Background Cities are an important driving force to implement the Sustainable Development Goals (SDGs) and the New Urban Agenda. The SDGs provide an operational framework to consider urbanization globally, while providing local mechanisms for action and careful attention to closing the gaps in the distribution of health gains. While health and well-being are explicitly addressed in SDG 3, health is also present as a pre condition of SDG 11, that aims at inclusive, safe, resilient and sustainable cities. Health in All Policies (HiAP) is an approach to public policy across sectors that systematically takes into account the health implications of decisions, seeks synergies, and avoids harmful health impacts in order to improve population health and health equity. HiAP is key for local decision-making processes in the context of urban policies to promote public health interventions aimed at achieving SDG targets. HiAPs relies heavily on the use of scientific evidence and evaluation tools, such as health impact assessments (HIAs). HIAs may include city-level quantitative burden of disease, health economic assessments, and citizen and other stakeholders’ involvement to inform the integration of health recommendations in urban policies. The Barcelona Institute for Global Health (ISGlobal)‘s Urban Planning, Environment and Health Initiative provides an example of a successful model of translating scientific evidence into policy and practice with regards to sustainable and healthy urban development. The experiences collected through ISGlobal’s participation implementing HIAs in several cities worldwide as a way to promote HiAP are the basis for this analysis. Aim The aim of this article is threefold: to understand the links between social determinants of health, environmental exposures, behaviour, health outcomes and urban policies within the SDGs, following a HiAP rationale; to review and analyze the key elements of a HiAP approach as an accelerator of the SDGs in the context of urban and transport planning; and to describe lessons learnt from practical implementation of HIAs in cities across Europe, Africa and Latin-America. Methods We create a comprehensive, urban health related SDGs conceptual framework, by linking already described urban health dimensions to existing SDGs, targets and indicators. We discuss, taking into account the necessary conditions and steps to conduct HiAP, the main barriers and opportunities within the SDGs framework. We conclude by reviewing HIAs in a number of cities worldwide (based on the experiences collected by co-authors of this publication), including city-level quantitative burden of disease and health economic assessments, as practical tools to inform the integration of health recommendations in urban policies. Results A conceptual framework linking SDGs and urban and transportplanning, environmental exposures, behaviour and health outcomes, following a HiAP rationale, is designed. We found at least 38 SDG targets relevant to urban health, corresponding to 15 SDGs, while 4 important aspects contained in our proposed framework were not present in the SDGs (physical activity, noise, quality of life or social capital). Thus, a more comprehensive HiAP vision within the SDGs could be beneficial. Our analysis confirmed that the SDGs framework provides an opportunity to formulate and implement policies with a HiAP approach. Three important aspects are highlighted: 1) the importance of the intersectoral work and health equity as a cross-cutting issue in sustainable development endeavors; 2) policy coherence, health governance, and stakeholders’ participation as key issues; and 3) the need for high quality data. HIAs are a practical tool to implement HiAP. Opportunities and barriers related to the political, legal and health governance context, the capacity to inform policies in other sectors, the involvement of different stakeholders, and the availability of quality data are discussed based on our experience. Quantitative assessments can provide powerful data such as: estimates of annual preventable morbidity and disability-adjusted life-years (DALYs) under compliance with international exposure recommendations for physical activity, exposure to air pollution, noise, heat, and access to green spaces; the associated economic impacts in health care costs per year; and the number of preventable premature deaths when improvements in urban and transport planning are implemented. This information has been used to support the design of policies that promote cycling, walking, public, zero and low-emitting modes of transport, and the provision of urban greening or healthy public open spaces in Barcelona (e.g. Urban Mobility, Green Infrastructure and Biodiversity Plans, or the Superblocks’s model), the Bus Rapid Transit and Open Streets initiatives in several Latin American cities or targeted SDGs assessments in Morocco. Conclusions By applying tools such as HIA, HiAP can be implemented to inform and improve transport and urban planning to achieve the 2030 SDG Agenda. Such a framework could be potentially used in cities worldwide, including those of less developed regions or countries. Data availability, taking into account equity issues, strenghtening the communication between experts, decision makers and citizens, and the involvement of all major stakeholders are crucial elements for the HiAP approach to translate knowledge into SDG implementation.

INTRODUCTION NlmCategory: BACKGROUND content: In the current mobility and globalization context, there is a growing need to identify potential changes on the pattern of diseases in the European Union (EU)/European Economic Area (EEA) and provide accurate diagnosis and treatment for the population. The pattern of rare communicable diseases that can affect people returning to EU/EEA from travel abroad, visiting EU/EEA or establishing in the EU/EEA is of special relevance. The objective of this manuscript is to give an overview about the EURaDMoG study and discuss the feasibility of establishing a European network on rare communicable diseases and other rare conditions linked to mobility and globalization. - Label: METHODS NlmCategory: METHODS content: We undertook a three-steps process where we first conducted a narrative review to estimate the prevalence and incidence and to list rare communicable and non-communicable diseases linked to mobility and globalization in the EU/EEA; second, we organized an international consultation workshop with experts in the diseases previously selected; and finally, the feasibility study analysed how successful a European expert network on rare diseases linked to mobility and globalization focused on health care provision would be, accounting for different operational and also sustainability criteria. - Label: RESULTS NlmCategory: RESULTS content: "First, considering the areas or topics that the network should cover, it was concluded that communicable and non-communicable rare diseases linked to mobility and globalization should be differentiated. Many actors in the response to COVID-19 are holding out for a vaccine to be developed. But in the meantime, tried and tested public-health measures for controlling outbreaks can be implemented. A scorecard can be used to assess governments’ responses to the outbreak.

Coronavirus disease 2019 (COVID-19) is a novel viral disease caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), which was first detected in Wuhan, China, in December, 2019.1 Given the fast spread, the severity of disease, the increasing number of cases outside China, and the number of affected countries, WHO declared the rapid spread of SARS-CoV-2 a pandemic on March 11, 2020.2 The availability of reliable surveillance platforms is crucial to monitor the COVID-19 epidemic in a timely manner and to respond with adequate control measures. Since the beginning of the outbreak, different countries have used different testing approaches and criteria, depending on their resources and capacity.

Background: The Health Product Profile Directory (HPPD), is an online database describing 8-10 key characteristics (such as target population, measures of efficacy and dosage) of product profiles for medicines, vaccines, diagnostics and other products that are intended to be accessed by populations in low- and middle-income countries. It was developed by TDR on behalf of WHO and launched on 15 May 2019. Methods: The contents of the HPPD was downloaded into an ExcelTM spreadsheet via the open access interface and analysed to identify the number of health product profiles by type, by disease, by year of publication, by status, by author organization and safety information. Findings: The HPPD contains summaries of 215 health product profiles published between 2008 and May 2019, 117 (54%) provide a hyperlink to the detailed publication from which the summary was extracted, the remaining 98 provide an email contact for further information. 55 target disease or health conditions are covered with 210 profiles describing a product with an infectious disease as the target. Only 5 product profiles in the HPPD describe a product for a non-communicable disease. Four diseases account for 40% of product profiles in the HPPD: TB 33 profiles (15%), Malaria 31 profiles (14%) HIV 13 profiles (6%) ad Chagas 10 profiles (5%). Interpretation: the HPPD provides a new tool to inform priority setting in global health - it includes all product profiles authored by WHO (51). There is a need to standardise nomenclature to more clearly distinguish between strategic publications (describing R&D priorities or preferred characteristics) compared to target product profiles to guide a specific candidate product undergoing R&D. Combining the analysis from HPPD to a mapping of funds available for R&D and those products in the R&D pipeline would create a better overview of global health priorities and how they are supported. Such analysis and increased transparency should take us another step closer to measuring and improving coordination of efforts in global health R&D. Resumen La reciente epidemia del virus Zika ha hecho evidente el riesgo de introducción de arbovirosis en Europa, especialmente en la región mediterránea, donde el vector Aedes albopictus se ha establecido como especie invasora. En dicho contexto, se realizó una evaluación integral del riesgo de introducción y transmisión autóctona del virus Zika y otros arbovirus de importancia para la salud pública en Cataluña. Se resumen los aspectos más importantes de dicha evaluación, así como las principales conclusiones y recomendaciones para la preparación y la respuesta en salud pública frente a la amenaza que suponen las arbovirosis emergentes.

Urban planning and related policies can contribute to improvement in health. Recent epidemiological and quantitative Health Impact Assessment (HIA) studies in Europe and North America suggest that a change from passive (car) to active transportation (cycling, walking) and public transport in daily life could improve health. HIA studies are still largely lacking in low and middle-income countries. We conducted a scoping study to evaluate the availability of data to conduct quantitative HIA in two cities from two low-income countries. We collected information through interviews with different local agents, from the National Institute of Statistics and by conducting field work to identify the built environment and mobility characteristics in the respective cities. Conducting a quantitative HIA in Maputo (Mozambique) is currently not possible, mainly because there is no appropriate data on mortality, road traffic accidents and physical activity of the general population. However, in Cochabamba (Bolivia) it might be possible when the mobility plan will be available (currently under development), in which data on traffic flows, mobility surveys and transport modal shares will become available. The current paper describes two examples of the opportunities and difficulties to conduct quantitative HIA in low- and middle-income countries, highlighting the limited availability of data (quantitatively and qualitatively) on transport and urban planning and health outcomes.

The prevalence of protective antibody levels (>160 mIU/ml) in neonates was 98.5%. The mean measles virus antibody level was 3,406 mIU/ml and increased with maternal age. Measles vaccination was reported by 42% of pregnant women and decreased with age.

In 2000, the circulation of indigenous measles was interrupted in Catalonia as a result of the activities of the elimination programme introduced in 1991. However, various transmission chains occurred in the indigenous population after the introduction of an imported case, and this caused a total change in the existing situation of elimination. The objective of this study was to analyze the epidemiological and clinical characteristics of the measles outbreak in Catalonia in 2006. Cases reported between August 28 2006 and July 8 2007 were collected. Cases were microbiologically confirmed by determination of measles-specific IgM and/or detection of the viral genome in urine samples. A total of 381 cases of measles were confirmed, representing an incidence of 6.6 per 100,000 inhabitants. Laboratory confirmation was made in 87.1% of the cases and all specimens sequenced belonged to the D4 genotype. Aims: To describe the prevalence of recent use of alcohol, medication, and illegal drugs among patients who attended emergency departments (EDs) as a result of suffering an injury due to any external mechanism and to identify factors associated with alcohol and drug use. Methods: A cross-sectional study was conducted in eight university hospitals in Spain. Participants were adult patients admitted to a trauma ED. Oral fluid was used to test for psychoactive substances analyzed by gas chromatography-mass spectrometry. Socio-demographic data and information on circumstances of the injury were collected through interviews. Results: The analysis included 1579 patients admitted to the EDs (56.4% men). Among Young people (= 40 years, 7.4% of men and 1.6% of women were positive for any illegal substance, and 16.3% and 11.0% respectively for alcohol. Prevalence of substance detected varied across mechanism of injury, gender and age group. Night-time injury was associated with substance use. Conclusions: A high proportion of injured patients who were treated in an EDs tested positive for psychoactive drugs. Routine testing at trauma departments would maximize the identification of patients who may benefit from referral to specialized addiction treatment centers, or brief interventions. Background In recent years there has been a substantial reduction in hepatitis B incidence as a result of routine vaccination of preadolescents and the selective vaccination of high risk groups and newborns of HBsAg+ mothers. Objectives To determine the prevalence of hepatitis B virus infection markers and of serologic markers of hepatitis B vaccination in a representative sample of pregnant women in Catalonia. Study design A representative sample was obtained by random cluster sampling (hospitals with maternity units) stratified by provinces. Anti-HBc, anti-HBs and HBsAg were determined using an ELISA test (Behring, Marburg, Germany). The crude and adjusted odds ratios for the sociodemographic variables analyzed were also calculated. Results The global prevalence of carriers of HBsAg+ was 0.1% (95% CI 0.0–0.3), that of hepatitis B infection (anti-HBc+) was 5% (95% CI 3.9–6.1) and that of serologic markers of vaccination (anti-HBs+ and anti-HBc−) was 16.4% (95% CI 14.5–18.2) The prevalence of infection increased with age from 3.0% (15–19 years age group) to 14.8% (40–44 years age group). The prevalence of anti-HBc+ was higher in women born in Asia (27.6%) and Africa (18.8%) than in those born in Spain (3.7%), Europe (3.3%) and America (4.6%), with the differences being statistically significant. Conclusions The results of this study confirm the change in the pattern of endemicity of hepatitis B infection in pregnant women in Catalonia after the introduction of vaccination programmes. They also confirm the increasingly important role played by immigration in the epidemiology of hepatitis B.

Summary. A universal vaccination program for preadolescents, aged 12 years, with the hepatitis A + B vaccine was introduced in 1998 in Catalonia (Spain) with the aim of protecting the whole population against hepatitis A. The hepatitis A + B vaccine program replaced the hepatitis B vaccination program for preadolescent started in 1991. The impact of the hepatitis A + B vaccination program was studied by assessment of the trend of reported cases of hepatitis A. All cases of viral hepatitis reported from 1992 to 2006 were included in the study. To evaluate changes in the epidemiology of hepatitis A, two periods were considered: a prevaccination period (1992–1998) and a post-vaccination period (2001–2006). The ratios of the rates were calculated according to age and sex. The comparison of rates and proportions was made by calculation of the normal z statistic. A total of 7536 cases of viral hepatitis were reported, of which 4109 (54.52%) were hepatitis A. The incidence rate of hepatitis A fell from 5.44 per 100 000 person-years in the prevaccination period to 3.02 in the post-vaccination period. In males, the rate fell from 6.85 to 3.89 and in females from 4.10 to 2.18. The male-female ratio of incidence rates was lower in the post-vaccination period. In males the global decline of incidence rate was 43.26% and in females 46.96%. The greatest decline occurred in the 15 to 19 years age group in both sexes (79.1% in men and 78.34% in women) but declines in the 10–14 years age group were also very important (69.21% and 67.88%, respectively). In conclusion, hepatitis A incidence fell in Catalonia in the post-vaccination period in vaccinated adolescents and also in other unvaccinated groups who have benefited from the indirect effects of the vaccination program.

The normalized difference vegetation index (NDVI) is often used as a marker of surrounding greenness in epidemiological studies aiming to evaluate the health effects of green space in urban settings. However, it is not clear the relationship between built environment characteristics, including green space, and NDVI. We aimed to evaluate the relationship between built environment characteristics, based on land-use and land-cover maps, and NDVI as a marker of surrounding greenness in the city of Barcelona. We used data from an already existing cohort of pregnant women in Barcelona (N = 8402). NDVI was derived and averaged within buffers of 100 m and 300 m for each participant, and categories of the built environment (m2) were derived from land-use and land-cover maps of Barcelona. We conducted ANOVA models to calculate the contribution (R2) of each land-use (or land-cover) category. The variability in NDVI in Barcelona was mainly explained by urban green (R2 between 0.32 and 0.53) and natural green areas (R2 between 0.19 and 0.52), although for the latter less than 4% of the participants were exposed to this. Both land-use and land-cover maps explained NDVI at 300 m better (full models explaining 76% and 78%, respectively) than at 100 m buffers (full models explaining 55% and 54%, respectively). Results of the present study indicate that NDVI can be a useful greenness metric depending on the hypothesis and area of study. However, for certain sizes of study areas (buffers smaller than 100 m), NDVI might have a lower predictive value. Results of the present study should be replicated in studies from other cities with different urban characteristics and climate conditions.

To analyze the prevalence of hepatitis B virus infection markers and hepatitis B vaccination in a representative sample of the juvenile and adult population of Catalonia and to evaluate the changes with respect to seroepidemiological surveys carried out in 1989 and 1996.In all subjects anti-HBc and anti-HBs antibodies and HBsAg were determined using an ELISA test. The possible association between sociodemographic variables and the prevalence of markers was analysed by calculating the adjusted odd ratio (simple logistic regression).The study was carried out in 2002 in representative samples of the juvenile (5-14 years) and adult population (or= 15 years) of Catalonia (Spain).In 2002 the global prevalence of HBsAg+ was 0.7% (95% CI: 0.4-1.0) and that of anti-HBc+ 8.7% (95% CI: 7.6-9.8), values higher than those obtained in 1989 of 1.5% (95% CI: 1.0-2.1) and 15.6 (95% CI: 13.9-17.3). The prevalence of markers of infection increased with age. The only sociodemographic variable significantly associated with the prevalence of hepatitis B virus infection was the place of birth. The risk of infection was twice as high in subjects born outside Catalonia (p0.01), adjusted OR 2.0 (95% CI: 1.34-2.98) compared with those born in Catalonia.The results of this study show that the prevalence of hepatitis B virus infection (anti-HBc+) in Catalonia (Spain) is currently the lowest it has ever been and suggest that there has been a change in the pattern of endemicity of hepatitis B virus infection in Catalonia, which has become a country of low endemicity.

The aim of this study was to determine the prevalence of varicella IgG antibodies in the population of Catalonia, and the variables associated. The study was carried out in 2002 in a representative sample of juvenile (5-14 years) and adult population (> or =15 years) of Catalonia (Spain). The global prevalence determined by ELISA test was 95.5% (95% CI 94.7-96.3). The prevalence increased with age, from 82% (95% CI 77.0-87.0) in the 5-9 years age up to 99.4% (95% CI 98.2-100) in >64 years of age. No statistically significant differences in the seroprevalence according to the sociodemographic variables were observed. The current strategy of vaccinating preadolescents at 12 years of age who have not suffered the disease will require more than 30 years to cover the current gap immunity in adolescents and young adults. Vaccination with two doses of vaccine for all adolescents and adults < or = 50 years who report not having suffered the disease during childhood should be considered.

Vaccination coverage in a representative sample of children under 3 years of age in Catalonia (Spain) was studied. Parents of children selected were contacted and asked for the child's vaccination card. A total of 630 children were included, of whom 570 were indigenous and 60 immigrants. Primary vaccination coverage was 96.5% for indigenous and 85% for immigrant children (p=0.0001), while the coverage of primary vaccination plus booster doses was 88.6% in indigenous and 78.3% in immigrant children (p=0.02). Efforts should be made to increase primary vaccination coverage in immigrant children and booster dose coverage in both indigenous and immigrant children.

Titres of anti-tetanus toxin antibodiesor = 0.1 IU/ml were determined using an enzyme linked immunosorbent assay in representative samples of the juvenile and adult population of Catalonia. The prevalence obtained in 1,316 juveniles and 1,296 adults was 99.4 and 68.3%, respectively. In adults, the prevalence in males (76.5%) was higher (P0.001) than in females (61.7%), fell with increasing age and was higher in subjects born in Catalonia (72.5%) than in those born outside Catalonia (57.9%) (P0.001). These results show that routine vaccination of children is successful. In adults agedor = 45 years, the prevalence is inadequate and efforts should be made to increase vaccination.

In most developed countries, nontyphoid Salmonella is an important cause of sporadic cases and outbreaks of foodborne gastroenteritis. The aim of this study was to investigate the trend of foodborne Salmonella-caused outbreaks and number of cases, hospitalizations, and deaths and compare them with those caused by other infectious agents. The study was carried out in Catalonia, a region in northeastern Spain with a population of 6.5 million inhabitants, in 2002. All information on reported outbreaks of foodborne disease from 1990 to 2003 was reviewed. For each outbreak, the following variables were collected: year; setting (household, restaurant, school, hospital, nursing home, and others); number of cases, hospitalizations, and deaths; causal agent; and food vehicle involved. Of 1652 reported outbreaks, 1078 had a known causal agent. Among them, 871 (80.8%) were caused by Salmonella, with 14,695 cases, 1534 hospitalizations, and 4 deaths. The rate of hospitalization was higher in outbreaks due to Salmonella than in those caused by other infectious agents (rate ratio, 2.54; 95% confidence interval, 2.20 to 2.94). Forty-eight percent of Salmonella-caused outbreaks were eggborne, compared with 5.3% of those caused by other infectious agents (rate ratio, 1.40; 95% confidence interval, 1.33 to 1.48). The annual number of cases in household outbreaks of eggborne Salmonella rose over time (R2 = 0.82), but the number of outbreaks produced in other settings did not. Eggborne outbreaks caused by Salmonella in households are a major cause of disease, and increased preventive efforts are necessary, especially consumer education and awareness of the risk of eating food containing raw or slightly cooked eggs.
