1. Use of Telehealth for Domiciliary Follow-up After Hematopoietic Cell Transplantation During the COVID-19 Pandemic: Prospective Pilot Study

Abstract

BackgroundPatients who have recently received a hematopoietic cell transplant (HCT) are at higher risk of acute complications in the first weeks after discharge, especially during the COVID-19 pandemic. ObjectiveThe aim of this study was to test the use of a telehealth platform for the follow-up of HCT patients during the first two weeks after discharge. MethodsIn total, 21 patients who received autologous or allogeneic HCT for hematological malignancies were screened from April 30, 2020, to July 15, 2020. The telehealth platform assisted in the daily collection of vital signs as well as physical and psychological symptoms for two weeks after hospital discharge. The required medical devices (oximeter and blood pressure monitor) were given to patients and a dedicated smartphone app was developed to collect this data. The data were reviewed daily through web-based software by a hematologist specializing in HCT. ResultsOnly 12 of 21 patients were able to join and complete the study. Technological barriers were the most frequent limiting factor in this study. Among the 12 patients who completed the study, adherence to data reporting was high. The patients’ experience of using such a system was considered good. In two cases, the system enabled the early recognition of acute complications. ConclusionsThis pilot study showed that telehealth systems can be applied in the early posttransplant setting, with evident advantages for physicians and patients for both medical and psychological aspects. Technological issues still represent a challenge for the applicability of such a system, especially for older adult patients. Easier-to-use technologies could help to expand the use of telehealth systems in this setting in the future.

Abstract

[ES]Los hospitales de nuestro entorno tienen un aspecto muy próximo al de los laboratorios científicos, alejado del ámbito humano, y no resultan especialmente acogedores. Es por ello que con este Trabajo Fin de Máster se pretende investigar el papel que pueda ejercer la madera en el proceso de humanización de la arquitectura hospitalaria y estudiar cómo hacerlo posible. Para ello se hace una revisión bibliográfica de la literatura publicada sobre el tema estudiando sus convenientes e inconvenientes, haciendo especial hincapié en lo que se refiere al comportamiento higiénico de la madera debido a la importancia de esté en un entorno hospitalario. De la misma manera, se dedica un apartado especial al estudio de los requerimientos técnicos a cumplir por el material en cuanto al fuego y las opciones que para ello nos ofrece el mercado actualmente. Para cerrar el apartado más teórico del trabajo, se hace una recopilación de referentes que han usado en mayor o menor medida la madera en el ámbito hospitalario en los últimos años. A modo de conclusión, se vuelca todo lo investigado mediante la resolución práctica de un caso teórico, el proyecto de la estructura realizada en madera de la ampliación del hospital San Eloy de Barakaldo., [EU]Gure inguruko ospitaleek zientzia laborategien antz handia dute, giza eremutik urrun, eta ez dira bereziki erosoak. That is the reason why this Master's Thesis aims to investigate the role timber could play in the process of hospital architecture humanisation and how to formalize it. To this end, a bibliographical review of the published literature is made, studying its advantages and disadvantages with special emphasis on the hygienic behaviour of wood due to its importance in a hospital environment. In the same way, a special section is dedicated to technical requirements wood needs to fulfil in terms of fire and the solutions the market currently offers. To close the more theorical portion of the document, a compilation of hospital architecture references that have used timber to a greater or lesser extent in recent years. As a conclusion, all the research previously done is presented through the practical resolution of a theoretical case, San Eloy Hospital's ampliation's timber structure project.

Abstract

Frailty is a crucial indicator in determining the well-being of older adults in terms of their health. With the growing number of elderly people, the demand for geriatricians is increasing, which means that they have less time to spend with each patient. The current methods for frailty assessment use simple tests that are time-consuming and do not require specific medical expertise. To address this issue, this paper proposes the use of social robots to assess frailty autonomously. It presents a practical proposal that defines the robot’s behavior and explains the design and implementation concepts. Tractament mèdic; Tractament amb irradiació; Mieloma múltiple Medical treatment; Irradiation treatment; Multiple myeloma Tratamiento médico; Tratamiento con irradiación; Mieloma múltiple El mieloma múltiple (MM) és una neoplàsia de cèl·lules plasmàtiques que representa al voltant de l’1% del total de neoplàsies i el 10% de les neoplàsies hematològiques. Té una incidència aproximada de 4-5 nous casos/100.000 habitants/any i presenta una incidència màxima entre els 70 i 75 anys d’edat. Un 35% dels afectats té menys de 65 anys. Els objectius d'aquesta guia són: desenvolupar i difondre la ICO-ICSPraxi per al tractament del mieloma múltiple; disminuir la variabilitat terapèutica entre els pacients tractats en els diferents centres d’aquesta institució; implementar i avaluar els resultats de la terapèutica en els pacients amb mieloma múltiple tractats d’acord amb les recomanacions d’aquesta guia.

Abstract

The study aimed to identify predictors of overall 30-day mortality in cancer patients with pulmonary embolism including suspected pulmonary embolism (SPE) and unsuspected pulmonary embolism (UPE) events. Secondary outcomes included 30- and 90-day major bleeding and venous thromboembolism (VTE) recurrence.The study cohort included 1033 consecutive patients with pulmonary embolism from the multicentre observational ambispective EPIPHANY study (March 2006–October 2014). A subgroup of 497 patients prospectively assessed for the study were subclassified into three work-up scenarios (SPE, truly asymptomatic UPE and UPE with symptoms) to assess outcomes.The overall 30-day mortality rate was 14%. The following variables were associated with the overall 30-day mortality on multivariate analysis: VTE history, upper gastrointestinal cancers, metastatic disease, cancer progression, performance status, arterial hypotension 110 beats·min−1, basal oxygen saturation versus overall UPE).The overall 30-day mortality was significantly lower in patients with truly asymptomatic UPE events (3%) compared with those with UPE-S (20%) and SPE (21%) (pIn conclusion, variables associated with the severity of cancer and pulmonary embolism were associated with short-term mortality. Our findings may help to develop pulmonary embolism risk-assessment models in this setting.

Abstract

BACKGROUND: Population ageing is a relevant concern in people diagnosed with rectal cancer. This study evaluates the adherence to clinical practice guidelines (CPGs) and patient outcomes in rectal cancer, with a particular focus on variation according to age. METHODS: This is a multicentre retrospective cohort study of all patients surgically treated for the first time for primary rectal cancer with curative intent in public hospitals in Catalonia during two study periods: first, with data from 2005 to 2007, and then with data from 2011 to 2012. We obtained the study data through a comprehensive review of patients' clinical records. Information on diagnosis, treatment and follow-up was collected and then compared with the relevant CPGs. We then performed a descriptive analysis of the categorical variables followed by a univariate and multivariate logistic regression analysis to calculate the odds ratio. Hole Drilling es una técnica de relajación de tensiones residuales conocida por su aplicación en otros materiales tecnológicos. Esta Tesis Doctoral afirma que la técnica es apta y útil para medir tensiones de origen estructural en elementos portantes de madera. Se ha ensayado en madera de pino radiata procedente de la CAPV pero es extensible a otras especies, como el roble.La técnica de aplicación in situ es capaz de determinar el estado tensional real y actualizado de un determinado elemento estructural de madera. La nueva aplicación de la técnica puede ayudar a los técnicos competentes en la inspección y la toma de decisiones en los proyectos de restauración o rehabilitación estructural. Se fomenta la conservación de elementos estructurales de madera, que de otra manera serían sustituidos por otros. En la Tesis Doctoral se han realizado ensayos de laboratorio en piezas estructurales tanto a compresión como a flexión y se ha aplicado, finalmente, en estructuras reales de edificios existentes, dando lugar a resultados muy satisfactorios.

Abstract

Background: We sought to develop and externally validate a nomogram and web-based calculator to individually predict the development of serious complications in seemingly stable adult patients with solid tumours and episodes of febrile neutropenia (FN). Patients and methods: The data from the FINITE study (n = 1133) and University of Salamanca Hospital (USH) FN registry (n = 296) were used to develop and validate this tool. The main eligibility criterion was the presence of apparent clinical stability, defined as events without acute organ dysfunction, abnormal vital signs, or major infections. Discriminatory ability was measured as the concordance index and stratification into risk groups. Results: The rate of infection-related complications in the FINITE and USH series was 13.4% and 18.6%, respectively. The nomogram used the following covariates: Eastern Cooperative Group (ECOG) Performance Status >= 2, chronic obstructive pulmonary disease, chronic cardiovascular disease, mucositis of grade >= 2 (National Cancer Institute Common Toxicity Criteria), monocytes 0.1). The concordance index was 0.855 and 0.831 in each series. Risk group stratification revealed a significant distinction in the proportion of complications. With a >= 116-point cutoff, the nomogram yielded the following prognostic indices in the USH registry validation series: 66% sensitivity, 83% specificity, 3.88 positive likelihood ratio, 48% positive predictive value, and 91% negative predictive value. Conclusions: We have developed and externally validated a nomogram and web calculator to predict serious complications that can potentially impact decision-making in patients with seemingly stable FN.

Abstract

We conducted a prospective study to assess the risk factors, molecular epidemiology and outcome of bloodstream infection (BSI) due to Enterococcus faecium in hospitalized cancer patients. Between 2006 and 2012, a significant increase in vancomycin-susceptible E. faecium BSI was observed among cancer patients. Comparison of 54 episodes of BSI due to E. faecium with 38 episodes of BSI due to E. faecalis showed that previous use of carbapenems was the only independent risk factor for E. faecium acquisition (OR 10.24; 95% CI, 1.35-77.66). All E. faecium isolates were susceptible to glycopeptides, whereas 97% showed high-level resistance to ampicillin and ciprofloxacin. All 30 isolates available for genotyping belonged to the hospital-associated E. faecium lineages 17, 18 and 78. After 2009, most of the isolates belonged to ST117 (lineage 78). Patients with E. faecium BSI were more likely to receive inadequate initial empirical antibiotic therapy than patients with E. faecalis BSI, and time to adequate empirical antibiotic therapy was also longer in the former group. No significant differences were found between the two groups regarding early and overall case-fatality rates. Independent risk factors for overall case-fatality were current corticosteroids (OR 4.18; 95% CI, 1.34-13.01) and intensive care unit admission (OR 9.97; 95% CI, 1.96-50.63). The emergence of E. faecium among cancer patients is a concern since there are limited treatment options and it may presage the emergence of vancomycin-resistant enterococci. Abstract

The study aimed to identify predictors of overall 30-day mortality in cancer patients with pulmonary embolism including suspected pulmonary embolism (SPE) and unsuspected pulmonary embolism (UPE) events. Secondary outcomes included 30- and 90-day major bleeding and venous thromboembolism (VTE) recurrence.The study cohort included 1033 consecutive patients with pulmonary embolism from the multicentre observational ambispective EPIPHANY study (March 2006-October 2014). A subgroup of 497 patients prospectively assessed for the study were subclassified into three work-up scenarios (SPE, truly asymptomatic UPE and UPE with symptoms) to assess outcomes.The overall 30-day mortality rate was 14%. The following variables were associated with the overall 30-day mortality on multivariate analysis: VTE history, upper gastrointestinal cancers, metastatic disease, cancer progression, performance status, arterial hypotension <100 mmHg, heart rate >110 beats·min -1 , basal oxygen saturation <90% and SPE (versus overall UPE).The overall 30-day mortality was significantly lower in patients with truly asymptomatic UPE events (3%) compared with those with UPE-S (20%) and SPE (21%) (p<0.0001). Thirty- and 90-day VTE recurrence and major bleeding rates were similar in all the groups.In conclusion, variables associated with the severity of cancer and pulmonary embolism were associated with short-term mortality.
