ABSTRACT Introduction: Patients with Class II, division 1 malocclusion generally seek treatment to improve facial esthetics. Therefore, the orthodontist needs to know the changes in the soft profile produced by functional appliances. Objective: This study evaluated the soft tissue profile changes in patients treated during the peak of the pubertal growth spurt. Methods: Thirty selected patients were randomized into three treatment groups: Twin Block (TB), Herbst with dental anchorage (HDA), and Herbst with skeletal anchorage (HSA). All patients had computed tomographic images: pretreatment (T1) and after 12 months of active treatment (T2). Twenty-four soft tissue cephalometric measures were analyzed. The normality of all data was assessed by the Shapiro-Wilk test. Intragroup comparisons were analyzed using the t-paired test; the inter-group comparisons were determined through ANOVA and the post-hoc Tukey test. Results: At T1, no significant differences were observed between groups. At T2, in the intragroup comparison, facial soft tissue changes were statistically significant in the three groups for the lower lip, sulcus inferioris, facial soft tissue convexity in HDA group and TB group, and H angle in HDA group and HSA group, and soft tissue pogonium in TB group. In the inter-group comparison, no statistically significant differences were observed. Conclusion: It can be concluded that there were significant changes in soft tissue measurements that benefited Class II, division 1 patient’s facial profile treated with the functional appliances Twin Block, Herbst, and Herbst with skeletal anchorage. The study aimed at evaluating the long-term soft tissue profile changes in Class II division 1 growing patients treated with cervical headgear appliance without tooth extraction. Lateral radiographs of 24 patients were analyzed at three different time points: pre-treatment (T1); post-treatment (T2); long-term post-treatment (T3) (mean follow-up period of 27 years). The following angles were evaluated: Nasolabial, Holdaway, Mentolabial, GPrPog and Soft Tissue Profile. The linear distances evaluated were: Upper Lip Thickness, Lower Lip Thickness, Upper Lip to E-plane, Lower Lip to E-plane, Nasal Prominence, Upper Lip Length, Lower Lip Length and Chin Thickness. In addition, cephalometric superimpositions were made to measure the distance between soft tissue landmarks over the study periods. A decrease in face convexity due to lip repositioning and mandibular development was found as a treatment result. Long-term follow-up showed substantial growth of the nose, increased labial vertical flaccidity and anterior residual growth of the chin. Early interception of Class II malocclusion allowed reduction of facial profile convexity, but the labial hypotonia and continued growth of the chin and nose promoted a concave profile in the long term. 9. Correlation between condylar repositioning, resorption, and mandibular relapse post-orthognathic surgery with the straight locking miniplate technique in patients with Class II and III malocclusion: a retrospective study.

This study investigated the relationship between condylar position, condylar volume, and mandibular relapse following orthognathic surgery in which the proximal segments were temporarily fixed with a positioning device to maintain the condylar position. Computed tomography data (pre-surgery, immediately post-surgery, and 1 year post-surgery) from 12 patients with Class II malocclusion and 20 with Class III were analysed. Condylar volume was significantly lower in Class II patients than in Class III patients at all time-points (P < 0.001), and only Class II patients showed a significant volume reduction from immediately post-surgery to 1 year post-surgery (P < 0.001). Three-dimensional condylar movements from pre-surgery to immediately post-surgery and from pre-surgery to 1 year post-surgery were significantly greater in Class II patients than in Class III patients (both P < 0.001). Additionally, Class II patients exhibited more restoration movements to the preoperative position from immediately post-surgery to 1 year post-surgery, correlating with the volume reduction (r = -0.53, P = 0.007), which in turn correlated significantly with mandibular relapse (r = 0.63, P < 0.001). These findings suggest an association between preoperative volume, intraoperative movement, condyle movement restoration, volume reduction, and postoperative mandibular relapse, particularly in the Class II group. 14. Changes in mandibular angle and intergonial width after bilateral sagittal split ramus osteotomy or bimaxillary surgery with/without counterclockwise rotation.

The aim of this retrospective study was to evaluate the morphological changes in the mandibular angle area after orthognathic surgery with or without mandibular counterclockwise rotation in Class II deformity patients, and to investigate the associated factors. Computed tomography scans obtained preoperatively (T0), within 1 month postoperatively (T1), and 6 months postoperatively (T2) were collected from 58 patients who underwent either bilateral sagittal split ramus osteotomy (group I), bimaxillary surgery with mandibular counterclockwise rotation (group II), or bimaxillary surgery without mandibular counterclockwise rotation (group III). The intergonial width increased after surgery, by 2.78 ± 2.02 mm in group I, 2.86 ± 2.81 mm in group II, and 2.53 ± 2.42 mm in group III (all P < 0.001). The mandibular angle (MA) increased in group I (ΔMA 4.76 ± 2.79°; P < 0.001) and group III (ΔMA 3.50 ± 2.58°; P < 0.001); however no significant increase was observed in group II. The increase in intergonial width was positively correlated with the lateral displacement of the proximal segment. Counterclockwise rotation of the proximal segment resulted in an increase in MA, while counterclockwise rotation resulted in a reduction in this increasing trend. ABSTRACT Objective: This article describes the orthodontic treatment performed on an adult patient with multiple dental losses. Case report: A female patient, 20 years and 4 months old, presented with the following conditions: absence of teeth #26, #35, #36 and #46; semi-impacted tooth #48; inclined molars adjacent to an edentulous space; canines and premolars in a Class II relationship; a convex profile; biprotrusion; and forced lip sealing. Results: Space in the region of tooth #26 was closed, as well the space of tooth #46; tooth #48 erupted and followed mesial movement passively; space of the region of tooth #35 was maintained for the placement of a dental implant; uprighting of tooth #37 was obtained. Aesthetic and functional goals of the treatment were achieved. Results remained stable 10 years after the end of the treatment. Conclusion: The modified helical loop could be effectively used in orthodontic mechanics to close edentulous spaces. Passive semi-impacted mandibular third molar eruption and mesialization can occur in adults when proper space is provided. more...

20. The case presentation of a phenytoin related gingival fibromatosis and the review of the related literature

The objective of this case presentation is to present a case of a phenytoin associated gingival fibromatosis in a 14 years old girl and the results of its comprehensive periodontal-orthodontic therapy. An attempt was also made to summarize the literature background of the clinical features and mechanism of drug induced gingival overgrowth. The gingival overgrowth is a common side effect, following the long term administration of anticonvulsant drugs, calcium channel blockers, and immunosuppressant drugs (phenytoin, nifedipine, and cyclosporine) to control epilepsy, hypertension, and organ transplant rejection respectively. The pathogenic mechanism has been intensively studied by numerous authors but it is still not totally understood. It is common in the pharmacological action of the three chemically totally different molecules that they suppress the cation channels (Na+, Ca++) in in the fibroblasts that control the active folic acid uptakes. The folic acid has a key role in controlling the MMP enzymes expression by the gingival fibroblast. Nevertheless alteration in the expression of different pro and anti-inflammatory cytokines have also been documented. Those molecules also have a direct stimulatory effect on the gingival fibroblast to synthesize excessive amount of collagen. In the presented case our patient had severe postnatal complications and at the age of three developed epilepsy (grand mal) that was difficult to control and finally at the age of 8 neurologist decided to administer phenytoin. The patient showed up at the Periodontal Department of Semmelweis University at the age of 14 complaining of severe gingival enlargement, speech difficulties and complicated tooth eruption. After initial motivation a series of external and internal bevel gingivectomy as well as apically repositioned flap surgeries were performed to remove the fibrotic gum, to assist permanent tooth eruption and re-establish the normal morphology of the gingiva. Originally patient had an Angle Class II malocclusion but due to the fibrotic gingiva the normal tooth eruption was also severely hampered. The orthodontic therapy was provided by the Department of Pediatric Dentistry and Orthodontics of Semmelweis University. The Angle class two malocclusion and the buccal position of the upper front teeth were corrected with bimaxillary fixed orthodontic appliances and after another three years the than 19 years old patient’s periodontal and dental condition as well as her self-esteem has been finally restored. more...

22. The Use of Mini-Plates for the Treatment of a High-Angle, Dual Bite, Class II Malocclusion

To present a case report of an orthodontic treatment of a high-angle, dual bite, Class II malocclusion without extractions and with the use of mini-plates. Class II malocclusion treatment protocols vary according to the morphological component of the malocclusion and the magnitude and direction of craniofacial growth. It is generally agreed that the cooperation of the patient and careful planning of anchorage are the key determinants of successful treatment. Protrusion of the upper and lower lip and a retrognathic mandible were the patient’s chief concerns. The patient had learned to project her mandible forward to disguise the overjet. The patient’s parents elected to correct the malocclusion with the use of bilateral infrazygomatic mini-plates. Pre-treatment condylar stabilization with an orthotic established a stable centric relation position, followed by mounting of the models on a semi-adjustable Panadent articulator. This allowed diagnosis and treatment planning from a stable condylar position and eliminated possible misdiagnosis due to the dual bite. Distal retraction and vertical control of the upper teeth enabled correction of the Class II malocclusion with minimal patient cooperation. Mini-plate-assisted treatment corrected the excessive overbite and overjet. The patient completed treatment with a stable occlusion and no longer postured her jaw forward. - Abstract
Objective To compare the vertical changes of dentition in patients with class II malocclusion treated by clear aligner and pendulum appliance. Methods Forty patients with class II malocclusion were enrolled. According to the different appliances used in the treatment, 40 cases were divided into two groups : invisible group and pendulum group, with 20 in each. The vertical displacement distance of the first molars ( U6, L6) in maxilla and mandible and the skeletal changes of the mandible were measured by Dolphin Imaging software before and after treatment. Results The difference of displacement distance between maxillary first molar crown and root increased significantly in the invisible group ( P< 0.05). Compared with the pendulum group, the differences of vertical distance of mandible first molars (LDPH), lower facial height (LFH), and the rate of lower faeial height and anterior facial height ( ANS-Me:N-Me) decreased in the invisible group (all P<0.05) . Conclusions Compared with pendulum appliance, clear aligner can better control the vertical change of the dentition of patients with class II malocelusion. When we put the molars toward the middle distance, it can depress the upper and lower molars. Abstract Objective This study was designed to evaluate the skeletal, dental and soft tissue effects of mandibular deficiency treatment with the mandibular protraction appliance (MPA) using 12 factors of the Ricketts analysis. Material and method This cross-sectional retrospective study sample consisted of a group (n = 27), with Class II malocclusion, convex facial profile, increased horizontal trespass and mandibular deficiency, with initial mean age of 12.27 and final of 15.18 years, treated with fixed appliance combined with the MPA, in an average time of 2.9 years. Initial and final radiographs were investigated using Ricketts analysis. The dependent t-test was used to compare the initial and final phases of the MPA group, with a significance level of 5%. Result Statistically significant differences were observed for dental changes such as retrusion (p=0.000) and palatal inclination of the maxillary incisors (p=0.000); protrusion (p=0.000) and buccal inclination of the mandibular incisors (p=0.000); increased interincisal angle (p=0.002) and improved molar ratio (p=0.003). There was also a restriction of the anterior displacement of the maxilla (p=0.000) and a decrease in the mandibular plane angle (p=0.024). The variable inferior labial protrusion with significance (p=0.000), reiterated the improvement in the profile. Conclusion The effects of MPA on correction of malocclusion Class II, verified by Ricketts analysis occurred predominantly by dentoalveolar changes, decrease in the Mandibular Plane Angle, and restriction of anterior displacement of the maxilla, which contributed to the improvement in the patient's profile. more...

本報告は、初診時年齢15歳2か月の女子で、過大なオーバージェットと著しい上顎歯列の叢生を伴うAngleⅡ級1類の治験例である。晩期残存していた上顎左側乳犬歯および両側第一小臼歯を抜去し、上下顎歯列にプリアジャストエッジワイズ装置を装着して、矯正歯科治療を行った。固定のため、上顎両側第二小臼歯・第一大臼歯間に歯科矯正用アンカースクリューを埋入した。その結果、1歯対2歯のⅡ級大臼歯関係を確立し、良好なオーバージェット、オーバーバイトおよび緊密な咬頭嵌合が得られた。保定開始から2年2か月を経過した後も、安定した咬合関係を保っており、患者の充分な満足が得られた。, This case report describes orthodontic treatment in a 15-year-2-month-old girl with an excessive overjet and severe crowing in the maxillary dental arch. Orthodontic correction with extraction of bilateral maxillary first premolars and left primary canine was performed using a preadjusted edgewise appliance and temporary anchorage devices. Objectives: To compare three-dimensional (3D) skeletal and dentoalveolar effects of the Herbst and Pendulum appliances followed by fixed orthodontic treatment in growing patients.Setting and Sample Population: A sample of 35 adolescents with cone-beam computed tomography scans obtained prior to Herbst and Pendulum treatment (T1) and immediately after fixed appliance treatment (T2).Materials and Methods: Patients with Class II malocclusion was assessed retrospectively and divided into two treatment groups: Herbst group (n = 17, age: 12.0 ± 1.6 years) and Pendulum group (n = 18, age: 12.1 ± 1.5 years), with a mean treatment duration of 2.8 ± 0.8 years and 2.5 ± 0.7 years, respectively. Reconstructions of the maxillomandibular and dentoalveolar regions and data in 3D were obtained relative to cranial base, maxillary and mandibular regional superimpositions. Treatment outcomes (T2-T1) were compared between both groups using t tests for independent samples (P<.05).Results: Significant increase in mandibular length was observed in the Herbst group (7.3 ± 3.5 mm) relative to the Pendulum group (4.6 ± 4.5 mm). Inferior and anterior displacements of Pogonion were 2.2 mm and 1.6 mm greater in the Herbst group, respectively. The mesial displacement of the lower first molars was significantly greater in the Herbst group (1.9 mm). The upper first molars had contrasting results in sagittal displacement, with 0.6 ± 1.7 mm of distal displacement with the Pendulum and 1.4 ± 2.1 mm of mesial displacement with the Herbst. Lower incisor projection and proclination were similar between groups.Conclusions: The Herbst and Pendulum appliances followed by comprehensive orthodontic treatment effectively corrected Class II malocclusion in growing patients, but with differing skeletal and dentoalveolar effects. Objective: This article describes the orthodontic treatment performed on an adult patient with multiple dental losses. Case report: A female patient, 20 years and 4 months old, presented with the following conditions: absence of teeth #26, #35, #36 and #46; semi-impacted tooth #48; inclined molars adjacent to an edentulous space; canines and premolars in a Class II relationship; a convex profile; biprotrusion; and forced lip sealing. Results: Space in the region of tooth #26 was closed, as well the space of tooth #46; tooth #48 erupted and followed mesial movement passively; space of the region of tooth #35 was maintained for the placement of a dental implant; uprighting of tooth #37 was obtained. Aesthetic and functional goals of the treatment were achieved. Results remained stable 10 years after the end of the treatment. Conclusion: The modified helical loop could be effectively used in orthodontic mechanics to close edentulous spaces. Background: Functional appliances have skeletal and dentoalveolar treatment effects, but knowledge on their effect on facial expressions is limited. Objective: To analyse changes in facial expressions, using three‐dimensional (3D) mimic muscle evaluation, in growing children with Class II malocclusion and large overjet, undergoing functional appliance treatment. Methods: Fifteen growing children with Class II division 1 malocclusion, selected prospectively, had dynamic 3D recordings of facial expressions prior to and 12 months after commencing functional appliance treatment. Facial expressions recorded were smile and lip pucker, and the movements of the oral commissures were analysed, as well as mouth width and its symmetry. A control group of fifteen age‐ and sex‐matched growing children without immediate need for orthodontic treatment had similar recordings prior to and after a 12‐month observation period. Results: In the treatment group, the Class II malocclusion in all children improved after 12 months of functional appliance wear. With regard to movements of the oral commissures, the initial recordings of the treatment group did not show any significant differences to the control group for neither smile nor lip pucker. Differences were noted however when looking at mouth width asymmetry, where this tended to become more symmetrical in the functional appliance treatment group, during rest and during smiling, while it became more asymmetrical in the untreated control group. Conclusion: The use of removable functional appliances in children with Class II division 1 malocclusion may have a positive effect on mouth width symmetry when smiling, making it more symmetrical. Objective: To evaluate whether osseous changes of the temporomandibular joint (TMJ) condyle affect backward rotation of the mandibular ramus in Angle Class II orthodontic patients with idiopathic condylar resorption (ICR). Methods: Twenty Japanese women with Class II malocclusion with ICR (ICR group) and 24 women with Class II malocclusion without ICR (non-ICR group) were examined. Pre-treatment panoramic radiographs were used to measure condylar ratios. Pre-treatment lateral cephalograms were used to evaluate maxillofacial morphology. Results: The ICR group had a significantly smaller condylar ratio, greater backward rotation of the ramus, less labially inclined upper incisors, and a steeper occlusal plane. The increased backward rotation of the ramus in the ICR group was significantly associated with a smaller condylar ratio. Conclusion: Angle Class II patients with ICR had shorter condylar height attributable to osseous changes of the TMJ condyle, and the shorter condylar height may affect subsequent backward rotation of the ramus. A Cirurgia Ortognática (CO) é o procedimento que corrige desproporções faciais da mandíbula, maxila e/ou mento, tendo impacto na dimensão das vias aéreas e nos aspectos funcionais e interferindo, por consequência, numa harmonia facial satisfatória. O objetivo deste estudo é descrever o ato cirúrgico de uma CO em paciente padrão facial II e analisar o planejamento virtual como auxiliar, assim como discutir sobre o diagnóstico e tratamento das deformidades dentofaciais, para um trabalho de conclusão de curso. Paciente do sexo feminino, M.P.S.C., 30 anos, compareceu ao Serviço de Cirurgia Bucomaxilofacial do Hospital das Clínicas – UFPE, queixando-se de dificuldades de mastigação, sono e fala, além da insatisfação com sua harmonia facial. Diante da análise facial, fotografias, radiografia e Tomografia Computadorizada, realizou-se o diagnóstico de padrão facial II de Angle da paciente que já se encontrava sob tratamento ortodôntico pré-operatório. A partir do software Dolphin Imaging®, simularam-se os movimentos cirúrgicos e planejou-se virtualmente a CO. Houve uma melhora significativa da harmonia facial, de mastigação, fala e qualidade do sono após o procedimento cirúrgico, realizado sob anestesia geral. Foram realizados exames radiográficos após a cirurgia e não foram observados indícios de quaisquer complicações. Dessa forma, a CO em combinação com o tratamento ortodôntico são a terapêutica de escolha para casos de deformidade dentofacial, melhorando as disfunções estéticas, psicossociais e funcionais causadas por ela. 40. The Use of Mini-Plates for the Treatment of a High-Angle, Dual Bite, Class II Malocclusion.

To present a case report of an orthodontic treatment of a high-angle, dual bite, Class II malocclusion without extractions and with the use of mini-plates. Class II malocclusion treatment protocols vary according to the morphological component of the malocclusion and the magnitude and direction of craniofacial growth. It is generally agreed that the cooperation of the patient and careful planning of anchorage are the key determinants of successful treatment. Protrusion of the upper and lower lip and a retrognathic mandible were the patient's chief concerns. The patient had learned to project her mandible forward to disguise the overjet. The patient's parents elected to correct the malocclusion with the use of bilateral infrazygomatic mini-plates. Pre-treatment condylar stabilization with an orthotic established a stable centric relation position, followed by mounting of the models on a semi-adjustable Panadent articulator. This allowed diagnosis and treatment planning from a stable condylar position and eliminated possible misdiagnosis due to the dual bite. Distal retraction and vertical control of the upper teeth enabled correction of the Class II malocclusion with minimal patient cooperation. Mini-plate-assisted treatment corrected the excessive overbite and overjet. The patient completed treatment with a stable occlusion and no longer postured her jaw forward. Background: The preference for the functional appliances to treat Class II and Class III malocclusions by orthodontists in Colombia and their reasons to select them is not well established, yet. Purpose: To compare the preferences in the use of functional appliances to treat Class II and Class III malocclusions between an orthodontic graduate program and the orthodontist members of the Colombian Society of Orthodontists (CSO) and to evaluate the association between the indicated functional appliance and the diagnostic and demographic characteristics of the patients in the orthodontic program. Methods: In this cross-sectional study were reviewed 565 clinical records of the orthodontic graduate program and the survey of 180 members of the CSO. Chi square and t test at a level of significance P < 0.05 were used to compare both groups. Results: the most frequently functional appliance used to treat Class II malocclusion in the orthodontic graduate program was Simoes Network (55, 42%) and to treat Class III was the Lazaro (28, 95%). CSO members preferred Planas indirect tracks (32,78%) to treat Class II and Facial Mask (62,22%) to treat Class III. Statistically significant differences (P < 0, 05) in the use preference of functional appliances between the orthodontic graduate program and the orthodontist from the CSO were found. Conclusions: The results suggest that the selection of functional appliances to treat Class II and Class III malocclusions is not only guided by the malocclusion but by the orthodontist's academic background and individual preferences as well. Objectives: To evaluate the influence of text messages on cooperation of Class II patients with the use of intermaxillary elastics. Materials and Methods: The sample consisted of 42 orthodontic patients (20 males and 22 females) aged between 14 and 34 years. They were randomly divided into one of the following two groups: a control group with 21 patients who did not receive messages and an experimental group with 21 patients who received motivational and reminder text messages. Messages were sent twice a week for a period of 3 months. The patients were instructed to wear the elastics all day, removing them only during meals and replacing them daily. All patients were instructed regarding the importance of cooperation. Measurements were performed with a digital caliper on plaster models at the beginning of elastics wear (T1) and 3 months later (T2). For intragroup and intergroup comparisons between T1 and T2, paired and unpaired t-tests, respectively, were used with a significance level of 5%. Results: Statistically significant differences were observed in the intra- and intergroup comparisons between T1 and T2. Both groups showed a decrease in the sagittal distance between upper and lower arches from T1 to T2, demonstrating the effective use of elastics. However, the experimental group showed a Class II correction that was 3.7 times greater than the control group (P = .001). To present a case report of an orthodontic treatment of a high-angle, dual bite, Class II malocclusion without extractions and with the use of mini-plates. Class II malocclusion treatment protocols vary according to the morphological component of the malocclusion and the magnitude and direction of craniofacial growth. It is generally agreed that the cooperation of the patient and careful planning of anchorage are the key determinants of successful treatment. Protrusion of the upper and lower lip and a retrognathic mandible were the patient's chief concerns. The patient had learned to project her mandible forward to disguise the overjet. The patient's parents elected to correct the malocclusion with the use of bilateral infrazygomatic mini-plates. Pre-treatment condylar stabilization with an orthotic established a stable centric relation position, followed by mounting of the models on a semi-adjustable Panadent articulator. This allowed diagnosis and treatment planning from a stable condylar position and eliminated possible misdiagnosis due to the dual bite. Distal retraction and vertical control of the upper teeth enabled correction of the Class II malocclusion with minimal patient cooperation. Mini-plate-assisted treatment corrected the excessive overbite and overjet. The patient completed treatment with a stable occlusion and no longer postured her jaw forward. Objectives: The purpose of this three-dimensional (3D) study was to assess retrospectively the middle cranial fossa and central skull base of patients treated with the Herbst appliance (HA). Materials and Methods: 3D surface virtual models of 40 Class II, division 1 malocclusion patients were generated from cone-beam computed tomography (CBCT) acquired before treatment (T0) and after 8 months of HA treatment (T1). T0 and T1 3D models were superimposed volumetrically at the anterior cranial fossa. Twenty subjects who had been treated with the Herbst appliance (HAG) were compared to 20 subjects who were not treated orthopedically. The latter group served as a comparison control group (CG). Quantitative assessments of the location and directional changes were made with linear and angular measurements between anatomical landmarks. Qualitative assessments of the spatial behavior of the middle cranial fossa and central skull base relative to the anterior cranial fossa were displayed graphically for visualization with color maps and semitransparent overlays. Non-parametric tests were performed to compare the between the HAG and CG. Results: Point-to-point linear measurements and skeletal rotation (pitch, roll, and yaw) changes were very small along the observational period and were not significantly different between HAG and CG. Visual analysis of color maps and overlays confirmed that no changes in the cranial base were associated with HA. A recent study conducted by researchers from the Department of Orthodontics explores the root length of maxillary and mandibular anterior teeth and central incisor crown-root morphology in patients with high-angle skeletal Class II open bite. The study, which included 81 untreated patients, found that the root length of certain teeth was shorter in high-angle Class II open bite patients compared to those with normal overbite. Additionally, the crown-root angle of the maxillary central incisor was smaller in high-angle Class II open bite patients, which may have implications for torque control or adduction movement of the anterior teeth. Objectives: To test the null hypotheses that the positions of the glenoid fossae and mandibular condyles are identical on the Class I and Class II sides of patients with Class II subdivision malocclusion. Materials and Methods: Retrospective three-dimensional (3D) assessments of the positions of the glenoid fossae and mandibular condyles were made in patients with Class II malocclusion. Relative to a fiducial reference at the anterior cranial base, distances from the glenoid fossae and condyles were calculated in pretreatment cone beam computed tomographic scans of 82 patients: 41 with Class II and 41 with Class II subdivision malocclusions. The 3D distances from glenoid fossae to sella turcica in the X (right-left), Y (anterior-posterior), Z (inferior-superior) projections were calculated. Results: Patients with Class II malocclusion displayed a symmetric position of the glenoid fossae and condyles with no statistically significant differences between sides (P . .05), whereas patients with Class II subdivision showed asymmetry in the distance between the glenoid fossae and anterior cranial base or sella turcica (P , .05), with distally and laterally positioned glenoid fossae on the Class II side. (P , .05). Male patients had greater distances between glenoid fossae and anterior cranial fossae (P , .05). The condylar position relative to the glenoid fossae did not differ between the two malocclusion groups nor between males and females (P . .05). Conclusions: The null hypotheses were rejected. Patients with Class II subdivision malocclusion displayed asymmetrically positioned right- and left-side glenoid fossae, with a distally and laterally positioned Class II side, although the condyles were symmetrically positioned within the glenoid fossae.
