[Background and purpose] Screening for and treatment of latent tuberculosis (TB) in patients with end-stage kidney disease (ESKD) are recommended. However, there is limited evidence on safety and treatment completion in this population. The objective of the study is to evaluate three short-course rifamycin-based regimens for the treatment of latent TB in ESKD patients., [Methods] Study design and setting. This is a prospective, open label, randomized clinical trial, that will be conducted at seven teaching hospitals in Spain. Study population, randomization, and interventions. Consecutive adult patients with ESKD requiring treatment for a latent TB infection will be randomly allocated (1:1:1) to receive one of the three treatment regimens of the study: three months of daily isoniazid plus rifampicin (3HR); three months of once-weekly isoniazid plus rifapentine (3HP); or four months of daily rifampicin (4R). Participants will be followed regularly through pre-established visits and a blood test schedule from enrolment to a month after finishing the assigned treatment. Outcomes. The primary outcome will be treatment completion, while the secondary outcomes will be discontinuation of the assigned treatment due to adverse events, related or unrelated to the study treatment; definitive discontinuation of the assigned treatment because of adverse events related to the treatment of the study, and death. Sample size. However, very few studies have assessed these associations. The aim of this work is to evaluate the association between thyroid dysfunction and body mass index (BMI) at baseline and after normalization of the hormone levels.Patients and methods: A retrospective, observational study of a cohort of otherwise healthy patients that were referred for evaluation of thyroid dysfunction to the Endocrine Department of Pontevedra University Complex Hospital, Spain was conducted. We collected data of BMI and thyroid hormone levels before treatment and after normalization of thyroid function within a follow-up period of 12 months.Results: A total of 330 patients were initially selected for the study. In order to exclude variables that for any reason could influence on BMI, 235 were excluded for further studies. Another 61 patients were also excluded because incomplete data on their medical records, failure to achieve euthyroidism, or lost to follow-up. Therefore, the eligible final study group consisted of 34 patients (17 with hypothyroidism and 17 with hyperthyroidism). No differences were observed in mean baseline BMI between hypo and hyperthyroid patients (27.07±3.22 vs 26.39±4.44, p=0.609). Overweight or obesity was observed in 76.5% and 58.8% of hypothyroid and hyperthyroid patients, respectively (p=0.23). After normalization of thyroid function, the weight of hypothyroid patients decreased from 70.93±10.06 kg to 68.68±10.14 (p=0.000), while the weight of hyperthyroid patients increased from 65.45±11.64 kg to 68.37±12.80 (p=0.000). Their mean BMI was 26.22±3.36 and 27.57±4.98 (p=0.361) for hypo- and hyperthyroid patients, respectively. 58.8% and 64.7% patients remained in the overweight/obesity range in each group (p=0.72).Conclusion: Untreated thyroid dysfunction is not associated with BMI. Normalization of thyroid levels significantly changed the weight of patients, but remaining most patients within overweight ranges.Keywords: hypothyroidism, hyperthyroidism, body mass index, obesity &nbsp

In Press., [EN]: [Objective] The objective of the study was to determine the trend of variables related to tuberculosis (TB) from the Integrated Tuberculosis Research Program (PII-TB) registry of the Spanish Society of Pulmonology and Thoracic Surgery (SEPAR), and to evaluate the PII-TB according to indicators related to its scientific objectives. [Method] Cross-sectional, population-based, multicenter study of new TB cases prospectively registered in the PII-TB between 2006 and 2016. The time trend of quantitative variables was calculated using a lineal regression model, and qualitative variables using the χy test for lineal trend. [Results] A total of 6,892 cases with an annual median of 531 were analyzed. Overall, a significant downward trend was observed in women, immigrants, prisoners, and patients initially treated with 3 drugs. Significant upward trends were observed in patients aged 40-50 and > 50 years, first visit conducted by a specialist, hospitalization, diagnostic delay, disseminated disease and single extrapulmonary location, culture(+), sensitivity testing performed, drug resistance, directly observed treatment, prolonged treatment, and death from another cause. The scientific objectives of the PII-TB that showed a significant upward trend were publications, which reached a maximum of 8 in 2016 with a total impact factor of 49,664, numbers of projects initiated annually, presentations at conferences, and theses. [Conclusions] PII-TB provides relevant information on TB and its associated factors in Spain. A large team of researchers has been created; some scientific aspects of the registry were positive, while others could have been improved., [ES]: [Objetivo] El objetivo del estudio fue conocer la tendencia de las variables relacionadas con la tuberculosis (TB) en España a partir del registro del Programa Integrado de Investigación en Tuberculosis (PII-TB) de la Sociedad Española de Neumología y Cirugía Torácica (SEPAR) y evaluar el PII-TB mediante indicadores relacionados con sus objetivos científicos. [Método] Estudio transversal multicéntrico de base poblacional de casos nuevos de TB registrados prospectivamente por el PII-TB entre 2006 y 2016. La tendencia temporal de variables cuantitativas se realizó mediante un modelo de regresión lineal y las cualitativas mediante la prueba de χ2 de tendencia lineal. [Resultados] Se analizaron 6.892 casos de TB con una mediana anual de 531. La tendencia general fue significativamente decreciente en mujeres, inmigrantes, privados de libertad y en tratados inicialmente con 3fármacos. Se incrementaron significativamente la tendencia de grupos de 40 -50 años y > 50 años, primera atención por especialista de zona, hospitalización, retraso diagnóstico, localización diseminada y extrapulmonar única, cultivo (+), realización de antibiogramas, resistencia a fármacos, tratamiento directamente observado, prolongación del tratamiento y muerte por otra causa. Los objetivos científicos del PII-TB que incrementaron significativamente fueron las publicaciones alcanzando un máximo de 8 en 2016 y con un factor de impacto total de 49,664, y también mejoraron los proyectos iniciados anualmente, presentaciones en congresos y las tesis o tesinas. [Conclusiones] El PII-TB proporciona información relevante sobre la TB y sus factores asociados en España. Se ha formado un amplio equipo de investigadores y se han detectado aspectos científicos positivos y otros mejorables.

La detección y el tratamiento precoz de infección tuberculosa en pacientes que han tenido contacto reciente con un enfermo de tuberculosis (TB), es una medida eficaz que previene la progresión a enfermedad tuberculosa en las personas infectadas. El diagnóstico de infección tuberculosa se ha basado históricamente en la Prueba de la Tuberculina (PT), aunque presenta inconvenientes de tipo operacional y ciertas limitaciones de fiabilidad diagnóstica. En los últimos años, se han introducido nuevas pruebas de determinación in vitro que intentan solventar, al menos parcialmente, las limitaciones de la PT. Son conocidas globalmente como IGRAs (de inglés "Interferon Gamma Release Assays"), por estar basadas bien en la determinación de interferon-gamma frente a antígenos de M. tuberculosis por parte de linfocitos sensibilizados (Quantiferon®-TB Gold In-Tube, QFT), o bien en el recuento de linfocitos productores de interferon-gamma (T-SPOT.TB®). Debido a la introducción en la práctica clínica aún relativamente reciente de estas pruebas, existen una serie de situaciones en los que la evidencia científica disponible es aún insuficiente y el empleo de los IGRAs está aún por definir de forma concreta. Una de esas situaciones es la que se presenta durante la realización del estudio en personas expuestas a un enfermo con capacidad de contagio. Se conocía la existencia de un "periodo ventana" en la PT, que es el tiempo que tarda en convertirse la prueba en positiva tras la infección (en torno a 8 semanas), pero nunca se había estudiado la existencia de este periodo de conversión con los IGRAs. Una segunda situación no suficientemente analizada es el comportamiento de los IGRAs en comparación con la PT, en pacientes con insuficiencia renal terminal. La tesis consta de 3 artículos que incluyen estudios prospectivos realizados en la Unidad de Tuberculosis del Hospital de Pontevedra en dos grupos de población: 1) Personas adultas sanas con contacto reciente con un enfermo de tuberculosis pulmonar y 2) Pacientes con insuficiencia renal terminal en programa de diálisis y contacto reciente con un caso de TB pulmonar bacilífera. Los objetivos de cada trabajo fueron: 1) Determinar la existencia de un "periodo ventana" de conversión en los IGRAs. (Anibarro L, et al. Eur Respir J 2011; 37:215) 2) Determinar la concordancia de la PT e IGRAs antes y después del "periodo ventana" y su correlación con el riesgo de infección. (Anibarro L, et al. Scand J Infect Dis 2011; 43:424) 3) Comparar la validez de PT con IGRAs en pacientes con insuficiencia renal terminal, para el diagnóstico de infección tuberculosa tras exposición a un caso de TB pulmonar. (Anibarro L, et al. BMC Infect Dis 2012; 12:195) Se estudiaron 184 contactos sanos a los que se realizaron PT y QFT en la primera visita a la Unidad, repitiéndose 8 semanas después si alguna prueba hubiera resultado inicialmente negativa. Se objetivó conversión a positiva tras el "periodo ventana" en el 29,6% de pacientes con PT inicialmente negativa y en el 21,2% con QFT inicialmente negativo. Entre los pacientes con conversión de la PT, el 37,5% (IC 95%:1,60-59,0%) ya tenían QFT positivo en la primera visita, lo que es sugestivo de una conversión más precoz de la QFT. Sin embargo, el 17,6% (IC 95:3,8-43%) de pacientes con conversión de QFT ya tenían PT inicialmente positiva. La concordancia entre ambas pruebas fue moderada (k=0,60) al inicio de los estudios, mejorando hasta k=0,73 (buena concordancia) al considerar el tiempo transcurrido tras el "periodo ventana", siendo incluso excelente (k=0,81) en pacientes no vacunados con BCG. El número de horas compartido con el enfermo y el menor espacio del lugar de contacto fueron las variables que se relacionaron con la positividad tanto de la PT como del IGRA (p=0,006 y 0,007 respectivamente para el número de horas; y p=0,022 y 0,02 respectivamente en lo referente al lugar de contacto). En el segundo grupo de población, se estudiaron 58 pacientes con Insuficiencia renal en programa de diálisis, 12 semanas después de haber estado en contacto con un caso de TB pulmonar bacilífera. La PT fue positiva en el 5,8% de pacientes y en otros 16,3% en una segunda PT realizada 2 semanas después (buscando un efecto "booster"). QFT fue positivo en el 34,6% de los pacientes. La concordancia entre ambas pruebas fue débil (k=0,21) si consideramos únicamente la primera PT y moderada (k=0,49) considerando también la segunda PT. Nueve pacientes presentaron eritema sin induración en la respuesta tuberculínica. Si se hubiera considerado el eritema como PT positiva, la concordancia PT-QFT habría sido buena (k=0,67). De los resultados de estos estudios, se concluye: 1) La existencia de un "periodo ventana" de conversión de IGRAs tras infección reciente por M. tuberculosis. 2) En la mayor parte de casos el "periodo ventana" de QFT es más corto o similar que el de la PT. 3) La validez de PT y QFT es similar para el diagnóstico de infección TB, especialmente en personas sanas no vacunadas con BCG. 4) Es preciso considerar un "periodo ventana" de conversión de al menos 8 semanas para descartar infección tuberculosa tanto con la PT como con los IGRAs. 5) QFT debe ser el método de referencia para el diagnóstico de infección tuberculosa en pacientes con insuficiencia renal terminal. 6) La presencia de eritema incluso sin induración en la respuesta a la PT, podría ser sugestiva de infección tuberculosa.
