13. Severe Respiratory Distress in a Child with Pulmonary Idiopathic Hemosiderosis Initially Presenting with Iron-Deficiency Anemia

Abstract

Idiopathic pulmonary hemosiderosis (IPH) is a rare cause of alveolar hemorrhage in children but should be considered in children with anemia of unknown origin who develop respiratory complications. It is commonly characterized by the triad of recurrent hemoptysis, diffuse parenchymal infiltrates, and iron-deficiency anemia. Pathogenesis is unclear and diagnosis may be difficult along with a variable clinical course. A 6-year-old boy was admitted to the hospital with a severe iron-deficiency anemia, but he later developed severe acute respiratory failure and hemoptysis requiring intubation and mechanical ventilation. The suspicion of IPH led to the use of immunosuppressive therapy with high dose of corticosteroids with rapid improvement in clinical condition and discharge from hospital.

14. Sutures vs. adhesives: two fixation methods for free gingival grafts. A six-year follow-up study.

Abstract

The article presents a research which compares the relative effectiveness of sutures and adhesives for fixing free autogenous gingival grafts on the basis of six-year postoperative clinical and biometrical data. 14 patients, as reported, were included in the study and 28 free autogenous gingival grafts were placed buccally around contralateral pairs of their teeth that had 1 mm or less of attached gingiva. According to the research, the pocket probing depth of adhesive grafts increased by 0.25 mm and that of sutured grafts increased by 0.32 mm after six years of surgery. The research concludes that, there was no statistically significant differences between suture and adhesive grafts for pocket probing depth, recession and shrinkage.

Il rapporto di ricerca raccoglie le elaborazioni prodotte ed i documenti di rendicontazione delle attività svolte come richiesto dall’incarico affidato ad Éupolis da parte di Regione Lombardia, DG Semplificazione e Digitalizzazione, DG Protezione Civile, Polizia Locale e Sicurezza e Funzione Specialistica Promozione del Territorio e Montagna. La parte introduttiva descrive il contesto normativo nazionale che ha introdotto l’obbligatorietà della gestione associata tra Comuni per l’esercizio delle funzioni fondamentali con approfondimento comparato sulle Regioni che hanno avviato un percorso normativo specifico. Le singole parti trattano i seguenti argomenti: Relazione di sintesi e di valutazione, relativa al triennio 2009/2011, sugli esiti delle gestioni associate incentivate ai sensi della L.r. 19/2008. I casi pilota: format di intervento, metodologia e strumenti. Abstract

Dendritic cells play a central role in the regulation of innate and adaptive immunity, and exert their sensor functions through a plethora of receptors, including the NLRP3-inflammasome. The role of NLRP3-inflammasome in sensing endogenous danger signals, such as MSU, has been deeply investigated in the last years. However the exact mechanism of MSU activation of DCs through NLPR3 is not completely defined yet. Moreover, little is known about the adjuvant role of MSU as promoter of DC- mediated priming of specific T cell subsets. Starting from these observations, the present work aims to describe evidences of the role of endogenous danger signals in the activation of the immune system. In particular, we investigated which is the role of MSU and its putative receptor NLRP3 in the activation of DCs and how this process is involved in starting both innate and adaptive immunity.

Abstract

Expression of receptors for IgA (FcαRs) was investigated on a panel or 35 human B cell lines by labelling with human secretory IgA (0.5 mg/ml) and flow cytometry analysis after staining with fluoresceinated goat anti-human secretory component and/or anti-α chain F(ab')2 fragments. Receptors for IgA could be demonstrated on one out of nine Burkitt's lymphoma cell lines, three out of five myeloma cell lines and five out of 21 lymphoblastoid cell lines. The percentage of FeαR-positive cells within the same B cell line varied upon repealed examination. Human dimeric IgA 1λ myeloma protein revealed the same number of IgA receptor positive cells as did secretory IgA, whereas monomeric IgA did not bind to FcαR. Detection of FcαR was not inhibited when the tests were carried out in the presence of human dimeric IgG, IgM, asialo-orosomucoid, and secretory component but it was abrogated by pre-treatment of the cells with trypsin. The binding characteristics of FcαRs were studied on the myeloma cell line Esteve, using 125I-labelled human dimeric IgA and secretory IgA. The binding was dose-dependent with rapid kinetics and specific inhibition by unlabelled secretory IgA. Scatchard plot analysis resulted in an equilibrium constant K ranging from 3.2 to 4.7 × 106 M/l. No correlation was observed between FcαR expression and differentiation stage, monoclonality, polyclonality of the cell lines, or Ig class produced by the B cells. [ABSTRACT FROM AUTHOR]

Abstract

Quantitative measurements of cultured human and murine thymic, and human thymoma reticulo-epithelial cells (REC), immunolabelled by different antibodies (Ab) (TE3, TE4, anti-HTLV p19(p19), lu5, K11 and Aks) and by thymic hormones (thymulin and thymosin alpha 1(Ta1)) within these cells, were performed using a flow cytometric technique. The anti-keratin polyclonal Ab labelled nearly the whole human or murine population. The p19 monoclonal Ab (MoAb), specific for the subcortical/medullary thymic regions, labelled 37-77% of the human REC. The TE3 MoAb, specific for the cortical region, labelled 54-83% of the REC. These percentages suggest that the cultured thymic REC (TREC) had markers of both regions together and therefore that these markers are not absolutely specific to determine their subcortical/medullary or cortical thymic origin. For the three populations there were more cells containing Ta1 than thymulin. The overlap of the percentage of labelled cells suggests that the same cell could synthesize the two hormones and that these hormones could be localized within the TE3 positive cells. 48. Porcine Reproductive and Respiratory Syndrome (PRRS) Virus-Specific Interferon-γ+ T-Cell Responses After PRRS Virus Infection or Vaccination with an Inactivated PRRS Vaccine

Abstract

Although field studies have found porcine reproductive and respiratory syndrome (PRRSV) inactivated vaccines to be beneficial in reducing losses linked to PRRSV infection, immune mechanisms induced by these vaccines need better understanding. In the study reported here, we examined the interferon-γ+ (IFNγ+) PRRS-specific T cell responses induced after infection and vaccination with an inactivated PRRS vaccine. Autologous monocyte-derived dendritic cells loaded with the PRRSV P120 strain were used to re-stimulate ex vivo T cells that had been primed in vivo by either the virus or the vaccine, or both. Virus-specific IFNγ+ T cells were quantified by using a porcine IFNγ- ELISpot assay. A specific but low live virus-induced response was observed between days 35 and 70 for most of the pigs tested, while a significant inactivated vaccine-induced PRRSV-specific IFNγ+ T-cell response was measured soon after vaccination. Moreover, we observed that vaccination of pre-challenged pigs clearly favoured the PRRSV-specific cell-mediated immunity primed by the live virus. To characterize further the nature of the PRRSV-specific T cells, the different T-cell subsets involved in PRRSV immunity were analyzed by flow cytometry. We showed that the inactivated vaccine was able to prime both CD4+CD8int+ and CD8high virus-specific T cells and that CD4+CD8int+ were preferentially recalled by the live virus.
