Solvent effects on the microwave-assisted synthesis of germanium nanoparticles are presented. A mixture of oleylamine and 1-dodecene was used as the reaction solvent. Oleylamine serves as a reducing agent in the synthesis while both molecules act as binding ligands. Increased concentrations of 1-dodecene in the solvent mixture were found to increase the size of the formed nanoparticles. Crystallinity was also dependent on the solvent mixture. Amorphous nanoparticles were obtained at lower 1-dodecene concentrations, whereas, at higher concentrations, particles contained crystalline and amorphous domains. 11-Methoxyundec-1-ene was synthesized to replace 1-dodecene in the reaction mixture for nuclear magnetic resonance (NMR) studies. 1H NMR of the reaction products shows that both solvent molecules in the system act as binding ligands on the nanoparticle surface. Introduction: Phase I of the Cancer Research UK Stratified Medicine Programme (SMP1) was designed to roll out molecular pathology testing nationwide at the point of cancer diagnosis, as well as facilitate an infrastructure where surplus cancer tissue could be used for research. It offered a non-trial setting to examine common UK cancer genetics in a real-world context. Methods: A total of 26 sites in England, Wales and Scotland, recruited samples from 7814 patients for genetic examination between 2011 and 2013. Tumour types involved were breast, colorectal, lung, prostate, ovarian cancer and malignant melanoma. Centralised molecular testing of surplus material from resections or biopsies of primary/metastatic tissue was performed, with samples examined for 3-5 genetic alterations deemed to be of key interest in site-specific cancers by the National Cancer Research Institute Clinical Study groups. Results: 10 754 patients (98% of those approached) consented to participate, from which 7814 tumour samples were genetically analysed. In total, 53% had at least one genetic aberration detected. From 1885 patients with lung cancer, KRAS mutation was noted to be highly prevalent in adenocarcinoma (37%). In breast cancer (1873 patients), there was a striking contrast in TP53 mutation incidence between patients with ductal cancer (27.3%) and lobular cancer (3.4%). Vast inter-tumour heterogeneity of colorectal cancer (1550 patients) was observed, including myriad double and triple combinations of genetic aberrations. Significant losses of important clinical information included smoking status in lung cancer and loss of distinction between low-grade and high-grade serous ovarian cancers. Conclusion: Nationwide molecular pathology testing in a non-trial setting is feasible. Acute mesenteric ischemia is a morbid disease process that is most common in elderly patients who often have multiple medical comorbidities. Intervention can progress to costly and futile care. The goal of this study was to develop a tool for practitioners to assess the risk of mortality. Patients treated at our institution over the past decade diagnosed with acute mesenteric ischemia were identified. Patients aged less than 65 years were excluded. Data were collected by retrospective chart review. Univariate analysis was used to identify significant risk factors for death. Decision tree analysis yielded a prognostic tool to assess death risk. Univariate analysis demonstrated that lactate (P ≤ 0.001) and pressor requirement (P ≤ 0.001) were predictive of death. Decision tree analysis showed that 79 per cent of patients with day of surgery (DOS) lactate ≥5.4 died postoperatively. Seventy per cent of patients with DOS lactate5.4 progressed to death if they required pressors and had a creatinine1.18. Only 6.1 per cent patients with a DOS lactate5.4, creatinine1.54, and no pressor requirement progressed to death. Several variables can be used to set expectations for families and help guide decision-making. Our tool was predictive of outcomes in 82 per cent of our study population.

Centers for disease control (CDC) Guidelines for Field Triage are effective when proper implementation by EMS personnel is paired with surgeon willingness to care for trauma victims. We hypothesized that in a state with an immature trauma system, a discrepancy exists between medic and surgeon perception of surgical readiness, coinciding with inconsistent implementation of protocols. Surveys were conducted among medics and general surgeons. Destination protocols, trauma center locations, surgeon readiness, and interest in trauma were assessed. A standard clinical trauma scenario was also used. Surgeon willingness to operate is not affected by working outside of trauma centers or interest in trauma. Medics working far from trauma centers are less confident in local surgeon's willingness to operate and less likely to have destination protocols. Trauma center proximity affects medic perception of surgeon willingness to operate, but mere presence of general surgeons does not. In a trauma scenario, surgeon willingness to operate was related to medic perception but not action. In rural states, most surgeons do not work in trauma centers and most medics do not work near them. Although most responding surgeons indicate willingness to operate, medics are confident of such willingness only half the time. This disparity results in inconsistent use of the CDC guidelines. Although most medics report protocols for destination determination, nearly one-fourth of victims are taken to the geographically closest centers, sometimes with no surgeon at all. Efforts at medic training, enhancing surgeon readiness, and alignment of goals are necessary for the CDC Guidelines to be effective.

The transfer of heat into wet sediments from magmatic intrusions or lava flows is not well constrained from field data. Such field constraints on numerical models of heat transfer could significantly improve our understanding of water–lava interactions. We use experimentally calibrated pollen darkening to measure the temperature profile around a basaltic sill emplaced into wet lakebed sediments. It is well known that, upon heating, initially transparent palynomorphs darken progressively through golden, brown, and black shades before being destroyed; however, this approach to measuring temperature has not been applied to volcanological questions. We collected sediment samples from established Miocene fossil localities at Clarkia, Idaho. Fossils in the sediments include pollen from numerous tree and shrub species. We experimentally calibrated changes in the color of Clarkia sediment pollen and used this calibration to determine sediment temperatures around a Miocene basaltic sill emplaced in the sediments. Results indicated a flat temperature profile above and below the sill, with T > 325 °C within 1 cm of the basalt-sediment contact, near 300 °C at 1–2 cm from the contact, and ~ 250 °C at 1 m from the sill contact. This profile suggests that heat transport in the sediments was hydrothermally rather than conductively controlled. This information will be used to test numerical models of heat transfer in wet sediments on Earth and Mars.

Death determined by neurologic criteria, commonly referred to as "brain death," occurs when function of the entire brain ceases, including the brain stem. Diagnostic criteria for brain death are explicit but controversy exists regarding nuances of the evaluation and potential confounders of the examination. Hospitals and ICU teams should carefully consider which clinicians will perform brain death testing and should use standard processes, including checklists to prevent diagnostic errors. Proper diagnosis is essential because misdiagnosis can be catastrophic. Timely, accurate brain death determination and aggressive physiologic support are cornerstones of both good end-of-life care and successful organ donation.

The wooden plank fence presents a deadly but unrecognized hazard to motorists. We hypothesize that fence plank injury is prevalent and results in significant morbidity and mortality. Databases of the University of Kentucky's Level I Trauma Center and the Fayette County Coroner were retrospectively analyzed over a 12-year period (1995-2006). One hundred and twenty-eight subjects were involved in vehicle contact with wooden plank fences. One hundred and twenty-three subjects were evaluated at the Emergency Department of our trauma center; 35 (27%) had a patient-plank interaction (PPI). Men (30/35) were more frequently involved (86%), and average age was 32.8 years. Thirty-two (91%) were drivers; 14/35 (40%) died from PPI-related injuries. The most common cause of death was blunt head trauma in 13 of these 14 fatally injured subjects (93%). This study provides new data underscoring the frequency, lethality, and economic consequences of this injury mechanism. Further research is needed to quantify the national prevalence of this problem and develop injury-mitigating strategies pertaining to roadway or fence design.

This paper examines the microstructure of import markets and the division of the gains from trade among consumers, importers and exporters. When exporters and importers transact through anonymous markets, double marginalization and business stealing among competing importers lead to lower profits. Trading parties can overcome these inefficiencies by investing in richer contractual arrangements such as bilateral contracts that eliminate double marginalization through fixed fees and joint contracts that internalize business stealing by maximizing joint profits of the exporter and its competing importers. Introducing these contractual choices into a trade model with heterogeneous exporters and importers, we show that trade liberalization increases the incentive to engage in joint contracts, thus raising the profits of exporters and importers at the expense of consumer welfare. We examine the implications of the model for prices, quantities and exporter-importer matches in Colombian import markets before and after the US-Colombia free trade agreement. US exporters that started to enjoy duty-free access were more likely to increase their average import price, decrease their quantity exported and reduce the number of import partners.

The red blood cell (RBC) transfusion trigger is a major driver of transfusion practice and affects health care costs and in some instances patient outcomes. Reducing the transfusion threshold will decrease RBC utilization and hospital costs.The hospital transfusion committee, endorsed by the medical staff executive committee, developed an educational program for physicians, nurses, and blood bank staff focusing on the scientific basis for a transfusion trigger of hemoglobin (Hb) of 7 g/dL rather than 8 g/dL as well as a program to discourage the routine 2-unit RBC transfusion. RBC transfusion practice review was performed and those physicians transfusing outside of the new variables were questioned as to the necessity for the transfusion.A total of 4492 RBC units were saved and 662 patients were not transfused over the three fiscal years (FYs), 2010, 2011, and 2012, compared to 2009 baseline. Direct cost savings over 3 years with a transfusion trigger of Hb of 7 g/dL was $943,320. If activity-based costing is used, the savings may have reached as high as $5,314,036. The number of single-unit RBC transfusions increased steadily over the course of the study while the number of 2-unit transfusions remained relatively stable over the three FYs 2010 to 2012.A Hb level of 7 g/dL is the transfusion threshold which is being adopted by many hospitals. Institutional culture change to a Hb level of 7 g/dL can be implemented with the right champion when endorsed by upper echelon medical leadership and hospital administration.

Mice that overexpress the human mutant amyloid precursor protein (hAPP) show learning deficits, but the apparent lack of a relationship between these deficits and the progressive beta-amyloid plaque formation that the hAPP mice display is puzzling. In the water maze, hAPP mice are impaired before and after amyloid plaque deposition. Here we show, using a new water-maze training protocol, that PDAPP mice also exhibit a separate age-related deficit in learning a series of spatial locations. This impairment correlates with beta-amyloid plaque burden and is shown in both cross-sectional and longitudinal experimental designs. Cued navigation and object-recognition memory are normal. These findings indicate that A beta overexpression and/or A beta plaques are associated with disturbed cognitive function and, importantly, suggest that some but not all forms of learning and memory are suitable behavioural assays of the progressive cognitive deficits associated with Alzheimer's-disease-type pathologies.

Red blood cells (RBCs) suppress T-cell responsiveness through a mechanism requiring cell-cell contact. Questions remain as to whether this effect is an allogeneic response, related to cell death, or dependent on particular components of the RBCs.Peripheral T cells were isolated from healthy donors and exposed to stored allogeneic RBCs or autologous RBCs after processing. RBCs were lysed by hypotonic solvent to produce cellular ghosts. Tritiated thymidine proliferation assays were utilized. Cultures were saturated with interleukin (IL)-2 to determine whether impaired IL-2 synthesis played a role.T-cell proliferation was suppressed by both autologous and allogeneic RBCs. RBC membrane integrity does enhance T-cell suppression. T-cell death is not responsible for the suppressive changes. IL-2 synthesis is suppressed in RBC-exposed T cells but addition of exogenous IL-2 does not rescue proliferative capabilities. Proliferation of T cells was inhibited with RBC exposure but mitigated with the addition of fresh RBCs.T-cell suppression is enhanced by intact RBCs but this effect is unrelated solely to alloantigens. Neither apoptosis nor necrosis of T cells contributes to this phenomenon. IL-2 synthesis is suppressed after RBC exposure as a consequence of T-cell inhibition, but is not the primary cause of suppression. Fresh RBCs do not mediate T-cell suppression, indicating that changes in the RBC and development of the storage lesion may occur during initial blood bank processing.

Showtime’s Dexter (2006-) is a television series about a serial killer named Dexter Morgan that has become a cult-hit in the landscape of popular North American television. The series’ narrative follows Dexter’s life as he struggles to manage his identity as a killer with his growing desire to evolve as a human being capable of experiencing love. In this thesis, I will offer an interpretation of Dexter that varies from most of what has been written on the series by focusing on the theme of love and its role in shaping Dexter’s developing sense of self. To consider the influence of love in the evolution of Dexter Morgan I will evaluate the significance of the figures of the Mother, the Father, and the Other throughout his life. Thus, I will be examining Dexter to explore the question: What is the significance of love in relation to the self? Dexter indicates that love is essential to the evolution of self. However, it also reveals the complex and contradictory nature of love as an experience that people rely on for personal fulfillment. The series has shown that Dexter’s efforts in love are often ill-fated. Yet, in spite of this he continues to be compelled to experience love. Evaluating the influence of Eros (the part of the psyche associated with love) on the self, it can be argued that love shapes the self on a fundamental level. As an element of the psyche, Eros is susceptible to a number of forces that structure it. So if Eros is defined based on one’s capacity for love, then love is a determining factor in regards to one’s psychological construction. With this in mind, I argue that love can be understood within the framework of psychopathy and, in this respect, a discussion of love is key to understanding how Dexter creatively shifts the typical popular representation of the serial killer as the archetype of a psychopath. My critical analysis in this thesis on the influence of love in the life of Dexter’s serial killer protagonist will illustrate that love is a form of psychopathy necessary for the existence of the self. Contemporary theories relating to subjectivity, selfhood, and love are critical to understanding and evaluating questions relating to the self. As such, a selection of these theories provided by Judith Butler, Adolf Guggenbuhl-Craig, and Jessica Benjamin will be utilized to interpret key scenes and sequences in Dexter that support my claim that the series indicates love is essential to Dexter Morgan’s self-development. Additionally, I will be examining the aesthetic construction of the series, specifically its use of voiceover narration, to interpret how this relates to Dexter’s evolution into a person capable of experiencing love. This thesis will demonstrate that, in significant ways, Dexter’s sense of self comes to exist through his relationships with other people because they allow him to experience love. With this, ironically, Dexter asks us to recognize that love is a fundamental element in regards to the evolution of self by demonstrating its significance in the selfdevelopment of a figure that is not traditionally associated with love: the serial killer. Recent research in international trade emphasizes the importance of firms extensive margins for understanding overall patterns of trade as well as how firms respond to specific events such as trade liberalization. In this paper, we use detailed U.S. trade statistics to provide a broad overview of how the margins of trade contribute to variation in U.S. imports and exports across trading partners, types of trade (i.e., arm’s-length versus related-party) and both short and long time horizons. Among other results, we highlight the differential behavior of related-party and arm’s-length trade in response to the 1997 Asian financial crisis.

This paper provides an integrated view of globally engaged U.S. firms by exploring a newly developed dataset that links U.S. international trade transactions to longitudinal data on U.S. enterprises. These data permit examination of a number of new dimensions of firm activity, including how many products firms trade, how many countries firms trade with, the characteristics of those countries, the concentration of trade across firms, whether firms transact at arms length or with related parties, and whether firms import as well as export. Firms that trade goods play an important role in the U.S., employing more than a third of the U.S. workforce. We find that the most globally engaged U.S. firms, i.e. those that both export to and import from related parties, dominate U.S. trade flows and employment at trading firms. We also find that firms that begin trading between 1993 and 2000 experience especially rapid employment growth and are a major force in overall job creation.

We describe the integration of MLRISC, a “generic” compiler back end, with TIL, a type-directed compiler for Standard ML. The TIL run-time system uses a form of type information to enable partially tag-free garbage collection. We show how we propagate this information through the final phases of the compiler, even though the back end is unaware of the existence of this information. Additionally, we identify the characteristics of MLRISC that enable us to use it with TIL and suggest ways in which it might better support our compiler. Preliminary performance measurements show that we pay a significant cost for using MLRISC, relative to a custom back end.
