Abstract

The fusion research facility ITER is currently being assembled to demonstrate that fusion can be used for industrial energy production, while several other programmes across the world are also moving forward, such as EU-DEMO, CFETR, SPARC and STEP. The high engineering complexity of a tokamak makes it an extremely challenging device to optimise, and test-based optimisation would be too slow and too costly. Instead, digital design and optimisation must be favored, which requires strongly-coupled suites of High-Performance Computing calculations. In this context, having surrogate models to provide quick estimates with uncertainty quantification is essential to explore and optimise new design options. Furthermore, these surrogates can in turn be used to accelerate simulations in the first place. This is the case of Parareal, a time-parallelisation method that can speed-up large HPC simulations, where the coarse-solver can be replaced by a surrogate. A novel framework, Neural-Parareal, is developed to integrate the training of neural operators dynamically as more data becomes available. For a given input-parameter domain, as more simulations are being run with Parareal, the large amount of data generated by the algorithm is used to train new surrogate models to be used as coarse-solvers for future Parareal simulations, leading to progressively more accurate coarse-solvers, and thus higher speed-up. It is found that such neural network surrogates can be much more effective than traditional coarse-solver in providing a speed-up with Parareal. This study is a demonstration of the convergence of HPC and AI which simply has to become common practice in the world of digital engineering design.

Abstract

Drought management largely depends on the availability of timely, accurate and integrated information about its characteristics. Concurrently, biostimulants could represent a sustainable measure to foster the resilience of cropping systems under water-limited conditions. Nevertheless, scientific recognition of the potential of biostimulants has not grown as fast as the interest from industry: therefore, there is an urgent need to investigate biostimulant action. In recent decades, remote sensing has been successfully applied to crop growth and stress monitoring. The use of radiative transfer models, often rooted in artificial intelligence, to estimate plant traits from remotely sensed data can be considered the link between the generalisation and the spatialisation of data, bringing field phenotyping and remote sensing closer together. In this framework, the present study was designed as a factorial combination of irrigation treatment (3 levels) and biostimulant treatment (3 levels) and conducted on processing tomato in open field during the summer of 2020. PROSAIL inversion was carried out to retrieve three major biophysical traits (LAI, LCC, CCC). The parameters dynamics during the season were investigated through GAM modelling. The validation of the model was carried out with a positive outcome in terms of accuracy. The PROSAIL inversion enabled the efficient retrieval of LAI and LCC at rates comparable to those in literature, while performing worse than literature findings only for CCC, probably due to the characteristics of tomato canopy. At the same time, the effect of irrigation was detected both for yield and quality data and detected through the GAM modelisation of the parameters. However, no biostimulant effect could be detected. The internal variability per plot of the retrieved biophysical traits was high. Abstract

The brain employs distinct circuitries to encode positive and negative valence stimuli, and dysfunctions of these neuronal circuits have a key role in the etiopathogenesis of many psychiatric disorders. The Dorsal Raphe Nucleus (DRN) is involved in various behaviors and drives the emotional response to rewarding and aversive experiences. Whether specific subpopulations of neurons within the DRN encode these behaviors with different valence is still unknown. Notably, microRNA expression in the mammalian brain is characterized by tissue and neuronal specificity, suggesting that it might play a role in cell and circuit functionality. However, this specificity has not been fully exploited. Here, we demonstrate that microRNA-34a (miR-34a) is selectively expressed in a subpopulation of GABAergic neurons of the ventrolateral DRN. Moreover, we report that acute exposure to both aversive (restraint stress) and rewarding (chocolate) stimuli reduces GABA release in the DRN, an effect prevented by the inactivation of DRN miR-34a or its genetic deletion in GABAergic neurons in aversive but not rewarding conditions. Finally, miR-34a inhibition selectively reduced passive coping with severe stressors. These data support a role of miR-34a in regulating GABAergic neurotransmitter activity and behavior in a context-dependent manner and suggest that microRNAs could represent a functional signature of specific neuronal subpopulations with valence-specific activity in the brain.

Abstract

Near-real-time, high-spatial-resolution leaf area index (LAI) maps would enable producers to monitor crop health and growth status, improving agricultural practices such as fertiliser and water management. LAI retrieval methods are numerous and can be divided into statistical and physically based methods. While statistical methods are generally subject to high site-specificity but possess high ease of implementation and use, physically based methods are more transferable, albeit more complex to use in operational settings. In addition, statistical methods need a large amount of data for calibration and subsequent validation, and this is only seldom feasible. Techniques based on predictive equations (PEphysical) represent a viable alternative, allowing the partial combination of statistical and physical methods merits while minimising their shortcomings. In this paper, predictive equation-based techniques were compared with four other methods: two radiative transfer model (RTM) inversion methods, one based on neural network (NNET) and one based on a look-up table (LUT), and two empirical methods (one using empirical models based on vegetation indices and in situ data and one based on empirical models found in the scientific literature). The methods were chosen based on common use. To evaluate the performance of the studied methods, the coefficient of determination (R2), root mean square error (RMSE), and normalised root mean square error (nRMSE, %) between the estimates and in situ LAI measurements were reported. The best PEphysical results, achieved by the OSAVI index (RMSE = 0.84 m2 m−2), provided better performance for LAI recovery than the NNET-based RTM inversions (0.86 m2 m−2) or the estimates made by LUT (0.94 m2 m−2). Furthermore, the best PEphysical produced accuracies comparable to the best empirical model (RMSE = 0.71 m2 m−2), calibrated through in situ data, and similar to the best literature model (RMSE = 0.76 m2 m−2). Abstract

Objectives: Functional movement disorders (FMD) refer to a heterogeneous group of manifestations incongruent with known neurological diseases. Functional neuroimaging studies in FMD indicate the overlap between cerebral regions in which abnormal activation occurs and those considered crucial for theory of mind (ToM), the ability to attribute mental states. The aim of this study was to explore whether FMD might be related to ToM disorders to the extent that they reduce the ability to make inferences about the mental states underlying motor behaviour during social interaction. Materials & Methods: Eighteen subjects with FMD and 28 matched healthy controls (HC) were given a ToM battery. The severity of FMD was rated by the Simplified-FMD Rating Scale (S-FMDRS). Dissociative symptoms were evaluated by the Dissociative Experiences Scale (DES-II). Results: FMD scored worse than the HC in most ToM tasks: second-order False Beliefs (p =.005), Faux-Pas Recognition Test (p <.001) and Reading the Mind in the Eyes Test (p =.020); control questions elicited normal scores. The DES-II indicated dissociative-borderline psychopathology and negatively correlated with accuracy on the second-order False Belief (Spearman's rho = −.444; p =.032); the positive correlation between DES-II and severity of motor symptoms (S-FMDRS) approached significance (Spearman's rho test =.392; p =.054). ToM disorders were not correlated with S-FMDRS, due to the typical variability in FMD over time with regard to the severity of symptoms and the district of body involved. Conclusions: Our results are consistent with the hypothesis that FMD are related to ToM deficits, and future studies are needed to define the specific nature of this relationship.

Abstract

COVID-19 has rapidly become a pandemic emergency, distressing health systems in each affected country. COVID-19 determines the need for healthcare in a large number of people in an extremely short time and, like a tsunami wave, overruns emergency, infectious diseases, and pneumology departments as well as intensive care units, choking healthcare services. Rehabilitation services are also affected by this epidemic which forces radical changes both in the organization and in the operating methods. In the absence of reference literature on this issue, this report aims to provide a background documentation to support physicians and healthcare personnel involved in neurorehabilitation and rehabilitation care.

Abstract

The number of children diagnosed with Autism Spectrum Disorder (ASD) has rapidly increased globally. Genetic and environmental factors both contribute to the development of ASD. Several studies showed linkage between prenatal, early postnatal air pollution exposure and the risk of developing ASD. We reviewed the available literature concerning the relationship between early-life exposure to air pollutants and ASD onset in childhood. We searched on Medline and Scopus for cohort or case-control studies published in English from 1977 to 2020. A total of 20 articles were selected for the review. We found a strong association between maternal exposure to particulate matter (PM) during pregnancy or in the first years of the children's life and the risk of the ASD. This association was found to be stronger with PM2.5 and less evident with the other pollutants. Current evidence suggest that pregnancy is the period in which exposure to environmental pollutants seems to be most impactful concerning the onset of ASD in children. Air pollution should be considered among the emerging risk factors for ASD. Further epidemiological and toxicological studies should address molecular pathways involved in the development of ASD and determine specific cause-effect associations.

Abstract

Background: The prevalence of anxiety is three times greater in patients with chronic diseases than in the general population. There has recently been growing interest in the roles of inflammation in contributing to the development of anxiety in people with Immune-mediated inflammatory diseases (IMID). Patient-reported outcome measures can facilitate the assessment of physical and psychological functioning. In recent years various patient outcome measures (PROMs) have been implemented. The National Institutes of Health (NIH)'s Patient-Reported Outcomes Measurement Information System (PROMIS®) is a set of publicly available and standardized Patient-Reported Outcomes (PROs) that cover physical appearance, mental health, and social health. The PROMIS® has been built through an Item Response Theory approach (IRT), a model-based measurement in which trait level estimates depend on both persons' responses and on the properties of the items that were administered. The aim of this study is to test the psychometric properties of the original anxiety items of the PROMIS® in a cohort of outpatients with IMIDs.Methods: We translated the original eight anxiety items from the PROMIS into Italian and administered them to consecutive outpatients affected by Inflammatory Bowel disease (n = 246), rheumatological (n = 100) and dermatological (n = 43) diseases, and healthy volunteers (n = 280). We analyzed the data through an Item Response Theory (IRT) analysis to evaluate the psychometric properties of the Italian adaptation of the PROMIS® anxiety short form.Results: Taken together, Confirmatory Factor Analysis and Exploratory Factor analysis suggested that the unidimensionality assumption if the instrument holds. The instrument has excellent reliability from a Classical Theory of Test (CTT) standpoint (Cronbach's ⍺ = .94, McDonald's ω = .95). The 1PL Graded Response Model (GRM) model provided a good balance between the goodness of fit and parsimony (BIC = 9099.82) as compared to the 2PL GRM model (BIC = 9111.02). We did not find signs of Differential Item Functioning (DIF), but we found that the local independence assumption was violated for some items. The analysis of the test reliability curve suggested that the instrument is most reliable for higher levels of the latent trait of anxiety.Discussion: The Italian adaptation of the PROMIS® anxiety short form shows acceptable psychometric properties both from a CTT and an IRT standpoint. Its compatibility with the 1 parameter logistic (1PL) model implies that its total score is a sufficient statistic for the latent anxiety trait. The TRC shows that this instrument is mostly informative for people with higher levels of anxiety, making it particularly suitable for clinical populations such as IMID patients.

Abstract

Biostimulants are emerging as a feasible tool for counteracting reduction in climate change-related yield and quality under water scarcity. As they are gaining attention, the necessity for accurately assessing phenotypic variables in their evaluation is emerging as a critical issue. In light of this, high-throughput phenotyping techniques have been more widely adopted. The main bottleneck of these techniques is represented by data management, which needs to be tailored to the complex, often multifactorial, data. This calls for the adoption of non-linear regression models capable of capturing dynamic data and also the interaction and effects between multiple factors. In this framework, a commercial glycinebetaine- (GB-) based biostimulant (Vegetal B60, ED&F Man) was tested and distributed at a rate of 6 kg/ha. Exogenous application of GB, a widely accumulated and documented stress adaptor molecule in plants, has been demonstrated to enhance the plant abiotic stress tolerance, including drought. Trials were conducted on tomato plants during the flowering stage in a greenhouse. The experiment was designed as a factorial combination of irrigation (water-stressed and well-watered) and biostimulant treatment (treated and control) and adopted a mixed phenotyping-omics approach. The efficacy of a continuous whole-canopy multichamber system coupled with generalized additive mixed modeling (GAMM) was evaluated to discriminate between water-stressed plants under the biostimulant treatment. Photosynthetic performance was evaluated by using GAMM, and was then correlated to metabolic profile. The results confirmed a higher photosynthetic efficiency of the treated plants, which is correlated to biostimulant-mediated drought tolerance. Abstract

SummaryEven though the Khorana risk score (KRS) has been validated to predict against the development of VTE among patients with cancer, it has a low positive predictive value. It is also unknown whether the score predicts outcomes in patients with cancer with established VTE. We selected a cohort of patients with active cancer from the RIETE (Registro Informatizado Enfermedad TromboEmbolica) registry to assess the prognostic value of the KRS at inception in predicting the likelihood of VTE recurrences, major bleeding and mortality during the course of anticoagulant therapy. We analysed 7948 consecutive patients with cancer-associated VTE. Of these, 2253 (28 %) scored 0 points, 4550 (57 %) 1–2 points and 1145 (14 %) scored ≥3 points. During the course of anticoagulation, amongst patient with low, moderate and high risk KRS, the rate of VTE recurrences was of 6.21 (95 %CI: 4.99–7.63), 11.2 (95 %CI: 9.91–12.7) and 19.4 (95 %CI: 15.4–24.1) events per 100 patient-years; the rate of major bleeding of 5.24 (95 %CI: 4.13–6.56), 10.3 (95 %CI: 9.02–11.7) and 19.4 (95 %CI: 15.4–24.1) bleeds per 100 patient-years and the mortality rate of 25.3 (95 %CI: 22.8–28.0), 58.5 (95 %CI: 55.5–61.7) and 120 (95 %CI: 110–131) deaths per 100 patient-years, respectively. The C-statistic was 0.53 (0.50–0.56) for recurrent VTE, 0.56 (95 %CI: 0.54–0.59) for major bleeding and 0.54 (95 %CI: 0.52–0.56) for death. In conclusion, most VTEs occur in patients with low or moderate risk scores. The KRS did not accurately predict VTE recurrence, major bleeding, or mortality among patients with cancer-associated thrombosis.

Abstract

OBJECTIVES: Previous literature showed that visual extinction and hemispatial neglect share common underlying mechanisms (Vuilleumier and Rafal, 2000), with common lesion patterns (Umarova et al., 2011). However, the relationship between visual extinction and hemispatial neglect is still not fully understood. In this study we used functional magnetic resonance imaging (fMRI) to identify brain regions underlying visual extinction in right-sided stroke patients with hemispatial neglect (N = 9) and related age-/gender-matched healthy control subjects (N = 25). MATERIALS AND METHODS: During fMRI, all participants were presented with a target stimulus (i.e., a black square of 2x2°; 10° apart from the central fixation point), that equiprobably appeared on either the left, right or both hemifields. Participants pressed one of three response buttons to indicate the hemifield(s) of target appearance, left, right, or bilateral. RESULTS: Control subjects were extremely accurate in target detection, irrespective of target side (left side: 99.5%, right side: 99.5%, bilateral: 97.8%). By contrast, neglect patients failed to detect bilateral targets (22.2% of accuracy). Importantly, â€oebilateral― targets were perceived by neglect patients as â€oeright― targets in the great majority of trials (56.3%). Moreover, neglect patients showed a decreased capability in detecting left (43.1%) vs. right targets (84.7%). The fMRI analysis revealed a greater involvement of the fronto-parietal cortex, bilaterally – despite most pronounced on the left hemisphere – in neglect patients as compared to healthy controls, irrespective of target side. DISCUSSION AND CONCLUSIONS: The increased activity of the fronto-parietal cortex appears to sustain the residual capabilities of neglect patients in detecting visual targets in the surrounding space. Overall, these findings provide new empirical evidence that visual extinction and hemispatial neglect are closely related, strengthening the notion that the lesion pattern underlying hemispatial neglect plays a major role in the phenomenology of visual extinction. Abstract

Background and purpose: The objective of this study was to assess the neurological manifestations in a series of consecutive severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2)-positive patients, comparing their frequency with a population hospitalized in the same period for flu/respiratory symptoms, finally not related to SARS-CoV-2. Methods: Patients with flu/respiratory symptoms admitted to Fondazione Policlinico Gemelli hospital from 14 March 2020 to 20 April 2020 were retrospectively enrolled. The frequency of neurological manifestations of patients with SARS-CoV-2 infection was compared with a control group. Results: In all, 213 patients were found to be positive for SARS-CoV-2, after reverse transcriptase polymerase chain reaction on nasal or throat swabs, whilst 218 patients were found to be negative and were used as a control group. Regarding central nervous system manifestations, in SARS-CoV-2-positive patients a higher frequency of headache, hyposmia and encephalopathy always related to systemic conditions (fever or hypoxia) was observed. Furthermore, muscular involvement was more frequent in SARS-CoV-2 infection. Conclusions: Patients with COVID-19 commonly have neurological manifestations but only hyposmia and muscle involvement seem more frequent compared with other flu diseases.

Abstract

Background The current global and national indications for antiretroviral treatment (ART, usually triple combination therapy) in adolescent and adults, including pregnant women, recommend early ART before immunologic decline, pre-exposure chemoprophylaxis (PrEP), and treatment of HIV-negative partners in serodiscordant couples. There is limited information on the implementation of these recommendations among pregnant women with HIV and their partners. Methods The present analysis was performed in 2016, using data from clinical records of pregnant women with HIV, followed between 2001 and 2015 at hospital or university clinics within a large, nationally representative Italian cohort study. The study period was divided in three intervals of five years each (2001-2005, 2006-2010, 2011-2015), and the analysis evaluated temporal trends in rates of HIV diagnosis in pregnancy, maternal antiretroviral treatment at conception, prevalence of HIV infection among partners of pregnant women with HIV, and proportion of seronegative and seropositive male partners receiving antiretroviral treatment. Results The analysis included 2755 pregnancies in women with HIV. During the three time intervals considered the rate of HIV diagnosis in pregnancy (overall 23.3%), and the distribution of HIV status among male partners (overall 48.7% HIV- negative, 28.6% HIV-positive and 22.8% unknown) remained substantially unchanged. Significant increases were observed in the proportion of women with HIV diagnosed before pregnancy who were on antiretroviral treatment at conception (from 62.0% in 2001-2005 to 81.3% in 2011-2015, P < 0.001), and in the proportion of HIV-positive partners on antiretroviral treatment (from 73.3% in 2001-2005 to 95.8% in 2011-2015, P = 0.002). Antiretroviral treatment was administered in 99.1% of the pregnancies that did not end early because of miscarriage, termination, or intrauterine death, and in 75.3% of those not ending in a live birth. No implementation of antiretroviral treatment was introduced among male HIV-negative partners. Conclusions The results suggest good implementation of antiretroviral treatment among HIV-positive women and their HIV-positive partners, but no implementation, even in recent years, of Pre-Exposure Prophylaxis (PrEP) among uninfected male partners. Further studies should assess the determinants of this occurrence and clarify the attitudes and the potential barriers to PrEP use.

Abstract

Purpose: To evaluate the association of hepatitis B virus (HBV) genotypes, basal core promoter (BCP)/precore (PC) and S gene mutations with the clinical-epidemiological characteristics of acute hepatitis B (AHB) in Italy. Methods: During July 2005–January 2007, 103 symptomatic AHB patients were enrolled and prospectively followed up at 15 national hospitals. HBV genotypes, BCP/PC and S gene variants were determined by nested-PCR and direct sequence analysis. Results: Genotype D, A and F were detected in 49, 45 and 6% of patients, respectively. BCP, PC, and BCP plus PC variants were found in 3.1, 11.3 and 7.2% of patients, respectively. At enrollment, 68.3% of patients were hepatitis B e antigen (HBeAg)-positive and 31.7% HBeAg-negative. Abstract

Background The current global and national indications for antiretroviral treatment (ART, usually triple combination therapy) in adolescent and adults, including pregnant women, recommend early ART before immunologic decline, pre-exposure chemoprophylaxis (PrEP), and treatment of HIV-negative partners in serodiscordant couples. There is limited information on the implementation of these recommendations among pregnant women with HIV and their partners. Methods The present analysis was performed in 2016, using data from clinical records of pregnant women with HIV, followed between 2001 and 2015 at hospital or university clinics within a large, nationally representative Italian cohort study. The study period was divided in three intervals of five years each (2001-2005, 2006-2010, 2011-2015), and the analysis evaluated temporal trends in rates of HIV diagnosis in pregnancy, maternal antiretroviral treatment at conception, prevalence of HIV infection among partners of pregnant women with HIV, and proportion of seronegative and seropositive male partners receiving antiretroviral treatment. Results The analysis included 2755 pregnancies in women with HIV. During the three time intervals considered the rate of HIV diagnosis in pregnancy (overall 23.3%), and the distribution of HIV status among male partners (overall 48.7% HIV- negative, 28.6% HIV-positive and 22.8% unknown) remained substantially unchanged. Significant increases were observed in the proportion of women with HIV diagnosed before pregnancy who were on antiretroviral treatment at conception (from 62.0% in 2001-2005 to 81.3% in 2011-2015, P < 0.001), and in the proportion of HIV-positive partners on antiretroviral treatment (from 73.3% in 2001-2005 to 95.8% in 2011-2015, P = 0.002). Abstract

The association between inferior vena cava filter (IVC) use and outcome in patients presenting with major bleeding during anticoagulation for venous thromboembolism (VTE) has not been thoroughly investigated. We used the RIETE registry to compare the 30-day outcomes (death, major re-bleeding or VTE recurrences) in VTE patients who bled during the first 3 months of therapy, regarding the insertion of an IVC filter. A propensity score matched (PSM) analysis was performed to adjust for potential confounders. From January 2001 to September 2016, 1065 VTE patients had major bleeding during the first 3 months of anticoagulation (gastrointestinal 370; intracranial 124). Of these, 122 patients (11%) received an IVC filter. Patients receiving a filter restarted anticoagulation later (median, 4 vs. 2 days) and at lower doses (95 +/- 52 IU/kg/day vs. 104 +/- 55 of low-molecular-weight heparin) than those not receiving a filter. During the first 30 days after bleeding (after excluding 246 patients who died within the first 24 h), 283 patients (27%) died, 63 (5.9%) had non-fatal re-bleeding and 19 (1.8%) had recurrent pulmonary embolism (PE). In PSM analysis, patients receiving an IVC filter (n = 122) had a lower risk for all-cause death (HR 0.49; 95% CI 0.31-0.77) or fatal bleeding (HR 0.16; 95% CI 0.07-0.49) and a similar risk for re-bleeding (HR 0.55; 95% CI 0.23-1.40) or PE recurrences (HR 1.57; 95% CI 0.38-6.36) than those not receiving a filter (n = 429). In VTE patients experiencing major bleeding during the first 3 months, use of an IVC filter was associated with reduced mortality rates. Clinical Trial Registration NCT02832245.
