Background: Although surgical treatments for breast cancer have exhibited advanced interventions, axillary lymph node dissection can limit functionality and compromise women’s self-care. This study aims to assess the effectiveness of a rehabilitation nursing program in improving self-care performance in women undergoing breast surgery with axillary lymph node dissection. Methods: This quantitative, quasi-experimental study involved 48 women recruited from a main hospital between 2018 and 2019. The participants completed a three-month rehabilitation program at home. The evaluation instrument used was the DASH questionnaire. This study was not registered. Results: The functionality of the upper limb ipsilateral to the surgery improved significantly (p < 0.001) after the implementation of the program, influencing the participants’ capacity for self-care, including washing/drying their hair, washing their back, and wearing a shirt. The average DASH total score increased from 54.4 to 8.1 after the program. Conclusions: The rehabilitation nursing program positively influenced the participants’ self-care ability. Incorporating rehabilitation nursing programs into breast cancer treatment can improve self-care performance and the overall quality of patients’ lives. This study was not registered.

Identifying adaptive or mitigating solutions for climate change is especially important for high conservation value ecosystems, such as kelp forests. The effects of Marine Heatwaves (MHWs), short-term increased temperature events, on the physiology of such organisms have been greatly overlooked when addressing thermal stress impacts, with a greater attention given to ocean warming. By testing the combined effect of heatwaves and ocean acidification, it has been demonstrated that the extra Carbon source helped organisms on their resilience to the thermal stressor, yet the effect extra light sources could have has never been analysed. In this study the combined effect of MHWs (+3ºC) and Artificial Light at Night (ALAN) (24hlight-0h dark), exposure was tested on the photosynthetic parameters (Fv, Fv/Fm, ETRm and Alfa), growth and respiration (R) rates and C/N tissue ratio of the species Laminaria digitata. A 14-day MHW simulation was performed followed by a 10-day recovery period, in short term mesocosm experiments at the Plymouth Marine Laboratory. A “collapsed factorial design”, was used with “SW Temperature” set as the main driver and “Light availability nested within SW Temperature” as the secondary one. As expected MHW alone caused significant negative impacts on growth and C/N tissue ratios, with organisms presenting the lowest rates, even in the recovery period, while it increased their fluorescence responses, with the highest’s values of Fv and Fv/FM observed only during the exposure period. On the other hand, MHW combined with ALAN treatments showed the highest values of growth and C/N ratios, while presenting the lowest values of Alfa, due to down regulations of light consumption mechanisms. It was concluded that by exposing organisms to ALAN this could help them on their resilience to MHW events, by enhancing their carbon fixation rates, and hence producing the extra amount of energy necessary to withstand the thermal stressor. No atual contexto das mudanças ambientais observadas a nível global, compreender o seu impacto para os ecossistemas tornou-se o principal foco da comunidade científica. Identificar soluções que levem à adaptação e mitigação dos efeitos causados pelas alterações climáticas é de especial importância tendo em conta ecossistemas de alto valor de conservação. Dentro deste grupo, algas marinhas, onde florestas de “Kelps” estão incluídas, desempenham um importante papel, sendo dos organismos marinhos fotossintéticos com maiores taxas de produção primária, atuando como “engenheiros ecossistémicos” e importantes sequestradores de carbono azul e servindo de abrigo para uma enorme diversidade de organismos marinhos, em variadas zonas costeiras. No entanto, tendo em conta a dependência direta de condições de temperatura baixas a moderadas observada para estes organismos, a maior parte dos estudos realizados neste contexto tendem a dar uma maior ênfase aos efeitos causados pelo aumento de temperatura a nível global, o aquecimento progressivo dos oceanos. Deixando assim de lado eventos locais e temporários de aquecimento das águas denominados “Ondas de Calor Marinhas” (no inglês “Marine Heatwaves”), que acabam por receber menor atenção por parte dos investigadores. Ondas de Calor Marinhas constituem eventos locais de águas anomalamente quentes, podendo resultar de processos de escalas temporais e espaciais variadas. Os seus impactos já foram demonstrados para uma série de organismos marinhos, incluindo florestas de kelps, no entanto, maioritariamente de um ponto de vista ecológico, com abordagens a nível dos efeitos fisiológicos causados nos indivíduos ainda bastante limitadas. Com o aumento da frequência de ocorrência e intensidade das ondas de calor observadas atualmente e previstas no futuro, torna-se imprescindível compreender melhor a dinâmica destes efeitos nos organismos e a respetiva capacidade de resiliência. Embora limitados, alguns estudos já realizaram tal abordagem, simulando condições de ondas de calor em ambientes controlados, observando o seu efeito isolado e, em alguns casos, combinado com outras pressões ambientais. Em estudos simulando a exposição a ondas de calor e níveis elevados de CO2-acidificação dos oceanos- por exemplo, foi observado que ao invés de atuar como uma fonte adicional de stress fisiológico, as condições de elevada concentração de CO2 aumentaram a resiliência dos organismos perante o stress térmico. No entanto nenhuma abordagem foi feita até agora explorando o mesmo contexto em relação a condições de elevada disponibilidade de luz. Desempenhando um papel igualmente importante ao do carbono inorgânico na fotossíntese, a intensidade e disponibilidade da luz, influenciam diretamente toda a atividade fotossintética dos organismos e, consequentemente, a sua sobrevivência. Embora a luz solar seja a fonte principal de luz usada por estes organismos, a luz artificial é também uma fonte opcional, e, neste contexto, o uso de Luz Artificial Noturna (em inglês “Artificial Light at Night”) ganha particular atenção. Com uma crescente atenção por parte da comunidade científica, o estudo da Luz Artificial Noturna tem ganho particular ênfase tendo em conta os seus efeitos em animais marinhos, negligenciando em parte os seus potenciais efeitos em organismos marinhos fotossintéticos, como nas macroalgas. Neste estudo o efeito combinado de Ondas de Calor (+3ºC) e Luz Artificial Noturna (24h claro-0h escuro), foi testado nos parâmetros fotossintéticos (Fv, Fv/Fm, ETRm e Alfa), taxas de crescimento e respiração (R) e razão percentual de C/N (Carbono por Azoto) nos tecidos da espécie Laminaria digitata. Foram simulados 14 dias de Ondas de Calor, seguidas por um período de recuperação de 10 dias, em experiências de curto prazo em mesocosmo no Plymouth Marine Laboratory (“PML”, Reino Unido). A experiência foi realizada no Inverno de 2022 e o desenho experimental foi definido como um “collapsed factorial design”, assim como sugerido por investigadores para diminuir o número de tratamentos e aumentar a possibilidade de replicados em estudos de múltiplos fatores. Foi definida a “Temperatura da água” como o fator principal e a “Disponibilidade de Luz aninhada dentro da Temperatura da água” como fator o secundário, tendo-se assim implementado dois tratamentos distintos: organismos expostos à onda de calor isolada e organismos expostos à onda de calor em combinação com a luz artificial noturna, além dos controlos expostos às condições ambientais de temperatura e luz. Como esperado, a exposição à Onda de Calor isolada causou impactos negativos significativos no crescimento e na relação tecidual de C/N dos organismos. Nesses tratamentos, valores mais baixos foram observados para estes parâmetros, estendendo-se ainda durante período de recuperação. O fator também causou o aumento da fluorescência dos organismos, com maiores valores de Fv e Fv/FM observados, embora apenas durante o período de exposição ao evento. Por outro lado, os tratamentos de Onda de Calor combinada com Luz Artificial Noturna, apresentaram os maiores valores de crescimento e razão C/N, enquanto observando-se os menores valores de Alfa, devido à regulação negativa dos mecanismos de consumo de luz. Para a maior parte dos parâmetros estudados foi observado um efeito apenas do fator “Disponibilidade de Luz aninhada dentro da Temperatura da água”, com a exceção do ETRm, onde não se observou efeito significativo de nenhum fator e de Fv/Fm, onde efeitos da “Temperatura da água” foram também observados. Concluiu-se então que, ao expor organismos de Kelp à Luz Artificial Noturna, a fonte adicional de energia luminosa pode contribuir para a sua maior resiliência a eventos de Ondas de Calor, aumentando a sua atividade fotossintética e fixação de carbono, e, portanto, produzindo a quantidade de energia necessária para suportar a pressão térmica. Esta descoberta é de extrema importância tendo em conta todos os serviços ecológicos e económicos prestados por florestas de Kelp, e outras algas marinhas. Ao identificar formas de aumentar a resiliência destes organismos às diferentes formas de pressão ambiental causados pelas alterações climáticas, contribui-se para o conhecimento de possíveis formas para melhorar a sua gestão, conservação e restauração. Em sequência, diminui-se também o impacto causado à diversidade de comunidades que estes ecossistemas abrigam e a toda sua teia trófica. Tal abordagem é também de grande interesse para indústrias de produção de florestas de Kelp. Particularmente para grandes produtores como a China, onde grandes números de eventos de ondas de calor são registados atualmente e previstos no futuro com o avanço das alterações climáticas, formas de mitigar os seus efeitos torna-se imprescindível. Exergames, which combine digital games and physical exercise, have become a popular alternative to traditional exercise programs and are increasingly used in the health domains. This study aimed to provide a systematic review to synthesize existing studies on the use of exergames for the mental wellbeing of healthy older adults, aggregating the collected data to identify effect size. We performed a Systematic Review and Grading of Recommendations, Assessment, Development, and Assessment (GRADE) evidence synthesis. We performed the search in the following databases: MEDLINE®, CINAHL®, SPORTDiscus, SCOPUS, SciELO, Psychology and Behavioral Sciences Collection, and Cochrane® Central Register of Controlled Trials from the first record until October 2021. Randomized controlled trials using exergames as an intervention were included. A total of 10 articles published between 2009 and 2021 were selected. Most studies used the Nintendo Wii® as the exergame in the intervention program. Interventions ranged from 6 to 12 weeks, with 8-week programs being most frequent. It was possible to observe a positive effect on mood, the reduction of apathy, anxiety and depression, self-esteem, and affection. info:eu-repo/semantics/publishedVersion

Objective: We aimed to systematically review and synthesise the impact of rehabilitation with games in people after knee arthroplasty. Methods: We conducted a systematic review following the Preferred Reporting Items for the declaration of Systematic Reviews (PRISMA – Preferred Reporting Items for Systematic Reviews and Meta- Analysis). The summary of evidence was developed using the Grading of Recommendations, Assessment, Development, and Assessment (GRADE). The review included randomised controlled trials that used characteristics of games in rehabilitation. Results: Eight articles from a total of 1289 identified articles were included after duplicates were removed. In total, 239 participants participated. There were no statistically significant changes between the groups using the exergames and control groups. The level of evidence was rated using GRADE and was very low or moderate. The difference in grouped means was not significant for Knee Flexion, Knee extension, Range of Motion, WOMAC (Western Ontario and McMaster Universities Arthritis Index), AKSS (American Knee Society Score), Self-Efficacy, Five Times Sit-to-Stand Test time, Pain, or Proprioception. Conclusions: The results of the different studies did not find significant changes in the intervention groups with exergames in the physical domains, especially in studies with shorter interventions. Therefore, further investment in future studies on developing and evaluating games is suggested to enhance training during the recovery process. info:eu-repo/semantics/publishedVersion

Palliative rehabilitation is defined as the process of helping a person with a progressive, commonly advanced, and/or incurable disease reach their physical, psychological, and social potential consistent with physiological and environmental limitations and life preferences. However, the evidence on this subject is dispersed in the literature.To examine and map interventions of palliative rehabilitation, implemented and evaluated in palliative care.A scoping review, using the Joanna Briggs Institute's guideline, was conducted. Multiple databases were searched: CINAHL Complete; PubMed; Scopus; SciELO; Cochrane Central Register of Controlled Trials; PEDro, as well as grey literature for studies that focus on qualified healthcare professionals caring for patients 18 years of age or older, working in palliative care, that focus on the concepts of palliative rehabilitation interventions.Of the 314 studies retrieved, two were included in this review. Both were conducted with physiotherapists, and none mentioned nursing rehabilitation. One of the studies implemented and evaluated an intervention of exclusively physical domain and another of physical and emotional domain. The interventions still differ in the number of treatments which ranged from 4 to 7 sessions. Both studies were implemented in oncological and non-oncological patients.Further research is needed to explore the rehabilitation strategies used by healthcare professionals working in palliative care that help patients. Moreover, since nurses are often the healthcare professionals who are in closest proximity to, and who spend the most time with, the patient, which rehabilitation interventions do these professionals should be focus of intervention.La riabilitazione palliativa è definita come il processo di aiutare una persona con una malattia progressiva, comunemente avanzata e / o incurabile a raggiungere il proprio potenziale fisico, psicologico e sociale coerente con i limiti fisiologici e ambientali e le preferenze di vita. Tuttavia, le prove su questo argomento sono disperse in letteratura OBIETTIVO: Esaminare e mappare gli interventi di riabilitazione palliativa, implementati e valutati nelle cure palliative.È stata condotta una scoping review, usando le linee guida del Joanna Briggs Institute. Sono stati cercati più database: CINAHL completo; PubMed; Scopus; SciELO; Cochrane Central Register of Controlled Trials; PEDro e letteratura grigia per studi incentrati su professionisti sanitari qualificati che si occupano di pazienti di età pari o superiore a 18 anni, che lavorano in cure palliative, che si concentrano sui concetti di interventi di riabilitazione palliativa.Dei 314 studi recuperati, due sono stati inclusi in questa revisione. Entrambi sono stati condotti con fisioterapisti e nessuno ha menzionato la riabilitazione infermieristica. Uno degli studi ha implementato e valutato un intervento dal punto di vista esclusivamente fisico e un altro dal punto di vista fisico ed emotivo. Gli interventi differiscono ancora nel numero di trattamenti che variavano da 4 a 7 sessioni. Entrambi gli studi sono stati condotti su pazienti oncologici e non oncologici.Sono necessarie ulteriori ricerche per esplorare le strategie di riabilitazione adottate dai professionisti sanitari che ricorrono a cure palliative per aiutare i pazienti. Inoltre, poiché gli infermieri sono spesso i professionisti sanitari che si trovano a più stretto contatto e trascorrono più tempo con il paziente, quali attività riabilitative facciano questi professionisti dovrebbe essere al centro dell'intervento.

Objetivo: Caracterizar os episódios de queda em pacientes internados em uma unidade cardiológica, quanto à ocorrência, fatores relacionados e risco. Método: Estudo descritivo, retrospectivo, por meio da análise de prontuários de pacientes internados em uma unidade de cardiologia que apresentaram episódio de queda entre janeiro de 2015 a dezembro 2016. O risco médio para quedas foi avaliado, conforme a Escala de Morse. Resultados: No período de estudo houve 32 episódios de quedas. A queda foi mais frequente em idosos (81,3%) e naqueles que faziam uso crônico de medicamentos, para controle e tratamento de comorbidades preexistentes. Dentre os fatores de risco, 34,4% apresentavam delirium, comprometimento neurológico e déficit de locomoção. O risco médio para quedas foi classificado como elevado (> 45), 25% das quedas resultaram em algum tipo de dano (leve ou moderado) e ocorreram em períodos matutinos. Conclusão: As contribuições fornecidas pelos registros de eventos adversos, deste estudo, facilitaram a identificação dos fatores de risco, demonstrando a necessidade de se propor intervenções de enfermagem preventivas, uma vez que assumir o evento e identificar suas causas são maneiras de praticar uma assistência de enfermagem segura ao paciente.

Rationale: Although proposed as a clinical prompt to sepsis based on predictive validity for mortality, the Quick Sepsis-related Organ Failure Assessment (qSOFA) score is often used as a screening tool, which requires high sensitivity.Objectives: To assess the predictive accuracy of qSOFA for mortality in Brazil, focusing on sensitivity.Methods: We prospectively collected data from two cohorts of emergency department and ward patients. Cohort 1 included patients with suspected infection but without organ dysfunction or sepsis (22 hospitals: 3 public and 19 private). Cohort 2 included patients with sepsis (54 hospitals: 24 public and 28 private). The primary outcome was in-hospital mortality. The predictive accuracy of qSOFA was examined considering only the worst values before the suspicion of infection or sepsis.Measurements and Main Results: Cohort 1 contained 5,460 patients (mortality rate, 14.0%; 95% confidence interval [CI], 13.1-15.0), among whom 78.3% had a qSOFA score less than or equal to 1 (mortality rate, 8.3%; 95% CI, 7.5-9.1). The sensitivity of a qSOFA score greater than or equal to 2 for predicting mortality was 53.9% and the 95% CI was 50.3 to 57.5. The sensitivity was higher for a qSOFA greater than or equal to 1 (84.9%; 95% CI, 82.1-87.3), a qSOFA score greater than or equal to 1 or lactate greater than 2 mmol/L (91.3%; 95% CI, 89.0-93.2), and systemic inflammatory response syndrome plus organ dysfunction (68.7%; 95% CI, 65.2-71.9). Cohort 2 contained 4,711 patients, among whom 62.3% had a qSOFA score less than or equal to 1 (mortality rate, 17.3%; 95% CI, 15.9-18.7), whereas in public hospitals the mortality rate was 39.3% (95% CI, 35.5-43.3).Conclusions: A qSOFA score greater than or equal to 2 has low sensitivity for predicting death in patients with suspected infection in a developing country. Using a qSOFA score greater than or equal to 2 as a screening tool for sepsis may miss patients who ultimately die. Using a qSOFA score greater than or equal to 1 or adding lactate to a qSOFA score greater than or equal to 1 may improve sensitivity.Clinical trial registered with www.clinicaltrials.gov (NCT03158493).

The Vazante Group, as originally described, is a thick marine carbonate-dominated succession adjacent to the Brasilia Fold Belt, on the western border of the Sao Francisco Craton, in south-central Brazil. The sedimentary dolomites of this group contain economically important Zn–Pb and phosphate deposits, but age constraints for the host rocks and mineralization have been controversial. New geochronological data and geological observations have indicated that the upper and middle sections of the Vazante succession belong to a Mesoproterozoic sequence that was thrust over a Neoproterozoic succession correlative with the Bambui Group. This new stratigraphic framework has significant implications for metallogenic exploration models in both intra-cratonic and passive-margin basins of the Sao Francisco Craton. Although hosted in Mesoproterozoic units, most of the Zn–Pb mineralization occurred in the Neoproterozoic by circulating hydrothermal fluids during the prolonged breakup of the Rodinia supercontinent. The possibility that an initial stage of mineralization occurred earlier is considered. Phosphorite generation in the Neoproterozoic units is conceivably related to glacial events. The refined stratigraphy combined with a new mineralization model will significantly contribute to the exploration strategy for phosphate and sulfide deposits in the Mesoproterozoic and Neoproterozoic successions of the Sao Francisco Craton and beyond.

This work focus on the corrosion and wear properties of titanium reinforced with 1% wt. alumina particles, produced by a combination of colloidal techniques and powder metallurgy. The alumina particles were added to control the grain growth of titanium during sintering, and simultaneously to increase hardness and wear resistance. Colloidal techniques permitted a homogeneous dispersion of alumina particles on the surface of fine Ti particles by the formulation of stable aqueous suspensions that were further processed by spray-dry to obtain spherical granules with improved compressibility. Ti-Alumina samples were produced by uniaxial pressing of granules and vacuum sintering leading to materials with homogeneous microstructure, a reduction of grain size higher than 50 % with respect to pure titanium, and a sensible increase in hardness. But the addition of ceramic particles can also have an influence on the corrosion behavior that is one of the most interesting properties of titanium alloys, and on wear resistance, that is one of the drawbacks of Ti. Moreover, the study of simultaneous action of wear and corrosion (tribocorrosion) is an area of highest interest in applications like biomedical or automotive. The corrosion behavior was evaluated by Electrochemical Impedance Spectroscopy (EIS) and Potentiodynamic Polarization (PP) in NaCl at two concentrations (0.9 % and 3.5 %) and temperatures (37 °C, and room temperature). Tribocorrosion tests were performed using a reciprocating ball-on-plate tribometer where a 10 mm diameter alumina ball was used as counter material, and 10 N normal load was applied during 30 min in the same concentrations and temperatures of NaCl as in the static corrosion test s.
