1. Asthma control with ICS-formoterol reliever versus maintenance ICS and SABA reliever therapy: a post hoc analysis of two randomised controlled trials

Abstract

Background In randomised controlled trials, as-needed inhaled corticosteroid (ICS)-formoterol reliever therapy reduces severe exacerbation risk compared with maintenance ICS plus short-acting beta2-agonist (SABA) reliever in adolescent and adult asthma, but results in slightly worse control of asthma symptoms, as measured by mean Asthma Control Questionnaire-5 (ACQ-5) score.Objective To assess the levels and changes in asthma control for as-needed budesonide–formoterol versus maintenance budesonide plus SABA in post hoc analyses from the Novel START and PRACTICAL clinical trials.Methods The number and proportion of participants at study end in each ACQ-5 category (‘well-controlled’, ‘partly controlled’ or ‘inadequately controlled’ symptoms), and in each responder category based on the minimal clinically important difference for ACQ-5 of 0.5 (improved, no change and worse) with as-needed budesonide–formoterol and maintenance budesonide plus SABA treatment were calculated.Results With last observation carried forwards, 189/214 (88.3%) and 354/434 (81.6%) of patients in the budesonide–formoterol group had ‘well-controlled’ or ‘partly controlled’ symptoms at the end of the study, vs 183/214 (85.5%) and 358/431 (83.1%) in the budesonide maintenance group, for Novel START and PRACTICAL, respectively. The proportion of patients whose symptom control was either improved or unchanged from baseline was 190/214 (88.8%) and 368/434 (84.8%) for budesonide–formoterol, vs 185/214 (86.4%) and 376/431 (87.2%) for maintenance budesonide, in Novel START and PRACTICAL respectively.Conclusions There were no clinically important differences in the proportions of patients with ‘well-controlled’ or ‘partly controlled’ asthma symptoms, or proportions who improved or maintained their level of control, with as-needed budesonide–formoterol versus maintenance budesonide plus SABA.

Abstract

Background Asthma is the most common chronic disease in children, many of whom are managed solely with a short-acting β2-agonist (SABA). In adults, the evidence that budesonide-formoterol as sole reliever therapy markedly reduces the risk of severe exacerbations compared with SABA alone has contributed to the Global Initiative for Asthma recommending against SABA monotherapy in this population. The current lack of evidence in children means it is unknown whether these findings are also relevant to this demographic. High-quality randomised controlled trials (RCTs) are needed. Objective The aim of this study is to determine the efficacy and safety of as-needed budesonide-formoterol therapy compared with as-needed salbutamol in children aged 5 to 15 years with mild asthma, who only use a SABA. Methods A 52-week, open-label, parallel group, phase III RCT will recruit 380 children aged 5 to 15 years with mild asthma. Participants will be randomised 1:1 to either budesonide-formoterol (Symbicort Rapihaler®) 50/3 µg, two actuations as needed, or salbutamol (Ventolin®) 100 µg, two actuations as needed. The primary outcome is asthma attacks as rate per participant per year. Secondary outcomes assess asthma control, lung function, exhaled nitric oxide and treatment step change. A cost-effectiveness analysis is also planned. Conclusion This is the first RCT to assess the safety and efficacy of as-needed budesonide-formoterol in children with mild asthma. The results will provide a much-needed evidence base for the treatment of mild asthma in children.

3. Successful Treatment of Actinic Keratosis with Kanuka Honey

Abstract

Actinic keratoses form as rough, scaly plaques on sun-exposed areas; they can be an important step in premalignant progression to squamous cell cancer of the skin. Currently, pharmacological treatments consist of topical immunomodulatory agents with poor side effect profiles. Use of honey has been common in both ancient and modern medicine, where it is now a key therapy in the management of wound healing. In vitro studies show the New Zealand native Kanuka honey to have immunomodulatory and antimitotic effects, with recent evidence suggesting efficacy of topical application in a variety of dermatological contexts, including rosacea and psoriasis. Here, we present a case report of a 66-year-old gentleman with an actinic keratosis on his hand, which had been present for years. Regular application of Kanuka honey over three months resulted in remission immediately following the treatment period with no signs of recurrence at nine months.

Abstract

Summary Background In adults with mild asthma, a combination of an inhaled corticosteroid with a fast-onset long-acting β-agonist (LABA) used as reliever monotherapy reduces severe exacerbations compared with short-acting β-agonist (SABA) reliever therapy. We investigated the efficacy of combination budesonide–formoterol reliever therapy compared with maintenance budesonide plus as-needed terbutaline. Methods We did a 52-week, open-label, parallel-group, multicentre, superiority, randomised controlled trial at 15 primary care or hospital-based clinical trials units and primary care practices in New Zealand. Participants were adults aged 18–75 years with a self-reported doctor's diagnosis of asthma who were using SABA for symptom relief with or without maintenance low to moderate doses of inhaled corticosteroids in the previous 12 weeks. We randomly assigned participants (1:1) to either reliever therapy with budesonide 200 μg–formoterol 6 μg Turbuhaler (one inhalation as needed for relief of symptoms) or maintenance budesonide 200 μg Turbuhaler (one inhalation twice daily) plus terbutaline 250 μg Turbuhaler (two inhalations as needed). Participants and investigators were not masked to group assignment; the statistician was masked for analysis of the primary outcome. Six study visits were scheduled: randomisation, and weeks 4, 16, 28, 40, and 52. The primary outcome was the number of severe exacerbations per patient per year analysed by intention to treat (severe exacerbations defined as use of systemic corticosteroids for at least 3 days because of asthma, or admission to hospital or an emergency department visit because of asthma requiring systemic corticosteroids). Safety analyses included all participants who had received at least one dose of study treatment. This trial is registered with the Australian New Zealand Clinical Trials Registry, number ACTRN12616000377437. Findings Between May 4, 2016, and Dec 22, 2017, we assigned 890 participants to treatment and included 885 eligible participants in the analysis: 437 assigned to budesonide–formoterol as needed and 448 to budesonide maintenance plus terbutaline as needed. Severe exacerbations per patient per year were lower with as-needed budesonide–formoterol than with maintenance budesonide plus terbutaline as needed (absolute rate per patient per year 0·119 vs 0·172; relative rate 0·69, 95% CI 0·48–1·00; p=0·049). Nasopharyngitis was the most common adverse event in both groups, occurring in 154 (35%) of 440 patients receiving as-needed budesonide–formoterol and 144 (32%) of 448 receiving maintenance budesonide plus terbutaline as needed. Interpretation In adults with mild to moderate asthma, budesonide–formoterol used as needed for symptom relief was more effective at preventing severe exacerbations than maintenance low-dose budesonide plus as-needed terbutaline. The findings support the 2019 Global Initiative for Asthma recommendation that inhaled corticosteroid–formoterol reliever therapy is an alternative regimen to daily low-dose inhaled corticosteroid for patients with mild asthma. 6. Simultaneous transcription of machine shorthand for the deaf

Abstract

People with severe or profound hearing loss are unable to make use of hearing aids and must therefore rely upon visual methods of communication. A review of communication techniques and aids for the Deaf is given which shows that there is often a need for a simultaneous visual transcription of speech for the Deaf to read. This thesis describes the development of such a trancription system based upon Palantype machine shorthand, a shorthand system used for verbatim reporting in the U.K. The Palantype transcription system described in this thesis was developed in two phases. In the first, an investigation was made into transliteration techniques for converting the pseudo-phonetic Palantype shorthand codes into orthographic English. Two prototype systems were built and subjected to lengthy practical trials with highly-motivated post-lingually deaf users. Though very successful, these trials showed that a higher quality transcription was required for most deaf users. In the second phase, an investigation of a limited dictionary of common English words to supplement the transliterations was made. This showed that a dictionary of about 1500 words could provide a transcript of approximately 80% orthographic English, and reconstitute over 90% of word boundaries. Experiments were carried out to compare the readability of this Palantype transcription with English orthography, and to estimate the effect on readability of operator keying errors and the limited dictionary. A microprocessor implementation of this system was built, and several systems are now in regular use. A Palantype switch keyboard, which replaced the original Palantype machine in this application, was also designed. A series of comparative tests showed that this keyboard substantially reduced operator errors. Finally, some consideration has been given to further development of the system to provide automated transcription in commercial applications such as verbatim reporting.

Abstract

Symptom-driven low-dose inhaled corticosteroid–formoterol is safe and effective in mild asthma and has been recommended as one of the preferred treatment regimens at steps 1 and 2 in the 2019 update of the Global Initiative for Asthma. However, there are no data on patient preferences for this regimen.A subgroup of participants in the PRACTICAL study (ACTRN12616000377437), a randomised controlled trial comparing symptom-driven budesonide–formoterol with maintenance budesonide plus as-needed terbutaline completed a survey on treatment preferences, satisfaction, beliefs and experience at their final study visit.306 (75%) out of 407 eligible participants completed the survey. Regimen preference was strongly associated with randomised treatment, as were preferences for and beliefs about preventer inhaler use. Combination preventer and reliever as-needed therapy was preferred by 135 (90%, 95% CI 85.2–94.8%) out of 150 who were randomised to as-needed budesonide–formoterol, and by 63 (40%, 95% CI 32.7–48.1%) out of 156 who were randomised to maintenance budesonide. By contrast, twice-daily preventer inhaler with a reliever inhaler as required was preferred by 15 (10%) out of 150 of those randomised to as-needed budesonide–formoterol and 93 (60%) out of 156 of those randomised to maintenance budesonide. Satisfaction with all study inhalers was high. Of patients randomised to as-needed budesonide–formoterol 92% (n=138) were confident using it as a reliever at the end of the study.Although most participants preferred the regimen to which they had been randomised, this association was much stronger for those randomised to budesonide–formoterol as needed, indicating that most patients preferred as-needed corticosteroid–formoterol therapy if they had experienced it.

Background: In adults with mild asthma, combination inhaled corticosteroid (ICS)/fast-onset long acting beta-agonist (LABA) used solely as reliever therapy is superior to short-acting beta agonist (SABA) reliever therapy. However, there is uncertainty regarding its comparative efficacy with maintenance low dose inhaled glucocorticoid plus as-needed SABA. Methods: A 52 week, open-label, parallel group, multicentre, phase III randomised controlled trial of 890 adults aged 18 to 75 with mild asthma who were using SABA for symptom relief, with or without maintenance ICS. Participants were randomised to either (i) budesonide-formoterol Turbuhaler 200/6μg, one inhalation as required for relief of symptoms or (ii) budesonide Turbuhaler 200μg twice daily, plus terbutaline Turbuhaler 250μg, two inhalations as required. The primary outcome was rate of severe exacerbations defined as hospital/emergency department systemic glucocorticoid treatment or at least 3 days of systemic glucocorticoids for asthma. Results: The analysis included 885 of 890 randomised patients; 70% were using ICS at entry. The annualised rate of severe exacerbations was lower in the as-needed budesonide-formoterol group than in the maintenance budesonide group (absolute rate 0.119 vs 0.172; relative rate, 0.69 [95% confidence interval [CI], 0.48 to 1.00]; p=0.049). The total severe and non-severe exacerbation rate was also lower in the as-needed budesonide-formoterol group (0.165 vs 0.237; relative rate, 0.70 [95% CI 0.51 to 0.95]; p=0.024). Conclusion: In adults with mild asthma, budesonide-formoterol used as required for relief of symptoms is more effective at preventing severe exacerbations than maintenance low dose budesonide plus as-needed terbutaline.

Abstract

Summary Background Whether blood eosinophil counts and exhaled nitric oxide (FeNO) are associated with important outcomes in mild asthma is unclear. In this prespecified subgroup analysis of a previously published open-label clinical trial, we aimed to assess associations between blood eosinophil counts and FeNO with outcomes and response to asthma treatment. Methods In the previously reported 52-week, open-label, randomised controlled trial, people with mild asthma receiving only β agonist reliever inhalers were enrolled at one of 16 clinical trials units in New Zealand, the UK, Italy, or Australia. Eligible participants were randomly assigned (1:1:1, stratified by country), to receive inhalers to take as-needed salbutamol (two inhalations of 100 μg in a pressurised metered dose inhaler), maintenance budesonide (200 μg twice per day by inhaler) plus as-needed salbutamol (two inhalations of 100 μg), or as-needed budesonide–formoterol (one inhalation of 200 μg budesonide and 6μg formoterol by inhaler). The primary outcome was the annual rates of asthma exacerbations per patient, and in this prespecified subgroup analysis, we assessed whether annual exacerbation rates in each treatment group were significantly different depending on levels of blood eosinophil count, FeNO, or a composite score of both. Analyses were done for patients with available biomarker measurements The study was registered with the Australian New Zealand Clinical Trials Registry, number ACTRN12615000999538. Findings 675 participants were enrolled between March 17, 2016, and Aug 29, 2017, of whom 656 had results for blood eosinophil analysis and 668 had results for FeNO. Of the patients who received as-needed salbutamol, the proportion of patients having a severe exacerbation increased progressively with increasing blood eosinophil count (two [4%] of 49 participants with Interpretation In patients with mild asthma, the effects of as-needed budesonide–formoterol on exacerbations are independent of biomarker profile, whereas the benefits of maintenance inhaled budesonide are greater in patients with high blood eosinophil counts than in patients with low counts. Abstract

Digital, Background: Two glucagon-like peptide-1 (GLP-1) receptor agonists reduced renal outcomes in people with type 2 diabetes at risk for cardiovascular disease. We assessed the long-term effect of the GLP-1 receptor agonist dulaglutide on renal outcomes in an exploratory analysis of the REWIND trial of the effect of dulaglutide on cardiovascular disease. Methods: REWIND was a multicentre, randomised, double-blind, placebo-controlled trial at 371 sites in 24 countries. Men and women aged at least 50 years with type 2 diabetes who had either a previous cardiovascular event or cardiovascular risk factors were randomly assigned (1:1) to either weekly subcutaneous injection of dulaglutide (1·5 mg) or placebo and followed up at least every 6 months for outcomes. Urinary albumin-to-creatinine ratios (UACRs) and estimated glomerular filtration rates (eGFRs) were estimated from urine and serum values measured in local laboratories every 12 months. The primary outcome (first occurrence of the composite endpoint of non-fatal myocardial infarction, non-fatal stroke, or death from cardiovascular causes), secondary outcomes (including a composite microvascular outcome), and safety outcomes of this trial have been reported elsewhere. In this exploratory analysis, we investigate the renal component of the composite microvascular outcome, defined as the first occurrence of new macroalbuminuria (UACR >33·9 mg/mmol), a sustained decline in eGFR of 30% or more from baseline, or chronic renal replacement therapy. Analyses were by intention to treat. This trial is registered with ClinicalTrials.gov, number NCT01394952. Findings: Between Aug 18, 2011, and Aug 14, 2013, 9901 participants were enrolled and randomly assigned to receive dulaglutide (n=4949) or placebo (n=4952). At baseline, 791 (7·9%) had macroalbuminuria and mean eGFR was 76·9 mL/min per 1·73 m2 (SD 22·7). During a median follow-up of 5·4 years (IQR 5·1-5·9) comprising 51 820 person-years, the renal outcome developed in 848 (17·1%) participants at an incidence rate of 3·5 per 100 person-years in the dulaglutide group and in 970 (19·6%) participants at an incidence rate of 4·1 per 100 person-years in the placebo group (hazard ratio [HR] 0·85, 95% CI 0·77-0·93; p=0·0004). Abstract

Digital, Background Three different glucagon-like peptide-1 (GLP-1) receptor agonists reduce cardiovascular outcomes in people with type 2 diabetes at high cardiovascular risk with high glycated haemoglobin A1c (HbA1c) concentrations. We assessed the effect of the GLP-1 receptor agonist dulaglutide on major adverse cardiovascular events when added to the existing antihyperglycaemic regimens of individuals with type 2 diabetes with and without previous cardiovascular disease and a wide range of glycaemic control. Methods This multicentre, randomised, double-blind, placebo-controlled trial was done at 371 sites in 24 countries. Men and women aged at least 50 years with type 2 diabetes who had either a previous cardiovascular event or cardiovascular risk factors were randomly assigned (1:1) to either weekly subcutaneous injection of dulaglutide (1·5 mg) or placebo. Randomisation was done by a computer-generated random code with stratification by site. All investigators and participants were masked to treatment assignment. Participants were followed up at least every 6 months for incident cardiovascular and other serious clinical outcomes. The primary outcome was the first occurrence of the composite endpoint of non-fatal myocardial infarction, non-fatal stroke, or death from cardiovascular causes (including unknown causes), which was assessed in the intention-to-treat population. This study is registered with ClinicalTrials.gov, number NCT01394952. Findings Between Aug 18, 2011, and Aug 14, 2013, 9901 participants (mean age 66·2 years [SD 6·5], median HbA1c 7·2% [IQR 6·6–8·1], 4589 [46·3%] women) were enrolled and randomly assigned to receive dulaglutide (n=4949) or placebo (n=4952). During a median follow-up of 5·4 years (IQR 5·1–5·9), the primary composite outcome occurred in 594 (12·0%) participants at an incidence rate of 2·4 per 100 person-years in the dulaglutide group and in 663 (13·4%) participants at an incidence rate of 2·7 per 100 person-years in the placebo group (hazard ratio [HR] 0·88, 95% CI 0·79–0·99; p=0·026). Abstract

Objectives To investigate the acceptability of topical medical grade kanuka honey for treatment of recurrent Herpes Simplex Labialis (HSL) and the feasibility of an individual crossover design. Design An open-label, single group, crossover randomised controlled trial. Methods Fifteen participants aged 16 years or older with a doctor's diagnosis of recurrent HSL were randomised to receive, according to a crossover design, either 5% acyclovir cream or medical grade kanuka honey for their next two episodes of HSL. Randomised treatments were applied to the lips five times per day from the first symptoms of developing the cold sore, until the lesion resolved. The primary outcome was the lesion healing time, defined as the time from development of first symptoms of the cold sore to return of normal skin. A key secondary outcome was acceptability of the honey treatment on visual analogue scale (VAS) (0 = completely unacceptable to 100 = completely acceptable). Kaplan–Meier survival plots, estimates of median healing times, and Cox Proportional Hazards with a random effect for participants to take into account the crossover design compared time to healing between treatments. Results 5/15 (33%) participants provided no data for either intervention, one provided complete data for only the honey intervention, and one provided data only on acceptability of the honey intervention. The Kaplan–Meier estimates of median healing time were similar for honey and acyclovir, both nine days. The hazard ratio for time to healing was 0.57 (95% CI 0.19 to 1.72), p = 0.32, favouring honey. The acceptability of honey was good, mean (SD) VAS 81.9 (24.2). Conclusions Topical medical grade kanuka honey was highly acceptable to patients for the treatment of HSL. A large trial of kanuka honey in HSL may require an alternative recruitment site such as pharmacies, a parallel group rather than crossover design, and closer ongoing supervision of treatment and documentation of outcomes than used in our study.

Introduction In adult asthma, combination inhaled corticosteroid (ICS)/fast-onset long-acting beta agonist (LABA) used solely as reliever therapy may represent an effective and safe alternative to ICS maintenance and short-acting beta agonist (SABA) reliever therapy. Objective To compare the efficacy and safety of ICS/fast-onset LABA reliever therapy with ICS maintenance and SABA reliever therapy in adults with asthma. Methods and analysis A 52-week, open-label, parallel group, multicentre, phase III randomised controlled trial with 1:1 randomisation to either budesonide/formoterol Turbuhaler 200/6 µg, one actuation as required for symptom relief, or budesonide Turbuhaler 200 µg, one actuation twice daily and terbutaline Turbuhaler 250 µg, two actuations as required for symptom relief. 890 adults aged 18–75 years with asthma for whom maintenance ICS and SABA reliever therapy is indicated by current guidelines will be recruited in New Zealand. The primary outcome variable is the rate of severe exacerbations per patient per year. This study will investigate a novel treatment regimen that might lead to a paradigm shift in asthma management for adults for whom guidelines currently recommend maintenance ICS and SABA reliever therapy. Ethics and dissemination Ethical approval has been granted (15/NTB/178). Study findings will be published according to Iinternational Committee of Medical Journal Editors9 recommendations. Abstract

Background Beta-agonist overuse is associated with adverse outcomes in asthma, however, the relationships between different metrics of salbutamol use and future risk are uncertain. Objective To investigate the relationship between metrics of salbutamol use and adverse outcome. Methods In a 24-week randomized controlled trial of 303 asthma patients at risk of severe exacerbations which compared the efficacy and safety of combination budesonide/formoterol inhaler according to a single inhaler regimen (SMART) with a fixed-dose regimen with salbutamol as reliever (‘Standard’), actual medication use was measured by electronic monitoring (Australian New Zealand Clinical Trials Registry Number ACTRN12610000515099). A nested cohort study explored the relationship between metrics of baseline salbutamol use over 2 weeks and future severe asthma exacerbations, poor asthma control (ACQ-5 ≥ 1.5) or ‘extreme’ salbutamol overuse (> 32 salbutamol actuations/24-h period). Results Higher mean daily salbutamol use (per two actuations/day) [Odds ratio (OR) (95% CI) 1.24 (1.06–1.46)], higher days of salbutamol use (per 2 days in 2 weeks) [OR 1.15 (1.00–1.31)] and higher maximal 24-h use (per two actuations/day) [OR 1.09 (1.02–1.16)] were associated with future severe exacerbations. Higher mean daily salbutamol use was associated with future poor asthma control [OR 1.13 (1.02–1.26)]. Higher mean daily salbutamol use [OR 2.73 (1.84–4.07)], number of days of use [OR 1.46 (1.24–1.71)], and maximal daily use [OR 1.57 (1.31–1.89)] were associated with an increased risk of future extreme salbutamol overuse. Conclusion and Clinical Relevance Electronically recorded frequency of current salbutamol use is a strong predictor of risk of future adverse outcomes in asthma, with average daily use performing the best. These findings provide new information for clinicians considering metrics of salbutamol as predictors of future adverse outcomes in asthma.

Abstract

In the law and society literature, it is widely accepted that national criminal trials often fail to produce an adequate account of the historical context in which mass crimes occur. This article evaluates whether this view is applicable to trials at the International Criminal Tribunal for the former Yugoslavia. Drawing on the responses provided by a survey of former prosecutor and defense team members and expert witnesses, empirical evidence is presented with respect to the motivations for including historical evidence in cases and an overall assessment of the historical discussions at the ICTY. Survey results demonstrate agreement among respondents that international criminal trials tend to include more historical evidence than domestic trials, on the grounds that the crimes are more widespread and systematic and involve a collective dimension that requires broader historical analysis. A majority of respondents thought prosecutors use historical evidence to develop their theory of the case and, in genocide trials, such evidence can be relevant to the mental state of the accused. Prosecution and defense respondents profoundly disagreed on whether the ICTY has produced an accurate and comprehensive account of the armed conflicts of the 1990s in the former Yugoslavia. Prosecution respondents generally held that the ICTY’s historical discussions provided valuable insights into the conflict and enhanced the legitimacy of the Tribunal. Defense respondents were more critical of their own expert witnesses, perceived the ICTY judges as unreceptive to defense experts and maintained that the Tribunal’s legitimacy had been undermined by historical discussions. Our correlation analyses identified a divergence between those with two years or less involvement with the tribunal and those with more than 2 years involvement, suggesting that Tribunal tends to "socialize" participants into a shared view of historical evidence. We conclude with a discussion of the potential implications of the survey for creating a shared historical account and promoting reconciliation in the region.

Abstract

Objective To investigate the efficacy of Honevo, a topical 90% medical-grade kanuka honey, and 10% glycerine (honey product) as a treatment for facial acne. Design Randomised controlled trial with single blind assessment of primary outcome variable. Setting Outpatient primary care from 3 New Zealand localities. Participants Of 136 participants aged between 16 and 40 years with a diagnosis of acne and baseline Investigator's Global Assessment (IGA) for acne score of ≥2.68, participants were randomised to each treatment arm. Interventions All participants applied Protex, a triclocarban-based antibacterial soap twice daily for 12 weeks. Participants randomised to the honey product treatment arm applied this directly after washing off the antibacterial soap, twice daily for 12 weeks. Outcome measures The primary outcome was ≥2 point decrease in IGA score from baseline at 12 weeks. Secondary outcomes included mean lesion counts and changes in subject-rated acne improvement and severity at weeks 4 and 12, and withdrawals for worsening acne. Results 4/53 (7.6%) participants in the honey product group and 1/53 (1.9%) of participants in the control group had a ≥ 2 improvement in IGA score at week 12, compared with baseline, OR (95% CI) for improvement 4.2 (0.5 to 39.3), p=0.17. There were 15 and 14 participants who withdrew from the honey product group and control group, respectively. Conclusions This randomised controlled trial did not find evidence that addition of medical-grade kanuka honey in combination with 10% glycerine to standard antibacterial soap treatment is more effective than the use of antibacterial soap alone in the treatment of acne. Abstract

A 70-kDa membrane protein (MP70) is a component of the lens fiber gap junctions. Its membrane topology and its N-terminal sequence are similar to those of the connexin family of proteins. Some features of MP70 containing fiber gap junctions are, however, distinct from gap junctions in other mammalian tissues: (i) Lens connexons form crystalline arrays only after cleavage of junctional proteins in vitro. These hexagonal arrays have a periodicity of 13.6 rim. which is significantly larger than the 8- 9-nm spacing of liver and heart gap junctions. (ii) Lens fiber gap junctions dissociate in low concentrations of nonionic detergent and this provides an avenue to purify MP70 directly from a membrane mixture. Isolated MP70 in the form of 17 S structures has an appearance consistent with connexon pairs. (iii) The C-terminal half of MP70 is cleaved in situ by a lens endogenous calcium-dependent protease. The processed form MP38 remains in the membrane and is abundant in the central region of the lens. A testable hypothesis for MP70 function is presented.

Abstract

OBJECTIVE: To investigate the efficacy of topical 90% medical-grade kanuka honey and 10% glycerine (Honevo) as a treatment for rosacea. DESIGN: Randomised controlled trial with blinded assessment of primary outcome variable. SETTING: Outpatient primary healthcare population from 5 New Zealand sites. PARTICIPANTS: 138 adults aged ≥16, with a diagnosis of rosacea, and a baseline blinded Investigator Global Assessment of Rosacea Severity Score (IGA-RSS) of ≥2. 69 participants were randomised to each treatment arm. 1 participant was excluded from the Honevo group, and 7 and 15 participants withdrew from the Honevo and control groups, respectively. INTERVENTIONS: Participants were randomly allocated 1:1 to Honevo or control cream (Cetomacrogol), applied twice daily for 8 weeks. MAIN OUTCOME MEASURES: The primary outcome measure was the proportion of participants who had a ≥2 improvement in the 7-point IGA-RSS at week 8 compared to baseline. Secondary outcomes included change in IGA-RSS and subject-rated visual analogue score of change in severity (VAS-CS) on a 100 mm scale (0 mm 'much worse', 100 mm 'much improved') at weeks 2 and 8. RESULTS: 24/68 (34.3%) in the Honevo group and 12/69 (17.4%) in the control group had a ≥2 improvement in IGA-RSS at week 8 compared to baseline (relative risk 2.03; 95% CI 1.11 to 3.72, p=0.020). The change in IGA-RSS for Honevo compared to control at week 2 minus baseline was -1 (Hodges-Lehman estimate, 95% CI -1 to 0, p=0.03), and at week 8 minus baseline was -1 (Hodges-Lehman estimate, 95% CI -1 to 0, p=0.005). Abstract

The updated 2011 Global Initiative for Chronic Obstructive Lung Disease (GOLD) guidelines introduced a new method to categorise chronic obstructive pulmonary disease (COPD) patients according to risk and symptom level, whereas previously categorisation was based solely on lung function impairment.1 Risk is classified as either ‘low’ and ‘high’ based on lung function impairment (FEV1) and exacerbation history. Symptom level can be assessed by using the modified Medical Research Council (mMRC) dyspnoea scale, the COPD Assessment Test (CAT) or the Clinical COPD Questionnaire (CCQ), the latter being added in the 2013 update.2 The guidelines state the recommended first choice pharmacologic therapy, an alternative choice and other possible treatments for each category: A—‘low risk, less symptoms’; B—‘low risk, more symptoms’; C—‘high risk, low symptoms’; or D—‘high risk, more symptoms’. It has been suggested that this new method of COPD patient classification should not be used in primary care because it could lead to inappropriate management recommendations and the categorisation process itself is too complex.3 As there are different methods to assess symptoms, with all three having different constructs and measurement properties,4 patients can fall into different categories according to the method used.5 As a result, clinicians can get different treatment recommendations depending on which method they use to assess symptoms. We performed an audit of COPD patients in primary care to assess whether there were significant differences in GOLD category depending on whether the mMRC or the CAT were used, and also to determine the appropriateness of the medications that these patients were prescribed, according to GOLD recommendations. Four hundred and fifty patients were assessed in three primary care practices in New Zealand, and data were collected on patient demographics, FEV1, exacerbation history, CAT and mMRC scores, and prescribed medication. The prescribed medication level was rated as ‘undertreated’, ‘well treated’, ‘over treated’ or ‘other regime’. To establish each patient’s treatment level, their current medication use was compared with each GOLD treatment recommendation (first and second choice for GOLD stage A, B, C and D, thus resulting in eight ‘yes’ or ‘no’ according to, e.g., ‘second choice treatment for stage B’). ‘Well treated’ patients received pharmacotherapy that is listed for GOLD’s recommendation for first or second choice therapy for their category. Patients who were prescribed medication that corresponded to a lower or higher COPD category were rated ‘undertreated’ or ‘over treated’, respectively. Those who did not receive any medication were rated as ‘undertreated’ and those who were prescribed a combination of medications that did not correspond to any of the GOLD recommendations were rated as ‘other regime’. Cohen’s kappa, a statistical method designed to assess agreement over and above that due to chance between two raters who each classify cases into categories, was used to assess agreement of classifications of GOLD category and treatment levels. A kappa of 1 represents perfect agreement, whereas 0 indicates no agreement. The mean age was 69 years (s.d.±10.1 years), 53% were males, 83% were New Zealand Europeans and 13% were Maori. The mean FEV1 was 56 (±18.5)% of predicted value. A graphical summary of the findings is shown in Figure 1. Figure 1 Global Initiative for Chronic Obstructive Lung Disease (GOLD) category by treatment level by (a) modified Medical Research Council Dyspnoea Scale and (b) COPD Assessment Test (CAT). Classifying patients using mMRC resulted in 30, 16, 16 and 38% in groups A, B, C and D, and using CAT resulted in 17, 29, 8 and 46%, respectively. The agreement between the categorisation using mMRC or CAT according to Cohen’s kappa was 0.62. More than 50% of patients included in this audit were prescribed medications that were not consistent with any first or second choice pharmacologic therapy listed in the GOLD guideline. A key finding was that 20% of patients were undertreated. In particular, when using the CAT, a large proportion in group B were undertreated, implying that a substantial proportion of patients with high levels of symptoms were not prescribed long-acting bronchodilators. In contrast, 68% of CAT-classified GOLD C patients received inhaled corticosteroids, which is not recommended for GOLD C patients. On average, patients in GOLD D used 2.7 (s.d. 1.2) different inhalers compared with 1.5 (s.d. 1.3) in GOLD A. The agreement in treatment level between patients categorised using mMRC or CAT was 0.94. The moderate agreement between using mMRC or CAT is similar to that seen in two other studies that had values of 0.51 (ref. 5) and 0.63 (ref. 6). In those studies and this audit, using the CAT led to a shift towards the ‘more symptoms’ categories, B and D. The likely reason for this is that the mMRC assesses only dyspnoea during exercise, whereas the CAT also assesses other parameters such as cough, phlegm, chest tightness, breathlessness going up hills/stairs, activity limitation, sleep, energy and confidence leaving home. Therefore, the CAT is more likely to reflect effects of COPD on a patient’s life than the mMRC. Also, the CAT might be more sensitive to effects of co-morbidity than the mMRC, e.g., orthopnea caused by heart failure might also affect sleep. In particular, use of the CAT resulted in the recategorisation of a substantial number of patients from group A to B. This results in more patients having long-acting bronchodilators recommended for their treatment, and in this audit, it meant that the use of CAT meant that more patients were classified as undertreated in group B. The risk versus benefit of earlier treatment with long-acting bronchodilators is currently uncertain. This audit and other similar studies are limited in that they have only assessed cross-sectional differences in GOLD categorisation and not changes in categorisation over time. The previous GOLD classification was solely based on FEV1 and as treatment did not greatly affect FEV1, there was only limited movement between categories and therefore treatment recommendations. However, data from primary care suggest that many people change categories over time,7,8 which could lead to step down of treatments. More studies are required to assess the effects of stepping down on outcomes. Although COPD is considered a progressive disease, a reduction in treatment could positively affect health by, e.g., reducing troublesome side effects. Also the medication(s) may have been initiated for the first, and perhaps, only exacerbation the patient will experience and therefore may not be required. The decision as to which questionnaire to use in clinical practice and how this would affect outcomes could not be answered based on this study. To answer this question, a prospective longitudinal study in which treatments are administered strictly according to the categorisation based on the two (and now three) questionnaires recommended by GOLD and assessing important clinical outcomes would be needed. However, based on feasibility and applicability for primary care, the International Primary Care Respiratory Group has previously recommended the use of the CCQ or CAT.9 In summary, this audit found that there was a marked difference in GOLD category depending on the method of symptom assessment and that many patients are undertreated or prescribed a treatment regime not consistent with the GOLD guideline.
