1. Perception and attitude toward online versus traditional anatomy teaching: An internet-based cross-sectional study among Indian medical students during COVID pandemic

Abstract

Background: The COVID-19 pandemic necessitated conduction of online medical education in India in both synchronized and asynchronized modes. Before the pandemic, the online teaching had very limited role in anatomy education in India. Aims and Objectives: This study was conducted to know the student’s perceptions and attitudes regarding online versus traditional anatomy teaching, the problems hindering online anatomy learning, and the areas of anatomy students enjoyed in virtual lessons. Materials and Methods: A cross-sectional study was done among the 1st year medical students of various institutions of India based on a questionnaire applied by Google Forms. Relevant information on student’s sociodemographic profile and their perception toward online versus traditional anatomy teaching were obtained. Four-point Likert scale was used for the responses on perception of teaching. Two hundred and fifty-two responses were studied and statistically analyzed. Results: Two hundred students (79.36%)used smartphones and 205 students (81.34%) used mobile internet in online learning. More than 90% of students preferred traditional anatomy teaching whereas less than 50% of students liked online anatomy teaching. In comparison with gross anatomy, students preferred online teaching of embryology, histology, and radiological anatomy. One hundred and six students (42.06%) opined that the internet problems were the major obstacle and 87 students (34.52%) considered the physical problems as main hindrances in online classes.Statistically significant increase in screentime after commencement of online class was seen among study participants. Conclusion: Majority of students preferred (statistically significant) traditional anatomy teaching over online anatomy teaching. Although students enjoyed online learning on selected topics of embryology, histology, and radiological anatomy, the main hindrances encountered in online anatomy classes were internet problems and physical problems.

Abstract

Background: Assessment has a very important role in curricular development by motivating students and target setting, providing appropriate feedback and quality assurance. During COVID-19 pandemic-induced lockdown, most of the medical schools throughout the world including India had to undergo a paradigm shift from face to face to online mode in medical education and assessment. This study was planned to assess and analyze the perception and opinion of 1st-year medical students from all over the country regarding the online assessment (OA) process that emerged during the COVID 19 pandemic. We also tried to find out the preferred resources that the students wanted to access via online mode and any major hindrance they faced. Methodology: It was a cross-sectional, Internet based, online survey conducted on 1st year medical students from all over India. Result: Students preferred the traditional (face to face) examinations over the OA in terms of usefulness, motivation, answering long answer questions, adequacy of allotted time, and ease of performance. However, they favored OA in terms of examination anxiety, examiner bias, and answering the objective type of questions. The lack of good Internet connectivity was considered a major hindrance. Conclusion: This information might be helpful for respective authorities to establish an effective online platform which could deliver an authentic and valid OA in future during any crisis.

Abstract

Introduction: Adnexal torsion contributes to 2.7-7.4% of all gynaecological emergencies and delayed or misdiagnosis results in loss of ovarian function. Diagnosis is not straight forward, both clinically and radiologically, and it can be confused with a number of other surgical and gynaecological emergencies. Aim: The aim of the present study was to correctly identify ovarian torsion among different cyst accidents, its overall incidence, the types of surgery involved and the histopathological diagnoses of the twisted adnexal masses in a prospective observation over two years. Materials and Methods: The clinical scoring was used in one group (n=14) and its reliability was assessed in correct identification of adnexal torsion by comparing with another group without the scoring (n=12). This scoring was developed in an Institute in Paris in (2000-04) using logistic regression model to select a combination of five best parameters namely: unilateral lumbar or abdominal pain, pain duration of less than 8 hours at first presentation, vomiting, absence of leucorrhoea and metrorrhagia, and cyst size more than 5 cm on ultrasound; to develop a combined score to diagnose torsion. A combined score of more than 60 was used to diagnose torsion in present study, and a score less than 40 to rule it out. For the intermediate range 41-60, clinical judgement was used in addition to scoring, to screen for torsion. Common differential diagnoses masquerading as torsion were also noted. Simple measures like specificity, sensitivity, PPV were calculated in both groups and compared between the two groups using Fischer’s-exact test for significance. Results: Cyst accidents and torsion accounted for 5% (n=19) and 3.7% (n=14) of all gynaecological emergencies (n=374). Lower abdominal pain and fever were the most common symptoms. Clinical diagnosis aided by the scoring correctly identified torsion in (n=8) 89% patients as compared to (n=6) 60% in group without scoring. The most common histopathological diagnosis was dermoid while the common procedures were detorsion and cystectomy and unilateral salpingo-oophorectomy. Several interesting scenarios like torsion and bilateral dermoid cysts, torsion of one ovary over the pedicle of contralateral ovarian cyst, a bulky ovary with an enlarged appendicular lump, endometriomas, tubo-ovarian masses mimicking torsion were also noted and diagnostic challenges were discussed. Conclusion: Clinical detection of adnexal torsion with perfect accuracy remains a challenge in emergency and a clinical scoring with careful imaging can help in refining diagnostic accuracy and plan appropriate surgery.

Abstract

Many modern applications of AI such as web search, mobile browsing, image processing, and natural language processing rely on finding similar items from a large database of complex objects. Due to the very large scale of data involved (e.g., users' queries from commercial search engines), computing such near or nearest neighbors is a non-trivial task, as the computational cost grows significantly with the number of items. To address this challenge, we adopt Locality Sensitive Hashing (a.k.a, LSH) methods and evaluate four variants in a distributed computing environment (specifically, Hadoop). We identify several optimizations which improve performance, suitable for deployment in very large scale settings. The experimental results demonstrate our variants of LSH achieve the robust performance with better recall compared with "vanilla" LSH, even when using the same amount of space.

Abstract

In this paper we discuss forestry issues related to land reform in Bolivia. We find that although the current land reform satisfies most of the conditions necessary for adequately addressing development issues in the agrarian sector, it does not deal with many challenges related to forest management, and in fact contains provisions conflicting with the objectives of sustainable forest management. Given that a large part of the land being titled is actually forest land, omissions of, and conflicts with, the objectives of sustainable forest management are critical, and may have harmful ramifications for the preservation of forest resources as well as poverty reduction within forest-dependent communities.

25. Why services cannot be the engine of growth for India

Abstract

India’s phenomenal service-led growth in recent decades has generated debate on the role of services vis-à-vis manufacturing as the engine of growth. With the rapidly increasing importance of Information and Communication Technology (ICT) in global production systems since the 1990s, there have been claims of services having developed a growth dynamism similar to manufacturing. This article examines the role of services in India’s growth process using the concept of inter-sectoral linkages to make comparison with the role of manufacturing. Input-Output linkages and time series analysis reveal that services have been much less integrated in India’s production structure than manufacturing. They were also less important in generating indirect employment spillovers through sectoral linkages, compared with manufacturing. Service sector growth is found to be autonomously driven by final demand and therefore less dependent on its interconnections with the rest of the economy from the production side. The findings also indicate that service sector growth has stimulated manufacturing growth but not vice versa. However, the impact of services on manufacturing from the demand-side is neither sustainable nor desirable going forward. India is in urgent need of strategically developing its manufacturing sector through integrating dynamic services like ICT and internalising productivity gains. At the same time measures to address India’s inequality are critical to broaden the country’s demand base and make the growth process more sustainable and inclusive. In this sense, inequality reduction is a prerequisite for growth and should not be seen as an alternative to it.

28. Effect of menstrual cycle on outer hair cells (OHC’s) and efferent auditory pathway in females – An Indian study

Abstract

Menstrual cycle occurs in females every months for ones where the endometrium which is the thick lining of the uterus wall where the fertilized egg is attached during pregnancy, this will shed every month. Sodium, chloride, and water reabsorption capacity increases by high-level progesterone during this period. When there is a change in water and sodium reabsorption which may in term affect the functioning of the auditory system especially peripheral auditory system. Otoacoustic emissions (OAE’s) arise from outer hair cells (OHC’s) which is situated on the basilar membrane that is seen inside the inner ear. The changes in the hormonal levels during the menstrual cycle has a clinically less significant effect on the suppression of TEOAE’s, one of the type of OAE. The current study targeted the testing during three different phases of the menstrual cycle (Day 1 to Day 28) - Follicular phase, Ovulation phase and Luteal phase. The amount of suppression (3dB) caused during different phases of the menstrual cycle for each individual frequencies was compared and analyzed. The study shows that TEOAE, CSTEOAE’s and suppression did not exhibit any statistically significant differences at all the frequencies between the different phases of the menstrual cycle. There are no significant correlation findings between menstrual cycle and outer hair cell function, and menstrual cycle and efferent auditory pathway functioning, considering the complaints of hearing and balance problem during the menstrual cycle period by.

Abstract

Vestibular disorders are the leading cause of dizziness in children. Vestibular disorders in youngsters with and due to Otitis Media Effusion have recently gained attention. The best way to identify vestibular disorders in youngsters is to use crude methods. The majority of the time, the parents’ history is used to form the view. As a result, questionnaires are the finest tool for estimating children. The Pediatric Vestibular Symptom Questionnaire was created and validated to measure children’s private vestibular symptom inflexibility (i.e. dizziness, shakiness), as a reliable and valid tool for assessing the presence and inflexibility of private vestibular symptoms in children with vestibular diseases or concussion provides a tool for assessing the presence and inflexibility of private vestibular symptoms in children with vestibular diseases or concussion. The questionnaire was used to distinguish between children with vestibular symptoms and healthy controls, and it should be used to detect and quantify vestibular symptoms that require further examination and treatment. The adaptation of the same in Malayalam Language among the natives of Kerala, the Southern Indian states were used widely to evaluate and identify the vestibular symptoms among the children for the effective treatment and better prognosis. Based on the statistics and analysis obtained the questionnaire demonstrated robust reliability, construct and discriminate validity.

34. Auditory training through cochlear habilitator (software) in children with cochlear implantation– A pre-post therapy comparison

Abstract

Cochlear Habilitator is an animated hypothetical cochlear pattern right and left with illumination from frequencies 125 Hz to 13000 Hz awareness and saving contours. This programmed has complete battery of training and testing on pure tones from 125 Hz to 10,000 Hz and environmental sounds numbering 79. The present study aimed to see the efficacy of cochlear habilitator in cochlear implant children. A total 10 cochlear implant children under which 4-female and, 6-male participants with in the age range of 3-7 years participated in this study with normal IQ and no history of any health problems. The cochlear habilitator (software) was installed in a laptop connected with two loudspeakers, which are configured with the cochlear habilitator (software) with azimuth angle of 180 degree. The 10 cochlear implant participants were selected based on the subject selection criteria, from the speech and hearing clinic and auditory training was provided for 28 days/sessions. The data was analyzed using SPSS software (version 17.0). T test values shows there is a significant difference in all tasks individually and on overall performance. The mean scores of Post therapy test is higher than the pre-therapy test. Hence cochlear habilitator computer based online software is a useful tool for cochlear implant children’s management in auditory training.
