In article analyzes the technical requirements for spherical washers used in threaded connections of pumps, which made it possible to highlight contradictions in paragraphs of the current standards for pump fasteners. They regarding recommendations for manufacturing technology and control of the working surfaces of washers. Publication analysis in the spherical surfaces lapping showed the absence of research in the spherical surfaces contact after lapping and the dependence of contact parameters by the technological parameters for this operation. The presence of a gap in conjunction with spherical washers after the lapping process was proved geometrically. Its location was determined, and a mathematical dependence was obtained to determine the maximum gap value in conjunction. It was found that the gap depends on the design parameters of the washers’ conjunction and the abrasive material grain size used for lapping. Recommendations for selecting the abrasive material grain size for the lapping operation for the most common values of the roughness parameters for the working surfaces of spherical washers have been formed. In the article was proposed to introduce into the technical requirements for the spherical washers drawings changes concerning the gap size allowable in the conjunction, and as well as tolerances value changes of the radii of the working surfaces of the washers, thus increasing their manufacturability without conflicting with the requirements of the current standards.

The efficiency of maintenance processes in an enterprise largely depends on ensuring adequate resources for its implementation. The main factor that affects the quality of these processes is competent employees. Their knowledge, skills and ability to respond to unexpected situations largely determine the efficiency of the functioning of the technical infrastructure in an enterprise. In the light of the prospects for the development of the Industry 4.0 concept, and, thus, for the development of highly automated systems, the demand for qualified maintenance employees will increase. Therefore, in order to ensure the right level of competency of maintenance workers, through the proper assessment and identification of their competency gap, is an important task of managers. In many enterprises this is not implemented. The aim of the presented work was to developed a comprehensive model of the competency assessment of maintenance workers. The implementation of the developed model enables the identification of the current level of employees' competencies and identification of the competency gap, as well as it allows to assess the effects of a failure to meet the required level of competency. Additionally, the results of the identification of the real activities taken by the surveyed enterprises concerning the competency assessment of maintenance services employees are presented in this article. This article presents an investigation of the use of machine learning methodologies for the prediction of surface roughness in milling operations, using sensor data as the primary source of information. The sensors, which included current transformers, a microphone, and displacement sensors, captured comprehensive machining signals at a frequency of 10 kHz. The signals were subjected to preprocessing using the Savitzky-Golay filter, with the objective of isolating relevant moments of active material machining and reducing noise. Two machine learning models, namely Elastic Net and neural networks, were employed for the prediction purposes. The Elastic Net model demonstrated effective handling of multicollinearity and reduction in the data dimensionality, while the neural networks, utilizing the ReLU activation function, exhibited the capacity to capture complex, nonlinear patterns. The models were evaluated using the coefficient of determination (R²), which yielded values of 0.94 for Elastic Net and 0.95 for neural networks, indicating a high degree of predictive accuracy. These findings illustrate the potential of machine learning to optimize manufacturing processes by facilitating precise predictions of surface roughness. Elastic Net demonstrated its utility in reducing dimensionality and selecting features, while neural networks proved effective in modeling complex data. Consequently, these methods exemplify the efficacy of integrating data-driven approaches with robust predictive models to improve the quality and efficiency of surface.

Interleukin-5 functions as a B-cell differentiation factor, but more importantly, in the context of this review, it plays a variety of roles in eosinophil biology, including eosinophil differentiation and maturation in the bone marrow, and facilitates eosinophil migration to tissue sites, usually in the context of an allergic reaction. Given the availability of selective anti-IL-5 drugs such as mepolizumab and reslizumab, as well as the IL-5 receptor antagonist benralizumab, it is worth investigating whether they could be used in some cases of allergic disease. Asthma has a well-documented involvement of IL-5 in its pathophysiology and has clear benefits in the case of anti-IL-5 therapy; therefore, current knowledge is presented to provide a reference point for the study of less-described diseases such as atopic dermatitis, chronic rhinosinusitis, chronic spontaneous urticaria, and its association with both IL-5 and anti-IL-5 treatment options. We then review the current literature on these diseases, explain where appropriate potential reasons why anti-IL-5 treatments are ineffective, and then point out possible future directions for further research.

The long COVID (coronavirus disease), a multisystemic condition following severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) infection, is one of the widespread problems. Some of its symptoms affect the nervous system and resemble symptoms of Alzheimer's disease (AD)-a neurodegenerative condition caused by the accumulation of amyloid beta and hyperphosphorylation of tau proteins. Multiple studies have found dependence between these two conditions. Patients with Alzheimer's disease have a greater risk of SARS-CoV-2 infection due to increased levels of angiotensin-converting enzyme 2 (ACE2), and the infection itself promotes amyloid beta generation which enhances the risk of AD. Also, the molecular pathways are alike-misregulations in folate-mediated one-carbon metabolism, a deficit of Cq10, and disease-associated microglia. Medical imaging in both of these diseases shows a decrease in the volume of gray matter, global brain size reduction, and hypometabolism in the parahippocampal gyrus, thalamus, and cingulate cortex. In some studies, a similar approach to applied medication can be seen, including the use of amino adamantanes and phenolic compounds of rosemary. Introduction: Comorbid insomnia and obstructive sleep apnea (COMISA) is not a well-identified sleep disorder, despite having a significant impact on health. This study investigates the relationship between sleep bruxism (SB) and sleep architecture in patients with COMISA, obstructive sleep apnea (OSA), and in those without any sleep disorders. Methods : 119 patients were included in the study and divided into three groups: OSA, COMISA, and a control group. Polysomnographic (PSG) examination provided parameters related to sleep architecture, OSA, and characteristics of SB. Results : The bruxism episode index (BEI) and other SB parameters were not found to be statistically different between the three groups ( p > 0.05). There was no statistical difference in measured sleep architecture between the COMISA and OSA groups ( p > 0.05). In comparison to the control group, participants in the COMISA group were found to have an increased apnea-hypopnea index (AHI), oxygen desaturation index (ODI), respiratory disturbance index (RDI), all arousals (AA), and respiratory arousals (RA) ( p < 0.05). Among COMISA patients, AA and RA were shown to have a positive linear correlation with the number of bradycardia events per hour (r = 0.49, r = 0.48, p < 0.05). Conclusions : SB does not occur in patients with COMISA more frequently than in patients with OSA or those without any sleep disorders. PSG parameters are not specific for COMISA; therefore, in order to differentiate this disorder from OSA alone, a comprehensive patient assessment has to be performed.

The composition of human breast milk is an ideal combination of substances necessary for the healthy development of an infant's body while protecting from pathogens and the balanced development of the microbiota. Its composition is dynamic and changes with the age of the child, meeting their current needs. The study provides a thorough overview of human milk components, such as immunological components, growth factors, hormones, carbohydrates, lipids, minerals, and vitamins. Authors focus on capturing the most important aspects of the effects of these substances on a newborn's body, while also looking for specific connections and describing the effects on given systems. Supplementation and the use of ingredients are also discussed. The purpose of this paper is to present the current state of knowledge about the bioactive components of human milk and their impact on the growth, development, and health of the young child.

This editorial investigates chronic traumatic encephalopathy (CTE) as a course of Alzheimer's disease (AD). CTE is a debilitating neurodegenerative disease that is the result of repeated mild traumatic brain injury (TBI). Many epidemiological studies show that experiencing a TBI in early or middle life is associated with an increased risk of dementia later in life. Chronic traumatic encephalopathy (CTE) and Alzheimer's disease (AD) present a series of similar neuropathological features that were investigated in this work like recombinant tau into filaments or the accumulation and aggregation of Aβ protein. However, these two conditions differ from each other in brain-blood barrier damage. The purpose of this review was to evaluate information about CTE and AD from various articles, focusing especially on new therapeutic possibilities for the improvement in cognitive skills.

To ensure the high reliability of aircraft structures, the Refill Friction Stir Spot Welding (RFSSW) process must be characterized by a high load capacity of the welds and a small standard deviation of the load capacity spread. This allows us to obtain uniform functional properties in the connections, ensuring the high quality of the process. This work aims to select the most favorable technological parameters for the welding process of EN AW-7075-T6 Alclad aluminum alloy sheets, which are used for the production of aircraft structures. The best networks were calculated using the Statistica 13.3 program. The obtained results were compared with the results of previous investigations. It has been shown that a model using neural networks allows for the determination of connection parameters with much greater accuracy than the classical model. The maximum error in estimating the load capacity of the connection for the mathematical model was 6.13%, and the standard deviation was 14.51%. In the case of neural networks, the maximum error value did not exceed 1.55%, and the standard deviation was 3.74%. It was shown that, based on the neural model, it is possible to determine the process parameters that ensure the required quality capacity of the process, ensuring a probability of obtaining the required load capacity of the connections amounting to P = 0.999935 with a defect rate of 0.0065%. This possibility is not provided by the classical model due to its large error in estimating the process spread and the high sensitivity of the process input parameters to the output parameters.

Manufacturing processes in industry applications are often controlled by the evaluation of surface topography. Topography, in its overall performance, includes form, waviness, and roughness. Methods of measurement of surface roughness can be roughly divided into tactile and contactless techniques. The latter ones are much faster but sensitive to external disturbances from the environment. One type of external source error, while the measurement of surface topography occurs, is a high-frequency noise. This noise originates from the vibration of the measuring system. In this study, the methods for reducing high-frequency errors from the results of contactless roughness measurements of turned surfaces were supported by machine learning methods. This research delves into optimizing filtration methods for surface topography measurements through the application of machine learning models, focusing on enhancing the accuracy of surface roughness assessments. By examining turned surfaces under specific machining conditions and employing a variety of digital filters, the study identifies the Gaussian regression filter and spline filter as the most effective methods at a 22.5 µm cut-off. Utilizing neural networks, support vector machines, and decision trees, the research demonstrates the superior performance of SVMs, achieving remarkable accuracy and sensitivity in predicting optimal filtration methods.

Objective: To evaluate temporal trends in the prevalence of gram-negative bacteria (GNB) with difficult-to-treat resistance (DTR) in the southeastern United States. Secondary objective was to examine the use of novel β-lactams for GNB with DTR by both antimicrobial use (AU) and a novel metric of adjusted AU by microbiological burden (am-AU)., Design: Retrospective, multicenter, cohort., Setting: Ten hospitals in the southeastern United States., Methods: GNB with DTR including Enterobacterales, Pseudomonas aeruginosa , and Acinetobacter spp. from 2015 to 2020 were tracked at each institution. Cumulative AU of novel β-lactams including ceftolozane/tazobactam, ceftazidime/avibactam, meropenem/vaborbactam, imipenem/cilastatin/relebactam, and cefiderocol in days of therapy (DOT) per 1,000 patient-days was calculated. Linear regression was utilized to examine temporal trends in the prevalence of GNB with DTR and cumulative AU of novel β-lactams., Results: The overall prevalence of GNB with DTR was 0.85% (1,223/143,638) with numerical increase from 0.77% to 1.00% between 2015 and 2020 ( P = .06). There was a statistically significant increase in DTR Enterobacterales (0.11% to 0.28%, P = .023) and DTR Acinetobacter spp. (4.2% to 18.8%, P = .002). Cumulative AU of novel β-lactams was 1.91 ± 1.95 DOT per 1,000 patient-days. When comparing cumulative mean AU and am-AU, there was an increase from 1.91 to 2.36 DOT/1,000 patient-days, with more than half of the hospitals shifting in ranking after adjustment for microbiological burden., Conclusions: The overall prevalence of GNB with DTR and the use of novel β-lactams remain low. The review describes correlations between impaired functioning of chaperones and co-chaperones in Alzheimer's disease (AD) pathogenesis. The study aims to highlight significant lines of research in this field. Chaperones like Hsp90 or Hsp70 are critical agents in regulating cell homeostasis. Due to some conditions, like aging, their activity is damaged, resulting in β-amyloid and tau aggregation. This leads to the development of neurocognitive impairment. Dysregulation of co-chaperones is one of the causes of this condition. Disorders in the functioning of molecules like PP5, Cdc37, CacyBP/SIPTRAP1, CHIP protein, FKBP52, or STIP1 play a key role in AD pathogenesis. PP5, Cdc37, CacyBP/SIPTRAP1, and FKBP52 are Hsp90 co-chaperones. CHIP protein is a co-chaperone that switches Hsp70/Hsp90 complexes, and STIP1 binds to Hsp70. Recognition of precise processes allows for the invention of effective treatment methods. Potential drugs may either reduce tau levels or inhibit tau accumulation and aggregation. Some substances neuroprotect from Aβ toxicity. Further studies on chaperones and co-chaperones are required to understand the fundamental tenets of this topic more entirely and improve the prevention and treatment of AD.

Asthma is a complex chronic respiratory disease characterized by airway hyperresponsiveness, inflammation, and obstruction. Many genes have been identified as associated with asthma but none with such substantial significance as the ADAM33 gene due to its role in airway remodeling and bronchial hyperresponsiveness. This review summarizes the current knowledge on the genetic and functional aspects of ADAM33 in asthma pathogenesis. We highlight its genetic variants associated with asthma susceptibility and severity, as well as the functional effects of ADAM33 on airway remodeling, smooth muscle cell proliferation, and its interplay with environmental factors. Additionally, we discuss the potential clinical implications of ADAM33 as a therapeutic target for asthma management.

Background: Compared with other facial fractures, mandibular fractures have the highest rate of postsurgical site infection. There is strong evidence to suggest that postoperative antibiotics do not reduce rates of surgical site infections (SSIs) regardless of duration. However, there are conflicting data in the literature regarding the role of prophylactic preoperative antibiotics in reducing rates of SSIs. This study reviews the rate of infections in patients who underwent mandibular fracture repair who received a course of preoperative prophylactic antibiotics compared with those who received no or 1 dose of perioperative antibiotics., Methods: Adult patients who underwent mandibular fracture repair at Prisma Health Richland between 2014 and 2019 were included in the study. A retrospective cohort review was conducted to determine the rate of SSI, comparing 2 groups of patients who underwent mandibular fracture repair. Patients who received more than 1 dose of scheduled antibiotics before surgery were compared with those who did not receive any antibiotic treatment before surgery or received only a single dose of antibiotics within 1 hour of incision time (perioperative antibiotics). The primary outcome was the rate of SSI between the 2 groups of patients., Results: There were 183 patients who received more than 1 dose of scheduled antibiotics before surgery and 35 patients who received a single dose of perioperative antibiotics or did not receive any antibiotics. Objective: Antimicrobial stewardship has special challenges in particular populations and facilities, including pediatrics. We sought to augment the information available to antimicrobial stewardship programs (ASPs) by created a cumulative statewide antibiogram for neonatal and pediatric populations., Methods: In the Antimicrobial Stewardship Collaborative of South Carolina (ASC-SC), we created statewide antibiograms, including a separate antibiogram accounting for the pediatric and neonatal intensive care unit (NICU) populations. We collated data from the 4 pediatric and 3 NICU facilities in the state to provide a cumulative statewide antibiogram., Results: Methicillin-susceptible Staphylococcus aureus was more prevalent than methicillin-resistant Staphylococcus aureus. Pseudomonas aeruginosa, Citrobacter koserii, and Acinetobacter baumannii were isolated in only 1 NICU., Conclusions: These antibiograms should improve empiric prescribing in both the inpatient and outpatient setting, providing data in some areas that historically do not have pediatric antibiogram to inform prescribing. Infections of the central nervous system (CNS) are complex to treat and associated with significant morbidity and mortality. Historically, antistaphylococcal penicillins such as nafcillin were recommended for the treatment of methicillin-susceptible staphylococcal CNS infections. However, the use of antistaphylococcal penicillins presents challenges, such as frequent dosing administration and adverse events with protracted use. This narrative reviews available clinical and pharmacokinetic/pharmacodynamic (PK/PD) data for cefazolin in CNS infections and produces a recommendation for use. Based on the limited available evidence analyzed, dose optimized cefazolin is likely a safe and effective alternative to antistaphylococcal penicillins for a variety of CNS infections due to methicillin-susceptible Staphylococcus aureus. Given the site of infection and wide therapeutic index of cefazolin, practitioners may consider dosing cefazolin regimens of 2 g IV every 6 h or a continuous infusion of 8-10 g daily instead of 2 g IV every 8 h to optimize PK/PD properties., (© 2023 Pharmacotherapy Publications, Inc.)

Traditionally, sustainable development has been seen as a combination of three pillars: economic, social and environmental development. In recent years, another one has been added to these three pillars, namely culture, as being indispensable in achieving sustainable development. This study proposes an integrated approach for the identification and classification of safety culture factors in the company in a sustainability context. The research design was based on the assumption that safety culture is part of organizational culture that should support the development of corporate sustainability. Firstly, the identification of the safety culture factors (SCFs) based on the literature review was presented. Then, the ISM method was used to identify the interaction between SCFs and to develop the hierarchical structure of these factors. In the next step, ISM was integrated with the MICMAC method to cluster the factors based on driving power and dependence power into four categories. Finally, safety culture factors with high driving power were rated using the fuzzy TOPSIS method from the sustainability dimension perspective. This approach was used in an automotive industry company to improve and develop the company's practices aimed at implementing a sustainable development strategy. A sensitivity analysis was also carried out to monitor the robustness of the approach.

Modern manufacturing engineering requires quick and reasonable solutions during the production planning stage, ensuring production efficiency and cost reduction. This research aims to create a scientific approach to the rational choice of a locating chart for complexly shaped parts. It is an important stage during the manufacturing technology and fixture design process. The systematization of the designed and technological features of complexly shaped parts and the definition of the features that impact a locating chart create the fundamentals for justification. A scientific approach has been developed using the complex combination of the part's features and a decision-making approach using the example of bracket-type parts. The matrix of design and technological features of parts was developed including steel AISI 3135 and cast iron DIN 1691. The classification of locating charts for bracket-type parts was defined. A mathematical model of the rational choice of the locating chart according to the structural code of the workpiece was verified in case studies from the practice. As a result, a decision-making approach was applied to the rational choice of the locating chart for any bracket-type part. The proposed solutions improve the production planning stage for machine building, automotive, and other industries.

The use of long-acting lipoglycopeptides (LaLGPs) in serious, deep-seated infections is of increasing interest. The purpose of this study is to evaluate the economic and clinical utility of LaLGPs in patients requiring protracted antibiotic courses who are not ideal candidates for oral transition or outpatient parenteral antibiotic therapy (OPAT). This is a retrospective, observational, matched cohort study of adult patients who received a LaLGP. Patients were matched 1:1 to those who received standard of care (SOC). Cost effectiveness was evaluated as total healthcare-related costs between groups. Clinical failure was a composite endpoint of mortality, recurrence, or need for extended antibiotics beyond planned course within 90 days of initial infection. There was no difference in clinical failure between the two cohorts (22% vs. 30%; p = 0.491). Six patients in the SOC cohort left against medical advice (AMA) prior to completing therapy. Among those who did not leave AMA, receipt of LaLGPs resulted in a decreased hospital length of stay by an average of 13.6 days. The average total healthcare-related cost of care was USD 295,589 in the LaLGP cohort compared to USD 326,089 in the SOC cohort ( p = 0.282). Receipt of LaLGPs may be a beneficial treatment option for patients with deep-seated infections and socioeconomic factors who are not candidates for oral transition or OPAT.

A multicenter case series of 21 patients were treated with imipenem-cilastatin-relebactam. There were mixed infection sources, with pulmonary infections (11/21,52%) composing the majority. The primary pathogen was Pseudomonas aeruginosa (16/21, 76%), and 15/16 (94%) isolates were multidrug-resistant. Thirty-day survival occurred in 14/21 (67%) patients. This article presents the results of tests aimed at determining the effect of slide burnishing parameters on the surface roughness of shafts made of 42CrMo4 heat-treatable steel. The burnishing process was carried out using tools with polycrystalline diamond and cemented carbide tips. Before burnishing, the samples were turned on a turning lathe to produce samples with an average surface roughness Ra = 2.6 µm. The investigations were carried out according to three-leveled Hartley's poly selective quasi D (PS/DS-P: Ha3) plan, which enables a regression equation in the form of a second-order polynomial to be defined. Artificial neural network models were also used to predict the roughness of the surface of the shafts after slide burnishing. The input parameters of the process that were taken into account included the values of pressure, burnishing speed and feed rate. Overall, the burnishing process examined leads to a reduction in the value of the surface roughness described by the Ra parameter. The artificial neural networks with the best regression statistics predicted an average surface roughness of the shafts with R 2 = 0.987. The lowest root-mean-square error and mean absolute error were obtained with all the network structures analysed that were trained with the quasi Newton algorithm.

The influence of irradiation should be considered in fatigue reliability analyses of reactor structures under irradiation conditions. In this study, the effects of irradiation hardening and irradiation embrittlement on fatigue performance parameters were quantified and a fatigue life prediction model was developed. Based on this model, which takes into account the cumulative effect of a neutron dose, the total fatigue damage was calculated according to Miner's linear cumulative damage law, and the reliability analysis was carried out using the Monte Carlo simulation method. The case results show that the fatigue life acquired by taking into account the cumulative effect of irradiation was reduced by 24.3% compared with that acquired without considering the irradiation effect. Irradiation led to the increase of the fatigue life at low strains and its decrease at high strains, which is in accordance with the findings of an irradiation fatigue test. The rate of increase in the fatigue life decreased gradually with the increase of the neutron dose. The irradiation performance parameters had a small influence on fatigue reliability, while the fatigue strength coefficient and the elastic modulus had a great influence on the fatigue reliability. Compared with the current method, which uses a high safety factor to determine design parameters, a fatigue reliability analysis method taking into account the cumulative effect of irradiation could be more accurate in the reliability analysis and life prediction of reactor structures.
