Background: The study aim was to analyse how mental health services are used in different parts of the Kainuu region in Finland and whether travel time to primary health care services is associated with the use of different contact types (in-person visits, remote contacts, home visits). Methods: The study population included adults who had used mental health services under primary health care (N = 7643) between 2015 and 2019. The travel times to the nearest health centre in a municipality were estimated as the population-weighted average drive time in postal code areas. The Kruskal–Wallis test and pairwise comparisons with Dunn-Bonferroni post hoc tests were used to assess the differences in mental health service use between health centre areas. A negative binomial regression was performed for the travel time categories using different contact types of mental health service use as outcomes. Models were adjusted for gender, age, number of mental health diseases and the nearest health centre in the municipality. Results: Distance was negatively associated with mental health service use in health centre in-person visits and in home visits. In the adjusted models, there were 36% fewer in-person visits and 83% fewer home visits in distances further than 30 min, and 67% fewer home visits in a travel time distance of 15–30 min compared with 15 min travel time distance from a health centre. In the adjusted model, in remote contacts, the incidence rate ratios increased with distance, but the association was not statistically significant. Conclusions: The present study revealed significant differences in mental health service use in relation to travel time and contact type, indicating possible problems in providing services to distant areas. Long travel times can pose a barrier, especially for home care and in-person visits. Remote contacts may partly compensate for the barrier effects of long travel times in mental health services. Especially with conditions that call for the continuation and regularity of care, enabling factors, such as travel time, may be important.

Objectives. To test the a priori hypothesis that out-of-hospital cardiac arrest (OHCA) is associated with cold weather during all seasons, not only during the winter. Methods. We applied a case–crossover design to all cases of nontraumatic OHCA in Helsinki, Finland, over 22 years: 1997 to 2018. We statistically defined cold weather for each case and season, and applied conditional logistic regression with 2 complementary models a priori according to the season of death. Results. There was an association between cold weather and OHCA during all seasons, not only during the winter. Each additional cold day increased the odds of OHCA by 7% (95% confidence interval [CI] 5 4%, 10%), with similar strength of association during the autumn (6%; 95% CI 5 0%, 12%), winter (6%; 95% CI 5 1%, 12%), spring (8%; 95% CI 5 2%, 14%), and summer (7%; 95% CI 5 0%, 15%). Conclusions. Cold weather, defined according to season, increased the odds of OHCA during all seasons in similar quantity. Public Health Implications. Early warning systems and cold weather plans focus implicitly on the winter season. This may lead to incomplete measures in reducing excess mortality related to cold weather.

While being an indispensable mode of transportation, the occupancy rate of taxi vehicles is generally low. In this study, we used a large data set on actual taxi trips to assess how ride sharing could provide means to decrease the number of trips and traveled distance. We implemented a method based the notion of a shareability network, allowing us to determine the set of trips associated with the highest benefit of sharing. Due to the complexity of the problem, we only considered pairwise ride sharing. Even with this limitation, we found that potentially a considerable number of taxi trips can be shared. Our analysis suggested that up to two out of three trips are shareable with at least one other trip. Although shareability is dependent on the length of the time window, a significant proportion of sharing opportunities can be realized even in short time windows.

Raportissa selvitettiin paikkatietoanalyysin avulla sairausvakuutuksen korvaamien taksimatkojen yhdistelyn vaikutuksia taksimatkojen kustannuksiin sekä arvioitiin, olisiko yhdistelyä voinut tapahtua vielä enemmän. Tutkimusaineistona käytettiin Tampereen yliopistollisen sairaalan (TAYS) erityisvastuualueen (erva) sairaanhoitopiirien alueella toimiviin tilausvälityskeskuksiin kertyneitä tietoja mm. matkojen osoitteista ja toteutumisajankohdista, ja Kelan rekisteritietoja niistä maksetuista sairausvakuutuksen korvauksista vuonna 2015. Yhteiskuljetusten osuus tutkimusaineiston taksimatkoista oli 12 %. Tulosten mukaan toteutuneilla yhteiskuljetuksilla säästettiin taksimatkojen kuluissa vuonna 2015 TAYS-erva-alueella arviolta 3,2 miljoonaa euroa eli noin kahdeksan prosenttia. Lisäksi osa ei-yhdistellyistä taksimatkoista olisi ollut mahdollista yhdistellä toisiinsa, jolloin saavutettu säästö olisi voinut olla vieläkin suurempi. Yhdistelyalueen laajentaminen yksittäisistä sairaanhoitopiireistä koko erva-alueen kattavaksi olisi kuitenkin lisännyt säästömäärää vain vähän. Taksimatkojen yhdistelyn laajuudessa oli melko suuria eroja sairaanhoitopiirien välillä sekä niiden sisällä. Yhdistelyaste ja matkaa kohti lasketut kustannussäästöt olivat suurimmat sairaanhoitopiirien reunakunnissa, mutta eniten yhdisteltyjä tehtiin ja suurimmat rahalliset säästöt tulivat sairaalakaupunkien sisäisillä matkoilla. Yli puolet yhteiskuljetuksiin kuuluneista taksimatkoista liittyi keskussairaalamatkaan. Kyseessä oli ensimmäinen kerta Suomessa, kun taksimatkojen yhdistelyä tehtiin paikkatietomenetelmien avulla näin laajalle rekisteriaineistolle. Aineiston suuri koko asetti haasteita erityisesti yhdistelypotentiaalin määrän arvioinnille, minkä vuoksi tässä tutkimuksissa pystyttiin tarkastelemaan pelkästään kahden matkan laskennallisia yhdistelyjä. Jatkoa ajatellen olisi tarkoituksenmukaista, että myös useamman matkan yhdistelyjen arviointi olisi mahdollista.

Perinteinen kuntarajoihin perustuva kaupunki-maaseutu-luokitus on kuntien koon kasvaessa osoittautunut epätarkaksi. Tässä raportissa kuvataan uuden paikkatietoihin perustuvan luokituksen toteuttaminen. Uusi luokitus perustuu paikkatietomenetelmään, jossa lähtöaineistot ovat koko maan kattavia 250x250 metrin tilastoruutuaineistoja. Luokitus perustuu maaseutualueiden osalta aiempaan maaseudun kolmijakoon, jossa tunnistetaan kaupungin läheinen maaseutu, ydinmaaseutu sekä harvaan asuttu maaseutu. Näiden luokkien lisäksi maaseudulta on erotettu maaseudun paikalliskeskukset. Kaupunkialueita ovat yli 15 000 asukkaan keskustaajamat, joihin on raportissa kuvatulla menetelmällä rajattu ydinkaupunkialue ja kehysalue. Ydinkaupunkialue jakautuu edelleen sisempään ja ulompaan kaupunkialueeseen. Luokituksen lähtöaineistona on käytetty väestö-, työvoima-, työmatka- ja rakennustietoja sekä Digiroad tieverkkoaineistoa ja CORINE maankäyttöaineistoa. Näiden perusteella on laskettu määrää, tiheyttä, tehokkuutta, saavutettavuutta, intensiteettiä, monipuolisuutta ja suuntautuneisuutta kuvaavia muuttujia, joiden luokitteluun ja yhdistelyyn luokitus perustuu. Luokitus on vapaasti saatavissa paikkatietomuodossa verkkosivuilta. Sen voi ladata paikkatietoaineistona ja siihen liittyvänä kuntatasolle laskettuna tilastoaineistona. Ensisijaisesti kaupunki-maaseutu-luokitus kuvaa alueiden välisiä eroja aluerakenteen tasolla. Luokitus ei kuvaa niinkään yksittäisen paikan ominaisuuksia vaan luonnehtii aluekokonaisuuksia. Alueluokkien rajat on yleistetty siten, että luokitus toimii parhaiten laajempien alueiden tarkasteluissa. Luokituksen avulla saadaan tietoa erityyppisten alueiden kehityksestä koko maan tasolla.

This research addresses the subject of cross-country terrain path finding using forest inventory route planning as a case study. Although the subject is closely related to the key concepts of geography, so far it has not been studied in academic geography in any significant extent. Accordingly, an additional motive for the research is to attempt to raise the importance of terrain path finding within the domain of academic geography and GIS. The research also strives to develop GIS-based methods that would be particularly useful in applications related to the environment, conditions and industries of the North and periphery.

Tourism is a key business sector for many geographically and economically marginal regions. The main form of tourism in these regions is nature-related tourism, consisting of activities such as hiking, fishing and various types of winter sports. The tourism resorts in marginal regions face the constant challenge of improving their tourist product to retain their position in the market while attempting to conserve the natural environment. In addition to the quest for pure excitement and adventure, the tourists of today expect better accessibility to information, services and nature. In response to this demand, a more widespread use of new information communication technologies, including mobile location-based services (LBS), is proposed in this paper. LBS have already been introduced to the tourism market, but mostly in urban areas. Adverse field conditions, inadequate availability of relevant data sources and high development costs are among the most common obstacles to the adoption of LBS in marginal regions. However, we conclude that LBS have the potential to add significant value to local tourism products and even create new business activities.
