiatric unit and all the procedures and difficulties that were necessary to achieve for a proper treatment for the two patients.(Results) At the end of the hospital stay, both patients were partially compensated and institutional placement was necessary. The clinical constellation had suffered a good evolution but some of the symptoms remain. Other pathologies were unveiled during hospitalization.(Conclusions) Neurosyphilis remains a diagnostic to keep in mind. Even if it is less frequent than in the past, it exists and needs a particular approach, at the same time that it poses considerable challenges.

39. A PARTICULAR CASE OF ABUSE OF NON-PSYCHOACTIVE SUBSTANCES

TitleA particular case of abuse of non-psychoactive substances(Introduction) As described in ICD -10, a wide variety of medicaments and folk remedies may be involved, but the particularly important groups are: (a) psychotropic drugs that do not produce dependence, such as antidepressants, (b) laxatives, and (c) analgesics that may be purchased without medical prescription, such as aspirin and paracetamol.(Objectives) In this poster presentation, the authors would like to report a particular situation of substance abuse that does not fall in the most used substances and that filled a particular aim for the patient.(Methods) The authors describe the case of a patient with a long story of dependence of loperamide hydrochloride, a synthetic antidiarrheal for oral use. In this specific situation, the patient needed a control over his intestinal transit and every little change in the rhythm that he had implemented was taken like a failure over bowel control or withdrawal symptoms, leading to an enormous difficulty to deal with the problem and help him to stop this kind of medication. The possible effect over the opioid receptors was not neglected.(Results) The patient was hospitalized for the first time in 2015 and the second time three years later. In both occasions, the patient was taking daily doses of loperamide over 50 tablets a day. A classic intervention with anxiolytic therapy covering the classic symptoms of withdrawal did not work and the patient left the hospital with a smaller daily dose but still taking loperamide at doses not clinically recommended.(Conclusion) In conclusion, the authors would like to share their experience in dealing with this complicated and rare situation of substance abuse, even though the therapeutic results were not encouraging.

41. Teor de iodo em alimentos: comparação entre os Açores e Portugal Continental

Neste trabalho pretende-se contribuir com novos dados sobre o teor de iodo em alimentos disponíveis para consumo em Portugal. O iodo é um oligoelemento indispensável para a síntese das hormonas da tiroide. O seu papel assume particular importância em grávidas e crianças. Estudos clínicos em Portugal demonstraram a existência de uma carência generalizada das grávidas no país com particular severidade nos arquipélagos da Madeira e Açores [1]. Em relação às crianças em idade escolar, cerca de metade apresenta iodúrias abaixo do recomendado [2]. Assim, optou-se por comparar o teor de iodo de alimentos produzidos no arquipélago dos Açores com alimentos oriundos de Portugal Continental. Foram selecionadas amostras de lacticínios, frutas e produtos hortícolas de ambas as regiões em estudo. No total analisaram-se 110 amostras, em triplicado. Destas, 50 eram oriundas da região dos Açores e 60 de Portugal Continental. A metodologia escolhida para a determinação foi a espectrometria de massa com plasma indutivo acoplado (ICP-MS). O leite apresentou um maior teor de iodo em Portugal Continental (255 µg/l) do que nos Açores (133 µg/l). Para o queijo foi igualmente em Portugal Continental que se obteve teores de iodo mais elevados, 275 µg/kg, quando comparado com os Açores, 117 µg/kg. Todas as frutas analisadas apresentaram valores abaixo do limite de quantificação. Os resultados evidenciam que os lacticínios analisados são uma boa fonte de iodo e devem fazer parte das dietas de populações carenciadas. Os dados analíticos estão, também, alinhados com estudos recentes, evidenciando que os alimentos ricos em iodo representam um beneficio claro no aporte diário deste micronutriente, para a população em geral. O iodo é um oligoelemento indispensável para a síntese das hormonas da tiroide. A alimentação é a maior fonte natural de iodo. Assim, o principal objetivo deste trabalho foi a determinação de iodo em alimentos como consumidos e representativos da dieta portuguesa. A metodologia escolhida para a quantificação deste nutriente foi a espectrometria de massa acoplada ao plasma indutivo (ICP-MS). Analisaram-se seis grupos de alimentos: peixes, mariscos e bivalves, leite e derivados, vegetais, fruta e refeições compostas. Os grupos do pescado (peixe, marisco e bivalves) foram os que apresentaram concentrações de iodo mais elevados, com um valor médio de 114 μg/100g. Os laticínios são também uma fonte importante para suprir o aporte diário de iodo (150 μg/dia) apresentado valores médios de 22 μg/100g. Os resultados permitem concluir que em Portugal uma alimentação rica em pescado e lacticínios supre a dose diária recomendada de iodo para um adulto saudável. Iodine is an essential trace element for the synthesis of thyroid hormones. Food is the largest natural source of iodine. Therefore, the main objective of this work was the determination of iodine in foods as consumed and representative of the Por tuguese diet. The methodology selected for the quantification of this nutrient was inductively coupled plasma mass spectrometry (ICP-MS). Six groups of food were analyzed: fish, seafood and shellfish, dairy products, vegetables, fruit and meals. Seafood groups (fish, shellfish and bivalves) showed the highest concentrations of iodine, with an average of 114 μg/100g. Dairy products are also a major source to supply the daily intake of iodine (150 μg/day) presented average values of 22 μg/100g. The results show that in Portugal a diet rich in fish and dairy products supplies the recommended daily intake of iodine for a healthy adult.

Neste trabalho que se enquadra na linha de pesquisa variacionista, pretende-se caracterizar o Vale do Jequitinhonha como um espaço de interseção e de hibridismo cultural no qual a variação linguística é veio fértil para o conhecimento da língua em uso. Nesse sentido, como relevante componente dessa variação, destacam-se seus traços arcaicos, que dizem respeito à concreta realização dos sons e à estrutura em que comportamentos lingüísticos da época da colonização são intensamente produtivos, conquanto seja também intenso o processo de urbanização por que passa essa região.

49. A case of severe Covid-19 infection as the first manifestation of Cushing's disease.

- Abstract
Summary: Cushing's disease (CD) is characterized by distinct syndromic features, often accompanied by obesity and depression. However, considering its gradual onset of symptoms, it is usually associated with diagnostic delays. In rare instances, CD may lead to severe infections due to the observed immunosuppression in affected individuals. We present a rare case of an undiagnosed CD in a 20-year-old male with a medical history of depression and obesity, complicated by severe COVID-19 infection. He presented to the Emergency Room with respiratory distress, hypertensive crisis, and fever, ultimately receiving the diagnosis of SARS-CoV-2 pneumonia. The patient required mechanical ventilation and intensive care unit (ICU) admission due to severe acute respiratory distress syndrome (ARDS). During ICU care, he received remdesivir and dexamethasone, subsequently developing severe hyperglycemia and worsened hypertension, requiring insulin and multiple antihypertensive agents to manage metabolic disruption. Upon physical examination, classic signs of hypercortisolism were noted. Subsequent laboratory tests and pituitary magnetic resonance imaging confirmed the diagnosis of CD. The patient underwent surgical resection with significant improvements in body composition and metabolic parameters postoperatively. After surgery, remission of hypercortisolism was evident, accompanied by notable improvements in mood and overall health. This case underscores the importance of recognizing hypercortisolism in the context of metabolic, physical, and mood changes. Timely diagnosis of CD is crucial to mitigate complications such as severe opportunistic infections and their outcomes., Learning Points: Despite some hallmark features such as proximal myopathy, easy bruising, purple striae, and facial plethora, Cushing's disease (CD) is a challenging diagnosis due to its nonspecific signs and symptoms and gradual onset. The case emphasizes the importance of recognizing subtle signs of CD, such as social isolation, depressive symptoms, and changes in body composition, which may be confounded by external factors like the COVID-19 pandemic. Patients with CD are prone to severe infections due to chronic hypercortisolism-induced immunosuppression. CD diagnostic delays are common, leading to worsening of metabolic and immune dysfunction over time. Heightened clinical suspicion and early intervention are essential to prevent diagnostic delays and optimize patient outcomes.

50. Ultrasound assessment of muscle mass in critically ill patients: A correlation with nutritional support and clinical outcomes.

- Abstract
Background: Critically ill patients are at high risk of Intensive Care Unit (ICU) Acquired weakness, which negatively impacts clinical outcomes. Traditional muscle mass and nutritional status assessments are often impractical in the ICU. Ultrasound offers a promising, non-invasive alternative. This study evaluates the relationship between ultrasound-based muscle assessments, patients' nutritional support, and clinical outcomes in the ICU., Methods: A prospective cohort study was conducted in three ICUs of a tertiary center. Daily nutritional intake, ultrasound measurements of the quadriceps muscle (rectus femoris cross-sectional area - RFCSA - and quadriceps muscle layer thickness - QMLT), and clinical data were collected on days 1, 3, and 7 of ICU., Results: A total of 128 patients were included in the analysis, with a mean age of 65.4 (±18.1) years and a median ICU stay of 6 (4-10) days. QMLT decreased by 5 % and 13 %, and RFCSA decreased by 10 % and 27 % on days 3 and 7, respectively. A significant correlation was found between lower caloric and protein intake and greater muscle mass loss within the initial 3 days of ICU admission. Multivariate logistic regression indicated that QMLT reduction significantly contributed to 28-day mortality (adjusted OR 1.088, 95 % CI: 1.018-1.113, p = 0.015). Lower daily caloric and protein intake was depicted in non-surviving patients (p < 0.001)., Conclusions: Our study demonstrates that critically ill patients experience significant muscle mass loss within the first 72 h of ICU.
