Abstract

Summary: Background: Acute myeloid leukaemia (AML) is a bone marrow malignancy with poor prognosis. One of several treatments for AML is midostaurin combined with intensive chemotherapy (MIC), currently approved for FLT3 mutation-positive (FLT3-MP) AML. However, many patients carrying FLT3 mutations are refractory or experience an early relapse following MIC treatment, and might benefit more from receiving a different treatment. Development of a stratification method that outperforms FLT3 mutational status in predicting MIC response would thus benefit a large number of patients. Methods: We employed mass spectrometry phosphoproteomics to analyse 71 diagnosis samples of 47 patients with FLT3-MP AML who subsequently received MIC. We then used machine learning to identify biomarkers of response to MIC, and validated the resulting predictive model in two independent validation cohorts (n = 20). Findings: We identified three distinct phosphoproteomic AML subtypes amongst long-term survivors. The subtypes showed similar duration of MIC response, but different modulation of AML-implicated pathways, and exhibited distinct, highly-predictive biomarkers of MIC response. Using these biomarkers, we built a phosphoproteomics-based predictive model of MIC response, which we called MPhos. Abstract

Abstract Background The combination of nivolumab + relatlimab is superior to nivolumab alone in the treatment of naive patients and has activity in PD-1 refractory melanoma. We had previously observed a reduced expression of LAG3 in melanoma tissue from patients with type 2 diabetes. Method To evaluate the impact of diabetes on oncological outcomes of patients with advanced melanoma treated with nivolumab plus the LAG3 inhibitor relatlimab we performed a retrospective multicenter study. Results Overall, 129 patients were included: 88 without diabetes before the treatment, 37 who were diagnosed with type 2 diabetes before the start of treatment, and 4 without diabetes before treatment who developed immune checkpoint inhibitor-induced diabetes (ICI-DM). PFS was 21.71 months (95% CI: 15.61–27.81) in patients without diabetes, 10.23 months (95% CI: 5.81–14.66) in patients with type 2 diabetes, and 50.85 months (95% CI: 23.04–78.65) in patients who developed ICI-DM. OS was 37.94 months (95% CI: 31.02–44.85) in patients without diabetes, 22.12 months (95% CI: 14.41–29.85) in those with type 2 diabetes and 57.64 months (95% CI: 42.29–72.99) in those who developed ICI-DM. Multivariate analysis showed that the presence of diabetes and LDH was correlated with OS and PFS. The mean OS was 64.63 months in subjects with low levels of glucose ( 1.5) had a worse prognosis than those whose glucose level had not increased. This result was observed also in subgroups treated either in first line or further lines. Patients who developed ICI-DM during the study period had better outcomes than the overall population and patients without diabetes. Conclusions LAG3 inhibition for treating metastatic or unresectable melanoma has a reduced efficacy in patients with type 2 diabetes, possibly due to a low expression of LAG3 in tumor tissue. Higher level evidence should be obtained.

Abstract

Summary: Background: Both dabrafenib/trametinib (D/T) and anti-PD-1 monotherapy (PD-1) are approved adjuvant therapies for patients with stage III BRAF V600-mutant melanoma. However, there is still a lack of head-to-head comparative data. We aimed to describe efficacy and toxicity outcomes for these two standard therapies across melanoma centers. Methods: This multicenter, retrospective cohort study was conducted in 15 melanoma centers in Australia, China, Germany, Italy, Japan, UK, and US. We included adult patients with resected stage III BRAF V600-mutant melanoma who received either adjuvant D/T or PD-1 between Jul 2015 and Oct 2022. The primary endpoint was relapse-free survival (RFS). Secondary endpoints included overall survival (OS), recurrence pattern and toxicity. Findings: We included 598 patients with stage III BRAF V600-mutant melanoma who received either adjuvant D/T (n = 393 [66%]) or PD-1 (n = 205 [34%]) post definitive surgery between Jul 2015 and Oct 2022. At a median follow-up of 33 months (IQR 21–43), the median RFS was 51.0 months (95% CI 41.0-not reached [NR]) in the D/T group, significantly longer than PD-1 (44.8 months [95% CI 28.5-NR]) (univariate: HR 0.66, 95% CI 0.50–0.87, P = 0.003; multivariate: HR 0.58, 95% CI 0.39–0.86, P = 0.007), with comparable OS with PD-1 (multivariate, HR 0.90, 95% CI 0.48–1.70, P = 0.75). Similar findings were observed using a restricted-mean-survival-time model. Among those who experienced recurrence, the proportion of distant metastases was higher in the D/T cohort. D/T had a higher incidence of treatment modification due to adverse events (AEs) than PD-1, but fewer persistent AEs. Interpretation: In patients with stage III BRAF V600-mutant melanoma post definitive surgery, D/T yielded better RFS than PD-1, with higher transient but lower persistent toxicity, and comparable OS. D/T seems to provide a better outcome compared with PD-1, but a longer follow-up and ideally a large prospective trial are needed. Abstract

Inverting papillomas (IP) are benign sinonasal tumours that can undergo transformation to squamous cell carcinoma (SCC). Low-Grade Papillary Schneiderian Carcinoma (LGPSC) is a unique malignancy that shares similar morphology to IP, but demonstrates invasive patterns different from SCC.Here we report the case of a patient who underwent excision of a unilateral sinonasal tumour with histopathology demonstrating bland cells and non-invasive “pushing” borders. The tumour recurred rapidly with extensive local invasion, however, further excisions continued to show non-malignant morphology, leading to diagnosis of LGPSC. The patient required radical maxillectomy to achieve complete disease clearance.LGPSC is rare with only two previous descriptions in the literature. LGPSC appears similar to IP, but has “pushing” borders and a propensity for rapid local recurrence. It is likely some existing cases of aggressive IP are LGPSC and we therefore advise MDT discussion for all unusual cases of IP. Keywords: Sinonasal malignancy, Inverted papilloma

Abstract

History of present illness: A 53-year-old male presented with worsening right knee pain and swelling over the past 48 hours. He denied recent trauma to the knee, history of IV drug use, and recent illness. He had no history of diabetes, immunodeficiency, chronic steroids, rheumatologic disease, or knee replacement. He described the pain as sharp, non-radiating, and worse with movement. He was unable to walk due to pain. Significant findings: Radiographs of the knee showed multiple radio-dense lines paralleling the articular surface (see red arrows) consistent with calcium pyrophosphate crystal deposition within the joint often seen in calcium pyrophosphate disease (CPPD) also known as pseudogout. Discussion: Patients commonly present to the emergency department with non-traumatic joint pain. Arthrocentesis is an important diagnostic tool to evaluate for septic arthritis, gout, or pseudogout. Arthrocentesis can demonstrate crystals or abnormal cell count, gram stain, and culture.[1] In the evaluation of joint pain, plain films are usually obtained to evaluate for fracture, dislocation, effusion, or secondary signs of infection. In this case the classic x-ray supported the diagnosis of CPPD.2 The patient was found to have positively birefringent rhomboid shaped crystals consistent with pseudogout on arthrocentesis. Gram stain and culture were both negative. The patient was discharged with NSAIDs and had significant improvement in symptoms upon follow up with primary care physician in 3 days.

8. Identification of nuclear protein targets for six leukemogenic tyrosine kinases governed by post-translational regulation.

Abstract

Mutated tyrosine kinases are associated with a number of different haematological malignancies including myeloproliferative disorders, lymphoma and acute myeloid leukaemia. The potential commonalities in the action of six of these leukemogenic proteins on nuclear proteins were investigated using systematic proteomic analysis. The effects on over 3600 nuclear proteins and 1500 phosphopeptide sites were relatively quantified in seven isogenic cell lines. The effects of the kinases were diverse although some commonalities were found. Comparison of the nuclear proteomic data with transcriptome data and cytoplasmic proteomic data indicated that the major changes are due to post-translational mechanisms rather than changes in mRNA or protein distribution. Analysis of the promoter regions of genes whose protein levels changed in response to the kinases showed the most common binding site found was that for NFκB whilst other sites such as those for the glucocorticoid receptor were also found. Glucocorticoid receptor levels and phosphorylation were decreased by all 6 PTKs. Whilst Glucocorticoid receptor action can potentiate NFκB action those proteins where genes have NFκB binding sites were in often regulated post-translationally. However all 6 PTKs showed evidence of NFkB pathway modulation via activation via altered IkB and NFKB levels. Validation of a common change was also undertaken with PMS2, a DNA mismatch repair protein. PMS2 nuclear levels were decreased in response to the expression of all 6 kinases, with no concomitant change in mRNA level or cytosolic protein level. Response to thioguanine, that requires the mismatch repair pathway, was modulated by all 6 oncogenic kinases. In summary common targets for 6 oncogenic PTKs have been found that are regulated by post-translational mechanisms. They represent potential new avenues for therapies but also demonstrate the post-translational regulation is a key target of leukaemogenic kinases.

11. Paediatric tracheobronchomalacia: Incidence, patient characteristics, and predictors of surgical intervention

Abstract

Objectives: Tracheobronchomalacia (TBM), a condition where an abnormality of the tracheal walls causes collapse during the respiratory cycle, is a common cause of airway obstruction in childhood. TBM can present with a large spectrum of disease severity and underlying pathologies that may be managed medically and surgically, and it is not always clear which patients would most benefit from surgical intervention. We aim to describe the incidence, patient characteristics, and predictors of surgical intervention in a large cohort of paediatric patients. Methods: We performed a retrospective review of all children diagnosed with TBM to a paediatric Otolaryngology unit in the west of Scotland between 2010 and 2020. Odds ratios for clinical predictors of surgery were calculated using logistic regression with uni- and multivariate analysis. Results: 249 patients were identified of which 219 proceeded to data collection. Primary malacia was noted in 161 (73.5%) and secondary in 58 (26.5%). Causes of secondary malacia included compression by the innominate artery (11%) and vascular rings (7.8%). Surgical interventions were performed in 28 patients (12.8%) including division of vascular ring, aortopexy, and surgical tracheostomy. Multivariate analysis showed secondary TBM, acute life-threatening events, and difficulty weaning from mechanical ventilation were independent risk factors for surgical intervention. Conclusions: TBM can present with a myriad of airway symptoms and is frequently associated with other airway and mediastinal pathologies necessitating multiple interventions. Children aged

13. Innovations in preoperative control of exsanguination in major trauma

Abstract

Exsanguination places a considerable strain on trauma systems worldwide, and is estimated to be involved in 20–40% of trauma deaths. This article evaluates innovations in the preoperative care of the injured patient to minimise the impact of bleeding. These include reform of organisations and structure of trauma networks, adoption of objective prehospital triage tools and changes in ambulance crews' approach to on-scene interventions. Developments of methods to mechanically control these bleeds—simple tourniquets, topical haemostatics developed in military settings and endovascular interventions such as resuscitative endovascular balloon occlusion of the aorta—are also analysed. This article has examined advancements in damage control resuscitation, including a possible future shift towards whole-blood transfusion and interventional radiology for primary haemorrhage control. Finally, a potential further development—the uptake of hybrid resuscitation suites—is examined.

18. A systematic review of the role of penicillin vs penicillin plus metronidazole in the management of peritonsillar abscess

Abstract

Background Peritonsillar abscess is a common clinical problem. Management involves drainage of the abscess and administration of antibiotics. The choice of antibiotic is related to the polymicrobial growth of aspirate cultures, leading to prescriptions of co-amoxiclav, or metronidazole in addition to phenoxymethylpenicillin. However there is little evidence to support this. Objectives The aim of this review was to assess clinical effectiveness of phenoxymethylpenicillin vs phenoxymethylpenicillin plus anaerobic cover in the management of peritonsillar abscess. Design/Setting A systematic review of literature and clinical trial databases in accordance with the PRISMA 2020 statement. Studies were screened for eligibility by two independent reviewers. Main outcome measure Three studies were included, two comparing oral penicillin to oral penicillin plus metronidazole, one comparing IM/oral penicillin to IM/oral sulbactam-ampicillin. Clinical outcomes were assessed in all, including recurrence rate, symptom improvement and duration of pyrexia. Results There was no significant difference in any clinical outcome across all studies between the two groups. One study found a significant increase in diarrhoea and vomiting as a side effect in the group receiving metronidazole plus penicillin compared to penicillin alone (p=0.01). Conclusion On reviewing the literature, no significant clinical benefit has been demonstrated in the addition of either metronidazole or more broad-spectrum antibiotic cover compared to oral penicillin monotherapy for peritonsillar abscess when combined with incision and drainage protocols. Moreover, unnecessary broad-spectrum antibiotics contribute to increased side effects, costs, and antimicrobial resistance.

Abstract

Cricopharyngeal spasm and pharyngeal pouch can result in severe dysphagia symptoms that can have a significant impact on a patient’s quality of life. Several surgical interventions have been demonstrated including balloon dilatation, botox injection, and open and endoscopic cricopharyngeal myotomy. The aim of this case series is to describe our technique for endoscopic cricopharyngeal myotomy and compare pre- and postoperative swallowing outcomes using objective swallowing questionnaires in a small series of patients. Seven patients diagnosed with cricopharyngeal spasm or grade 1 pharyngeal pouch on barium swallow were prospectively observed. MD Anderson Dysphagia inventory (MDADI) and Dysphagia handicap index (DHI) were given to patients before and after their operation. All patients underwent the same surgical technique with the cricopharyngeal bar divided and the defect closed with dissolvable sutures. Mean inpatient stay was 7 days (range 1-8) and mean time to resumption of soft diet was 4 (range 1-7) days. All patients resumed oral diet before discharge. MDADI scores showed improvement across all domains, however only the emotional score achieved statistical significance ( P = .04). Dysphagia handicap index scores showed a statistically significant improvement across functional, emotional, and total domains. Postoperative chest infection treated with oral antibiotics occurred in 3 patients (43%) and no other complications were documented. In conclusion, endoscopic cricopharyngeal myotomy with primary sutured endoscopic closure can result in a significant benefit to patient’s swallowing symptoms, emotional well-being, and quality of life.

Description, Brain proteins altered in young APOE ε4 carriers are found decades later in Alzheimer’s disease and present novel drug targets., Aptamer-based proteomics revealed differentially abundant proteins in Alzheimer’s disease (AD) brains in the Baltimore Longitudinal Study of Aging and Religious Orders Study (mean age, 89 ± 9 years). A subset of these proteins was also differentially abundant in the brains of young APOE ε4 carriers relative to noncarriers (mean age, 39 ± 6 years). Several of these proteins represent targets of approved and experimental drugs for other indications and were validated using orthogonal methods in independent human brain tissue samples as well as in transgenic AD models. Using cell culture–based phenotypic assays, we showed that drugs targeting the cytokine transducer STAT3 and the Src family tyrosine kinases, YES1 and FYN, rescued molecular phenotypes relevant to AD pathogenesis. Our findings may accelerate the development of effective interventions targeting the earliest molecular triggers of AD.

Abstract

The British government has ambitions to achieve net-zero carbon emissions countrywide by 2050, with their largest challenge being emissions from the construction industry. Approved Document L sets standards for the fabric performance of buildings to regulate their consumption of fuel and power, thereby allowing easier transition to all-renewable grid electricity and the phasing out of fossil fuels. Whilst this approach has shown success in new builds, its effectiveness on retrofits, especially regarding built heritage, is significantly reduced. Responding to this, the paper investigates alternative sustainable design solutions that could feasibly justify revisions to Approved Document L, to improve the sustainable performance of existing buildings and bring them in line the government’s 2050 targets. Trialing solutions on a listed building case study, benchmark figures are used to analyse the energy, carbon, and cost implications of sustainable design approaches relating to passive design, low-carbon technologies, renewable energy, and additional considerations. Using this method, it is reasonable to conclude that the standards of Approved Document L for existing buildings are currently underperforming but can feasibly be revised to encompass the full breadth of contemporary sustainable design solutions.

Abbot, Rich, et al. (Virgo and MAGIC Collaboration), Advanced LIGO and Advanced Virgo are monitoring the sky and collecting gravitational-wave strain data with sufficient sensitivity to detect signals routinely. 32. Eagle syndrome and carotid artery dissection: a rare skull base cause of stroke

Abstract

Eagle’s syndrome is a rare collection of symptoms that occur secondary to an elongated styloid process or calcified stylohyoid ligament irritating its surrounding structures. Classically, this presents as unilateral throat pain or rarely, as acute neurological symptoms secondary to compression of the internal carotid artery: so called ‘stylocarotid syndrome’. Significant neurological events in teenagers, secondary to Eagle syndrome have not been reported. We discuss the rare case of a teenage boy, diagnosed with right internal carotid artery dissection and middle cerebral artery infarction, with no cause initially identified. Following further admission with a transient neurological episode, he was noted to have elongated styloid processes with the right abutting the site of carotid dissection. He underwent styloidectomy and has since remained symptom free. This case highlights the importance of considering anatomical variants when assessing young patients with neurological symptoms, and the potential morbidity and mortality benefit that early surgical intervention may have.

Abstract

This article investigates the influence of the Decalogue commandment not to murder in 1 Corinthians as a case study of Paul’s dependence on the law for ethics. Paul does not quote or allude to the murder commandment in the letter. However, there is good evidence that the murder commandment, in line with its interpretation in the Jewish Scriptures and early Jewish moral teaching, is a fundamental presupposition of Paul’s moral teaching in 1 Corinthians. This can be seen in Paul’s use of murder as a metaphor and of antisocial vices as synecdoches for the murder commandment, his concern about the moral impurity of such murder, and the close relationship between Paul’s call to love and his call to forsake such antisocial vices. Reading 1 Corinthians with the murder commandment in mind, especially as it was understood in ancient Judaism, sheds light on the nature of the problems in the church of God in Corinth and clarifies Paul’s response to the problems in the church.

Abstract

Surface meltwater on ice shelves can exist as slush, it can pond in lakes or crevasses, or it can flow in surface streams and rivers. The collapse of the Larsen B Ice Shelf in 2002 has been attributed to the sudden drainage of ∼3000 surface lakes and has highlighted the potential for surface water to cause ice-shelf instability. Surface meltwater systems have been identified across numerous Antarctic ice shelves, although the extent to which these systems impact ice-shelf instability is poorly constrained. To better understand the role of surface meltwater systems on ice shelves, it is important to track their seasonal development, monitoring the fluctuations in surface water volume and the transfer of water across ice-shelf surfaces. Here, we use Landsat 8 and Sentinel-2 imagery to track surface meltwater across the Nivlisen Ice Shelf in the 2016–2017 melt season. We develop the Fully Automated Supraglacial-Water Tracking algorithm for Ice Shelves (FASTISh) and use it to identify and track the development of 1598 water bodies, which we classify as either circular or linear. The total volume of surface meltwater peaks on 26 January 2017 at 5.5×107 m3. At this time, 63 % of the total volume is held within two linear surface meltwater systems, which are up to 27 km long, are orientated along the ice shelf's north–south axis, and follow the surface slope. Over the course of the melt season, they appear to migrate away from the grounding line, while growing in size and enveloping smaller water bodies. This suggests there is large-scale lateral water transfer through the surface meltwater system and the firn pack towards the ice-shelf front during the summer.

Abstract

Introduction: Human papilloma virus (HPV)–positive oropharyngeal cancer carries a good prognosis when managed with primary chemoradiotherapy. However, the dramatically increasing rate of this disease means more patients are now developing recurrence, with surgery remaining the mainstay of treatment. Despite this, there is no agreed technique for excision of recurrent oropharyngeal cancer. Objective: We describe the transoral robotic technique employed by our head and neck multidisciplinary team (MDT) in the management of patients with recurrent HPV positive oropharyngeal cancer and assess their symptom severity using quality of life and swallowing questionnaires. Method: Three (2 males:1 female, mean age 60.7 years) patients with recurrent or residual p16 positive oropharyngeal cancer following radical chemoradiotherapy were identified. All patients underwent selective neck dissection, tracheostomy, and transoral robotic surgery (TORS)–assisted partial oropharyngeal resection with the resultant defect closed with a robotic assisted radial forearm free flap (RFFF). Patient quality of life, symptom severity, and swallowing were assessed pre- and postoperatively using the University of Washington Quality of Life score and MD Anderson Dysphagia Index (MDADI). Results: Histopathological examination revealed complete clearance of the primary lesion in all cases. Two patients made uneventful recoveries, while one patient developed a chest infection and tracheocutaneous fistula managed conservatively. Mean inpatient stay was 15 days (range 8-27). University of Washington Quality of Life and MDADI scores showed a mild improvement in symptoms following surgery. Conclusion: Surgical management of recurrent oropharyngeal cancer remains a technical challenge; however, MDT discussion and judicious use of TORS oropharyngeal resection and RFFF can result in good oncological and quality of life outcomes with acceptable postoperative complications and symptoms.

Abstract

Purpose: This review provides an overview and quality assessment of existing interven-tions, assessing the intervention types that are most effective at increasing enrolment and adherence to cardiac rehabilitation in older patients aged ≥65 years Methods: The review of the literature was performed using electronic databases to search for ran-domised controlled trials that aimed to increase enrolment and/or adherence to cardiac rehabilitation in older patients aged ≥65 years. The main key words were cardiac rehabilitation, enrolment, adherence and older patients. Studies were included if; (1) the intervention targeted improving enrolment and/or adherence to at least one of the following components of the cardiac rehabilitation programme: exer-cise, education or maintaining lifestyle changes; (2) assess the effectiveness of an intervention on in-creasing enrolment and/or adherence to a cardiac rehabilitation programme or any of its components; (3) include measures for assessing enrolment and/or adherence to a cardiac rehabilitation programme or any of its components; (4) the study included patients with a mean age of ≥65 years who were deemed eligible to participate in a cardiac rehabilitation programme. Included studies could be published in any language and there were no date restrictions for included studies. Studies focusing on pharmaceutical adherence were not included for the purpose of this review. Results: Seven studies were included, with four investigating enrolment (1944 participants) and three assessing adherence to intervention programmes (410 participants). Three studies (1919 participants) reported higher enrolment to cardiac rehabilitation in the intervention group. Two studies that reported increases in enrolment to cardiac rehabilitation were deemed to have an unclear or high risk of bias. All three studies (410 participants) reported better adherence to cardiac rehabilitation in the interven-tion group when compared to the control group. Two studies that reported better completion of cardi-ac rehabilitation were deemed to have an unclear or high risk of bias. No formal meta-analysis was conducted due to the observed multiple heterogeneity among outcome measures, the low number of included studies and variability in study designs. Conclusion: This review found only weak evidence to suggest that interventions can increase enrol-ment or adherence to cardiac rehabilitation programmes for patients aged ≥65 years, therefore no practice recommendations could be made and further high-quality research is needed in this popula-tion group.

Abstract

Background Small bowel obstruction is a common surgical emergency, and is associated with high levels of morbidity and mortality across the world. The literature provides little information on the conservatively managed group. The aim of this study was to describe the burden of small bowel obstruction in the UK. Methods This prospective cohort study was conducted in 131 acute hospitals in the UK between January and April 2017, delivered by trainee research collaboratives. Adult patients with a diagnosis of mechanical small bowel obstruction were included. The primary outcome was in‐hospital mortality. Secondary outcomes included complications, unplanned intensive care admission and readmission within 30 days of discharge. Practice measures, including use of radiological investigations, water soluble contrast, operative and nutritional interventions, were collected. Results Of 2341 patients identified, 693 (29·6 per cent) underwent immediate surgery (within 24 h of admission), 500 (21·4 per cent) had delayed surgery after initial conservative management, and 1148 (49·0 per cent) were managed non‐operatively. The mortality rate was 6·6 per cent (6·4 per cent for non‐operative management, 6·8 per cent for immediate surgery, 6·8 per cent for delayed surgery; P = 0·911). The major complication rate was 14·4 per cent overall, affecting 19·0 per cent in the immediate surgery, 23·6 per cent in the delayed surgery and 7·7 per cent in the non‐operative management groups (P < 0·001). Cox regression found hernia or malignant aetiology and malnutrition to be associated with higher rates of death. Malignant aetiology, operative intervention, acute kidney injury and malnutrition were associated with increased risk of major complication. Conclusion Small bowel obstruction represents a significant healthcare burden. Patient‐level factors such as timing of surgery, acute kidney injury and nutritional status are factors that might be modified to improve outcomes. © 2019 The Authors. In this study, we explored how our understanding of hydrologic flow pathways can be improved and expanded in both space and time by the simultaneous application of engineered synthetic DNA tracers. In this study, we compared the advective-dispersive transport properties and mass recovery rates of two types of synthetic DNA tracers, one consisting of synthetic DNA strands encapsulated into biodegradable microspheres and another consisting of 'free' DNA, i.e. not encapsulated. The DNA tracers were also compared with a conservative fluorescent dye. All tracers were injected into a small (3.2-km2) valley glacier, Storglaciaren, in northern Sweden. Seven of the nine DNA tracers showed clear recovery during the sampling period and similar peak arrival times and dispersion coefficients as the conservative fluorescent dye. However, recovered DNA tracer mass ranged only from 1% to 66%, while recovered fluorescent dye mass was 99%. Resulting from the cold and opaque subglacial environment provided by the glacier, mass loss associated with microbial activity and photochemical degradation of the DNA is likely negligible, leaving sorption of DNA tracers onto suspended particles and loss of microtracer particles to sediment storage as probable explanations. Despite the difference in mass recovery, the advection and dispersion information derived from the DNA tracer breakthrough curves provided spatially explicit information that allowed inferring a theoretical model of the flow pathways that water takes through the glacier.

Abstract

Many commonly used atmospheric dispersion models are limited to continuous or instantaneous releases only, and cannot accurately simulate time-varying releases. The current paper discusses a new enhanced dispersion formulation accounting for time-varying effects resulting from a pressure drop in a vessel or pipe, and presuming no rainout. This new formulation is implemented in the Unified Dispersion Model (UDM), and is planned to be included in a future version of Phast. First existing methods are summarised for modelling finite-duration and time-varying releases, and limitations of these methods are identified. Secondly the new mathematical model is summarised. The new formulation presumes a number of ‘observers’ to be released at successive times from the point of discharge. The UDM carries out pseudo steady-state calculations for each observer, where the release data correspond to the time at which the observer is released. Subsequently the model applies a correction to the observer concentrations to ensure mass conservation when observers move with different velocities. Finally effects of along-wind diffusion (due to ambient turbulence) are included by means of Gaussian integration over the downwind distance. This results in reduced concentrations while the cloud travels in the downwind direction. The benefits of the new UDM methodology are illustrated for the case of a H2S toxic release from a long pipeline representative of some extremely sour fields in the Middle East that are now being developed. Using corrected observer concentrations and along-wind diffusion significantly reduces toxic effect distances when compared to the current Phast 7.1 approach.
