Abstract

The article presents a directory of medical education courses related to women's health. The mission of the Association of Academic Women's Health Programs is to improve the health of women through leadership in research, education, clinical models, and community partnerships. The "General Internal Medicine: Women's Health Residency" program is offered by Jacobi Medical Center, New York. In addition to the required aspects of the internal medicine residency training program, junior/senior residents have weekly gynecology continuity clinic co-supervised by an internist and a gynecologist. They are seeing women for routine gynecological care as well as women for preoperative evaluations prior to surgery. A residency program is being offered by the Harvard University School of Medicine. In addition to inpatient rotations in internal medicine, residents are required to participate for three to four months each year in a multidisciplinary primary care practice for women. This experience provides ambulatory subspecialty training in gynecology, orthopedics, and dermatology.

Abstract

ObjectivesWe sought to determine whether angiotensin-converting enzyme inhibition (ACE-I) (i.e., quinapril) prevents transient ischemia (exertional and spontaneous) in patients with coronary artery disease (CAD).BackgroundIt is known that ACE-I reduces the risk of death, myocardial infarction (MI), and other CAD-related outcomes in high-risk patients. Numerous studies have confirmed that ACE-I improves coronary flow and endothelial function. Whether ACE-I also decreases transient ischemia is unclear, because no studies have been adequately designed or sufficiently powered to evaluate this issue.MethodsUsing a randomized, double-blinded, placebo-controlled, multicenter design, we enrolled 336 CAD patients with stable angina. None had uncontrolled hypertension, left ventricular (LV) dysfunction, or recent MI, and all developed electrocardiographic (ECG) evidence of ischemia during exercise. They were randomly assigned to one of two groups: 40 mg/day quinapril (n = 177) or placebo (n = 159) for 8 weeks. Patients then entered an additional eight-week treatment phase to examine the full dose range. Those assigned to 40 mg quinapril continued that dose and those assigned to placebo were titrated to 80 mg/day. Treadmill testing, the Seattle Angina Questionnaire, and ambulatory ECG monitoring were used to assess responses at baseline and at 8 and 16 weeks.ResultsThe groups did not differ significantly at entry or in terms of indexes assessing myocardial ischemia at 8 or 16 weeks of treatment. In this low-risk population, ACE-I was not associated with serious adverse events.ConclusionsOur findings suggest short-term ACE-I in CAD patients without hypertension, LV dysfunction, or acute MI is not associated with significant effects on transient ischemia.

10. Water Intake as it Relates to Mood, Psychosocial Well-being and Behavior Patterns in an Extended Care Facility

Abstract

Water is a basic human need. The literature reports that residents of extended care facilities are at risk for inadequate water intake. For nurses involved with the care of the extended care resident providing adequate amounts of fluid is a basic responsibility. The purpose of this observational study was to determine water intake as it relates to mood, psychosocial well-being and behavior patterns in extended care facility residents. A convenience sample of 39 subjects were observed for two twenty-four hour periods. All foods and fluids ingested were recorded. Information on each subject's mood, behavior and psychosocial well-being was obtained from the Minimum Data Set. The data revealed that only four of the subjects exceeded 100% of water intake adequacy based on the requirement of 1,600 ml/m^ of body surface area. The mean percentage of water intake adequacy among the subjects was 66.2%. Twenty-one of the subjects were below the mean of water intake adequacy. In this study no significant relationship between mood, behavior and psychosocial wellbeing and water intake of the subjects could be found.
