IntroductionCancer survivors experienced poorer health-related quality of life (HRQoL) and greater psychological distress during the COVID-19 pandemic than those without cancer. However, the underlying mechanisms that may explain how negative experiences during the pandemic are associated with distress and HRQoL remain unknown. We examined whether psychosocial risk factors (i.e., healthcare disruption, disruption to daily activities and social interaction [DDASI], and financial hardship) mediated the relationship between negative COVID-19-related experiences and cancer survivors’ HRQoL and psychological distress (i.e., depressive symptoms, and anxiety) and whether the mediating effects were moderated by psychosocial protective factors (i.e., stress management ability and social support).MethodsA total of 9,651 cancer survivors completed a questionnaire assessing negative COVID-19-related experiences, psychosocial and practical experiences, and HRQoL. Conditional process analysis was used to evaluate the proposed moderated mediation models.ResultsParticipants had a mean age of 63.8 years (SD = 12.3) and were mostly non-Hispanic White (82.3%). DDASI and financial hardship mediated the relationship between negative COVID-19-related experiences and cancer survivor’s HRQoL and psychological distress. Stress management ability buffered the indirect effect of DDASI on cancer survivors’ HRQoL and psychological distress. Social support buffered the indirect effect of financial hardship on HRQoL and depressive symptoms.ConclusionFinancial resources and social interactions may buffer negative effects of major disruptions such as the COVID-19 pandemic. Future studies should assess the longitudinal impact of these associations.

Summary: Background: Consumption of fibre, fruits and vegetables have been linked with lower colorectal cancer (CRC) risk. A genome-wide gene-environment (G × E) analysis was performed to test whether genetic variants modify these associations. Methods: A pooled sample of 45 studies including up to 69,734 participants (cases: 29,896; controls: 39,838) of European ancestry were included. To identify G × E interactions, we used the traditional 1--degree-of-freedom (DF) G × E test and to improve power a 2-step procedure and a 3DF joint test that investigates the association between a genetic variant and dietary exposure, CRC risk and G × E interaction simultaneously. Abstract In a mobile ad hoc network (MANET), all nodes are communicated with one another across wireless networks to create a temporary network without the support of centralised management. Due to dynamic topology in MANET, secure routing is a crucial issue. The existing secure routing protocol and their security concerns are analysed in this work. The suggested Swarm Intelligence‐based Secure Ad‐hoc On‐demand Distance Vector (SIS‐AODV) algorithm offers security by applying a secret key and hash mechanism to prevent the involvement of malicious nodes in routing operations. A secure routing system of MANET guards against internal and external network attacks. The proposed SIS‐AODV algorithm consists of two sections: the secret key value generated by applying Elliptical Curve Cryptography (ECC)‐based algorithm and the PRESENT algorithm to encrypt the data packets. Besides, authentication and non‐repudiation are applied using the H‐PRESENT 128 algorithm. The PRESENT algorithm and H‐PRESENT 128 hash function require less computational power. Centralised management is optional in this scheme, so overhead decreases. The second section of SIS‐AODV consists of Ant Colony Grey Wolf Optimization over the AODV algorithm to improve network performance while implementing a security algorithm over MANET. Analysis results show maximum performance with a packet delivery ratio of 98% and throughput of 85%. In addition, end‐to‐end delay is reduced by up to 25%, and routing overhead decreases by up to 20%. Four sustainable feeds were formulated with locally available ingredients of Benin, produced by extrusion, chemically and physically characterized and their performance was explored. Formulated feeds (F1, F2, F3 and F4) and a Control Feed (CF) were fed to Clarias gariepinus juveniles for 16 weeks and fish growth parameters were monitored. Nutritional and sensorial qualities of the fresh and smoked fish were evaluated. Formulated feeds had the same levels of protein and carbohydrate contents and were similar to CF in this concern. The fat content of F2, F3 and F4 feeds was lower than that of the CF except for F1, which had similar fat content. All tested feeds had floatability rates exceeding 80%. The mean weight gain of the fish ranged from 168 g to 401 g. The best Feed Conversion Ratio was recorded with CF (1.01) followed by F2 (1.21). The Protein Efficiency Ratio of F2 (2.14) and CF (2.51) did not differ statistically. Fish fed with F2 had the best nutritional profile (protein: 682 g/kg, and fat: 231 g/kg) compared with fish fed on other feed formulas and the CF (protein: 634 g/kg, and fat: 274 g/kg). Smoked fish fed F2 and CF recorded similar acceptability from consumers.

This empirical research on children’s poverty and the accompanying risk behavior was conducted in the Baktalórántháza micro-region, in one of the most disadvantaged micro-regions of Hungary. The study, completed in 2023, was conducted utilizing three methods, a questionnaire for families, interviews, and focus group interviews with social professionals working in the settlements. The region is one of the ten micro-regions with the highest poverty rate in the country. The majority of the population only has an elementary education, and the proportion of graduates is much lower than the national average. The proportion of households with three or more children is higher than the national average and the proportion of unemployed people in households with children is twice as high as the national average. Based on the experience of social workers working in the area, in addition to smoking and drinking alcohol, the consumption of psychoactive and psychotropic substances has increased among adolescents and young adults. Based on various indicators, children regularly consume illegal drugs. The origin and composition of these drugs are typically unknown. According to the reports by drug users, everyday life is easier, and they can escape from problems when under the influence of drugs. Based on the observations of experts, the consumption of various psychoactive substances has harmful effects on behavior, health, learning, and family life. School performance and the ability to think and learn decrease. Drug users are dissatisfied with their lives, have problems with social relationships, engage in partner violence, and may develop antisocial behavior in their lives.

Abstract The Human Reference Atlas (HRA) is defined as a comprehensive, three-dimensional (3D) atlas of all the cells in the healthy human body. It is compiled by an international team of experts who develop standard terminologies that they link to 3D reference objects, describing anatomical structures. The third HRA release (v1.2) covers spatial reference data and ontology annotations for 26 organs. Experts access the HRA annotations via spreadsheets and view reference object models in 3D editing tools. This paper introduces the Common Coordinate Framework (CCF) Ontology v2.0.1 that interlinks specimen, biological structure, and spatial data, together with the CCF API that makes the HRA programmatically accessible and interoperable with Linked Open Data (LOD). We detail how real-world user needs and experimental data guide CCF Ontology design and implementation, present CCF Ontology classes and properties together with exemplary usage, and report on validation methods. The CCF Ontology graph database and API are used in the HuBMAP portal, HRA Organ Gallery, and other applications that support data queries across multiple, heterogeneous sources.

Abstract Background Ovarian cancer is the fifth most common female cancer in the United States. There have been very few studies investigating mental health diagnoses among ovarian cancer survivors with long‐term follow up. The aim of this study is to examine the incidence of mental illness among ovarian cancer survivors compared to a general population cohort. A secondary aim is to investigate risk factors for mental illnesses among ovarian cancer survivors. Patients and methods Cohorts of 1689 ovarian cancer patients diagnosed between 1996 and 2012 and 7038 women without cancer matched by age and birth state from the general population were identified. Mental health diagnoses were identified from electronic medical records and statewide healthcare facilities data. Cox proportional hazard models were used to estimate hazard ratios (HRs). Results Ovarian cancer survivors experienced increased risks of mental illnesses within the first 2 years after cancer diagnosis (HR = 3.55, 95% CI = 3.04–4.14). The risks of depression among ovarian cancer survivors were nearly 3‐fold within the first 2 years of cancer diagnosis (HR = 2.59, 95% CI = 1.94–3.47), and 1.69‐fold at 2–5 years after cancer diagnosis (HR = 1.69, 95% CI = 1.18–2.42). Ovarian cancer survivors experienced an 80% increased risk of death with a mental illness diagnosis (HR = 1.80, 95% CI = 1.48–2.18) and a 94% increased risk of death with a depression diagnosis (HR = 1.94, 95% CI = 1.56–2.40). Conclusions Higher risks of mental illnesses were observed among ovarian cancer survivors throughout the follow‐up periods of 0–2 years and 2–5 years after cancer diagnosis. Multidisciplinary care is needed to monitor and treat mental illnesses among ovarian cancer survivors.

Respirometric microbial assays are gaining popularity, but their uptake is limited by the availability of optimal O2 sensing materials and the challenge of validating assays with complex real samples. We conducted a comparative evaluation of four different O2-sensing probes based on Pt-porphyrin phosphors in respirometric bacterial assays performed on standard time-resolved fluorescence reader. The macromolecular MitoXpress, nanoparticle NanO2 and small molecule PtGlc4 and PtPEG4 probes were assessed with E. coli cells in five growth media: nutrient broth (NB), McConkey (MC), Rapid Coliform ChromoSelect (RCC), M-Lauryl lauryl sulfate (MLS), and Minerals-Modified Glutamate (MMG) media. Respiration profiles of the cells were recorded and analyzed, along with densitometry profiles and quenching studies of individual media components. This revealed several limiting factors and interferences impacting assay performance, which include probe quenched lifetime, instrument temporal resolution, inner filter effects (mainly by indicator dyes), probe binding to lipophilic components, and dynamic and static quenching by media components. The study allowed for the ranking of the probes based on their ruggedness, resilience to interferences and overall performance in respirometric bacterial assays. The ‘shielded’ probe NanO2 outperformed the established MitoXpress probe and the small molecule probes PtGlc4 and PtPEG4.

Bleaching and softening of the hard fiber “Coir” renders it suitable for use in the coir industry, which presently adopts various chemical treatment processes that are not eco-friendly. The present study aims at treatment of coir fibers with Averrhoa bilimbi for enhanced bleaching and softening. The quality of the treated fibers with respect to physical and chemical characterization viz. variation in lignin content, flexural rigidity to assess the degree of softness, light fastness, tensile strength and brightness index has been carried out. The treated fibers displayed remarkable increase in brightness index from 9.45 to 11.19. Removal of surface projections on the coir fiber upon treatment is evident from Scanning Electron Microscope images. These surface changes can be attributed to the fiber softening.

Abstract Mesoporous, bifunctional MgO-SnO2 nanocatalysts with enhanced surface area are used for producing biodiesel from waste cooking oil. Biodiesel with yield of 80% is achieved within the first 20 min when transesterification is carried out at an optimum condition of 18:1 methanol to oil ratio, 2 wt% of nanocatalyst, and at a reaction temperature of 60 °C. The conversion gives a maximum yield of 88% when transesterification is allowed to continue for 120 min. The waste cooking oil used in this work is dominated with linoleic acid and oleic acid, which during transesterification gets converted into methyl linoleate and 9-octadecenoic acid methyl ester. These nanocatalysts are fabricated using a composite of rutile (tetragonal) phase SnO2 and cubic phase MgO nanostructures with prominent crystal orientation along [211] and [200] plane respectively. The MgO-SnO2 nanocomposites with an enhanced surface area of 31 m2/g, basic sites of 2 mmol/g, and particle size of ~15 nm are synthesized by novel sequential thermal decomposition and sol-gel technique. The synthesized wide band gap nanocomposites have Mg and Sn in the ratio of 15:1 and do not have any impurity phases as observed in the X-ray diffraction pattern and EDS spectrum. The presence of surface oxygen states and Mg2+ and Sn4+ oxidative states is responsible for the catalytic activity and recyclability displayed by the composites. This work signifies the role of nanocomposites and their synthesis conditions in improving the rate of transesterification. These metal oxide nanocomposites which are nontoxic, stable, cost effective, and easier to synthesis are promising catalysts for large-scale transesterification of waste cooking oil to biodiesel.

Sub-valvular aortic stenosis (SAS) occurs due to a fibrous membrane or a muscular narrowing causing left ventricular outflow tract obstruction. The physiological changes of pregnancy may exacerbate the cardiac condition posing significant challenges for anaesthesia and surgery. A 34 years primigravida, with 32 weeks gestation, a known case of sub-valvular aortic stenosis presented in the emergency room in view of leaking/bleeding per-vagina. Risk factors such as tachycardia, decrease in afterload, preload and increased left ventricle contractility lead to exacerbation of the obstruction and should be avoided. We report a successful anaesthetic management of her lower segment cesarean section while balancing the physiological changes of pregnancy superimposed by pathology of the disease.

Rising pollution levels, altered lifestyles, and poor diets are contributing to a dramatic increase in the prevalence of cancer worldwide. When it comes to threats to human health, cancer is among the top concerns. For decades, natural products, particularly secondary metabolites derived from fungi, have been used to treat a range of chronic diseases, including cancer. Red yeast rice (RYR), is a type of traditional Chinese medicine that has been utilized throughout Asia for about one thousand years, both in culinary and curative contexts. The principal active compound in RYR is Monacolin K, a secondary fungal metabolite that is a leading drug for lowering blood lipid levels and treating hypercholesterolemia. Monacolin K also prevents gastric cancer, colon cancer, breast cancer, acute myeloid leukemia, and nervous system disorder like depression, Parkinson's disease, and schizophrenia. This review article explains the biosynthetic pathway as well as the mechanism of action of lovastatin in reducing blood cholesterol levels, cancer, neurological disorders, and antifungal activities. It also highlights the futuristic role of lovastatin in the food and drug industry.

This research aims to determine the effect of activated carbon obtained from cocoa husks (Theobroma cacao L.) on the adsorption of iron (II) present in water of an aquifer for human consumption. Charcoal was prepared at different carbonization temperatures (500, 600 and 700 °C) for 30 minutes and activated with phosphoric acid in the proportions of 1:1. Iron (II) adsorption was determined as a function of variations in mixing speed, contact time, charcoal dosage and stirring speed. It was determined that, over the range considered, agitation speeds had no significant effect on the percentage of iron (II) removal, being the dose of activated carbon and temperature, the most influential variables. The water samples had an initial iron concentration of 3.15 mg/L and 4 mg/L. The best iron (II) adsorption result was obtained with activated carbon at the carbonization temperature of 700 °C, with a mass of 1.5 g of carbon, with efficiencies of 93 % and 98 % for both samples considered. Based on the results, it was concluded water for human consumption is treatable with activated carbon derived from Theobroma cacao L. for the adsorption of iron (II), considering that this parameter is below the maximum limit of 0.3 mg/L allowed by current regulations.

Abstract Nutritious complementary feeding is often not affordable in Benin, and iron deficiency exists. This research aimed at formulating an affordable and sensory acceptable complementary food supplement using local food ingredients to increase iron intake among children aged 6–59 months in Benin. The complementary food supplement was formulated to ensure that 10 g would cover 25% of the estimated average requirements for iron for children aged 6 to 12 months. Adansonia digitata fruit pulp, Moringa oleifera leaf powder, and Cochlospermum tinctorium root powder were used to compose the complementary food supplement, which was mixed with maize and sorghum ogi porridges before being presented to the mothers and children for the acceptability test. The mineral contents of Adansonia digitata fruit pulp in mg/100 g dw were 9.9 ± 0.1 for iron and 0.9 ± 0.1 for zinc. The iron and zinc contents of Moringa oleifera leaf powder and Cochlospermum tinctorium root powder in mg/100 g dw were 34.1 ± 2.2 and 26.8 ± 2.7 and 9 ± 0.0 and 0.9 ± 0.0, respectively. The complementary food supplement contained, in mg/100 g dw, 17.4 ± 1.1 of iron and 1.2 ± 0.1 of zinc. The maize and sorghum ogi porridges enriched with the complementary food supplement at substitution rates of 15% and 16% (in dry weight), respectively, were acceptable to 85% of children for sorghum ogi porridge and 87% for maize ogi porridge. The present study demonstrated the potential of local food ingredients in the formulation of an iron‐rich and acceptable complementary food supplement for children aged 6–59 months in Benin.

Abstract The COVID-19 pandemic is still raging in most countries. Although the recent mass vaccination campaign has opened a new chapter in the battle against SARS-CoV-2, the world is still far from herd immunity. There is an urgent need to identify healthy people at high risk of contracting COVID-19, as well as supplements and nutraceuticals that can reduce the risk of infection or mitigate symptoms. In the present study, a metabolic phenotype that could protect individuals from SARS-CoV-2 infection or predispose them to developing COVID-19 was investigated. Untargeted metabolomics was performed on serum samples collected from 51 healthcare workers who were in good health at the beginning of the COVID-19 outbreak in Italy, and who were later exposed to the same risk of developing COVID-19. Half of them developed COVID-19 within three weeks of the blood collection. Our results demonstrate the presence of a specific signature associated with protection from SARS-CoV-2. Circulating monolaurin, which has well-known antiviral and antibacterial properties, was higher in protected subjects, suggesting a potential defensive role against SARS-CoV-2 infection; thus, dietary supplements could boost the immune system against this infection. In addition, our data demonstrate that people with higher levels of cholesterol are at higher risk of developing COVID-19. The present study demonstrates that metabolomics can be of great help for developing personalized medicine and for supporting public healthcare strategies. Studies with larger cohorts of subjects are necessary to confirm our findings.

The aim of this research was to determine the efficiency of activated carbon obtained from coconut (Cocos nucifera) endocarp in the reduction of color in runoff water to improve its quality. For this purpose, activated carbon was produced at a carbonization temperature of 700 °C in a time of 30 minutes and phosphoric acid at a concentration of 85% was used as an activating agent. A completely randomized design was used, consisting of 3 treatments (100, 50 and 25 g of activated carbon) and a control (without treatment), with three replicates of each treatment, where 1 L of water from the uptake was filtered. It was determined that the best percentage removal was obtained with treatment 3 (T3) in the reduction of color and turbidity with average values of 97.56% and 97.11%, respectively. An activated carbon filter was installed in the San Lorenzo - Moyobamba sector in a similar way to the laboratory tests, in which the efficiency of this product in reducing the color and turbidity of the water was demonstrated, achieving values suitable for human consumption.

Colorectal cancer (CRC) is increasingly recognized as a heterogeneous disease. No studies have prospectively examined associations of blood metabolite concentrations with all-cause mortality in patients with colon and rectal cancer separately. Targeted metabolomics (Biocrates AbsoluteIDQ p180) and pathway analyses (MetaboAnalyst 4.0) were performed on pre-surgery collected plasma from 674 patients with non-metastasized (stage I–III) colon (n = 394) or rectal cancer (n = 283). Metabolomics data and covariate information were received from the international cohort consortium MetaboCCC. Cox proportional hazards models were computed to investigate associations of 148 metabolite levels with all-cause mortality adjusted for age, sex, tumor stage, tumor site (whenever applicable), and cohort; the false discovery rate (FDR) was used to account for multiple testing. A total of 93 patients (14%) were deceased after an average follow-up time of 4.4 years (60 patients with colon cancer and 33 patients with rectal cancer). After FDR adjustment, higher plasma creatinine was associated with a 39% increase in all-cause mortality in patients with rectal cancer. HR: 1.39, 95% CI 1.23–1.72, pFDR = 0.03; but not colon cancer: pFDR = 0.96. Creatinine is a breakdown product of creatine phosphate in muscle and may reflect changes in skeletal muscle mass. The starch and sucrose metabolisms were associated with increased all-cause mortality in colon cancer but not in rectal cancer. Genes in the starch and sucrose metabolism pathways were previously linked to worse clinical outcomes in CRC. In summary, our findings support the hypothesis that colon and rectal cancer have different etiological and clinical outcomes that need to be considered for targeted treatments.

Algae are existing macroscopic materials with substantial benefits, including as important growth regulators and macronutrients and micronutrients for the growth of healthy crop plants. Biofertilizers obtained from algae are identified as novel production fertilizers or innovative biofertilizers without the detrimental impacts of chemicals. Seaweeds contain many water-soluble minerals and nutrients that plants can easily absorb and that are valuable for crop plants’ growth. At present, Ascophyllum nodosum (L.) Le Jolis extract outperforms chemical fertilizers in terms of increasing seed germination, plant development, and yield, as well as protecting plants from severe biotic and abiotic stresses. A. nodosum contains bioactive compounds that exhibit an array of biological activities such as antibiotic, anti-microbial, antioxidant, anti-cancer, anti-obesity, and anti-diabetic activities. A. nodosum extract (AnE) contains alginic acid and poly-uronides that improve soil’s water-carrying ability, morsel structure, aeration, and capillary action, stimulating root systems in plants, increasing microbial activity in soil, and improving mineral absorption and availability. The scientific literature has comprehensively reviewed these factors, providing information about the different functions of A. nodosum in plant growth, yield, and quality, the alleviation of biotic and abiotic stresses in plants, and their effects on the interactions of plant root systems and microbes. The application of AnE significantly improved the germination rate, increased the growth of lateral roots, enhanced water and nutrient use efficiencies, increased antioxidant activity, increased phenolic and flavonoid contents, increased chlorophyll and nutrient contents, alleviated the effects of abiotic and biotic stresses in different crop plants, and even improved the postharvest quality of different fruits.

Acute respiratory diseases (ARIs), a group of infectious diseases with similar epidemiological and clinical characteristics, are the most common reason for seeking medical care, children skipping attendance of educational institutions and parents skipping work. The clinical picture of ARI is characterized by a combination of the following symptoms: increased body temperature, nasal discharge, mostly mucous, nasal congestion, sore or scratchy throat and cough. In some cases, myalgia and manifestations of asthenia such as weakness, rapid fatigue, headache and decreased appetite may be noted. Pathogenetic therapy of ARI is based on clinical syndromes and includes symptomatic, antiviral and antibacterial drugs, also mucolytics, expectorants, cough suppressants; H1-histamine receptor blockers and immunocorrectors if necessary. Hyperthermia is the most significant problem faced by parents of young patients and pediatricians when dealing with ARI. The production of interferon gamma, interleukin-2 and tumor necrosis factor, which stimulate the production of IgG, is significantly increased with fever. Therefore, it is especially important to know the algorithms of behavior and criteria for understanding when and what antipyretics should be prescribed to a child. Administration of antipyretics for fever is aimed not only at stopping the fever, but also to improve the child’s wellbeing and reduce the pain syndrome during ARI. When choosing antipyretics, it is necessary to consider the age of the child, the preferred method of administration, the allowable doses, the frequency of administration, the prevention of complications in children at risk. Antipyretics may be prescribed in the form of combined drugs, not only able to reduce fever, but also eliminate other symptoms that occur against the background of acute respiratory infection.

Introduction: Coronavirus disease 2019 (COVID-19) is a viral disease which originated in the city of Wuhan (China) and progressively spread to all the continents and brought the world to a standstill. Various severity markers have been seen to play a role in the COVID-19. International studies have demonstrated role of serum electrolyte in COVID-19 as a potential severity marker. Hence, this study was undertaken in Indian patients. Material and methods: 100 COVID-19 reverse transcription polymerase chain reaction (RT-PCR) positive patient of non-severe and severe disease of either gender getting admitted to hospital and above 18 years were enrolled in the month of August to September 2020. The mean values of electrolytes and high-sensitivity C-reactive protein (HsCRP) in non- severe and severe disease were compared and correlated. Results: 100 patients including 70 of non-severe and 30 of severe disease were evaluated. The mean sodiumwas 134.03±6.77mEq/L and 135.5± 6.77mEq/l in the non-severe and the severe groups respectively (p value=0.91); potassium was 4.24±0.61mEq/l and 4.52±0.80 mEq/l respectively (p value=0.75); corrected serum calcium was 8.38±0.74 mg/dl and 8.48±0.74mg/dl respectively (p value = 0.95); phosphorus levels was 3.66±1.99mg/dl and 3.45±1.63 respectively (p value = 0.52); and serum magnesium level was 2.15±0.3 mEq/l and 2.03±0.56 mEq/l (p value = 0.18). The mean level of HsCRP was 30.95±49.41 mg/L in non- severe while 94.78±79.62 mg/L in severe infection (p value=0.03). In the severe group, the electrolyte values were found to be poorly correlated with the hsCRP levels. Conclusion: Electrolytes does not serve as severity markers in COVID-19 in an Indian population.

With food insecurity rising dramatically in Sub-Saharan Africa, promoting the use of sorghum, cowpea and cassava flours in staple food such as bread may reduce wheat imports and stimulate the local economy through new value chains. However, studies addressing the technological functionality of blends of these crops and the sensory properties of the obtained breads are scarce. In this study, cowpea varieties (i.e., Glenda and Bechuana), dry-heating of cowpea flour and cowpea to sorghum ratio were studied for their effects on the physical and sensory properties of breads made from flour blends. Increasing cowpea Glenda flour addition from 9 to 27% (in place of sorghum) significantly improved bread specific volume and crumb texture in terms of instrumental hardness and cohesiveness. These improvements were explained by higher water binding, starch gelatinization temperatures and starch granule integrity during pasting of cowpea compared to sorghum and cassava. Differences in physicochemical properties among cowpea flours did not significantly affect bread properties and texture sensory attributes. However, cowpea variety and dry-heating significantly affected flavour attributes (i.e., beany, yeasty and ryebread). Consumer tests indicated that composite breads could be significantly distinguished for most of the sensory attributes compared to commercial wholemeal wheat bread. Nevertheless, the majority of consumers scored the composite breads from neutral to positive with regard to liking. Using these composite doughs, chapati were produced in Uganda by street vendors and tin breads by local bakeries, demonstrating the practical relevance of the study and the potential impact for the local situation. Overall, this study shows that sorghum, cowpea and cassava flour blends can be used for commercial bread-type applications instead of wheat in Sub-Saharan Africa.

Abstract Background Pediatric Advanced Life Support provides guidelines for resuscitating children in cardiopulmonary arrest. However, the role physicians’ attitudes and beliefs play in decision-making when terminating resuscitation has not been fully investigated. This study aims to identify and explore the vital “non-medical” considerations surrounding the decision to terminate efforts by U.S.-based Pediatric Emergency Medicine (PEM) physicians. Methods A phenomenological qualitative study was conducted using PEM physician experiences in terminating resuscitation within a large freestanding children’s hospital. Semi-structured interviews were conducted with 17 physicians, sampled purposively for their relevant content experience, and continued until the point of content saturation. Resulting data were coded using conventional content analysis by 2 coders; intercoder reliability was calculated as κ of 0.91. Coding disagreements were resolved through consultation with other authors. Results Coding yielded 5 broad categories of “non-medical” factors that influenced physicians’ decision to terminate resuscitation: legal and financial, parent-related, patient-related, physician-related, and resuscitation. When relevant, each factor was assigned a directionality tag indicating whether the factor influenced physicians to terminate a resuscitation, prolong a resuscitation, or not consider resuscitation. Seventy-eight unique factors were identified, 49 of which were defined by the research team as notable due to the frequency of their mention or novelty of concept. Conclusion Physicians consider numerous “non-medical” factors when terminating pediatric resuscitative efforts. Factors are tied largely to individual beliefs, attitudes, and values, and likely contribute to variability in practice. An increased understanding of the uncertainty that exists around termination of resuscitation may help physicians in objective clinical decision-making in similar situations.

The expression of some microRNAs (miRNA) is modulated in response to cigarette smoke (CS), which is a leading cause of major preventable diseases. However, whether miRNA expression is also modulated by the aerosol/extract from potentially reduced-risk products is not well studied. The present work is a meta-analysis of 12 in vitro studies in human organotypic epithelial cultures of the aerodigestive tract (buccal, gingival, bronchial, nasal, and small airway epithelia). These studies compared the effects of exposure to aerosols from electronic vapor (e-vapor) products and heated tobacco products, and to extracts from Swedish snus products (in the present work, will be referred to as reduced-risk products [RRPs]) on miRNA expression with the effects of exposure to CS or its total particulate matter fraction. This meta-analysis evaluated 12 datasets of a total of 736 detected miRNAs and 2775 exposed culture inserts. The t-distributed stochastic neighbor embedding method was used to find similarities across the diversity of miRNA responses characterized by tissue type, exposure type, and product concentration. The CS-induced changes in miRNA expression in gingival cultures were close to those in buccal cultures; similarly, the alterations in miRNA expression in small airway, bronchial, and nasal tissues resembled each other. A supervised clustering was performed to identify miRNAs exhibiting particular response patterns. The analysis identified a set of miRNAs whose expression was altered in specific tissues upon exposure to CS (e.g., miR-125b-5p, miR-132-3p, miR-99a-5p, and 146a-5p). Finally, we investigated the impact of RRPs on miRNA expression in relation to that of CS by calculating the response ratio r between the RRP- and CS-induced alterations at an individual miRNA level, showing reduced alterations in miRNA expression following RRP exposure relative to CS exposure (94 % relative reduction). No specific miRNA response pattern indicating exposure to aerosols from heated tobacco products and e-vapor products, or extracts from Swedish snus was identifiable.

Background and Objective: The new study model of the Medical Council of India 2015, suggested the introduction of a module of early clinical exposure (ECE) to 1st year medical students to improve their understanding of the subjects taught. Thus, the main objective of the study was to assess; how students respond to the modified curriculum of ECE in the 1st year. Methodology: Cross-sectional study was conducted among 1st year medical students (n = 140) and were brought to the hospital; followed by the demonstration of a visual relay of an appendectomy procedure, which in turn was followed by an interactive session with the facilitators. Following this, the students filled a questionnaire (designed as per the Likert scale) about their opinion of the module. In addition, a test was conducted before and after the module to assess the comprehension of the subject by the students. Statistical analysis was performed using descriptive statistics (for qualitative variables) and t-test, for determining the significance between pre- and post-test scores. Results: Ninety-five percent of the students responded extremely well to ECE and was appreciated by the students. This was clear through the positive feedback received on improvement in understanding and interest in the subject. The pre- and post-test evaluation also resulted in a significant increase in test scores, (P = 2.20e−16). Conclusion: ECE module significantly helped 1st year medical students to comprehend the topic better. It made learning interesting and helped those correlate basic sciences with their clinical applications and relevance in practical medicine.

Abstract Background The presence of an extra testis is referred to as polyorchidism and is a rare congenital anomaly. There are with less than 200 cases reported, globally. It carries with it, a risk of malignancy making astute diagnosis and follow-up important. The management protocol of polyorchidism is ambiguous with factors such as age of the patient, location, size and anatomical organization of the testicular system playing an important role. Case presentation A 28-year-old male who presented with a non- tender scrotal swelling which on clinical and sonographic examination was diagnosed to be a case of triorchidism (the most common type of polyorchidism). Type 2 polyorchidism with normal epididymis and vas deference was observed. The case was conservatively managed and regular follow-up was advised. Conclusion Polyorchidism is a rare congenital anomaly. Patients with Polyorchidism can be conservatively managed. The reproductive potential and possible concomitant symptoms and complications including cryptorchidism, inguinal hernia and torsion will affect the management strategy. However, if the supernumerary testis is suspected of undergoing neoplastic transformation, it should be resected.

BackgroundSurveys play a vital role in cancer research. During the COVID-19 pandemic, the use of electronic surveys is crucial to improve understanding of the patient experience. However, response rates to electronic surveys are often lower compared with those of paper surveys. ObjectiveThe aim of this study was to determine the best approach to improve response rates for an electronic survey administered to patients at a cancer center during the COVID-19 pandemic. MethodsWe contacted 2750 patients seen at Moffitt Cancer Center in the prior 5 years via email to complete a survey regarding their experience during the COVID-19 pandemic, with patients randomly assigned to a series of variations of prenotifications (ie, postcard, letter) or incentives (ie, small gift, modest gift card). In total, eight combinations were evaluated. Qualitative interviews were conducted to understand the level of patient understanding and burden with the survey, and quantitative analysis was used to evaluate the response rates between conditions. ResultsA total of 262 (9.5%) patients completed the survey and 9 participated in a qualitative interview. Interviews revealed minimal barriers in understanding or burden, which resulted in minor survey design changes. Compared to sending an email only, sending a postcard or letter prior to the email improved response rates from 3.7% to 9.8%. Similarly, inclusion of an incentive significantly increased the response rate from 5.4% to 16.7%, especially among racial (3.0% to 12.2%) and ethnic (6.4% to 21.0%) minorities, as well as among patients with low socioeconomic status (3.1% to 14.9%). ConclusionsStrategies to promote effective response rates include prenotification postcards or letters as well as monetary incentives. This work can inform future survey development to increase response rates for electronic surveys, particularly among hard-to-reach populations.

Natural (microbial) communities are complex ecosystems with many interactions and cross-dependencies. Among other factors, selection pressures from the environment are thought to drive the composition and functionality of microbial communities. Fermented foods, when processed using non-industrial methods, harbor such natural microbial communities. In non-alcoholic fermented foods the fermenting microbiota is commonly dominated by 4–10 species of bacteria, which make them suitable model systems to study ecosystem assembly and functioning. In this study, we assess the influence of the environment on the composition of microbial communities of traditional fermented products from Africa. We compare differences between microbial communities that are found in similar products but come from different countries, hypothesizing they experience different environmental selection pressures. We analyzed bacterial community composition in 36 samples of various cereal-based fermented foods from Benin, Tanzania and Zambia using 16S rDNA amplicon sequencing. The differential abundance analysis indicates that the bacterial communities of fermented foods from the three countries are dominated by mostly lactic acid bacteria belonging to the genera of Lactobacillus, Weisella and Curvibacter. The samples from Zambia contain the most dissimilar microbial communities in comparison with samples from Benin and Tanzania. We propose this is caused by the relatively low temperature in Zambia, suggesting that indeed environmental selection can shape community composition of fermenting microbes.

English learners (ELs) in K–12 schools must acquire English while simultaneously mastering content knowledge. Educational technology may support students’ learning through the affordance of individualized language practice. The current randomized controlled trial intervention study examined the effects of Rosetta Stone Foundations software on English learning among middle school ELs. The study took place in Grades 6 to 8 of an urban U.S. school district ( N = 221). Predictors of interest included time of testing (pretest vs. posttest) and software usage, and covariates included grade level, sex, and attendance. Additionally, socioeconomic status and home language were accounted for due to sample homogeneity. Multilevel models indicated that treatment group students showed larger gains than control group students on oral/aural outcomes. These results indicate that the software intervention enables individualized practice that can produce proficiency-related gains over and above the typical classroom curriculum.

Abstract Food fortification is an important nutrition intervention to fight micronutrient deficiencies and to reduce their incidence in many low‐ and middle‐income countries. Food fortification approaches experienced a significant rise in the recent years and have generated a lot of criticism. The present review aimed to shed light on the actual effect of food fortification approaches on the reduction of malnutrition. A set of 100 articles and reports, which have dealt with the impact of food fortification on malnutrition, were included in this review. This review identified a broad selection of local raw materials suitable for a food‐to‐food fortification approach.

Abstract Background Ischemic stroke results in a robust inflammatory response within the central nervous system. As the immune-inhibitory CD200-CD200 receptor 1 (CD200R1) signaling axis is a known regulator of immune homeostasis, we hypothesized that it may play a role in post-stroke immune suppression after stroke. Methods In this study, we investigated the role of CD200R1-mediated signaling in stroke using CD200 receptor 1-deficient mice. Mice were subjected to a 60-min middle cerebral artery occlusion and evaluated at days 3 and 7, representing the respective peak and early resolution stages of neuroinflammation in this model of ischemic stroke. Infarct size and behavioral deficits were assessed at both time points. Central and peripheral cellular immune responses were measured using flow cytometry. Bacterial colonization was determined in lung tissue homogenates both after acute stroke and in an LPS model of systemic inflammation. Results In wild-type (WT) animals, CD200R1 was expressed on infiltrating monocytes and lymphocytes after stroke but was absent on microglia. Early after ischemia (72 h), CD200R1-knockout (KO) mice had significantly poorer survival rates and an enhanced susceptibility to spontaneous bacterial colonization of the respiratory tract compared to wild-type (WT) controls, despite no difference in infarct or neurological deficits. While the CNS inflammation was resolved by day 7 post-stroke in WT mice, brain-resident microglia and monocyte activation persisted in CD200R1-KO mice, accompanied by a delayed, augmented lymphocyte response. At this time point, CD200R1-KO mice displayed greater weight loss, more severe neurological deficits, and impaired motor function compared to WT. Systemically, CD200R1-KO mice exhibited signs of persistent infection including lymphopenia, T cell activation and memory conversion, and narrowing of the TCR repertoire. These findings were confirmed in a second model of acute neuroinflammation induced by systemic endotoxin challenge. Conclusion This study defines an essential role of CD200-CD200R1 signaling in stroke. Loss of CD200R1 led to high mortality, increased rates of post-stroke infection, and enhanced entry of peripheral leukocytes into the brain after ischemia, with no increase in infarct size. This suggests that the loss of CD200 receptor leads to enhanced peripheral inflammation that is triggered by brain injury.

Improving the technological functionality of climate-resilient crops (CRCs) to promote their use in staple foods, such as bread, is relevant to addressing food and nutrition security in Africa. Dry heating of cowpea flour (CPF) was studied as a simple technology to modulate CPF physicochemical properties in relation to bread applications. For this purpose, the melting behavior of cowpea starch and proteins in CPF was first studied and modeled using Flory–Huggins theory for polymer melting. Next, dry-heating conditions were investigated based on the predicted biopolymer melting transitions in CPF to be well below starch and protein melting. The pasting properties (i.e., peak viscosity, final viscosity, breakdown and setback) of CPF could be selectively modulated depending on temperature-time combinations without altering the thermal behavior (i.e., melting enthalpies) of CPF. Water-binding capacity and soluble solids decreased with the increased severity of the temperature-time combinations. Dry-heated CPF added to CRC-based bread significantly improved crumb texture. In particular, dry heating at 100 °C for 2 h provided bread with the highest crumb softness, cohesiveness and resilience. The positive effects on the crumb texture could be largely related to enhanced starch integrity, as indicated by a reduction in breakdown viscosity after treatment. Overall, dry heating of CPF under defined conditions is a promising technology for promoting the use of CPF as a techno-functional and protein-rich ingredient in bread-type products.

Cancer-related fatigue (CRF) is considered one of the most frequent and distressing symptoms for cancer survivors. Despite its high prevalence, factors that predispose, precipitate, and perpetuate CRF are poorly understood. Emerging research focuses on cancer and treatment-related nutritional complications, changes in body composition, and nutritional deficiencies that can compound CRF. Nutritional metabolomics, the novel study of diet-related metabolites in cells, tissues, and biofluids, offers a promising tool to further address these research gaps. In this position paper, we examine CRF risk factors, summarize metabolomics studies of CRF, outline dietary recommendations for the prevention and management of CRF in cancer survivorship, and identify knowledge gaps and challenges in applying nutritional metabolomics to understand dietary contributions to CRF over the cancer survivorship trajectory.

A társadalmak idősödésének jelensége mára már közismertté vált, a világ számos országában érzékelteti demográfiai, gazdasági, társadalmi hatását. A születéskor várható átlagos élettartam növekedése, az alacsony gyermekszám magától értetődő módon hívta életre a csökkenő, elöregedő társadalmak megjelenésének folyamatát. Mindez számos feladat elé állította a szakpolitikák, a hazai és nemzetközi szervezetek képviselőit, elsősorban az aktív, egészséges időskor elősegítése érdekében. Tanulmányunkban egy 167 fős empirikus vizsgálat eredményeinek egy részét mutatjuk be, melyet a Szent Lukács Görögkatolikus Szeretetszolgálattal közösen bonyolítottunk le 2019 telén azzal a céllal, hogy felmérjük az idősek helyzetét, szükségleteit. Jelen írásunkban azon eredményeit ismertetjük, melyek a közösségi tevékenységekre, a szabadidő eltöltésére, valamint a szociális kapcsolatok jellemzőire koncentrálnak. Elemző munkánk során azt tapasztaltuk, hogy az érintettek, a bentlakásos idősek otthonában élők, fokozottan nyitottak a fizikai és mentális aktivitás megőrzését szolgáló közösségi tevékenységekre, melyek hosszútávon esélyt biztosítanak jóllétük fenntartásához, javításához.

Objectives: Anxiety and anticipatory stressors are commonly experienced by children visiting the Pediatric Emergency Department (PED), but little research exists that addresses the efficacy of interventions to decrease this stress. This one-sample pretest-postest pilot study gathered preliminary data on the feasibility and effectiveness of utilizing audiobooks to reduce fear and state anxiety in children in the PED.Methods: Participants were 131 children in kindergarten through 8th grade (M = 9.4 years, 54% female), triaged urgent or emergent, presenting to the PED. Participants self-reported fear (Children's Fear Scale) and state anxiety (modified State-Trait Anxiety Inventory for Children; mSTAIC) before and after listening to an age-appropriate audiobook (two options). Data regarding patient experience were also collected. Paired samples t-test was used to examine pre–post intervention changes in fear and state anxiety.Results: Significant, albeit small, improvements in fear and the mSTAIC states of nervous, calm, happy, and relaxed were found after use of the audiobook (Cohen's dz = 0.22–0.35). Small, yet significant correlations were found between child age/grade level and improvements in fear and in the mSTAIC states of scared and relaxed, suggesting that the audiobook was more beneficial for older participants. Over 60% of participants liked the audiobook content “a lot” as well as enjoyed listening to the audiobook “a lot.” Without prompting, 15% of participants requested to listen to an additional audiobook.Conclusions: Listening to an audiobook is feasible and could be effective in decreasing fear and state anxiety for children during a waiting period in the PED. The technology is low-cost, simple, and portable. The results of this study should be interpreted with prudence due to the lack of a control group and results that, although significant, were modest based on effect size conventions; future studies should explore the impact of audiobooks on patient stress with an expanded sample size and control group.

Consumption of pepeta, a traditionally processed immature rice product, is common in Tanzania, where it contributes to food security as an early rice food i.e. when there is no other food available at the household while the crops in the field not yet fully ripe. Many production steps are needed to guarantee a consistent, good quality pepeta product, and this hinders its utilization in other rice-growing sub-Saharan regions. This study aims to gain insight into the pepeta processing knowledge and final product, and assess variations in the processing conditions and parameters across the study area. A survey among 257 Tanzanian processors and consumers revealed that the pepeta product is widely known, rated second (73.5% respondents) as rice-based food after wali (cooked white rice, (100%)) and linked to traditions of the communities in the study area. Harvest of immature rice grain, roasting, pounding, cleaning, and packing are the main process steps of pepeta production. Method of rice harvest, rice suitability for pepeta production after optimum harvest, dryness of grains and number of pounding as indicator to terminate roasting and pounding process respectively, and packaging materials used varied significantly across respondents in the study area. Reported criteria considered by respondents for product acceptability did not vary significantly across study area. The criteria include colour (76.5%), general appearance (60.8%), texture (64.7%) and taste (52.9%). Immature rice paddy and pepeta were sold at a higher price than mature rice paddy and white rice, respectively, which implies that options to facilitate pepeta processing through, for instance, standardization of processing conditions and parameters could lead to increased income.

Recent research has generated important new insights into the existence, behavior, and violent consequences of armed actors in civil wars. However, the lack of suitable information on actor relationships with the state means that studies have been forced to assume that armed groups are either pro-government or anti-government and remain that way for the duration of their existence. Both assumptions severely limit our understanding of the armed actors themselves, as well as the violent dynamics they produce. This article introduces the Government and Armed Actors Relations Dataset (GAARD), which provides detailed information on all major armed groups and their fluctuating alignment with the state between 1989 and 2007. GAARD identifies when armed groups fight with or against the government, and when they lack relationships with the government altogether. It further provides information on all changes in group alignments and documents when and how these occurred. We demonstrate that more than 25% of armed groups changed their alignments and showcase how this allows researchers to pursue original research on the drivers, dynamics, and outcomes of civil conflicts.

The cost of fish feed is a major constraint to fish farming in Sub-Sahara Africa. In the aquaculture value chain, feed is a determining factor and accounts for 60-75% of the total cost of fish production in many African countries. Therefore, 284 actors from all eight agro-ecological areas of Benin were interviewed and 28 local feed ingredients were collected as alternative ingredients for new fish feed formulations for, predominantly, Clarias gariepinus and Tilapia niloticus. Three categories of feeds were used, namely imported (84% of farmers), locally produced to complement imported feeds (76%) and natural ingredients (81%). The main imported feeds were from the Netherlands (59% of farmers), Ghana (52%) and France (15%). Natural ingredients were mostly Moringa leaves (52%), cassava leaves (26%) and maggots (43%). The best available ingredients were cereal bran, soybean meal, cottonseed meal, cassava chips, palm kernel cake, soybean and maize. Regarding proteins, the most promising ingredients were trash fish (680 g/kg), poultry viscera (590 g/kg), soybean meal (450 g/kg) and cottonseed meal (410 g/kg). Oyster shell had the highest ash content (960 g/kg), followed by whole garden snail meal (700 g/kg). The highest carbohydrate contents were for tapioca (890 g/kg), lafun (880 g/kg) and cassava chips (810 g/kg). Overall, this study revealed a diversity of local feed ingredients available in Benin to formulate fish feeds with adequate nutritional composition to enable efficient fish farming.

Many traditionally fermented milk products such as mabisi involve spontaneous fermentation, which can result in bacterial community composition variation due to selection pressure. The aim of this study was to determine the composition of bacterial communities in the different types of mabisi produced across Zambia and identify the factors that influence their composition. Samples of mabisi were collected across the country, and analyzed for pH and bacterial communities using 16S rRNA amplicon sequencing. We found that the bacterial community composition was dominated by members of two phyla, i.e., Firmicutes and Proteobacteria, from which the top 10 most abundant genera were Lactococcus, Lactobacillus, Streptococcus, Enterobacter, Citrobacter, Klebsiella, Kluyvera, Buttiauxella, Aeromonas, and Acinetobacter. The most dominant genus was Lactococcus, which was present in all types of mabisi produced from all regions. The mabisi products from traditional mabisi production regions (TMPRs) were dominated by lactic acid bacteria (LAB) whereas products from non-TMPRs were dominated by non-LAB species. Tonga mabisi, the most popular type of mabisi produced in non-TMPRs, had the most complex and diverse bacterial community composition compared to the other types, which included barotse, backslopping, creamy, and thick-tonga mabisi. Other factors that influenced bacterial community composition were geographical location, fermentation duration and pH while the type of fermentation container and producer did not. This study provides new insights that can be applied in starter culture development as well as microbial functionality studies.

The seaweed, Ascophyullum nodosum is the focus of the research these days and is looked upon as an important alternative bio-fertilizer. For this purpose the interaction of the seaweed with different plants including legumes is important to understand. We previously noticed lesser pigmented nodules in legume plant Vigna aconitifolia, when grown in presence of A. nodosum Extract (ANE). We therefore studied the effect of the ANE on the tyrosinase inhibition activity, an enzyme known to produce melanin pigment. In addition, the effect of the treatment of ANE on the photosynthetic pigments was also recorded. The seeds of Vigna aconitifolia were treated with 0%, 0.01%, 0.05%, 0.10%, 0.50% and 1.0% of A. nodosum extract for different time periods from 0–24 hours. It was observed that with the increased exposure to the seaweed (ANE), the tyrosinase inhibition activity also increased up to 0.05% of the seaweed concentration at 24 h of soaking and decreased after increasing the concentration of ANE from 0.1–1.0% at the same time period. Seed treatment with 0.05% seaweed was found to be the most effective concentration showing highest tyrosinase inhibition.

The study was undertaken to understand the propensity for increased engagement with open educational practices (OEP), to include methods prioritizing student-centered teaching & learning, and awareness, use, and development of open educational resources (OER) among higher education faculty in Kyrgyzstan. The study employed a mixed-methods research design, combining qualitative and quantitative data obtained from 35 faculty, librarians, and administrators in institutions of higher education in Kyrgyzstan. This study aimed to identify current teaching practices and learning resource usage practices, gauge levels of knowledge regarding Kyrgyzstan’s Copyright law, Creative Commons licenses, and Open Educational Resources, and investigate perceptions regarding potential roles for libraries in enabling others’ learning regarding Copyright and Creative Commons, and open educational resources. Analysis of the results revealed a higher than expected gravitation toward student-centered pedagogy than previously assumed. The study also identified broad use of digital downloads as learning materials, conflation of open educational resources with free online resources, and positive perceptions of libraries’ potential to instruct regarding Kyrgyz copyright, Creative Commons, and open educational resources, and needs for further professional development training for librarians.

The fertilizers that are derived from seaweed are known as Seaweed Liquid Fertilizers (SLF). SLF is a modern, cheap, non-toxic, and natural bioactive fertilizer. Among different studied seaweeds, Ascophyllum nodosum is significant as having bioactive ingredients that potentially regulate the molecular, physiological, and biochemical processes of crop plants. In the present study, the effects of the application of different concentrations (0.00%, 0.01%, 0.02%, 0.05%, 0.10%, 0.50%, and 1.00%) of A. nodosum Extract (ANE) to the Vigna aconitifolia through roots (Pot Root Application, PRA) and on the leaves (Pot Foliar Application, PFA) were monitored via the plant growth. The lower concentrations of ANE in both the PRA and PFA experiments showed positive growth on V. aconitifolia. The 0.10% ANE stimulated the maximum shoot growth when applied through the roots, while 0.05% ANE in both PFA and PRA experiments led to an increase in the number of pods, nodules, organic content, and moisture percentage. The 0.10% ANE also increased the leaf numbers, leaf area, and photosynthetic pigments. Hence, the application of 0.05% and 0.10% of A. nodosum extract in two ways (i.e., Pot Foliar Application, PFA, and Pot Root Application, PRA) ameliorated the growth capabilities of V. aconitifolia.

Abstract Background Initial serological testing for chronic hepatitis B virus (HBV) and hepatitis C virus (HCV) infection is conducted using either rapid diagnostic tests (RDT) or laboratory-based enzyme immunoassays (EIA)s for detection of hepatitis B surface antigen (HBsAg) or antibodies to HCV (anti-HCV), typically on serum or plasma specimens and, for certain RDTs, capillary whole blood. WHO recommends the use of standardized testing strategies – defined as a sequence of one or more assays to maximize testing accuracy while simplifying the testing process and ideally minimizing cost. Our objective was to examine the diagnostic outcomes of a one- versus two-assay serological testing strategy. These data were used to inform recommendations in the 2017 WHO Guidelines on hepatitis B and C testing. Methods Few published studies have compared diagnostic outcomes for one-assay versus two-assay serological testing strategies for HBsAg and anti-HCV. Therefore, the principles of Bayesian statistics were used to conduct a modelling exercise to examine the outcomes of a one-assay versus two-assay testing strategy when applied to a hypothetical population of 10,000 individuals. The resulting model examined the diagnostic outcomes (true and false positive diagnoses; true and false negative diagnoses; positive and negative predictive values as a function of prevalence; and total tests required) for both one-assay and two-assay testing strategies. The performance characteristics assumed for assays used within the testing strategies were informed by WHO prequalification assessment findings and systematic reviews for diagnostic accuracy studies. Each of the presumptive testing strategies (one-assay or two-assay) was modelled at varying prevalences of HBsAg (10%, 2% and 0.4%) and of anti-HCV (40%, 10%, 2% and 0.4%), aimed at representing the range of testing populations typically encountered in WHO Member States. When the two-assay testing strategy was considered, the model assumed the independence of the two assays. Results Modeling demonstrated that applying a single assay (HBsAg or anti-HCV), even with high specificity (99%), may result in considerable numbers of false positive diagnoses and low positive predictive values (PPV), particularly in lower prevalence settings. Even at very low prevalences shifting to a two-assay testing strategy would result in a PPV approaching 1.0. When test sensitivity is high (>99%) false negative reactions are rare at all but the highest prevalences; but a two-test strategy might yield more false negative diagnoses. The order in which the tests are used has no impact on the overall accuracy of a two-assay strategy though it may impact the total number of tests needed to complete the diagnostic strategy, incurring added cost and complexity. Bridging the gap between genetic variations, environmental determinants, and phenotypic outcomes is critical for supporting clinical diagnosis and understanding mechanisms of diseases. It requires integrating open data at a global scale. The Monarch Initiative advances these goals by developing open ontologies, semantic data models, and knowledge graphs for translational research. The Monarch App is an integrated platform combining data about genes, phenotypes, and diseases across species. Monarch's APIs enable access to carefully curated datasets and advanced analysis tools that support the understanding and diagnosis of disease for diverse applications such as variant prioritization, deep phenotyping, and patient profile-matching. We have migrated our system into a scalable, cloud-based infrastructure; simplified Monarch's data ingestion and knowledge graph integration systems; enhanced data mapping and integration standards; and developed a new user interface with novel search and graph navigation features. Furthermore, we advanced Monarch's analytic tools by developing a customized plugin for OpenAI's ChatGPT to increase the reliability of its responses about phenotypic data, allowing us to interrogate the knowledge in the Monarch graph using state-of-the-art Large Language Models. Rapid advances in nanotechnology have led to its emergence as a tool for the development of green synthesized noble metal nanoparticles, especially silver nanoparticles (AgNPs), for applications in diverse fields such as human health, the environment and industry. The importance of AgNPs is because of their unique physicochemical and antimicrobial properties, with a myriad of activities that are applicable in various fields, including the pharmaceutical industry. Countries with high biodiversity require the collection and transformation of information about biological assets into processes, associations, methods and tools that must be combined with the sustainable utilization of biological diversity. Therefore, this review paper discusses the applicable studies of the biosynthesis of AgNPs and their antimicrobial activities towards microorganisms in different areas viz. medicine and agriculture. The confirmed antiviral properties of AgNPs promote their applicability for SARS-CoV-2 treatment, based on assimilating the virus’ activities with those of similar viruses via in vivo studies. In this review, an insight into the cytotoxicity and safety issues of AgNPs, along with their future prospects, is also provided.
