Abstract

Background and Aims: COVID-19 has infected more than 77 million people worldwide and impacted the lives of many more, with a particularly devastating impact on vulnerable populations, including people with substance use disorders (SUDs). Quarantines, travel bans, regulatory changes, social distancing, and “lockdown” measures have affected drug and alcohol supply chains and subsequently their availability, price, and use patterns, with possible downstream effects on presentations of SUDs and demand for treatment. Given the lack of multicentric epidemiologic studies, we conducted a rapid global survey within the International Society of Addiction Medicine (ISAM) network in order to understand the status of substance-use patterns during the current pandemic. Design: Cross-sectional survey. Setting: Worldwide. Participants: Starting on April 4, 2020 during a 5-week period, the survey received 185 responses from 77 countries. Measurements: To assess addiction medicine professionals' perceived changes in drug and alcohol supply, price, use pattern, and related complications during the COVID-19 pandemic. Findings: Participants reported (among who answered “decreased” or “increased”) a decrease in drug supply (69.0%) and at the same time an increase in price (95.3%) globally. With respect to changes in use patterns, an increase in alcohol (71.7%), cannabis (63.0%), prescription opioids (70.9%), and sedative/hypnotics (84.6%) use was reported, while the use of amphetamines (59.7%), cocaine (67.5%), and opiates (58.2%) was reported to decrease overall. Conclusions: The global report on changes in the availability, use patterns, and complications of alcohol and drugs during the COVID-19 pandemic should be considered in making new policies and in developing mitigating measures and guidelines during the current pandemic (and probable future ones) in order to minimize risks to people with SUD.

Abstract

Background: The coronavirus disease 2019 (COVID-19) pandemic has impacted people with substance use disorders (SUDs) worldwide, and healthcare systems have reorganized their services in response to the pandemic. Methods: One week after the announcement of the COVID-19 as a pandemic, in a global survey, 177 addiction medicine professionals described COVID-19-related health responses in their own 77 countries in terms of SUD treatment and harm reduction services. The health responses were categorized around (1) managerial measures and systems, (2) logistics, (3) service providers, and (4) vulnerable groups. Results: Respondents from over 88% of countries reported that core medical and psychiatric care for SUDs had continued; however, only 56% of countries reported having had any business continuity plan, and 37.5% of countries reported shortages of methadone or buprenorphine supplies. Participants of 41% of countries reported partial discontinuation of harm-reduction services such as needle and syringe programs and condom distribution. Fifty-seven percent of overdose prevention interventions and 81% of outreach services were also negatively impacted. Conclusions: Participants reported that SUD treatment and harm-reduction services had been significantly impacted globally early during the COVID-19 pandemic. Based on our findings, we highlight several issues and complications resulting from the pandemic concerning people with SUDs that should be tackled more efficiently during the future waves or similar pandemics. The issues and potential strategies comprise the following: (1) helping policymakers to generate business continuity plans, (2) maintaining the use of evidence-based interventions for people with SUDs, (3) being prepared for adequate medication supplies, (4) integrating harm reduction programs with other treatment modalities, and (5) having specific considerations for vulnerable groups such as immigrants and refugees.

Abstract

The extracellular matrix comprises a network of macromolecules such as collagens, proteoglycans and glycoproteins. VWA1 (von Willebrand factor A domain containing 1) encodes a component of the extracellular matrix that interacts with perlecan/collagen VI, appears to be involved in stabilizing extracellular matrix structures, and demonstrates high expression levels in tibial nerve. Vwa1-deficient mice manifest with abnormal peripheral nerve structure/function; however, VWA1 variants have not previously been associated with human disease. By interrogating the genome sequences of 74 180 individuals from the 100K Genomes Project in combination with international gene-matching efforts and targeted sequencing, we identified 17 individuals from 15 families with an autosomal-recessive, non-length dependent, hereditary motor neuropathy and rare biallelic variants in VWA1. A single disease-associated allele p.(G25Rfs*74), a 10-bp repeat expansion, was observed in 14/15 families and was homozygous in 10/15. Given an allele frequency in European populations approaching 1/1000, the seven unrelated homozygote individuals ascertained from the 100K Genomes Project represents a substantial enrichment above expected. Haplotype analysis identified a shared 220 kb region suggesting that this founder mutation arose >7000 years ago. A wide age-range of patients (6-83 years) helped delineate the clinical phenotype over time. The commonest disease presentation in the cohort was an early-onset (mean 2.0 ± 1.4 years) non-length-dependent axonal hereditary motor neuropathy, confirmed on electrophysiology, which will have to be differentiated from other predominantly or pure motor neuropathies and neuronopathies. Because of slow disease progression, ambulation was largely preserved. Neurophysiology, muscle histopathology, and muscle MRI findings typically revealed clear neurogenic changes with single isolated cases displaying additional myopathic process. We speculate that a few findings of myopathic changes might be secondary to chronic denervation rather than indicating an additional myopathic disease process. Duplex reverse transcription polymerase chain reaction and immunoblotting using patient fibroblasts revealed that the founder allele results in partial nonsense mediated decay and an absence of detectable protein. CRISPR and morpholino vwa1 modelling in zebrafish demonstrated reductions in motor neuron axonal growth, synaptic formation in the skeletal muscles and locomotive behaviour. In summary, we estimate that biallelic variants in VWA1 may be responsible for up to 1% of unexplained hereditary motor neuropathy cases in Europeans. The detailed clinical characterization provided here will facilitate targeted testing on suitable patient cohorts. This novel disease gene may have previously evaded detection because of high GC content, consequential low coverage and computational difficulties associated with robustly detecting repeat-expansions. Reviewing previously unsolved exomes using lower QC filters may generate further diagnoses.

21. The 'glass ceiling' and empowerment of Qatari women for employment in Qatar

Abstract

Qatar is rapidly transforming from a traditional conservative and modest society to a more open, modern and technologically advanced society. Qatar has made significant strides over the recent years in increasing the participation of women in higher education and in the Qatari workforce (Scott-Jackson, 2015). Women comprise 24% of the Qatari population and 18% of the workforce (MDPS, 2015), however, they remain concentrated in entry-level administrative jobs (Al Ansari, 2012) and mostly in the public sector. Is this inequality of participation of women in higher levels of Qatari organizations, a Glass Ceiling phenomenon that has been noticed in other countries and global organizations? The answer from similar previous research is “Yes” and this dissertation is an endeavor to further build on these reasons and highlight how this problem has sprouted in Qatar. Using an employment effectiveness route, to make employees both men and women, express their views of their current work experiences and their likelihood to continue or leave employment and complete their career objectives in the current organization, this dissertation will help understand whether the Glass Ceiling has external reasons or is it self-inflicted. Self-infliction can be a reason from the lack of enthusiasm to progress into senior roles by women themselves due to the Islamic and patriarchal society values and the influence that this has had on women in Qatar and the entire middle east region in general (Ermis-Mert, A., 2018). The objective of this research is to find not only the reasons for the Glass Ceiling but also the solutions of how to use public policies and driving cultural changes to overcome this problem and to see Qatar fulfilling its vision 2030 in its entirety.

29. Is There Any Difference in the Levels of Antibodies against Helicobacter Pylori Infection between the Patients with Migraine Headache with or without Aura?

Abstract

Background: Recent studies have focused on a positive correlation between Helicobacter pylori (H. pylori) infection and occurrence of migraine headache. This study aimed to determine the role of H. pylori infection in the two common types of migraine headache including with (MA) and without aura (MO). Methods: In a case-control study, 84 patients (with MA or MO migraine headache) and 49 healthy individuals were enrolled. The enzyme-linked immunosorbent assay (ELISA) was used to measure the levels of immunoglobulin G (IgG) and M (IgM) against H. Pylori in patients and controls. Findings: The mean age was 35.8 ± 11.1 and 33.4 ± 18.9 years in patient and control groups, respectively. The mean level of IgM antibody was significantly more in patients (26.3 ± 23.1 U/ml) compared to control group (17.5 ± 11.2 U/ml) (P = 0.004). It was not any significant difference between the levels of IgM and IgG antibodies in patients with MA or MO migraine headache. In addition, the mean HIT6 score was significantly more in patients with MA (65.5 ± 4.7) compared to those with MO (54.9 ± 5.3) migraine headache (P < 0.001). The only significant correlation was found between the IgG antibody level and the HIT6 score in patients with MA (r = 0.407; P = 0.011) and MO (r = 0.499; P = 0.002) migraine headache. There was significant relationship between the normal and high levels of IgG antibody and the severity of headache (P = 0.002) in all the patients. The risk of migraine occurrence in patients was not significantly associated with the levels of IgG and IgM antibodies. Conclusion: Further investigation should be designed to gain better conclusion about the role of H. Pylori treatment and eradication in the course of migraine headaches.

Abstract

Every student has different thinking abilities, such as thinking activities to receive and process information. The students' thinking ability is cognitive thinking ability or known as metacognition, such as monitoring and controlling each stage conducted. Therefore, teachers need to make efforts to improve students' metacognition by using probing prompting learning techniques. This technique can encourage students' metacognition abilities in solving mathematical problems. This study aims to determine the development of students' metacognition in mathematical problem-solving. This classroom action research applied a qualitative descriptive method which was carried out in five cycles, involving four out of 22 Year 7 students in one of the islamic junior high schools in Aceh Besar, Indonesia. Data analysis of students' metacognition development was investigated through metacognition question sheets which were carried out after solving mathematical problem solving problems. Students' and teacher's activities were investigated in each lesson. The results showed that the students' metacognition in mathematical problem-solving increased in Cycle I to V based on metacognition question sheets, students' activity sheets, and teacher's observation sheets in using probing prompting techniques. The development of students' metacognition can also be seen through other supporting aspects, such as students' activities and teacher's efforts in the learning, both increased in each cycle. This chapter discusses the resiliency aspect of electric power distribution systems considering the role of distributed generation. A restoration tool is developed that can emulate the recovery path of the distribution system after a contingency situation, then engineering resiliency assessment framework is applied to (semi)-quantitatively measure the dimensions of resiliency. The proposed procedure is implemented on a test system to demonstrate the key factors affecting the resiliency of a distribution system, including distributed generation.

Abstract

Ambient air quality is defined not only by the source strength but a variety of meteorological parameters as well. In the current study, ambient concentrations of PM along with temperature and relative humidity levels were monitored at seven different locations of Pakistan. A DustTrak DRX (Model 8533, TSI Inc.) was employed for twenty four hours real time monitoring of particulate matter at the selected sites. A considerable variation was observed in the 24 hour trend of particulate matter (PM) at different locations owing to variation in meteorological conditions due to different altitudes and seasons, and natural and anthropogenic sources in the vicinity. The highest average concentrations of PM2.5 (407 mu g/m(3)) were observed at highest elevation (Makra Peak, Shogran, 3089 m) while lowest averages (102 mu g/m(3)) were obtained at the seaside (Hawks Bay, Karachi, 0 m). On the other hand PMTotal fraction exhibited highest levels at site B (506 mu g/m(3)) and lowest at Site A (121 mu g/m(3)). Correlation factors were determined for PM and meteorological parameters at each location. More research needs to be conducted to have a comprehensive knowledge about the physical parameters controlling particulate dispersal at different altitudes within the country.
