Damaging GATA6 variants cause cardiac outflow tract defects, sometimes with pancreatic and diaphragmic malformations. To define molecular mechanisms for these diverse developmental defects, we studied transcriptional and epigenetic responses to GATA6 loss of function (LoF) and missense variants during cardiomyocyte differentiation of isogenic human induced pluripotent stem cells. We show that GATA6 is a pioneer factor in cardiac development, regulating SMYD1 that activates HAND2, and KDR that with HAND2 orchestrates outflow tract formation. LoF variants perturbed cardiac genes and also endoderm lineage genes that direct PDX1 expression and pancreatic development. Remarkably, an exon 4 GATA6 missense variant, highly associated with extra-cardiac malformations, caused ectopic pioneer activities, profoundly diminishing GATA4, FOXA1/2, and PDX1 expression and increasing normal retinoic acid signaling that promotes diaphragm development. These aberrant epigenetic and transcriptional signatures illuminate the molecular mechanisms for cardiovascular malformations, pancreas and diaphragm dysgenesis that arise in patients with distinct GATA6 variants.

Objetivo: relatar a experiência de elaboração de um algoritmo de gerenciamento de casos para pessoas com hipertensão arterial sistêmica atendidas na atenção primária. Método: relato de experiência em que a construção do algoritmo foi baseada no modelo da Community Access Ageing, Disability and Home Care, Department of Human Services NSW9, que prevê sete etapas: Compromisso, Avaliação, Planejamento, Implementação, Monitoramento, Revisão e Encerramento. Resultados: compuseram o algoritmo questionários validados e atividades como visitas domiciliares, consultas de enfermagem, plano terapêutico individualizado e pactuação de metas, educação em saúde, contato telefônico e redirecionamento para a rede atenção à saúde. O tempo de elaboração foi de 12 meses. Conclusão: o algoritmo desenvolvido representa uma ferramenta simples e dinâmica de gerenciamento de casos, que orienta as atividades de cuidado de pessoas com hipertensão atendidas na atenção primária, mediante sete etapas, e facilita a leitura dos resultados.

Abstract Engineered epithelial cell sheets for clinical replacement of non-functional upper aerodigestive tract mucosa are regulated as medicinal products and should be manufactured to the standards of good manufacturing practice (GMP). The current gold standard for growth of epithelial cells for research utilises growth arrested murine 3T3 J2 feeder layers, which are not available for use as a GMP compliant raw material. Using porcine mucosal tissue, we demonstrate a new method for obtaining and growing non-keratinised squamous epithelial cells and fibroblast cells from a single biopsy, replacing the 3T3 J2 with a growth arrested primary fibroblast feeder layer and using pooled Human Platelet lysate (HPL) as the media serum supplement to replace foetal bovine serum (FBS). The initial isolation of the cells was semi-automated using an Octodissociator and the resultant cell suspension cryopreservation for future use. When compared to the gold standard of 3T3 J2 and FBS containing medium there was no reduction in growth, viability, stem cell population or ability to differentiate to mature epithelial cells. Furthermore, this method was replicated with Human buccal tissue, providing cells of sufficient quality and number to create a tissue engineered sheet.

Introdução: o letramento em saúde auxilia no aumento do conhecimento e das habilidades para acessar informações e gerenciar os cuidados de saúde, bem como no desenvolvimento de mecanismos para o autocuidado, especialmente na recuperação em pós-operatório de cirurgias cardíacas. Portanto existe a necessidade de interligar ações voltadas a saúde e mudança de comportamento das pessoas submetidas a cirurgia cardíaca, com a finalidade de auxiliar na sua recuperação, melhorar a qualidade de vida e identificar precocemente possíveis complicações. Objetivos: descrever o conhecimento e o letramento em saúde de adultos no pós-operatório tardio de revascularização do miocárdio. Métodos: estudo qualitativo com abordagem retrospectiva, realizado no período de dezembro de 2020 a abril de 2021, com 24 pessoas submetidas a cirurgia cardíaca no último semestre de 2019. Na coleta de dados utilizou-se um questionário semiestruturado, a Eight-Health Literacy Assessment Tool e um roteiro de entrevista. Para o tratamento dos dados, utilizou-se um software de pesquisa qualitativa, para posterior análise do conteúdo. Resultados: As entrevistas foram transcritas e possibilitaram a construção de um corpus geral constituído por 24 textos, separados em 430 segmentos de texto com aproveitamento de 86,5%. Após verificação da similitude emergiram em três categorias: “Entendimento, busca e compreensão das informações de saúde”; “Rede de apoio familiar e amigos, no cuidado após a cirurgia cardíaca” e “Momento Cirúrgico”. Conclusões. O letramento em saúde insatisfatório impactou na compreensão das informações de saúde fornecidas pela equipe multiprofissional, vindo a resultar na dificuldade de entendimento em relação a sua doença e execução dos cuidados no domicílio. Observou-se nos depoimentos, a ausência dos profissionais de saúde no planejamento e orientação dos cuidados no domicílio, sendo delegada aos familiares a responsabilidade da execução da terapêutica.

Objetivo: analisar a relação do letramento em saúde, conhecimento da doença e risco para o desenvolvimento do pé diabético em adultos com Diabetes Mellitus, de acordo com as características sociodemográficas e clínicas. Método: estudo transversal, descritivo e analítico, realizado de janeiro a março de 2020. Participaram 33 adultos com alterações glicêmicas cadastrados em unidade de saúde da Região Metropolitana de Curitiba, Paraná, Brasil. Aplicou-se questionário sociodemográfico e clínico e os instrumentos Eight-Item Health Literacy Assessment Tool e Spoken Knowledge in Low Literacy Patients with Diabetes. Resultados: houve predominância de letramento satisfatório e conhecimento adequado, associados à idade (≤59 anos), tempo de escolaridade (>4 anos) e diagnóstico (>5 anos). Ao cruzar as variáveis de letramento em saúde e conhecimento da doença com o risco para pé diabético não houve significância estatística. Conclusão: níveis satisfatórios de letramento foram associados ao conhecimento adequado, sem relação com o risco para pé diabético. Descritores: Diabetes Mellitus Tipo 2. Letramento em Saúde. Adulto. Introdução: O enfarte do miocárdio é uma doença aguda cujo cuidado deve ser imediato e a optimização da sua situação clínica está relacionada com as próprias crenças e a maneira como as pessoas vivenciam o processo de doença. Objetivo: Compreender o caminho percorrido pelas pessoas que vivenciam um enfarte agudo do miocárdio na procura da sua melhor condição de saúde. Métodos: Pesquisa qualitativa descritiva, realizada no período de fevereiro a maio de 2018, com 12 adultos jovens. Para a coleta de dados foi utilizado um questionário semi-estruturado, um roteiro de entrevista com questões abertas e diário de campo. Realizou-se análise de conteúdo temático, após tratamento dos dados, em software de pesquisa qualitativa. Resultados: Emergiram três temas centrais, que foram caraterizados como: experiências de saúde e adoecimento (compreensão da doença), percepção do risco e interação com as instituições. Conclusão: Verificou-se a demora no reconhecimento dos sinais e sintomas do enfarte agudo do miocárdio, facto este que impediu a pronta procura de cuidados de saúde.

ABSTRACT Objective: To identify the perception of post-stroke patients on the case management conducted by nurses. Method: Qualitative descriptive study with 13 post-stroke patients, followed-up with case management conducted by a nurse for six months, and interviewed at home one month after the end of the follow-up. The interviews were recorded, transcribed in full and analyzed with the help of a software. Results: Of the 13 participants, 57.14% were men and 50% were between 50 and 59 years. Content analysis resulted in three categories: “Self-management of care”, which addressed changes in attitudes towards treatment, mainly due to concern of recurrence in a short period of time; “Sequelae and late repercussions of the stroke”, which presents participants’ difficulties after the event; and “Support received after a stroke”, which emphasizes the actions and attitudes of the case manager nurse during the follow-up period. Conclusion: Case management was perceived by the participant as a health-promoting tool capable of increasing treatment adherence.

Objective. To synthesize the best available evidence regarding the effectiveness of nursing case management in primary health care, compared to usual care, in improving blood pressure in adults over 18 years with hypertension. Methods. Systematic review that includes studies carried out with adult patients diagnosed with hypertension, with or without other concomitant chronic diseases, followed-up by a case manager nurse, who evaluated the effectiveness of case management in the improvement of blood pressure. A critical evaluation of the studies was made and the results of interest were described using the instruments and tools from the Joanna Briggs Institute. Due to the heterogeneity of the included studies, the results of similar measures were not grouped in statistical meta-analysis. A narrative and tabular format was used to synthesize and present them. Results. Six randomized controlled trials were critically evaluated and included in the review. The total sample was 1963 participants. The results showed the outcomes compared at baseline and at the end of follow-up (six or twelve months). Regarding the main outcome, systolic and diastolic blood pressure, there was some reduction in the group followed-up through case management in studies lasting six months; however, the impossibility of comparing the findings poses limitations to answering the questions in this review. Conclusions. Despite the heterogeneity of the studies, the narrative and tabular analysis demonstrated that short-term case management in primary care (six-month studies) helped to reduce blood pressure levels, although the level of evidence for these results is low or very low.

ABSTRACT Objective: To validate the construct and test the reliability of the Brazilian version of Hypertension Knowledge-Level Scale. Methods: Methodological research with 220 participants in a primary health care unit from Curitiba, Paraná. The data were collected with a social demographic questionnaire and the Brazilian version of the scale. RESUMO Objetivo: comparar a autoconfiança para avaliação e intervenção de enfermagem em cenários clínicos simulados de emergência e verificar a satisfação dos estudantes de Enfermagem em participar do método. Métodos: pesquisa de intervenção, de braço único, longitudinal do tipo antes e depois, realizado com 35 estudantes de Enfermagem de uma universidade pública do Sul do Brasil. ABSTRACT Objectives: to describe the Adult's perception of healthcare after Myocardium Infarction. Methods: qualitative descriptive analysis carried out with 12 adults who had myocardial infarction and conducted from February to May 2018. The research design was based on the “Consolidated Criteria for Reporting Qualitative Research”. Data collection was performed through semi-structured questionnaire, interview script and field diary. The interviews were encoded, and their content was analyzed using software support. Results: three classes were obtained: “Fear, (un) certainty and (un) awareness”; “Lifestyle and beliefs of illness” and “Health care search”. Unhealthy lifestyle habits, lack of health prevention / promotion, difficulties in accessing services and lack of knowledge about the disease generated fear and uncertainty about the future. Final Considerations: the perception of acute myocardial infarction is related to the trajectory of illness, lifestyle and the belief that the infarction only affects others.

ABSTRACT Objectives: To acknowledge the social representations of aggressive men denounced for violence against women. Methods: A qualitative study based on the Theory of Social Representations, carried out with 12 men denounced for violence against women. Data were collected from February 2015 to April 2016, through an interview focused on the Special Women’s Police Station (Delegacia Especial da Mulher) in of Guarapuava. The analysis was based on the transcription, coding and use of the software Interface de R pour les Analyses Multidimensionnelles de Textes et de Questionnaire. Results: Four categories were defined: from generation to generation: the spread of violence; feelings and behaviors in the face of violence; repercussions of violence on the aggressor; and, the aggressor and his victimization. Final considerations: Men represent conjugal violence through victimizing and blaming women. The living of these men with violence comes from their childhood and the consequence feared by them is the prison.

Abstract  We report a case of foreign body ingestion of a blister pill pack, causing small bowel obstruction. A 76-year-old woman on multiple medications presented with 3 days of progressive abdominal distention, nausea, and vomiting. A computed tomography (CT) scan demonstrated small bowel obstruction with a distinctive metallic foreign body in the distal ileum with associated wall thickening and mesenteric inflammatory changes. At exploratory laparotomy, an impacted, intact blister pill pack was removed from the distal ileum. The ingestion of blister pill packs has been associated with a range of clinical and imaging findings. To our knowledge, this is the only reported case of CT diagnosis of small bowel obstruction caused by blister pack ingestion. Early recognition of the imaging findings of an ingested blister pill pack is important to expedite appropriate management. [ABSTRACT FROM AUTHOR]

RESUMO Objetivo: comparar a percepção de competência clínica dos estudantes de Enfermagem ao início e ao final dos estágios supervisionados do último semestre da graduação. Método: trata-se de estudo descritivo e longitudinal, com abordagem quantitativa realizada nas dependências do curso de Enfermagem de uma universidade pública. Participaram 17 acadêmicos do último período da graduação em Enfermagem. A coleta de dados foi realizada no período de fevereiro a junho de 2017, em dois momentos, no início e término do semestre letivo, mediante aplicação do questionário de competência clínica traduzido e adaptado transculturalmente, que possui 48 questões divididas em dois domínios: comportamentos profissionais de Enfermagem e competências/habilidades. Objective: To identify in the nursing literature the health care systems used by individuals going through myocardial infarction.Method: Integrative review conducted in five national and international databases from 2007 to 2017. In the review, 398 studies were identified and 13 were analyzed according to the classification of the health care systems into formal and informal.Results: Health care was impacted by values, beliefs and social norms constructed throughout the lives of the individuals, and which guide the search for different health care systems during the acute episode of infarction, with emphasis to the informal system structured by the individuals’ families and community.Conclusion: The understanding of health care systems by nursing professionals facilitates the elaboration of educational health actions and the management of care to patients going through myocardial infarction.

Os dessalinizadores têm sido adotados com cada vez mais frequência haja vista que as medidas convencionais, tais como construção de cisternas e açudes, não têm sido suficientes para suprir o consumo humano durante o período da estiagem. Este trabalho teve como objetivo comparar os tipos de dessalinizadores. Para tanto, foi feita uma ampla compilação de várias trabalhos das literaturas nacional e internacional disponíveis em bases de dados sobre a dessalinização e os tipos de dessalinizadores. Consideraram-se as vantagens e desvantagens, com o objetivo de identificar qual as principais características de cada método de dessalinização. Através da vasta revisão bibliográfica, foi possível identificar as principais características dos métodos de dessalinização. Dentre as tecnologias estudadas, cada uma se destaca em um meio diferente como água do mar ou poços, apresentando vantagens e desvantagens únicas. Portanto, a utilização de uma ou outra deve ser analisada caso a caso. Em comum, todas elas apresentam um grande consumo de energia elétrica, sendo este insumo o maior responsável direto pelo preço da água dessalinizada. A osmose reversa é tida como a melhor solução na maioria dos casos. É importante salientar que a dessalinização das águas é uma solução para o abastecimento de comunidades cuja relevância deverá se acentuar nos próximos anos, face à carência crescente de água potável no mundo.

Qualitative study, undertaken in three phases between 2012 and January 2014 to verify the use of clinical protocols when giving advice to chronic patients. The initial data collection took place upon discharge from the hospital, the second at home after 12 months and the third after 24 months. The sample consisted of 20 hypertensive and/or diabetic adults hospitalized at a teaching hospital in Curitiba, Brazil. Based on the data analysis, three categories were constituted: “Knowledge about the disease”, “Care with regard to the disease” and “Barriers for treatment adherence”. It was identified that the advice, through the use of clinical protocols, furthered the knowledge about the disease and the care the participants developed. In conclusion, the use of the protocols is a health education strategy for chronic patients. They make the care feasible and reveal the progress made and difficulties found.

ABSTRACT Objective: to make the translation, cross-cultural adaption and content and face validation of the Hypertension Knowledge-Level Scale for use in Brazil. Methods: methodological research carried out in six stages: translation, synthesis, back-translation, expert committee’s assessment, pre-test and validation. Validation was performed through the Delphi technique in two rounds. The participants were two translators and two back-translators, eight professionals in the expert committee, 40 adult participants in the pre-test, 35 experts in the first validation round and 28 in the second validation round. Data analysis included Cronbach’s alpha, content validity index and one-tailed t-test. Results: the translation and cross-cultural adaptation allowed for language adjustments so that the items were comprehensible and suitable for use in Brazil. The content validity index of the Brazilian version of Hypertension Knowledge-Level Scale was 0.96 and Cronbach’s alpha was 0.92. Conclusions: the scale was translated, cross-culturally adapted to Brazilian Portuguese, had its content and face validated and proved reliable to evaluate the knowledge of adults about hypertension.

Resumo OBJETIVO Elaborar uma escala preditiva de determinantes para complicações em adultos com hipertensão e ações para o autocuidado apoiado na atenção primária. MÉTODOS Pesquisa metodológica realizada no município de Curitiba-PR em 2013 e 2014, em duas etapas, a primeira mediante coleta de dados com 387 adultos com hipertensão, por meio de uma entrevista estruturada e escalas de ansiedade, depressão, qualidade de vida, adesão medicamentosa e apoio social. A segunda etapa foi à construção da escala a partir das variáveis estatisticamente significantes após a análise multivariada. RESULTADOS A escala foi composta pelas variáveis: idade, sexo, tabagismo, tempo de diagnóstico, classificação de risco na unidade de saúde, medicamentos em uso e depressão, posteriormente por meio de revisão da literatura foram sugeridas ações para o autocuidado apoiado. CONCLUSÃO A escala permite identificação de fatores que podem predizer o desenvolvimento de complicações da hipertensão e fornece ações para o autocuidado apoiado.

The present study aimed to investigate the relationship between depression and quality of life in hypertensive adults. Quantitative study with cross-sectional design, with 387 adults with hypertension, conducted in 2014-2015, in 18 Basic Health Units (UBS) in the city of Curitiba, State of Paraná. The tools used to assess quality of life and depression were, respectively, the Medical Outcomes Study 36-Item Short Form Health Survey and Beck Inventory. A significant correlation was found between the worst scores of quality of life and the presence of depression, with great differences between the means in the domains emotional aspects (Confidence Interval = 95%: 26.72% - 42.08%) and mental health (Confidence Interval = 95%: 20.21% - 28.73%). Knowledge of this relationship can help nurses in the planning of care for individuals with hypertension, in the improvement of depressive symptoms and quality of life.

RESUMO Trata-se de pesquisa metodológica de natureza quantitativa que utilizou a técnica Delphi com o objetivo de validar o conteúdo e a aparência de uma escala de predição para complicações da hipertensão com ações para o gerenciamento do cuidado. A coleta de dados ocorreu no período de fevereiro a julho de 2015, em duas rodadas de questionários via Google Docs® e uma reunião de consenso. Os participantes foram mestres e doutores experts em hipertensão arterial sistêmica. Os dados foram analisados por meio de estatística descritiva e pelo índice de validade de conteúdo. Na primeira rodada do processo de validação, 15 (47%) participantes eram mestres, 17 (53%) doutores e a concordância entre eles sobre os itens da escala foi de 87%; na segunda participaram 13 (50%) mestres, 13 (50%) doutores e a concordância aumentou para 98%. Na reunião de consenso, seis experts discutiram as sugestões fornecidas na segunda rodada. Concluiu-se que a escala de predição para complicações da hipertensão com ações para o gerenciamento de cuidado mostrou evidências de validade de conteúdo, indicando que pode ser utilizada para medir o risco do desenvolvimento de complicações e fornecer ações que possibilitem o gerenciamento do cuidado.

Objective. To identify the care models and the impact of the use of these in the care of people with chronic diseases reported in the literature in the years 2000 to 2014. Methods. Integrative literature review in which the following guiding question was adopted: Which care models are used in the care of patients with chronic diseases and what impacts can be verified through their application? We consulted the bibliographic databases Virtual Health Library, LILACS, MEDLINE, Spanish Bibliographic Index of Health Sciences and the Database of Nursing. Results.The sample consisted of 17 articles on the topic of interest. Three categories emerged from the analysis: healthcare costs, model-based care experience, and patient autonomy. The articles addressed self-management, case management and care model for people with chronic diseases. The major impacts on the use of the models were: a better relationship between the patient and the health professional, an increase in the autonomy of the person with chronic illness, and a reduction in personal and health care expenditure. Conclusion. The use of care models for people with chronic diseases presents benefits to the patient and to the health system. Objetivo: identificar as terapias complementares utilizadas por pessoas com hipertensão arterial sistêmica. Método: pesquisa quantitativa, descritiva com corte transversal realizada nos domicílios de 387 pacientes com hipertensão, no período de abril de 2013 a fevereiro de 2015. A coleta de dados foi realizada mediante roteiro de entrevista semiestruturada e os dados foram analisados de forma descritiva e inferencial com auxílio do Statistical Package for the Social Sciences. Resultados: dos participantes da pesquisa, 20,7% faziam uso de terapias complementares, as quais foram indicadas principalmente pelos familiares e amigos, sendo a fitoterapia (85%) a mais citada pelos pacientes, seguida pela acupuntura, com o objetivo de tratar ou controlar a hipertensão. Conclusões: o tipo de terapia complementar mais utilizado pelas pessoas com hipertensão arterial sistêmica foi a fitoterapia. Descritores: Terapias Complementares; Hipertensão; Adulto; Enfermagem

ABSTRACT Objective: Evaluate the contribution of debriefing after clinical simulations for nursing students. Method: Quantitative study, conducted with 35 nursing students who participated in five clinical simulation scenarios with planned debriefings based on the model of the National League Nursing/Jeffries Simulation Theory. After the fifth scenario, students answered the Debriefing Evaluation Scale associated with the Simulation. Results: The items evaluated involved the psychosocial, cognitive, and affective values, and within a scale from one to five, the highest mean was found in cognitive value with 4.23 (±0.56) points, then in psychosocial value with 3.77 (±0.53), and finally in affective value with 3.71 (±0.63) points. Conclusion: The debriefing conducted after the clinical simulation scenarios was a reflective exercise that contributed to the student integrating multiple knowledges in affective, cognitive and psychosocial values, and thus develop the competencies required.

ABSTRACT Objective: To standardize the duty shift in a General Adult Intensive Care Unit. Method: Multi-method research, which used action research, descriptive study and content validation. Participants included 11 care nurses and 4 intensive care nurses. For the data collection, a semi-structured questionnaire was used, meetings with participants and validation with specialists. In this step, the modified online Delphi Technique was used. For data treatment, the Discourse of the Collective Subject (DCS) was used, descriptive analysis and Content Validity Index. Results: Three DCS on duty change, a Standard Operating Procedure (SOP) and an information registration instrument, validated in appearance, clarity, suitability and content. Final considerations: The instrument assists in the transmission of information, strengthening patient safety and SOP will outline the shift, these tools can improve ICU shift, minimizing the risks of communication failure.

ABSTRACT Objective: translating and transculturally adapting the Clinical Competence Questionnaire to Brazilian senior undergraduate Nursing students, as well as measuring psychometric properties of the questionnaire. Method: a methodological study carried out in six steps: translation of the Clinical Competence Questionnaire instrument, consensus of the translations, back-translation, analysis by an expert committee, pre-testing and then presentation of the cross-cultural adaptation process to the developers. Psychometric properties were measured using Cronbach's alpha, intraclass correlation coefficient and content validity index. Results: the instrument was translated, transculturally adapted and its final version consisted of 48 items. Cronbach's alpha coefficient was 0.90, and the agreement index of the items was 99% for students and 98% for evaluators. Conclusion: the Clinical Competence Questionnaire was translated and adapted to Brazilian students, and the psychometric properties of the Portuguese version of the questionnaire presented satisfactory internal consistency regarding the studied sample.
