19. Elevated Serum Leptin Levels: A Risk Factor for Non-Small-Cell Lung Cancer?

Abstract

Objective: Leptin is intimately intertwined in the molecular pathophysiology of several cancer types; with regard to lung cancer, however, limited research has been conducted, with overall conflicting results. Methods: The present case-control study comprises 66 non-small-cell lung cancer (NSCLC) cases and 132 healthy controls matched for gender and age. Lifestyle, sociodemographic and medical history information has been obtained in addition to body mass index (BMI) measurements and weight change during the last 2 months. Serum leptin and adiponectin levels were determined following a standard protocol. Results: In multiple logistic regression analyses, elevated serum leptin emerged as a risk factor for NSCLC independent of central obesity, more pronounced after controlling for BMI and recent weight loss (odds ratio = 4.58, 95% confidence interval: 1.94-10.82). Additionally, smoking and animal foods consumption were strongly associated with the disease, whereas plant foods consumption showed a protective association. Conclusions: The observed higher serum leptin levels in NSCLC cases might be attributed to direct or indirect effects mediated by cancer-or cachexia-related cytokines. In line with the growth-promoting properties of leptin in the lung tissue documented elsewhere, increased serum leptin concentration may represent a tumor-promoting event during non-small-cell lung carcinogenesis. Abstract

An important question in geophysics is whether earthquakes (EQs) can be anticipated prior to their occurrence. Pre-seismic electromagnetic (EM) emissions provide a promising window through which the dynamics of EQ preparation can be investigated. However, the existence of precursory features in pre-seismic EM emissions is still debatable: in principle, it is difficult to prove associations between events separated in time, such as EQs and their EM precursors. The scope of this paper is the investigation of the pre-seismic EM activity in terms of complexity. A basic reason for our interest in complexity is the striking similarity in behavior close to irreversible phase transitions among systems that are otherwise quite different in nature. Interestingly, theoretical studies (Hopfield, 1994; Herz and Hopfield 1995; Rundle et al. 1995; Corral et al. 1997) suggest that the EQ dynamics at the final stage and neural seizure dynamics should have many similar features and can be analyzed within similar mathematical frameworks. Motivated by this hypothesis, we evaluate the capability of linear and non-linear techniques to extract common features from brain electrical activities and pre-seismic EM emissions predictive of epileptic seizures and EQs respectively. Abstract

ObjectivesThe purpose of this study was to quantitatively assess the relationship between therapy-induced changes in left ventricular (LV) remodeling and longer-term outcomes in patients with left ventricular dysfunction (LVD).BackgroundWhether therapy-induced changes in left ventricular ejection fraction (LVEF), end-diastolic volume (EDV), and end-systolic volume (ESV) are predictors of mortality in patients with LVD is not established.MethodsSearches for randomized controlled trials (RCTs) were conducted to identify drug or device therapies for which an effect on mortality in patients with LVD was studied in at least 1 RCT of ≥500 patients (mortality trials). Then, all RCTs involving those therapies were identified in patients with LVD that described changes in LVEF and/or volumes over time (remodeling trials). We examined whether the magnitude of remodeling effects is associated with the odds ratios for death across all therapies or associated with whether the odds ratio for mortality was favorable, neutral, or adverse (i.e., statistically significantly decreased, nonsignificant, or statistically significantly increased odds for mortality, respectively).ResultsIncluded were 30 mortality trials of 25 drug/device therapies (n = 69,766 patients; median follow-up 17 months) and 88 remodeling trials of the same therapies (n = 19,921 patients; median follow-up 6 months). The odds ratio for death in the mortality trials was correlated with drug/device effects on LVEF (r = −0.51, p < 0.001), EDV (r = 0.44, p = 0.002), and ESV (r = 0.48, p = 0.002). In (ordinal) logistic regressions, the odds for neutral or favorable effects in the mortality RCTs increased with mean increases in LVEF and with mean decreases in EDV and ESV in the remodeling trials.ConclusionsIn patients with LVD, short-term trial-level therapeutic effects of a drug or device on LV remodeling are associated with longer-term trial-level effects on mortality.
