Os bisfosfonatos (BFs) têm sido indicados para o tratamento de doenças do metabolismo ósseo. Atualmente, seu emprego terapêutico aumentou e, com ele, os efeitos adversos, dos quais um dos mais importantes é a indução da osteonecrose dos maxilares, uma complicação de difíceis tratamento e solução. Até o presente, não se sabe ao certo qual é o mecanismo de desenvolvimento da osteonecrose dos maxilares induzida por bisfosfonatos (ONMB), nem qual deve ser o tratamento estabelecido perante essa manifestação. Apesar de a literatura apresentar formas variadas de tratamento, não existe um protocolo definido. Apresentamos uma revisão sobre a ONMB, enfocando sua etiopatogenia e as formas reportadas de tratamento.Bisphosphonates (BPs) have been used for the management of bone metabolic diseases. Currently their therapeutic use has increased, as also have their adverse effects, one of the most important being the bisphosphonate-related osteonecrosis of the jaw (BRONJ), a complication of difficult treatment and solution. Until now, the physiopathology of BRONJ remains unclear, and its treatment is uncertain. Although the literature provides several treatment options, there is no defined protocol. We present a review about BRONJ, focusing on its pathogenesis and its reported forms of treatment.

Objective: To assess the occurrence and characteristics of the reported accidents with perforating-cutting materials involving students, staff and faculty members, between 2000 and 2005 at the Dental Clinic of the School of Dentistry of the University of São Paulo. Methods: A survey of the records of reported occurrences of accidents was made, considering the material that caused the accident, time of day of the occurrence, the discipline in which it occurred, and clinical conduct adopted in the emergency room. When available, the results of the laboratory exams of the accident victim and the source patient were also taken into consideration. Results: The data assessed showed there were 40 accident reports, of which 39 reports involved undergraduate students and 1 staff member. The instrument that caused most accidents was the anesthetic needle and largest number of these accidents occurred in the Surgery discipline. However, 50% of the records did not present complete information, which prevented a more accurate epidemiological assessment. Conclusion: The data obtained led to the conclusion that the rate of accidents is extremely low considering the number of clinical attendances provided in the period and raises the hypothesis that many cases were not reported.

medical and dental treatment in the form of ozonized water and oxygen / ozone gas mixture and associated with photobiomodulation therapy. The evidence map was based on the 3iE evidence searched in the databases: LILACS, PubMed, EMBASE and Cochrane Library, seeking systematic and narrative reviews, analyzed in an excel characterization spreadsheet. Three independent literature reviewers screened the systematic review search output blinded at the software Rayyan. The full-text publications were screened against the specified inclusion criteria by thre independent reviewers; disagreements were resolved through discussion. Forty-two studies were found, 16 of which were systematic reviews and 26 narrative reviews, published between 2006 and 2021, with different levels of confidence. The intervention that most appeared in the literature was oral ozone therapy as a bacterial colony reduction and decontamination agent, with emphasis on topical oxygen-ozone gas mixtures, topical ozonized water. Oral ozone therapy can bring benefits to clinical practice and patients. The evidence map provides an easy visualization of valuable information for patients, health practitioners and managers, in order to promote evidence-based complementary therapies. Este estudio tuvo como objetivo hacer el mapa de evidencias sobre la ozonoterapia, como abordaje integrador y / o complementario al tratamiento médico y odontológico en forma de agua ozonizada y mezcla de oxígeno / ozono gaseoso y asociado a la terapia de fotobiomodulación. El mapa de evidencia se basó en la evidencia 3iE buscada en las bases de datos: LILACS, PubMed, EMBASE y Cochrane Library, buscando revisiones sistemáticas y narrativas, analizadas en una hoja de cálculo de caracterización en Excel. Tres revisores independientes de la literatura examinaron el resultado de la búsqueda de la revisión sistemática cegados en el software Rayyan. Tres revisores independientes examinaron las publicaciones de texto completo en función de los criterios de inclusión especificados; Los desacuerdos fueron resueltos mediante una discusión. Se encontraron 42 estudios, 16 de los cuales fueron revisiones sistemáticas y 26 revisiones narrativas, publicados entre 2006 y 2021, con diferentes niveles de confianza. La intervención que más apareció en la literatura fue la ozonoterapia oral como agente reductor de colonias bacterianas y descontaminación, con énfasis en mezclas tópicas de oxígeno-ozono gaseoso, agua ozonizada tópica. La ozonoterapia oral puede aportar beneficios a la práctica clínica y a los pacientes. El mapa de evidencia proporciona una visualización fácil de información valiosa para pacientes, profesionales de la salud y administradores, con el fin de promover terapias complementarias basadas en evidencia. Este estudo teve o objetivo de realizar um mapa de evidências sobre a ozonioterapia, como abordagem integrativa e / ou complementar ao tratamento médico-odontológico na forma de água ozonizada e mistura de oxigênio / gás ozônio e associada à fotobiomodulação. 5. Effect of photobiomodulation on the viability of osteoblasts and fibroblasts submitted to alendronate sodium or zoledronic acid

O objetivo deste estudo e avaliar o efeito da terapia de fotobiomodulacao (TFBM) na viabilidade de osteoblastos e cultura de fibroblastos em diferentes concentracoes de alendronato ou acido zoledronico. Duas linhagens celulares – celulas de camundongos semelhantes a osteoblastos (OSTEO 1) e fibroblastos de mucosa bucal humana (FMM1) – foram utilizadas. As celulas foram submetidas a diferentes concentracoes de bisfosfonatos (1 μM, 10 μM e 100 μM de alendronato de sodio e 3 μM, 5 μM e 10 μM de acido zoledronico) por 24 horas. Em seguida, as culturas receberam TFBM. As irradiacoes foram aplicadas com laser de diodo (InGaAIP, 660 nm, 30 mW, spot 0,028 cm2) em modo continuo, pontual e de contato, em duas densidades de energia: 5 J/cm2 (4,5 s) ou 10 J/cm2 (9s) com intervalos de 6 horas. A viabilidade celular foi determinada pelo ensaio de atividade mitocondrial (MTT) 24 h apos a ultima irradiacao. Os dados foram comparados pelo ANOVA One-Way, complementado pelo teste de Tukey (p < 0,05). O alendronato de sodio nas concentracoes de 100 μM e 10 μM e o acido zoledronico na concentracao de 10 μM apresentaram maior toxicidade a longo prazo. A viabilidade celular do grupo tratado com TFBM foi significativamente maior que a do grupo de controle negativo. O mesmo ocorreu com os osteoblastos tratados com as maiores concentracoes do farmaco (5 e 10 μM), apesar de nao atingir a viabilidade celular do grupo de controle positivo, apresentou maior viabilidade do que o controle negativo no qual as celulas nao foram irradiadas. Nos grupos submetidos ao acido zoledronico, controles positivos apresentaram maior viabilidade celular. Concluimos que sob os parâmetros aplicados neste estudo, a TFBM, com uma densidade de energia de 5 J/cm2, foi capaz de reverter a toxicidade do alendronato sodico aplicado nas concentracoes mais altas em ambos os tipos celulares, enquanto a toxicidade do acido zoledronico, independentemente de suas concentracoes, foi nao influenciada pela TFBM.

A odontologia reabilitadora está passando por uma grande evolução em todas as áreas de conhecimento, sejam pelas pesquisas cientificas, desenvolvimento das técnicas cirúrgicas e desenvolvimento de materiais. Com o surgimento de novas tecnologias, mais recursos vem sendo adregados na clínica diária, já que materiais e técnicas auxiliam desde o diagnóstico até a confecção das próteses sobre implantes. Objetiva-se, neste relato de caso clínico, demonstrar a importância da associação de várias áreas do conhecimento na reabilitação implantossuportada, valorizando os benefícios da utilização das tomografias computadorizadas cone beam, do sistema CAD/CAM, dos scaners intraorais e das técnicas adesivas nas reabilitações. Paciente com 54 anos procurou tratamento na Fundecto-USP (Fundação Faculdade de Odontologia conveniada à FOUSP) com queixa de ausência dentária na região dos elementos 14 e 15, além de queixas referentes à estética na região anterior. Foi realizado o diagnóstico através de montagem em articulador semi-ajustável, fotografias e enceramento diagnóstico. Apesar da ausência de dois elementos, foi planejada a reabilitação com implante apenas do elemento 15 e aumento dos elementos 16 e 13. A tomografia computadorizada cone beam, permitiu identificar ausência de altura óssea suficiente para instalação de implante na região do 15, então foi planejado a instalação do implante com levantamento da membrana do seio maxilar de forma simultânea. Através de escaneamento intraoral, tecnologia CAD/CAM e informações clínicas, foi realizado o projeto do enceramento virtual das futuras coroas. Após aprovação foi realizada a impressão dos modelos, fresagem das coroas em cera, prova das coroas em cera e fresagem em cerâmica das coroas. Com base na experiência vivenciada parece lícito concluir que os desenvolvimentos tecnológicos têm tornado as reabilitações odontológicas complexas mais rápidas, mais efetivas e previsíveis.

8. Effects of the unilateral removal and dissection of the masseter muscle on the facial growth of young rats

This study analyzed the effects of the unilateral removal and dissection of the masseter muscle on the facial growth of young rats. A total of 30 one-month-old Wistar rats were used. Unilateral complete removal of the masseter muscle was performed in the removal group, and detachment followed by repositioning of the masseter muscle was performed in the dissection group, while only surgical access was performed in the sham-operated group. The animals were sacrificed at three months of age. Axial radiographic projections of the skulls and lateral projections of the hemimandibles were taken. Cephalometric evaluations were made and the values obtained were submitted to statistical analyses. In the removal group, there were contour alterations of the angular process, and a significant homolateral difference in the length of the maxilla and a significant bilateral difference in the height of the mandibular body and the length of the mandible were observed. Comparison among groups revealed significance only in the removal group. It was concluded that the experimental removal of the masseter muscle during the growing period in rats induced atrophic changes in the angular process, as well as asymmetry of the maxilla and shortening of the whole mandible.

ResumoO acto cirúrgico deve ser sempre precedido de uma avaliação das condições de saúde local e sistémica do paciente, sendo os exames complementares o subsídio que utilizamos para confirmar as suspeitas clínicas e hipóteses de diagnóstico, adequando o paciente à terapêutica proposta. A literatura é ampla no que diz respeito a testes de hemostasia por meio de diferentes métodos, contudo, não há estudos na literatura revista, que comparem o tempo de sangramento pelo método de Ivy com o tempo de sangramento aferido na mucosa bucal. Propusemos neste estudo, a avaliação da técnica de aferição do tempo de sangramento na mucosa bucal e comparámos com o tempo de sangramento pelo método de Ivy convencional, em pacientes com história de diátese hemorrágica relacionada com procedimentos cirúrgicos anteriores. Tais pacientes apresentaram-se no ambulatório de Cirurgia da Faculdade de Odontologia da Universidade de São Paulo com a necessidade de procedimentos de cirurgia oral menor. Foi realizado o tempo de sangramento pelo método de Ivy previamente ao teste do tempo de sangramento da mucosa bucal em 30 pacientes. Os resultados avaliados pelo teste de KOLMOGOROV-SMIRNOV seguiram uma distribuição normal em ambas as variáveis (p>0,15).O tempo médio de sangramento na pele e na mucosa bucal foi de 295 segundos e 291 segundos respectivamente, demonstrando semelhança entre os tempos aferidos.Concluiu-se que o tempo de sangramento da mucosa bucal não apresentou diferença estatística quando comparado com o tempo de sangramento de Ivy (p=0,755).AbstractSurgical procedures must be preceded by an accurate evaluation of the local and systemic health status and complementary exams can confirm or not clinical suspect and diagnosis hypothesis in order to adequate the patient to the proposed surgical treatment.The literature is generous with respect of hemostasia tests employing several methodologies although there are not studies that compare the Bleeding Time Test by Ivy's method with bleeding time accessed at the oral mucosa. We propose at this study the evaluation of a bleeding time method in the oral mucosa comparing the results with the conventional Ivy's test in patients with bleeding disorders history in past surgical procedures.The patients were those attending the Oral Surgery Clinic of the Dental School of University of São Paulo which underwent oral minor surgery. The Ivy's Bleeding time test was previously obtained before the evaluation of the bleeding time of the mucosa in 30 patients. It was concluded that the bleeding time of the mucosa did not present statistical difference when compared to the Ivy's test (p=0,755). The results evaluated by KOLMOGOROV-SMIRNOV's method followed a normal distribution in both samples (p>0,15) and the mean bleeding time at the skin and at the oral mucosa was 295 seconds and 291 seconds respectively showing similarity between the groups.

10. 'Estudo radiográfico das características dos terceiros molares e suas correlações com a impactação óssea'

Diante da importância da avaliação radiográfica, foram estudadas as classificações de várias características dos terceiros molares, por meio de panorâmicas, sendo investigado suas possíveis correlações com o estado de impactação óssea. Informações referentes aos dentes foram coletadas em relação ao estado de impactação ou inclusão óssea (de acordo com a formação radicular), espaço disponível, posição do longo eixo, profundidade óssea, relações com o dente adjacente, com o seio maxilar e com o nervo alveolar inferior. Além disso, foram considerados as ausências de dentes no hemiarco, a presença de supranumerários, de outros dentes inclusos/impactados, o gênero e a idade do paciente. Ao total, foram analisadas 802 panorâmicas com 2687 terceiros molares, sendo o estado de impactação pesquisado em 2119 desses, nos quais foi possível identificar correlações do espaço inadequado e de algumas posições com a etiologia das impactações, principalmente nos dentes inferiores. As análises dos resultados revelaram que não houve diferenças estatisticamente significantes das características do terceiros molares entre os gêneros e que a freqüência de impactação óssea diminuiu com o avanço da idade. Foi também observada boa concordância entre os dentes do mesmo arco do paciente, o que não ocorreu entre seus dentes do mesmo lado, nos quais os inferiores apresentaram tendência de impactação. As classificações estabelecidas mostraram-se satisfatórias e de relevante importância na avaliação dos terceiros molares, tanto para a determinação do seu tratamento como para o seu planejamento cirúrgico. The aim of this retrospective study was to investigate the panoramic radiographic features of third molar and to analyze the relationship between its classification with the state of bone impaction. The considered radiographic features were the status of impaction, the space for the third molar, the angulation of tooth, the level of depth, the development of root, the relation to the second molar, to the maxillary sinus and to the inferior alveolar nerve, and also others radiological characteristics. Moreover, the relationship between the state of impaction with gender, age and absence of teeth were studied, and either observations of supernumerary or any impacted teeth. 802 radiographs were evaluated with 2687 third molars, and the status of impaction was analyzed in 2119 teeth. It was observed a significant convergence between the upper or the lower tooth from the same patient but it was not significant between the sides. This study had demonstrated that the viable space and some teeth angulations had strong relationship with the etiology of impaction, mainly considering the lower teeth. The established classifications were accurate and showed be useful for the study and surgical planning of third molars.

The handling of vinyl polysiloxane (addition silicone) impression putties with latex gloves is said to interfere with the setting of these impression materials. The aim of this study is to evaluate the effect of handling techniques on the setting of vinyl polysiloxane impression putties using several types of gloves. The setting of these materials was evaluated by means of an elasticimeter. Four vinyl polysiloxane putty impression materials and five brands of gloves (one made of vinyl, one of synthetic rubber, and three of natural rubber) were studied. Based on the type of glove, they were previously washed or not, and a spatula was used or not for initial mixing (before handmixing). The vinyl, the synthetic and one of the natural rubber gloves did not require the previous washing procedure and/or the use of a spatula for initial mixing. Two other natural rubber gloves - depending on the silicone -, showed satisfactory results only when the initial mixing was performed with a spatula. It was concluded that setting inhibition depends on the kind of vinyl polysiloxane impression material and the kind of gloves used, but when the initial mixing was performed with the spatula this setting inhibition was overcome. The results of this study also showed that it is possible to associate cross-contamination control and satisfactory performance of addition silicone putty materials. When doubts arise from the compatibility between vinyl polysiloxane impression putties and gloves, the initial mixing should be performed with a spatula. A finalidade da presente pesquisa foi avaliar a influência na elasticidade da manipulação de silicones de adição em consistência de massa, por meio de várias técnicas, empregando-se luvas. A polimerização dos materiais foi avaliada por meio de um elasticímetro. Ensaiaram-se quatro silicones, empregando cinco marcas de luvas (uma de vinil, uma de borracha sintética e três de borracha natural). Dependendo do tipo de luva, ela foi lavada ou não e ainda foi utilizada ou não uma espátula para a mistura inicial dos silicones. Os resultados demonstraram que no emprego das luvas de vinil, uma das de borracha sintética e uma das de borracha natural não precisaram ser lavadas, nem a utilização de espátula para a mistura inicial foi necessária. Duas luvas de borracha natural não conduziram a resultados satisfatórios com todos os silicones sem que a mistura inicial fosse feita com a espátula mas, com essa prática, todas as luvas foram compatíveis com todos os silicones. Pode-se concluir que, em caso de dúvida sobre a compatibilidade entre tipo de luva e marca de silicone, o emprego de espátula na mistura inicial contorna o problema.

Traumas to deciduous teeth may have severe consequences. This article addresses a dentigerous cyst case report associated with an upper permanent incisor, unusual site of occurrence, which was impacted and dislodged from its natural site of eruption after trauma at the deciduous predecessor.The main aspects of etiopathology and its clinical characteristics are also discussed, with especial focus on the radiographic features of the diagnosis in order to allow for an accurate surgical planning.

Bisphosphonates (BPs) have been used for the management of bone metabolic diseases. Currently their therapeutic use has increased, as also have their adverse effects, one of the most important being the bisphosphonate-related osteonecrosis of the jaw (BRONJ), a complication of difficult treatment and solution. Until now, the physiopathology of BRONJ remains unclear, and its treatment is uncertain. Although the literature provides several treatment options, there is no defined protocol. We present a review about BRONJ, focusing on its pathogenesis and its reported forms of treatment.

Dentigerous cyst (DC) is one of the most common odontogenic cysts of the jaws and rarely recurs. On the other hand, keratocystic odontogenic tumor (KCOT), formerly known as odontogenic keratocyst (OKC), is considered a benign unicystic or multicystic intraosseous neoplasm and one of the most aggressive odontogenic lesions presenting relatively high recurrence rate and a tendency to invade adjacent tissue. Two cases of these odontogenic lesions occurring in children are presented. They were very similar in clinical and radiographic characteristics, and both were treated by marsupialization. The treatment was chosen in order to preserve the associated permanent teeth with complementary orthodontic treatment to direct eruption of the associated permanent teeth. At 7-years of follow-up, none of the cases showed recurrence.

The calcifying epithelial odontogenic tumor (CEOT) is a rare benign odontogenic neoplasm that accounts for approximately 1% of all odontogenic tumors. Its origin is controversial as well as its true malignant potential. A case of an advanced CEOT associated with an impacted right second molar in the mandible of a young black female patient is presented. Computed tomography imaging, radiographic and pathological findings, surgical patterns, gross anatomy of the lesion and the performed treatment are discussed. O tumor odontogênico epitelial calcificante (TOEC) é uma neoplasia odontogênica rara de natureza benigna, representando aproximadamente 1% de todos os tumores odontogênicos. Sua origem é controversa bem como seu potencial de transformação maligna. Esse artigo relata um caso avançado de TOEC associado a um segundo molar direito incluso na mandíbula deuma paciente negra jovem. As imagens de tomografia computadorizada, achados radiográficos e patológicos, padrões cirúrgicos, aspectos anatômicos lesão e o tratamento realizado são discutidos.

The main aspects related to its classification, diagnosis and clinical characteristics are also discussed.

17. Effects of the unilateral removal and dissection of the masseter muscle on the facial growth of young rats

This study analyzed the effects of the unilateral removal and dissection of the masseter muscle on the facial growth of young rats. A total of 30 one-month-old Wistar rats were used. Unilateral complete removal of the masseter muscle was performed in the removal group, and detachment followed by repositioning of the masseter muscle was performed in the dissection group, while only surgical access was performed in the sham-operated group. The animals were sacrificed at three months of age. Axial radiographic projections of the skulls and lateral projections of the hemimandibles were taken. Cephalometric evaluations were made and the values obtained were submitted to statistical analyses. In the removal group, there were contour alterations of the angular process, and a significant homolateral difference in the length of the maxilla and a significant bilateral difference in the height of the mandibular body and the length of the mandible were observed. Comparison among groups revealed significance only in the removal group. It was concluded that the experimental removal of the masseter muscle during the growing period in rats induced atrophic changes in the angular process, as well as asymmetry of the maxilla and shortening of the whole mandible.
