The history of hypertension the past hundred years is a successful story of the fall of myths, beliefs, and assumptions under the weight of evidence. The recent editorial by Kreutz et al. (2020),"Blood pressure medication should not be routinely dosed at bedtime. We must disregard the data from the HYGIA project", published in Blood Pressure, conveys unjustified concerns founded on baseless doubts and suspicions about the Hygia Chronotherapy Trial. The physicians of Portugal are beginning to incorporate into routine clinical practice the proven methods of the Hygia Chronotherapy Trial - 48-hour ambulatory blood pressure monitoring and bedtime hypertension chronotherapy - to improve in a cost-effective matter the diagnosis and management of hypertension and to reduce the overwhelming burden of cardiovascular morbidity and mortality in our country.

Current hypertension guidelines fail to provide a recommendation on when-to-treat, thus disregarding relevant circadian rhythms that regulate blood pressure (BP) level and 24 h patterning and medication pharmacokinetics and pharmacodynamics. The ideal purpose of ingestion-time (chronopharmacology, i.e. biological rhythm-dependent effects on the kinetics and dynamics of medications, and chronotherapy, i.e. the timing of pharmaceutical and other treatments to optimize efficacy and safety) trials should be to explore the potential impact of endogenous circadian rhythms on the effects of medications. Such investigations and outcome trials mandate adherence to the basic standards of human chronobiology research. In-depth review of the more than 150 human hypertension pharmacology and therapeutic trials published since 1974 that address the differential impact of upon-waking/morning versus at-bedtime/evening schedule of treatment reveals diverse protocols of sometimes suboptimal or defective design and conduct. Many have been “time-of-day,” i.e. morning versus evening, rather than circadian-time-based, and some relied on wake-time office BP rather than around-the-clock ambulatory BP measurements (ABPM). Additionally, most past studies have been of too small sample size and thus statistically underpowered. As of yet, there has been no consensual agreement on the proper design, methods and conduct of such trials. This Position Statement recommends ingestion-time hypertension trials to follow minimum guidelines: (i) Recruitment of participants should be restricted to hypertensive individuals diagnosed according to ABPM diagnostic thresholds and of a comparable activity/sleep routine. (ii) Tested treatment-times should be selected according to internal biological time, expressed by the awakening and bed times of the sleep/wake cycle. (iii) ABPM should be the primary or sole method of BP assessment. (iv) The minimum-required features for analysis of the ABPM-determined 24 h BP pattern ought to be the asleep (not “nighttime”) BP mean and sleep-time relative BP decline, calculated in reference to the activity/rest cycle per individual. (v) ABPM-obtained BP means should be derived by the so-called adjusted calculation procedure, not by inaccurate arithmetic averages. (vi) ABPM should be performed with validated and calibrated devices at least hourly throughout two or more consecutive 24 h periods (48 h in total) to achieve the highest reproducibility of mean wake-time, sleep-time and 48 h BP values plus the reliable classification of dipping status. (vii) Calculation of minimum required sample size in adherence with proper statistical methods must be provided. (viii) Hypertension chronopharmacology and chronotherapy trials should preferably be randomized double-blind, randomized open-label with blinded-endpoint, or crossover in design, the latter with sufficient washout period between tested treatment-time regimens.

Poster distinguido com o 1º lugar no "XVIII Encontro Anual da APECSP", Introdução: Um em cada cinco adolescentes tem problemas de saúde mental que persistem na idade adulta (Lee et al., 2014), verificando-se um crescente número de situações graves e de adolescentes medicados com ansiolíticos e antidepressivos (Olfson, Druss, & Marcus, 2015). Em Portugal, a percentagem de adolescentes que referem mal-estar físico e psicológico e comportamentos autolesivos tem vindo a aumentar (Matos et al., 2015). Objetivos: Avaliar a prevalência e caracterizar a população de adolescentes com perturbações do foro mental numa Unidade de Saúde Familiar. Metodologia: Estudo retrospetivo da casuística de perturbações do foro mental em adolescentes inscritos numa Unidade de Saúde Familiar do norte de Portugal. Recurso ao sistema de monitorização das unidades funcionais (MIM@UF) para identificação dos adolescentes entre os 10 e 19 anos com codificação para o diagnóstico de sensação de depressão, perturbação depressiva, sensação de ansiedade, distúrbio ansioso ou tentativa de suicídio/suicídio. Resultados: O distúrbio ansioso foi apenas diagnosticado a uma adolescente de 15 anos de idade, num universo de 1212 adolescentes (570 do sexo masculino e 642 do sexo feminino). Não se confirmaram casos de outras perturbações do foro mental. Discussão: Face às estatísticas nacionais e internacionais conhecidas a este respeito, é de considerar estarmos perante uma situação de subdiagnóstico das perturbações do foro mental na população adolescente. Esta é também uma preocupação da Direção Geral da Saúde, que, no âmbito do Programa Nacional para a Saúde Mental 2017, pretende aumentar em 25% o registo de pessoas com ansiedade e depressão nos cuidados de saúde primários, e sensibilizar os profissionais de saúde para o seu diagnóstico (DGS, 2017). Conclusão: A identificação precoce dos adolescentes com perturbação mental deve ser uma prioridade. Aortic coarctation (AC) represents ∼7% of congenital cardiac diseases and is usually diagnosed in childhood or early adult life, depending on the severity of obstruction and associated malformations. Left untreated fewer than 20% of patients survive to age 50. We describe a case of thoracic AC, diagnosed at age 61, in a woman with known hypertension since age 45. At age 56 the patient was admitted with a subarachnoid hemorrhage and, during cerebral angiography, a thoracic aortic aneurysm was detected. Four years later the patient was referred to the outpatient hypertension clinic due to uncontrolled hypertension and cardiac failure. The echocardiogram disclosed left ventricular hypertrophy and aggressive treatment failed to control her hypertension. At age 61, due to lower limb muscular fatigue, arterial Doppler ultrasound was performed that revealed symmetrically decreased ankle/brachial pressure index, suggesting aortic stenosis. MRI angiography enabled a diagnosis of AC with a large poststenotic dilation which had been interpreted as an aortic aneurysm in successive CT scans. The authors highlight the unusually late clinical presentation and misdiagnosis despite extensive radiologic investigation. The subarachnoid hemorrhage was probably a disease manifestation, since berry aneurysms are among the noncardiac malformations associated with AC. Resumo: A coartação da aorta (CA) representa ∼7% das cardiopatias congénitas sendo habitualmente diagnosticada na infância ou no início da vida adulta, dependendo da gravidade da obstrução e das malformações associadas. Não tratados, menos de 20% dos doentes sobrevivem até aos 50 anos. Descreve-se um caso de CA torácica, diagnosticada aos 61 anos, numa mulher com HTA conhecida desde os 45. Aos 56 anos foi internada por hemorragia subaracnoideia e, aquando da realização de angiografia cerebral, foi detetado aneurisma da aorta torácica. Quatro anos mais tarde foi referenciada à Consulta de Hipertensão por HTA não controlada e insuficiência cardíaca. O ecocardiograma mostrava hipertrofia ventricular esquerda e a HTA mantinha-se não controlada apesar de polimedicada. Aos 61 anos, por queixas de fadiga muscular intensa dos membros inferiores, efetuou Doppler Arterial que identificou índices de pressão sistólica diminuídos, bilateralmente e de forma simétrica, sugerindo estenose da aorta abdominal. A angio-RMN permitiu o diagnóstico de CA torácica com importante dilatação pós-estenótica, que foi interpretada em TAC seriadas como correspondendo a aneurisma da aorta descendente. Destaca-se a apresentação clínica tardia e diagnóstico erróneo, apesar de extensa investigação radiológica. Salienta-se ainda que a hemorragia subaracnoideia foi, provavelmente, uma manifestação da doença já que os aneurismas do polígono de Willis são uma das malformações extracardíacas associadas à CA. Pheochromocytoma is a catecholamine-secreting tumor, for which the treatment of choice is complete surgical resection, when possible. Some form of preoperative pharmacological preparation is indicated. Most centers use alpha blockade, phenoxybenzamine being the preferred drug. There is little experience with the use of other alpha-blocking agents. The authors report three different cases in which doxazosin was used: as preoperative preparation of a patient with pheochromocytoma, as preparation for therapy in I-131-MIBG of a patient with a malignant pheochromocytoma, and as a pre-cesarean preparation in a pregnant woman with multiple endocrine neoplasia type 2A (MEN-2A). Doxazosin provided a safe alternative to phenoxybenzamine in the three cases described, with some advantages that the authors summarize.

Tuberculosis affects one-third of the world's population. The incidence of extrapulmonary tuberculosis including cutaneous and skeletal manifestations has increased in the last decades. The authors report a clinical case of concomitant metastatic cutaneous abscesses and Pott disease in an immunocompetent patient, a rare presentation of tuberculosis.

Tese de Doutoramento em Arquitectura (Teoria e História da Arquitectura) apresentada à Faculdade de Ciências e Tecnologia da Universidade de Coimbra Esta dissertação formula uma proposta de leitura da história e identidade espaciais da cidade com a qual em 1537 se fundiu a Universidade. É isso o que, de forma clara, exprime o título, Divercidade (por oposição a Univercidade), especificando de imediato tratar-se de uma urbanografia do espaço de Coimbra até ao estabelecimento definitivo da Universidade. Porém esse desígnio interagiu com o que o autor tem vindo a manter e desenvolver como estratégia global de investigação, usando a História segundo dois vectores: num revela-se a demiurgia da sua formação de arquitecto, a qual consiste em ansiar conhecer e dominar o todo pela sua acção; o outro consiste no paradigma de que o todo é a contemporaniedade, ou seja, a síntese do passado no momento presente à capacidade de transformação que caracteriza a Vida. De ambos resulta a necessidade de desenvolver Conhecimento no campo onde a Arquitectura efectivamente se relaciona com a realidade do tempo:a Cidade. Surgiu assim um trabalho onde o tempo discorre sobre o núcleo urbano em análise com o ritmo necessário à leitura contínua da sua transformação até à conformação da matriz da correspondente identidade espacial. Trata-se também de um texto onde, com uma imaginação integrada, se intentou cerzir os hiatos onde a História, a Arqueologia e a realidade ainda não lograram fornecer dados seguros, isso enquanto se inquirem as especificidades dos momentos das grandes mudanças (os tempos fortes) e o que é mais comum nos grandes momentos de mudança (os tempos fracos). Claro que por trás de tanta ambição esteve a vontade de continuar a reflectir essencialmente sobre a problemática da cidade portuguesa no seu todo e não apenas no caso coimbrão, clarificando-se, desta feita, alguns dados essenciais à compreensão da cidade medieval portuguesa, bem como ao dealbar da modernidade no pensamento urbanístico português.
