Purpose This retrospective study aimed to study the applicability of 2015 adult American Thyroid Association (ATA) differentiated thyroid cancer (DTC) postoperative risk stratification and guidelines in the pediatric population for evaluating the number of metastatic lymph nodes in the postoperative risk stratification and postradioactive iodine (RAI) treatment dynamic risk stratification (DRS) using response to treatment (RTT) reclassification. In addition, the effect of pubertal status and gender was assessed on disease presentation and prognosis. Methods Data of 63 DTC patients aged 20 years or less, stratified into prepubertal, pubertal, and postpubertal age groups, was divided into low, intermediate, and high-risk groups using pediatric ATA recurrence risk stratification. Forty-seven patients were classified as responders (excellent and indeterminate responses) and incomplete responders (biochemical and structurally incomplete responses) by assessing the RTT at 1.5 years follow-up similar to recommendation of 2015 adult DTC ATA guidelines. Results Female-to-male ratio showed a trend of gradual increase with increasing age. Significantly more responders were observed in low- and intermediate-risk groups than in high-risk group (p = 0.0013; p = 0.017, respectively), while prepubertal group had more extensive (N1b) disease. Using DRS at follow-up of 1.5 year, pubertal and postpubertal groups showed significantly better response to RAI. More female than male patients showed response and took significantly less time to respond to RAI (p = 0.003). Conclusion RAI response in pediatric DTC depends on pubertal status, gender, and number of malignant nodes. DRS using RTT classification may be applicable early at 1.5 years after initial therapy in different pubertal age and risk groups.

We propose a discrete analogue of null geodesics in causal sets that are approximated by a region of 2d Minkowski spacetime, in the spirit of Kronheimer and Penrose's "grids" and "beams" for an abstract causal space. The causal set analogues are "ladder molecules", whose rungs are linked pairs of elements corresponding loosely to Barton et al's horizon bi-atoms. In 2d a ladder molecule traps a ribbon of null geodesics corresponding to a thickened or fuzzed out horizon. The existence of a ladder between linked pairs of elements in turn provides a generalisation of the horismotic relation to causal sets. Simulations of causal sets approximated by a region of 2d Minkowski spacetime show that ladder molecules are fairly dense in the causal set, and provide a light-cone like grid. The rapidly evolving clinical utility of prostate-specific membrane antigen (PSMA) PET/CT as an imaging modality for prostate cancer (PCa) has brought to the forefront a multitude of non-prostatic diseases also exhibiting PSMA radioligand uptake. We report a case of a 71-year-old male with PCa who underwent PSMA PET/CT for recurrence evaluation. The scan demonstrated [68 Ga]Ga-PSMA-11 uptake in the distal ileum. Retrospective enquiry revealed that the patient was a known case of Crohn’s disease. This case highlights an uncommon finding of PSMA radioligand uptake in the distal small bowel (non-prostatic benign pathological uptake) in a patient with known Crohn’s disease, which may be misinterpreted in the evaluation of PCa.

OBJECTIVE The aim of this study was to compare the performance of angiogenesis-specific 68Ga-RGD2 PET/CT with Disease Activity Score 28-erythrocyte sedimentation rate (DAS28-ESR) in assessing disease activity and treatment response in rheumatoid arthritis (RA). METHODS This was a prospective study comparing the performance of 68Ga-RGD2 PET/CT and DAS28-ESR in 30 RA patients. All of them underwent 68Ga-RGD2 PET/CT scan from head to toe and clinical examination at the baseline. A repeat scan and clinical examination were done in 27 patients after 3 months of treatment with disease-modifying antirheumatic drugs ± steroids. Three PET parameters, that is, PJC (PET-positive joint count), aSUVmax (average SUVmax), and hSUVmax (highest SUVmax), of positive joints were compared with the DAS28-ESR for disease activity assessment and response evaluation. RESULTS Among the 3 PET parameters, PJC showed a significant correlation with the DAS28-ESR (0.64, P < 0.01). A significant change was observed with treatment in the DAS28-ESR and PET parameters of 27 patients at follow-up. There was significant correlation between percentage changes in DAS28-ESR and scan parameters such as PJC (0.689, P < 0.001), aSUVmax (0.712, P < 0.001), and hSUVmax (0.555, P = 0.003) values. The absolute change in aSUVmax value could accurately discriminate (area under the curve, 0.98; P = 0.001) European League Against Rheumatism responders from nonresponders. CONCLUSIONS 68Ga-RGD2 PET/CT is a promising tool for objective assessment of disease activity and treatment response in patients with RA.

Doppler ultrasonography and testicular scintigraphy are sensitive techniques to detect the adequacy of blood flow to testicular tissue. Because Doppler ultrasonography is an operator-dependent imaging modality, it can result in equivocal findings. Testicular scintigraphy proves to be more accurate in such cases, but it may be inconclusive on occasion. The authors report the case of a young man with a left undescended testis where testicular scintigraphy was inconclusive for viability and 18F-FDG PET/CT provided important diagnostic information.

Due to its importance in human health and environmental monitoring, ethanol sensing has generated large research interest across the globe. In this work, we report a simple co-precipitation method to synthesize La-doped BaSnO3 and evaluate its ethanol sensing properties. Electron microscopy and X-ray diffraction analyses revealed formation of crystalline BaSnO3 having rod-like shape, few microns in length and 1–2 µm in width. The optimal sensing performance was achieved when operated at 220 °C for 4 wt% La-doped BaSnO3 microrods for which a response as high as 48 was obtained against 100 ppm of ethanol exposure, whereas the undoped BaSnO3 exhibited its best performance (although much lower response) at 260 °C. Multiple characterization techniques revealed that the enhancement of the sensor performance by incorporation of La was due to changes in the physico-chemical properties like specific surface area, oxygen content and electronic bandgap of the BaSnO3. The high repeatability, high selectivity to ethanol, fast response and recovery times, and low detection limit of 0.01 ppm suggest good potential for the 4% La-doped BaSnO3 film as a low cost ethanol sensor.

PURPOSE This study was conducted to identify the role of hepatobiliary scintigraphy (HBS) in the management of biliary tract injuries. METHODS We retrospectively studied 54 patients (28 male and 26 female patients) aged 3 to 78 years with evidence of bile leak on HBS performed between January 2015 and October 2017. Following intravenous injection of Tc-mebrofenin, dynamic images were acquired for 30 minutes followed by static images until 24 hours. SPECT/CT was performed in patients with suspicion of bile leak on the planar images. Patients were classified as those with free intraperitoneal or localized bile leak. Any abdominal drain output was documented, and its statistical significance was assessed using Mann-Whitney U test. Subsequent management was also documented. RESULTS Of the 54 patients, 28 demonstrated free intraperitoneal bile leak and 26 localized leak on HBS. Thirty-four patients (24 with free intraperitoneal leak and 10 with localized leak) had an abdominal drain. Drain output was significantly higher in patients with free intraperitoneal leak compared with patients with localized leak (370 vs 78 mL/d per patient, P < 0.01). All patients with free intraperitoneal bile leak underwent interventional procedures such as endoscopic retrograde cholangiopancreatography and stenting or Roux-en-Y hepaticojejunostomy later on. Patients with localized bile leak were managed conservatively and were free of symptoms at 1-year follow-up. CONCLUSIONS Our findings indicate that, in addition to detecting biliary leak, HBS may be used to identify patients with localized bile leak (48% of patients in our study) who do not require active intervention.

Coronavirus disease 2019 (COVID-19) is caused by the novel viral pathogen, severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). COVID-19 primarily involves the lungs. Nucleic acid testing based on reverse-transcription polymerase chain reaction of respiratory samples is the current gold standard for the diagnosis of SARS-CoV-2 infection. Imaging modalities have an established role in triaging, diagnosis, evaluation of disease severity, monitoring disease progression, extra-pulmonary involvement, and complications. As our understanding of the disease improves, there has been substantial evidence to highlight its potential for multi-systemic involvement and development of long-term sequelae. Molecular imaging techniques are highly sensitive, allowing non-invasive visualization of physiological or pathological processes at a cellular or molecular level with potential for detection of functional changes earlier than conventional radiological imaging. The purpose of this review article is to highlight the evolving role of molecular imaging in evaluation of COVID-19 sequelae. Though not ideal for diagnosis, the various modalities of molecular imaging play an important role in assessing pulmonary and extra-pulmonary sequelae of COVID-19. Perfusion imaging using single photon emission computed tomography fused with computed tomography (CT) can be utilized as a first-line imaging modality for COVID-19 related pulmonary embolism.

Aim: This prospective study was performed to compare the identification rate, sensitivity and specificity of blue dye and filtered Technetium 99m labelled sulphur colloid (Tc99m SC), individually and in combination for the detection of sentinel lymph nodes in clinically node negative early-stage breast cancer. Materials and methods: Thirty-two women with biopsy-proven breast cancer (cT1N0M0-T2N0M0) were included in this prospective study over a period of 18 months (2019-2020). Four peri-areolar intradermal injections of filtered Tc99m SC were administered 2-3 hours before surgery. Sequential dynamic anterior images of the chest were acquired on a gamma camera, followed by anterior and anterior oblique static images and hybrid single photon emission computed tomography / computed tomography (SPECT/CT) of the affected breast and ipsilateral axilla. Pre-operatively, the sites of detected sentinel lymph nodes (SLNs) were marked on the skin surface using a hand-held gamma probe. SLNs were then identified intra-operatively after peri-tumoral methylene blue injection (blue nodes) and with the gamma probe (hot nodes). These SLNs were removed and sent for frozen section histopathology separately, labelled as hot only, blue only and hot and blue. All patients underwent axillary lymph node dissection (ALND). The final histopathology of all lymph nodes excised during ALND and SLN biopsy were analysed. Identification rate, sensitivity and specificity of both the methods for SLN detection individually and in combination were compared. Results: SLN data of 32 female patients aged 54.7±10.6 years with cT1N0M0-T2N0M0 was analysed. In 87.5% of patients (n=28) the same lymph node was identified by blue dye and filtered Tc99m SC and discordance was seen in 12.5% (n=3) patients. No SLN was detected by any method in one patient. Identification rate of filtered Tc99m SC alone, blue dye alone and combination techniques was 96.9% (n=31), 93.8% (n=30) and 96.9% (n=31) respectively. Of total 593 dissected lymph nodes including 78 SLNs (average 2.4±1.7 per patient), metastasis was found in 198 lymph nodes (12/32 patients). The sensitivity and specificity of the procedure considering only filtered Tc99m SC, blue dye alone and combination of the two tracers was found to be 83.3% (95% CI of 51.6-97.9) and 100% (95% CI of 82.4-100), 75% (95% CI of 42.8- 94.5) and 100% (95% CI of 81.5 -100) and 83.3% (95% CI of 51.6-97.9) and 100% (95% CI of 82.4-100) respectively. Conclusion: SLNB with filtered Tc99m SC using the intra operative gamma probe has better identification rate and sensitivity than blue dye in detecting SLN, while preoperative SPECT/CT helped the surgeon to locate the SLN during surgery. Blue dye has a complementary role in identifying the SLN due to ease of visualisation while operating. Fanconi anemia (FA) is a genetic disease associated with the risk of different congenital malformations, bone marrow failure, and predisposition to cancer. Congenital abnormalities of the kidney and urinary tract are not infrequent in FA with renal ectopia being one of them. Hodgkin’s lymphoma most commonly presents as painless lymphadenopathy and systemic B symptoms like fever, night sweats, and weight loss. But rarely, it can present with unusual paraneoplastic manifestations. In the present case, we report a 30-year-old woman who presented with sub-acute onset pan-cerebellar symptoms. The serum neuronal antibody panel was negative. She underwent F-18 FDG PET/CT to evaluate the neoplastic etiology as the underlying cause, which demonstrated both the primary etiology and the paraneoplastic changes in the brain.

131I scan plays a crucial role in the management of patients with differentiated thyroid cancer for the evaluation of remnant thyroid tissue, residual/recurrent metastatic disease, posttherapy tracer distribution, and response assessment to high-dose 131I therapy. Different causes secondary to physiological, pathological, and anatomical variations have been described for false-positive findings in the whole-body planar images. This case report of a patient of differentiated thyroid cancer with undocumented trauma to the left knee region a day before receiving the high-dose radioiodine therapy showed an interesting image finding of tracer uptake at unusual site in the posttherapy whole-body 131I scan.

For robotic systems to succeed in high risk, real-world situations, they have to be quickly deployable and robust to environmental changes, under-performing hardware, and mission subtask failures. These robots are often designed to consider a single sequence of mission events, with complex algorithms lowering individual subtask failure rates under some critical constraints. Our approach utilizes common techniques in vision and control, and encodes robustness into mission structure through outcome monitoring and recovery strategies. In addition, our system infrastructure enables rapid deployment and requires no central communication. This report also includes lessons in rapid field robotic development and testing. We developed and evaluated our systems through real-robot experiments at an outdoor test site in Pittsburgh, Pennsylvania, USA, as well as in the 2020 Mohamed Bin Zayed International Robotics Challenge. All competition trials were completed in fully autonomous mode without RTK-GPS. Our system placed fourth in Challenge 2 and seventh in the Grand Challenge, with notable achievements such as popping five balloons (Challenge 1), successfully picking and placing a block (Challenge 2), and dispensing the most water onto an outdoor, real fire with an autonomous UAV (Challenge 3)., 29 pages, 26 figures. 99mTc-ethylenedicysteine (99mTc-EC) diuretic renal scintigraphy is an established, safe, and noninvasive study to evaluate relative renal function and drainage. We present a known case of bilateral renal stone disease who presented with acute urinary obstruction detected intraprocedurally on 99mTc-EC diuretic renography. He was managed subsequently with emergency double J stenting. The development of acute obstruction intraprocedurally in this patient may be resultant of forced diuresis caused by furosemide, dislodging the renal stones in the distal ureters. In the setting of renal calculi, cautious use of furosemide with watchful monitoring may be needed to see for the development of obstruction and manage accordingly.

Objective To evaluate 18F-fluorodeoxyglucose (FDG) PET/CT in the assessment of disease activity, extent of the disease and response to therapy in relapsing polychondritis. Methods Twenty-five patients (9 men, 16 women) with a mean age of 38.2 years (s.d. 13.7; range 18–62), diagnosed to have relapsing polychondritis according to Damiani and Levine’s modification of McAdam’s criteria, who underwent PET/CT examination were included. Ten patients underwent a second PET/CT examination after therapy or during follow-up. Clinical symptoms and auxiliary examination findings were recorded. PET/CT findings were reviewed and correlated with the clinical symptoms. Results The major symptoms were aural pain (n = 21), nasal pain (n = 10), stridor (n = 5), cough (n = 9), fever (n = 8) and laryngeal tenderness (n = 8). The initial PET/CT was positive in 23/25 patients. PET/CT revealed involvement of auricular (n = 14), nasal (n = 8), laryngeal (n = 7), tracheobronchial (n = 6) and Eustachian (n = 3) cartilages with a mean maximum standardized uptake value (SUVmax) of 4.1 (s.d. 2.5; range 1.7–12.7). Fair correlation of aural/nasal pain/stridor with FDG avidity of cartilage involvement on PET/CT was noted. The key finding was detection of asymptomatic large airway involvement in seven patients (28%). Re-examination PET in 10 patients revealed complete therapeutic response (n = 5), partial response (n = 1), stable disease (n = 1), progressive disease (n = 1) and disease recurrence (n = 2). Conclusion FDG PET/CT is a useful tool for the assessment of the disease activity and extent. It identified activity in clinically inaccessible sites that are of clinical significance. It is also useful in assessing treatment response and finding relapse.

La3+-doped BaSnO3 microtubes (La3+–BaSnO3) have been synthesized by electrospinning method, and the influence of La3+ content on the sensing properties of BaSnO3 for detection of formaldehyde vapor has been investigated. The as-prepared materials have been characterized using XRD, SEM, DSC, XPS, and UV-Vis. The La3+–BaSnO3 sample doped with 4 wt% La exhibited a response as high as 220 to formaldehyde vapor (1000 ppm concentration) along with a very low detection limit of 0.1 ppm at 270 °C, whereas at 140 °C, it exhibited a response of 80 and detection limit of 1 ppm. In addition, the sensor showed excellent selectivity of 57 to formaldehyde at 140 °C when compared with other vapors. Further, the sensor also showed good repeatability and stability over a long period of time suggesting its strong potential as a commercial formaldehyde sensor.

Malignant melanoma is a highly aggressive tumor and surgical resection is the primary treatment. However, the chances of recurrence are quite high despite complete resection. The aim of study was to evaluate the 18F-fluorodeoxyglucose(18F-FDG) positron emission tomography–computed tomography (PET/CT) in detection of recurrent melanoma after curative surgery and its prognostic value. Fifty-four melanoma patients (32 women) with prior primary lesion resection were evaluated with 18F-FDG PET/CT for clinically suspicious recurrent disease. The diagnostic accuracy of 18F-FDG PET/CT (visual interpretation as well as semi-quantitative parameter) was determined on the basis of subsequent imaging and clinical follow-up. Melanoma-specific survival and risk of progression (hazard ratio [HR]) were assessed using Kaplan–Meier method and Cox regression analysis.18F-FDG PET/CT detected recurrent diseases in 36 (66%) patients including distant metastases in 13 patients and second synchronous malignancy in 2 patients. Overall, the sensitivity, specificity, positive predictive value, and negative predictive value of 18F-FDG PET/CT were 91.2%, 80.0%, 88.6%, and 84.2%, respectively, with area under the curve of 0.86 (95% confidence interval: 0.74–0.97; P < 0.05). Positive 18F-FDG PET/CT study was associated with a significantly shorter overall survival than negative study (30.8 ± 4.6 vs. 64.5 ± 6.9 months, P < 0.05). Apart from positive 18F-FDG PET/CT scan, maximum standardized uptake value (SUVmax) >2.7 and combination of both were independently associated with an increased risk of disease progression (HR = 7.72, 21.58, and 11.37, respectively; P < 0.05).18F-FDG PET/CT showed enhanced diagnostic performance in patients with suspicious recurrent malignant melanoma leading to appropriate management. FDG positivity along with SUVmax >2.7 provides important prognostic value in predicting the survival outcomes and assessing the risk of disease progression.

BACKGROUND Currently, there is no consensus regarding the medical treatment of chronic Charcot neuroarthropathy (CN) of foot, except for effective off-loading. Because tarsal bones are predominantly trabecular, teriparatide may improve the macroarchitecture of foot bones in chronic CN. METHODS People with diabetes and chronic CN were randomized to receive either 20 μg teriparatide or placebo subcutaneous daily for 12 months. Thirty-eight patients were screened and data were analyzed for 20. The maximum standardized uptake (SUVmax ) value of 18 F-FDG PET/CT the region of interest, bone turnover markers and foot bone mineral density BMD were determined. The primary outcome measure was change in SUVmax g/ml. RESULTS Mid-foot was the most common region involved. After 12 months, SUVmax increased from 30.6 ± 14.7 to 37.7 ± 18.0 (P = 0.044) in the teriparatide group, but decreased from 27.6 ± 12.2 to 22.9 ± 10.4 with placebo (P = 0.148). The estimated treatment difference (ETD) was 11.9 ± 4.3 (95% CI 2.9, 20.8; P = 0.012). Similarly, P1NP increased with teriparatide (19.8 ± 5.5; P = 0.006) but decreased with placebo (-5.1 ± 3.8 ng/mL; P = 0.219); ETD was 24.8 ± 6.6 (95% CI 10.8, 38.8; P < 0.001) and CTX increased in both the teriparatide and placebo groups. Foot BMD increased by 0.06 ± 0.04 g/cm2 (P = 0.192) with teriparatide, but decreased by -0.06 ± 0.08 g/cm2 with placebo (P = 0.488; intergroup comparison, P = 0.096). CONCLUSION Teriparatide increases foot bone remodeling by an osteoanabolic action in people with CN.

ZnFe2O4 /graphene quantum dots (GQDs) nanocomposites were prepared via hydrothermal method . The as-prepared nanocomposites were characterized by XRD, SEM, TEM, HRTEM , TG, FTIR, Raman, XPS and N 2 adsorption-desorption, respectively. The effect of GQDs content on the gas-sensing responses and the gas-sensing selectivity of the ZnFe2O4/GQDs nanocomposites was investigated. It was found that the sensor based on ZnFe2O4/GQDs nanocomposite (S-15) exhibited good response and good selectivity to acetone vapor at room temperature, the responses of the sensor based on ZnFe2O4/GQDs nanocomposite (S-15) to 1000 ppm acetone and 5 ppm acetone reached 13.3 and 1.2, respectively; the response times and the recovery times for 1000-5 ppm acetone were all shorter than 12 s. The ZnFe2O4/GQDs nanocomposite (S-15) will be a potential acetone sensing materials if its long-term gas sensing stability is improved.

Skull base osteomyelitis is 1 of the complications of a middle ear infection seen mainly in immunocompromised individuals. A 3-phase bone scan and clinical, laboratory, and other radiological imaging are used for the diagnosis. However, in previously operated cases, bone scan findings are not reliable. We present a case of a 70-year-old man with persistent ear discharge postsurgery and with 3-phase bone scan negative for active infection. However, a subsequent 18F-FDG-labeled autologous leukocyte PET/CT study was able to diagnose the presence of active infection.

Background Gallbladder (GB) hypomotility has been reported in adults with celiac disease (CD), but there is no literature on GB dysfunction in children with CD. We aimed to study GB motility in children with CD, before and after a gluten-free diet (GFD), using ultrasonography (USG) and technetium-99 labeled mebrofenin hepatobiliary scintigraphy (HBS). Methods Children with newly diagnosed CD were enrolled and evaluated for GB ejection fraction (GBEF) using HBS and USG. Those with reduced GBEF on initial HBS were again evaluated after 6 months of strict GFD and the results were compared. Results Of the 50 children with CD (mean age 9±2.1 years, 54% boys), 16% (n=8) had a low GBEF at baseline (19±13%). Continuous ambulatory peritoneal dialysis is a convenient alternate form of treatment in patients receiving hemodialysis for renal failure. Sometimes during the procedure, patients may develop scrotal swelling because of patent processus vaginalis. The diagnostic utility of the peritoneal scintigraphy, an infrequently performed nuclear medicine scan to identify peritoneoscrotal communication, has been reported by many authors. Most of the previous case reports in the literature demonstrated unilateral peritoneoscrotal communication. We present an interesting Tc-sulfur colloid peritoneal scintigraphic image finding of bilateral peritoneoscrotal communication in a 67-year-old man who developed scrotal swelling while undergoing continuous ambulatory peritoneal dialysis.
