1. Reading Comprehension Instruction: Evaluating Our Progress since Durkin's Seminal Study

Abstract

Purpose: Nearly 50 years ago, Durkin (1978-1979) conducted a seminal observation study on reading comprehension teaching in Grades 3 through 6. She reported that teachers rarely taught reading comprehension (less than 1% of instructional time). Since then, we have gained substantial knowledge about teaching reading comprehension. We aimed to comprehensively review direct observation studies of reading instruction to understand the extent to which recommended comprehension practices are implemented in schools. Method: A systematic search of the extant literature identified 66 observation studies of reading comprehension instruction for children in K-12 schools, including a total of 1,784 teachers. We employed a mixed-method systematic review to address the research questions. Results: Integrated analyses revealed that reading comprehension instruction infrequently aligned with research-based practices. Findings revealed that, on average, 23% of instructional time during reading/language arts instruction was dedicated to reading comprehension. Like Durkin's study (1978-1979), the results indicated that teachers spent much of this time engaging in initiation-response-evaluation conversation patterns rather engaging students in extensive discussion of text or teaching knowledge or practices (e.g. text structure, reading comprehension strategies) that support reading comprehension. However, analyses suggested that studies conducted after 2000 reported more research-based reading comprehension practices than earlier studies. Conclusions: These findings suggest a substantial gap persists between the reading comprehension practices identified as research-based and those observed in typical practice. There is a need for renewed efforts to prioritize reading comprehension instruction in policy and practice.

2. Instructional Practices for Secondary Social Studies Teachers: Describing a Curricular Program Designed to Improve Language, Content Knowledge and Literacy Outcomes for Emergent Bilinguals

Abstract

This article describes a curricular program for use in middle school social studies classrooms that supports the development of emergent bilinguals' language, literacy skills, and acquisition of content knowledge. The program leverages four supports found to be impactful for emergent bilinguals: 1) foregrounding content to increase students' readiness to learn language through content instruction; 2) making meaningful and relevant connections to students' cultural and linguistic backgrounds in complex disciplinary texts and activities; 3) engaging students in discussions and collaborations through heterogeneous teams that include students with a variety of linguistic proficiencies; and 4) using formative assessment to increase feedback and instructional responsiveness. The article presents illustrative examples of how these supports were embedded within a unit on ancient Greece and offers guidance on how middle school social studies teachers can incorporate supports in their units.

3. Quantum Simulation of Nonlinear Dynamical Systems Using Repeated Measurement

Abstract

We present a quantum algorithm based on repeated measurement to solve initial-value problems for nonlinear ordinary differential equations (ODEs), which may be generated from partial differential equations in plasma physics. We map a dynamical system to a Hamiltonian form, where the Hamiltonian matrix is a function of dynamical variables. To advance in time, we measure expectation values from the previous time step, and evaluate the Hamiltonian function classically, which introduces stochasticity into the dynamics. We then perform standard quantum Hamiltonian simulation over a short time, using the evaluated constant Hamiltonian matrix. This approach requires evolving an ensemble of quantum states, which are consumed each step to measure required observables. We apply this approach to the classic logistic and Lorenz systems, in both integrable and chaotic regimes. Abstract

Clinical trials are pivotal in the drug discovery process to determine the safety and efficacy of a drug candidate. The high failure rates of these trials are attributed to deficiencies in clinical model development and protocol design. Improvements in the clinical drug design process could therefore yield significant benefits for all stakeholders involved. This paper examines the current challenges faced in clinical trial design and optimization, reviews established classical computational approaches, and introduces quantum algorithms aimed at enhancing these processes. Specifically, the focus is on three critical aspects: clinical trial simulations, site selection, and cohort identification. This study aims to provide a comprehensive framework that leverages quantum computing to innovate and refine the efficiency and effectiveness of clinical trials.

5. Supporting Knowledge and Language Acquisition of Secondary Emergent Bilinguals through Social Studies Instruction

Abstract

This study examined the initial efficacy of World Generation (WorldGen), a Tier I social studies instructional approach for emergent bilingual (EB) students and their native English-speaking (non-EB) peers in Grades 6 and 7. WorldGen builds on prior research on instructional practices that have been associated with improved content knowledge and literacy outcomes for EBs in classes of students with varying English proficiency. Using a within-teacher design, middle grades world history teachers' classes were randomly assigned to WorldGen treatment (17) or comparison conditions (16) for three to four approximately two-week units. The student sample included 42% EBs. Students in the treatment condition (n = 373) scored higher, on average, on world history content (Hedges' g = 0.47) and vocabulary knowledge (Hedges' g = 0.41) than students in the comparison condition (n = 343) but no statistically significant findings were yielded regarding disciplinary literacy skills at the end of WorldGen instruction. Of primary interest, the statistically significant main effects indicated that world history content knowledge and vocabulary learning was similar for both current EB and non-EB students in the treatment condition. The findings provide initial support for the use of the WorldGen instructional practices for improving content acquisition and vocabulary in general education social studies classes with students with a range of English proficiency. Furthermore, teachers perceived the WorldGen instructional practices and materials as providing the information and learning experiences necessary to support students in meeting grade-level expectations.

11. Effects of Social Studies-Embedded Literacy Instruction on the Informational Writing of Adolescent Emergent Bilinguals

Abstract

The literacy instruction that adolescent emergent bilingual (EB) students currently receive is insufficient to improve their writing proficiency (Graham et al., 2021; National Center for Education Statistics, 2012). To promote improvements in EBs' writing, evidence-based practices that are culturally and linguistically responsive must be provided not only in English language arts classes but across content areas at the Tier I level (Baker et al., 2014; Linan-Thompson et al., 2018). The purpose of this dissertation study was to (a) examine the effects of a set of social studies-embedded integrated literacy instructional practices and materials that were modified for EBs (i.e., CSEL PACT) on the total words written and writing quality produced by EBs in Grade 11 as measured by an informational writing prompt, and (b) determine the extent to which pretest vocabulary, social studies content knowledge, and reading comprehension skills predicted EBs' total words written and writing quality. A secondary analysis of data was conducted using a subset of data collected as part of a larger randomized controlled trial. EBs were randomly assigned to a treatment condition (n = 98) in which two units of CSEL PACT were taught consecutively or a business-as-usual condition (n = 59) in which they received their typical U.S. History instruction. Results of mixed effects models in which participants were nested within teachers yielded small, nonsignificant (p > 0.05) effects of CSEL PACT instruction on EBs' total words written (g = 0.05) and writing quality (g = 0.07). Additionally, pretest vocabulary predicted (p < 0.05) total words written and writing quality was predicted by pretest reading comprehension and fixed effects of teachers. The small, nonsignificant effect sizes related to CSEL PACT instruction on adolescent EBs' writing skills correspond to findings in previous research (De La Paz et al., 2017; van Drie et al., 2015). Moreover, the significance of reading comprehension skills as a predictor of writing quality matches prior research and theory (Kim et al., 2022). Further investigation into how the role of vocabulary differs in predicting total words written between EBs and non-EBs is needed. 13. The conserved noncoding RNA ModT coordinates growth and virulence in Clostridioides difficile.

Abstract

Bacterial noncoding RNAs fulfill a variety of cellular functions as catalysts, as scaffolds in protein complexes or as regulators of gene expression. They often exhibit complex tertiary structures that are a key determinant of their biochemical function. Here, we characterize the structured "raiA motif" RNA from Clostridioides difficile, which is conserved in more than 2,500 bacterial species from the phyla Bacillota and Actinomycetota. We show that its transcript abundance and stability in exponentially growing bacteria rivals that of ribosomal RNAs. Deletion of the "raiA motif" RNA is associated with delayed transition into stationary phase, and changes in stationary phase pathways such as spore formation, hence we rename it ModT (modulator of transition phase). Mechanistically, we show that ModT-mediated changes in cellular cyclic di-GMP levels are linked to the pronounced sporulation defect in the modT mutant. Importantly, we show that expression profiles and isoform patterns of ModT are conserved in Clostridium perfringens and Paeniclostridium sordellii, and that these orthologs can functionally complement ModT in C. difficile. Chemical structure probing of ModT in vivo reveals dynamic refolding and provides initial evidence for a potential association of ModT with proteins. In summary, our findings indicate that ModT fulfills a conserved role in regulating growth transitions in bacteria and provide a crucial step towards delineating its molecular mechanism.

14. Longitudinal Predictors of Bilingual Language Proficiency, Decoding, and Oral Reading Fluency on Reading Comprehension in Spanish and in English

Abstract

This study examined the longitudinal prediction of decoding, oral reading fluency (ORF), and bilingual language proficiency (BLP) on student reading comprehension (RC) outcomes in Spanish and in English. Participants were first-grade Latinx students attending bilingual programs. Findings indicated that BLP initial status and gains were significant predictors of Spanish and English RC at the end of second grade. English and Spanish decoding in first grade, in addition to BLP, explained 27 percent of the variance in English reading comprehension. However, only Spanish decoding was a significant predictor of Spanish reading comprehension. Once English and Spanish second grade ORF scores were added to the model, decoding no longer explained any of the variance in RC. BLP, English ORF initial status and gains, and Spanish gains explained 47 percent of the variance in English reading comprehension. BLP and Spanish ORF initial status explained 46 percent of the variance in Spanish RC. [This is the online first version of an article published in "School Psychology Review."]

Abstract

BackgroundSeveral studies have demonstrated an association between tricuspid regurgitation (TR) and organ dysfunction including hepatic and renal insufficiency. Improvement of liver function following transcatheter edge-to-edge repair (T-TEER) has already been linked to reduction of venous congestion due to TR reduction. This study analyzes whether TR-reduction using T-TEER is also associated with improved renal function.Methods and resultsThe TRIC-ULM registry includes 92 selected patients undergoing T-TEER between March 2017 and May 2023. Estimated glomerular filtration rate (eGFR) improvement was evident in 53 patients (57%) at 3-months follow-up (FU) and defined by FU eGFR > baseline eGFR. Median age was 80 [interquartile range 75–83] years, pre- and postinterventional TR grades were 4 [3–5] and 1 [1–2], baseline eGFR was 36 [30–53] ml/min and New Yeark Heart Association (NYHA) IV was evident in 15% of patients. Multiple logistic regression analysis revealed TR vena contracta reduction (Odds ratio (OR) 1.35 [95% CI: 1.12–1.64] per mm, p = 0.002) and reduced preinterventional tricuspid annular plane systolic excursion (TAPSE) [OR 0.89 (95% CI: 0.79–0.99) per mm, p = 0.033] to independently predict renal improvement at FU. An eGFR improvement threshold of >9 ml/min was associated with reduced 1-year heart failure hospitalization rates [adjusted hazard ratio 0.22 (95% CI: 0.07–0.62) p = 0.005].ConclusionEffective tricuspid edge-to-edge repair is associated with improved renal function and reduced heart failure hospitalization. In patients without renal improvement at 3-months follow-up, residual tricuspid regurgitation should be reevaluated for reintervention.

Abstract

This systematic review aims to synthesize the literature on the effects of cross-linguistic transfer on Spanish/English bilinguals' reading comprehension skills. The search yielded at least 90 studies in which participants were Spanish/English bilinguals aged birth to Grade 12. Researchers assessed participants' decoding and/or linguistic comprehension skills in one of the target languages (i.e., English and Spanish) and their reading comprehension skills in the other language (e.g., cross-linguistic transfer). Included is an analysis of the research methodologies implemented in the identified studies and the assessments used to measure bilinguals' language and literacy development. Preliminary meta-analytic findings comparing studies that implemented methods that allow for causal inferences to those that provided only correlations will be included.

23. The Impact on Inpatient Stays, Crisis and Emergency Department Assessments In Patients With Emotionally Unstable Personality Disorder Who Complete an 18-Month Mentalization-Based Therapy Programme in a Tertiary Personality Disorder Service in Northern Ireland

Abstract

Aims The Personality Disorder Service in the Northern Health & Social Care Trust was originally set up to deliver evidence-based treatment for people with the diagnosis of personality disorder. This group of people historically have been stigmatised, excluded and let down by services, despite their complex needs and frequent history of childhood trauma. The team developed a Mentalization Based Therapy (MBT) programme originally commencing in 2013. To identify recent completers of the MBT 2 18 month programme and to assess whether there was any reduction or change in pattern to the number of days spent as inpatient both during and after having completed the programme, whether there was a reduction in the frequency of same day assessments with community mental health teams or unscheduled care and finally whether there was any reduction in terms of volume of crisis assessments and presentations to Emergency Department. Methods Using validated Quality Improvement Methods, a Plan Do Study Act Cycle was commenced which involved identifying patients who had begun and finished the MBT programme and minimum of 12 months had passed since completion in order to follow-up. We then broke down this data into 3 domains. By using EPEX, Paris and Electronic Care Record computer systems, it was possible to analyse days spent as inpatient, same day assessments and crisis assessments as well as Emergency Department attendance. For these periods of time, they were split into pre-commencement of programme (18 months), during programme (18 months) and post-completion of programme (12 months) to see if there was any tangible decrease in these numbers. 19 service users were identified that had initially been referred to Personality Disorder Service between 2016 and 2018 and who subsequently began MBT2 programme between 2017 and 2019. Given the length of completion of the programme, this allowed us to gather a full set of data with regard to these patients up to completion of programme in 2021. Subsequent period of 12 months was then analysed post-completion of treatment taking us up to 2022. Results The average time spent in inpatient admission days prior to starting therapy for 18 months (n = 19) was 21.74 days, this decreased to 6.53 during therapy and 3.68 post-therapy (12 month follow-up) = 5.52 adjusted for 18 months. This represents a reduction of 74.61%. The average number of same day assessments and unscheduled care (n = 8) seeking prior to admission was 1.38. This decreased to 0.75 during therapy and 0.88 post-therapy adjusted to 1.32 for 18 months, which represents a small decline of 4.35%. Finally, the average number of Crisis contacts and Emergency Department assessments were 2.63 in the 18 months before commencing therapy, 1.26 during therapy and 0.58 in the 12 months post-therapy, 0.87 adjusted for 18 months. This represents a reduction of 66.92% Conclusion It is clear from analysis of the data that there has been a substantial decrease in time spent as admitted inpatient as well as number of contacts with Crisis Assessors and Emergency Departments in association with completion of the MBT 18 month programme. This demonstrates that, by using an evidence-based and well-established programme, which carries a high time commitment for both service users and practitioners, it is possible to considerably reduce use of other, more acute services and keep patients with a diagnosis of EUPD out of hospital longer and on a sustained basis and also to reduce presentations to Emergency Departments which was often on the basis of self-harm and/or overdoses. The dual result is that it can be validated objectively that service users are suffering less distress after having completed the programme, which will lead to better quality of life, whilst also reducing the burden on costly inpatient services with the end result being an important investment in mental health services in Northern Ireland and the prototype for the developing regional service.

24. Trends and Issues in Student-Run Peer-Reviewed Journals of Education

Abstract

The purpose of this mixed methods bibliometric study was to identify the trends and issues in student-run peer-reviewed journals of education and to determine predictive factors of journal longevity. To that end, a search for journals was conducted, yielding 11 journals and 1,105 records. Journals were coded for editorial procedures, goals, and guidelines. Records were coded for publication characteristics, author characteristics, and keywords and abstracts. The analysis consisted of calculating descriptive statistics for journals, authors, and affiliations, implementing text mining on titles, keywords, and abstracts, and fitting a logistic regression model to predict journal longevity. Results revealed that student-run journals are somewhat prone to gaps in publication, but that effective editorial practices such as requiring abstracts and publishing issues consistently predict journal longevity. Three themes emerged across journals: a transformative focus, leaning into the margin, and inconsistent guidelines & processes. Recommendations for student-run peer-reviewed journals of education are to (a) form sustainable editorial structures, (b) follow consistent publication patterns, (c) solicit authors purposefully, and (d) continue leaning into the margin.

Abstract

Tubo-ovarian high-grade serous carcinomas (HGSC) are highly proliferative neoplasms that generally respond well to platinum/taxane chemotherapy. We recently identified minichromosome maintenance complex component 3 (MCM3), which is involved in the initiation of DNA replication and proliferation, as a favorable prognostic marker in HGSC. Our objective was to further validate whether MCM3 mRNA expression and possibly MCM3 protein levels are associated with survival in patients with HGSC. MCM3 mRNA expression was measured using NanoString expression profiling on formalin-fixed and paraffin-embedded tissue (N = 2355 HGSC) and MCM3 protein expression was assessed by immunohistochemistry (N = 522 HGSC) and compared with Ki-67. Kaplan-Meier curves and the Cox proportional hazards model were used to estimate associations with survival. Abstract

Tensegrity structures have been extensively studied over the last years due to their potential applications in modern engineering like metamaterials, deployable structures, planetary lander modules, etc. Many of the form-finding methods proposed continue to produce structures with one or more soft/swinging modes. These modes have been vividly highlighted and outlined as the grounds for these structures to be unsuitable as engineering structures. This work proposes a relationship between the number of rods and strings to satisfy the full-rank convexity criterion as a part of the form-finding process. Using the proposed form-finding process for the famous three-rod tensegrity, the work proposes an alternative three-rod ten-string that is stable. The work demonstrates that the stable tensegrities suitable for engineering are feasible and can be designed.

Abstract

In the context of multimodal treatments for abdominal cancer, including procedures such as cytoreductive surgery and intraperitoneal chemotherapy, recurrence rates remain high, and long-term survival benefits are uncertain due to post-operative complications. Notably, treatment-limiting side effects often arise from an uncontrolled activation of the immune system, particularly peritoneally localized macrophages, leading to massive cytokine secretion and phenotype changes. Exploring alternatives, an increasing number of studies investigated the potential of plasma-activated liquids (PAL) for adjuvant peritoneal cancer treatment, aiming to mitigate side effects, preserve healthy tissue, and reduce cytotoxicity towards non-cancer cells. To assess the non-toxicity of PAL, we isolated primary human macrophages from the peritoneum and subjected them to PAL exposure. Employing an extensive methodological spectrum, including flow cytometry, Raman microspectroscopy, and DigiWest protein analysis, we observed a pronounced resistance of macrophages towards PAL. This resistance was characterized by an upregulation of proliferation and anti-oxidative pathways, countering PAL-derived oxidative stress-induced cell death. The observed cellular effects of PAL treatment on human tissue-resident peritoneal macrophages unveil a potential avenue for PAL-derived immunomodulatory effects within the human peritoneal cavity. Our findings contribute to understanding the intricate interplay between PAL and macrophages, shedding light on the promising prospects for PAL in the adjuvant treatment of peritoneal cancer.

Abstract

Objective Deferral of non-emergency cardiac procedures is associated with increased early emergency cardiovascular hospitalisation. This study aimed to identify predictors of worse clinical outcome after deferral of non-emergency cardiovascular interventions.Methods This observational case-control study included consecutive patients whose non-emergency cardiac intervention has been postponed during COVID-19-related lockdown between 19 March and 30 April 2020 (n=193). Cox regression was performed to identify predictors of the combined 1-year end point emergency cardiovascular hospitalisation and death. All patients undergoing non-emergency interventions in the corresponding time period 2019 served as control group (n=216).Results The combined end point of death and emergency cardiovascular hospitalisation occurred in 70 (36.3%) of 193 patients with a postponed cardiovascular intervention. The planned intervention was deferred by a median of 23 (19–36) days. Abstract

Background: This study assessed the potential cost-effectiveness of high (80–100%) vs low (21–35%) fraction of inspired oxygen (FiO2) at preventing surgical site infections (SSIs) after abdominal surgery in Nigeria, India, and South Africa. Methods: Decision-analytic models were constructed using best available evidence sourced from unbundled data of an ongoing pilot trial assessing the effectiveness of high FiO2, published literature, and a cost survey in Nigeria, India, and South Africa. Effectiveness was measured as percentage of SSIs at 30 days after surgery, a healthcare perspective was adopted, and costs were reported in US dollars ($). Results: High FiO2 may be cost-effective (cheaper and effective). In Nigeria, the average cost for high FiO2 was $216 compared with $222 for low FiO2 leading to a −$6 (95% confidence interval [CI]: −$13 to −$1) difference in costs. In India, the average cost for high FiO2 was $184 compared with $195 for low FiO2 leading to a −$11 (95% CI: −$15 to −$6) difference in costs. In South Africa, the average cost for high FiO2 was $1164 compared with $1257 for low FiO2 leading to a −$93 (95% CI: −$132 to −$65) difference in costs. The high FiO2 arm had few SSIs, 7.33% compared with 8.38% for low FiO2, leading to a −1.05 (95% CI: −1.14 to −0.90) percentage point reduction in SSIs. Conclusion: High FiO2 could be cost-effective at preventing SSIs in the three countries but further data from large clinical trials are required to confirm this.

Abstract

ObjectiveCervical cancer represents the fourth leading cause of cancer among women and is associated with over 311,000 annual deaths worldwide. Timely diagnosis is crucial given the lengthy pre-cancerous phase, which is typified by cervical intraepithelial neoplastic lesions. However, current treatment methods are often tissue-destructive and can be accompanied by severe side effects. To address these concerns, our study introduces a novel, gentle approach for the tissue-preserving treatment of CIN lesions.ResultsWe present findings of a controlled, prospective, single-armed phase IIb clinical trial performed at the Department for Women’s Health, Tübingen, Germany. From September 2017 to March 2022 we assessed 570 participants for study eligibility. Of the screened patients, 63 participants met with CIN1/2 lesions met the inclusion criteria and were treated with non-invasive physical plasma (NIPP). Assessment of treatment efficacy was based on a comprehensive analysis of histological and cytological findings, along with high-risk HPV infection load at 3 and 6 months post-treatment. Comparative analyses were performed retrospectively with data obtained from 287 untreated patients in the control group. Our findings indicate that patients treated with NIPP experienced an 86.2% rate of full remission, along with a 3.4% rate of partial remission of CIN lesions, which compares favorably to the control group’s rates of 40.4% and 4.5%, respectively. Additionally, we observed a twofold reduction in high-risk HPV infections following NIPP treatment. Minor side effects were observed, such as mild pain during treatment and short-term smear bleeding or increased vaginal discharge within 24 h after treatment. Given the experimental nature of NIPP treatment and the availability of established standard treatments, our study was designed as a non-randomized study.ConclusionNIPP treatment offers a highly flexible and easy-to-apply method for treating pre-cancerous CIN1/2 lesions. This non-invasive approach is notable for its tissue-preserving nature, making it a promising alternative to current excisional and ablative treatments.
