4. A QUALITY INITIATIVE TO INCREASE HOSPICE REFERRALS FOR STARS PATIENTS DURING THE SECOND COVID SURGE

Abstract

Background CMS uses the Overall Hospital Quality Star Rating program to stratify hospitals based on specific quality criteria (e.g., 30-day readmissions of older adults with pneumonia). “STARS patients” experience more readmissions, longer LOS and often have more complex discharge plans. During the second surge of COVID we implemented a program to increase hospice referrals through early identification and implementation of goals of care (GOC) conversations. Methods Electronic Medical Records reviewed for STARS patients from pre- (1/2019-7/2020) and post (3/2021–2/2022) program implementation. Location: 2 community-based hospitals. Data collected: demographics, hospital outcomes, discharge disposition. Data compared with Student’s t tests and Chi square. Results 459 patients, 177 pre-program and 282 post-program were included. Groups were similar in age (83.0 vs 83.6), LACE score (13.0 vs 12.8), principal diagnoses (PNA: 41.5% vs 46.0%, HF/COPD/AMI: 58.5% vs 54.0%), and mortality (3.5 vs. 4.0%). LOS increased 4.9 days vs. 6.1 days (p < 0.01), readmission rates unchanged: 12.6% vs 13.0% (p=0.90). GOC conversations increased, 48.6% to 75.0% (p < 0.01), DNR from 24% to 44% (p < 0.01), and comfort measures from 0.5% to 5% (p < 0.01). Hospice referrals increased from 0.5% to 11.2% (p < 0.01). Discussion: Early identification of STARS patients increased GOC conversations, DNR, comfort measures and hospice referral. Patients across time periods were similar in age, LACE and admitting diagnoses. LOS increased by a day, likely reflecting time needed to arrange discharge disposition. Increased hospice at end-of-life is associated with better quality care and patient/family satisfaction. This program may be adapted to larger, academic medical centers within the health system.

Abstract

Background and Aims The Observational Postmarketing Ulcerative colitis Study [OPUS] was conducted to obtain the first long-term [5 years] safety data assessing treatment with originator infliximab versus conventional therapies in patients with ulcerative colitis [UC] in real-world clinical practice. Methods The OPUS registry was a prospective, non-randomised, observational study that measured adverse events in nine prespecified categories of interest in UC patients whose treatment with either originator infliximab or conventional therapy [defined as initiation or dose-increase of corticosteroids and/or immunosuppressants] was determined by their treating physician. Results Data for 2239 patients were available: N = 1180 enrolled to conventional therapy [including N = 296 who switched to originator infliximab during follow-up] and N = 1059 enrolled to originator infliximab. Patients in the originator infliximab group, compared with the conventional therapy group, had more severe disease at baseline, based on partial Mayo score [PMS]: 46.0% of patients in the originator infliximab group had severe disease (PMS of 7–9 [out of 9]), compared with 30.5% in the conventional therapy group. In adjusted time-to-event analyses, enrolment into the originator infliximab group was associated with a higher risk of serious infection (hazard ratio = 1.98 [95% confidence interval: 1.34, 2.91; p Conclusions UC patients treated with infliximab had higher risk for serious infection, compared with conventional therapies. Methods Cox proportional hazards models compared time to colectomy for both treatment groups. A secondary analysis examined colectomy incidence rates based on IFX exposure timing (defined by a 90-day window after the last IFX dose date). Results Of 2239 patients with data, 1059 enrolled in IFX and 1180 enrolled in ConvRx (including 296 patients who switched to IFX). Patients in the IFX group had more severe disease at baseline vs the ConvRx group (percentage with baseline partial Mayo score 7-9: 46.0% vs 30.5%, respectively). During 5 years of follow-up, 271 patients (12.1% of enrolled patients) had colectomy. Enrollment in the IFX group was associated with a higher risk of colectomy (hazard ratio = 3.12; 95% confidence interval, 2.25-4.34; P Conclusions Colectomy was reported at a higher rate in the IFX group than in the ConvRx group, although patients in the IFX group had more severe disease at baseline and most of the colectomies occurred after patients had been off of IFX for ≥90 days.

Abstract

This study explored the concept of ‘giving up’ from the perspective of care staff working in care homes, and their everyday communication and hidden knowledge concerning what they think about this taboo topic and the context it reflects. Moving to a care home is a major transition where cumulative losses can pose risks to mental health in later life. If not recognised, this vulnerability can lead to depression which extends to suicide ideation and behaviours in the form of self-harm and self-neglect. Care homes are a significant place of care until death, yet a discourse of silence means that self-harm and suicide is under-reported or not attended to with specialist expertise. The layperson's concept of an older person ‘giving up’ on life is hardly discussed in the literature. This co-produced qualitative study used an inductive approach to explore this phenomenon through focus groups with 33 care staff across four care homes in South-East England. Findings paint a complex picture, highlighting tensions in providing the right support and creating spaces to respond to such challenging situations. ‘Giving up’ requires skilled detailed assessment to respond to risks alongside improved training and support for paid carers, to achieve a more holistic strategy which capitalises on significant relationships within a wider context.

Abstract

Rationale Accumulation and activation of mast cells and eosinophils have been implicated in the pathogenesis of several chronic inflammatory gastrointestinal (GI) diseases, including eosinophilic gastrointestinal diseases (EGIDs) and inflammatory bowel disease (IBD). Despite the strong association of mast cell and eosinophil numbers and activation with the pathogenesis of IBD, no further characterization of these cells has been performed. Current treatment options for IBD include aminosalicylates, antibiotics, immunomodulators, biologic agents and small molecules. These therapies are only moderately effective. A significant proportion of patients fail to respond, do not fully respond, or lose response over time. Therefore, there is significant need for more selective and effective therapy options. Siglec-8 is an inhibitory receptor selectively expressed on human eosinophils and mast cells and represents a novel target for the treatment of IBD with the anti-Siglec-8 mAb, antolimab (AK002). We aimed to quantify and evaluate the activation state of mast cells and eosinophils in colonic tissue from IBD or non-diseased patients. In addition, we quantified the production of TNFa from human colon tissue mast cells and evaluated the inhibitory activity of antolimab (AK002) on these cells. Methods Single-cell suspensions were prepared by enzymatic digestion of fresh colonic biopsies from patients clinically diagnosed with IBD (n=29) or non-diseased control tissues (n=16). Multi-color flow cytometry was performed to identify major immune cell populations and evaluate the activation state of mast cells and eosinophils. Mast cells were FACS-sorted from human colon tissue to evaluate cytokine production and inhibitory activity of antolimab. Results The percentage of mast cells and the expression of the mast cell degranulation marker CD107a were significantly increased in ulcerative colitis (UC) patient biopsy tissue compared to Crohn’s disease (CD) and non-diseased colonic tissue (Figure 1A and B). Furthermore, FACS-sorted mast cells from human colon tissue produced significant quantities of TNFa that was reduced after ex vivo antolimab treatment. Colonic tissue eosinophils were also elevated in a subset of UC and CD patient biopsies, and all UC and CD tissue eosinophils displayed increased expression of the activation marker CD11b compared to control colonic tissue. Conclusions Mast cells and eosinophils may play a significant role in driving the pathogenesis of ulcerative colitis through the production of inflammatory mediators. The high expression of Siglec-8 and the inhibitory activity against mast cells suggests that antibodies that target this receptor, such as antolimab (AK002) represent a potential novel approach for the treatment of IBD.

Abstract

Objective: The well documented demographic shift to an aging population means that more people will in future be in need of long term residential care. Previous research has reported an increased risk of mental health issues and suicidal behaviour among older people living in residential care settings. However, there is little information on the actual prevalence of depression, self-harm, and suicidal ideation in this population, how it is measured and how care homes respond to these issues. \ud Method: This systematic review of international literature addressed three research questions relating to; the prevalence of mental health problems in this population; how they are identified and; how care homes try to prevent or respond to mental health issues. \ud Results: Findings showed higher reported rates of depression and suicidal ideation (but not self-harm) in care home residents compared to matched age groups in the community, variation in the use and appropriateness of standardised measures across studies and, interventions almost exclusively focused on increasing staff knowledge about mental health but with an absence of involvement of older people themselves in these programmes. \ud Conclusion: We discuss the implications of these findings in the context of addressing mental health difficulties experienced by older people in residential care and future research in this area.

Abstract

Introduction There is significant variation in processes and outcomes of care for patients with inflammatory bowel disease (IBD), suggesting opportunities to improve quality of care. Recent efforts to define quality measures for IBD have identified emergency room (ER) visits, hospitalizations, corticosteroid use, and opioid use as indicators of care quality. We hypothesized that IBD care could be improved through a structured quality improvement (QI) program. Methods We utilized the Breakthrough Series Collaborative approach developed by the Institute for Healthcare Improvement to improve care for adults with IBD. We identified primary and secondary drivers of urgent care need for patients including those at high risk for ER use, and a multi-stakeholder panel developed 19 practice change ideas that could influence those drivers. Between January 2018 and May 2019, clinical sites participating in a QI collaborative across the United States tested and implemented various change ideas, shared ongoing results during coached monthly webinars, and participated in 3 in-person meetings to learn QI methods and share best practices. Patient-reported outcomes (PROs) were collected at clinical visits, including recent ER use and hospitalizations, use of steroids and narcotics, and measures of care utilization. Providers rated whether patients were at high risk for urgent care needs. Site performance on key measures were monitored using statistical control charts, with assessment for common cause (due to chance) variation and special cause (non-random) variation. Results We collected data prospectively from 20,382 discrete visits at twenty-six participating clinical practices (14 academic/university, 12 private/community). Disease type included Crohn’s disease (58%), ulcerative colitis (39%), and other (3%); 54% were female. During the 15-month project period, improvement with special cause variation was noted across multiple measures. Collaborative-wide decreases were seen in ER utilization (18% to 14%, relative reduction of 22%; Figure), hospitalization (14% to 11%, relative reduction of 21%), steroid use (14% to 10%, relative reduction of 29%), and narcotic utilization (8% to 4%, relative reduction of 50%). Successful change ideas tested by sites included proactive maintenance of a “high risk” patient list, reserved outpatient visits for urgent needs, “morning-after” contact with patients who went to the ER, patient education about how and when to get help, and proactively scheduling earlier follow-up for high risk patients. Conclusions Outcomes of IBD care were improved using a structured QI program that facilitates small changes in practice structure, sharing of best practices across sites, and ongoing feedback. Spread of successful change ideas may facilitate broad improvement in IBD care and significant cost savings when applied to a large population. Relational aggression is often perceived as a female issue. Less is known about relational aggression in adolescent boys. This study examines whether the issues associated with relational aggression in girls are similar for boys to determine whether an intervention designed for girls would be relevant for boys. Focus group discussions illustrate that boys engage in relational aggression and that it appears to be lined to maintaining power and status amongst their friendship groups. After the intervention the boys reported having a greater understanding of relational aggression, in particular the effects of exclusion. They reported that conflict resolution and managing anger were difficult for them to do. They admitted that some of the techniques might be useful, but were less sure about whether they would use them. They expressed a desire to explore anger management in more detail. Aim To investigate Irish neonatal nurses' knowledge and beliefs of Kangaroo care. Background Although kangaroo care existed in other countries for 25 years, it is a new occurrence in Irish neonatal care. A review of the literature suggests that, while it demonstrates benefits for both infants and parents, some neonatal nurses do not exhibit an awareness of current kangaroo care research, or hold positive beliefs towards its use with preterm infants. As they have the most parent-infant contact and influence over whether kangaroo care is carried out, their knowledge and beliefs are of importance. Method A quantitative, descriptive design with neonatal nurses ( n = 62) was used. Findings Fifty six neonatal nurses (90.3%) believed kangaroo care a safe alternative for stable growing preterm infants, agreeing on the benefits for both infants and parents The overall level of neonatal nurses' knowledge of kangaroo care varied from good to excellent, the lowest score being 35/51. Results indicated nurses' uncertainty regarding kangaroo care with intubated infants, and infants requiring blood pressure support, umbilical lines and phototherapy. This suggests the need to provide education on kangaroo care to foster the development of more positive beliefs and increase staff knowledge of potential adverse effects.

Abstract

Relational aggression is common among girls. It can be distressing for the victim and may not always be recognised as a form of bullying. This paper evaluates a school–based intervention designed to help girls understand relational bullying. Girls from a single-sex school completed a two-stage evaluation of the intervention. Students showed greater awareness of relational bullying after the intervention and indicated that it would influence them to change their behaviour. At follow-up, retention of knowledge was good. Students reported greater self-monitoring of behaviour and less exclusivity. While this is encouraging, we cannot conclude that all negative behaviours ceased. Abstract

Teachers have an important role to play in the management and prevention of bullying. Although many anti-bullying programmes advocate a ‘whole school approach’, they tend to focus on student-student behaviours and rarely examine the nature of other relationships which exist in the school. This study examines bullying between students and teachers at two time points. Thirty per cent of students said they were bullied by teachers at both times. Numbers reporting they bullied teachers were 28 per cent and 16 per cent, respectively. The paper explores the important role that teachers play in dealing with bullying in a school setting by modelling appropriate behaviours and dealing with it effectively. If teachers are victimised by or engage in bullying students this has implications for the ethos within the school and may also impact on the success of anti-bullying programmes. In this article, the authors report on the evaluation of an anti-bullying programme, the cool school programme (CSP), conducted in a secondary school in Ireland. The study explored how participation in the anti-bullying programme affected students and teachers in one second-level school. The programme focused on students' faith in the teachers' ability to deal with bullying effectively, their willingness and confidence in telling a teacher about bullying and whether they felt safe in school. Teacher confidence and behaviour with regard to bullying were also explored. The study found that 91 per cent of students felt that teachers took bullying seriously. Eighty-nine per cent thought that teachers knew how to deal with bullying and 75 per cent felt able to tell a teacher if they were bullied. We report the effects of stimulus pre-exposure in combination with either non-specific or imagery instructions on face-name learning in 15 memory-impaired subjects. Pre-exposure involved viewing the face alone (for 6 seconds) and making various judgements about the person depicted (e.g. honesty), prior to being shown the face-name pair for learning (for 4 seconds). When the face-name pairs were shown for 4 seconds accompanied by non-specific instructions, pre-exposure to the faces alone led to performance levels as good as when face-name pairs were shown for 10 seconds, and significantly greater performance than when the face-name pairs were shown for just 4 seconds. In line with the results of previous studies, imagery was found significantly to enhance face-name association learning, compared to non-specific instructions. Purpose of review To provide a critical review of the literature concerning the significance of mucosal healing as a predictor of future clinical and endoscopic course in patients with inflammatory bowel disease. Particular focus is given to articles published within the last year. Recent findings In patients with Crohn's disease, mucosal healing appears to predict future endoscopic activity. The ability to predict future clinical activity is uncertain, likely limited by present techniques for assessment of clinical activity. Recent studies provide support for escalation of treatment in response to ongoing endoscopic disease activity. However, guidance regarding de-escalation of medical therapy is lacking. In patients with ulcerative colitis, mucosal healing appears to be an important predictor of future endoscopic and clinical activity. Recent evidence suggests that mucosal healing is also an important predictor of long-term outcomes such as need for future colectomy. Summary Assessment of mucosal healing is an important tool in the management of patients with inflammatory bowel disease. The pursuit of endoscopic remission has a clear role in the management of patients with ulcerative colitis, but the application of this parameter to the care of patients with Crohn's disease requires further research.

Abstract

Purpose The long-term outcome in spina bifida–Chiari II– hydrocephalus complex is poorly understood. Traditional neurosurgical outcome measures are crude. Neuropsychological testing is increasingly important in outcome assessment. We investigated the health, disability, lifestyle and cognitive function in adults who had myelomeningocoele closure at birth. Methods Adult patients under routine follow-up were assessed in a joint neurosurgery/neuropsychology clinic. Patientscompletedlifestylequestionnaires,thehydrocephalus outcome questionnaire (HOQ) and underwent cognitive testing. Clinical variables including number of shunt revisions, shunt infection and surgical decompression of foramen magnum, which may influence outcome, were investigated. Results Twenty-one adults with a median age of 35 years were investigated. All had treated hydrocephalus, and eight had foramen magnum decompression for headache or progressive brainstem symptoms with stabilisation of symptoms in seven and improvement in one. Only eight patients were living independently, five were in paid employment and five work voluntarily. HOQ scores for cognitive function were lower (0.56 ± 0.20; mean ± standard deviation (SD)) than those for physical (0.64 ± 0.15) and social–emotional (0.65 ± 0.17) health. Cognitive function varied across the cohort with attention most severely affected (73.9 ± 17.0; mean ± SD). Repeated episodes of shunt malfunction or foramen magnum decompression were not associated with a worse cognitive function. Conclusions Despite intervention in childhood and adequate cerebrospinal fluid diversion the prognosis for independent living into adulthood remains poor. All patients have elements of cognitive impairment. Structural brain abnormalities may be more important determinants of cognitive outcome than shunt malfunction.

This letter to the editor discusses a literature review to assess the evidence on the efficacy of oral contraceptives when combined with antibiotic therapy. It formulates a rational guideline for patients with inflammatory bowel disease receiving concomitant oral contraceptives and antibiotic therapy.
