Zusammenfassung Ziel Analyse mundgesundheitsbezogener Diagnoseprävalenzen bei Kindern, Jugendlichen (KiJu), jungen Erwachsenen mit psychischen Erkrankungen/Verhaltensstörungen (PEVS) und jenen der Allgemeinbevölkerung in Deutschland anhand von kassenärztlichen Abrechnungsdaten Methode Anonym vom ZI (Zentralinstitut) bereitgestellte bundesweite Abrechnungsdaten (2019) von GKV-Versicherten (0–44 Jahre) wurden u. a. nach Geschlecht, Altersgruppen sowie ICD-K00-K14-Diagnose stratifiziert. Ergebnis Über diese Datenbank wurden keine verlässlichen mund- bzw. zahngesundheitsbezogenen Aussagen generiert. Laut Datensatz wiesen unabhängig von einer F-Diagnose 1,8% aller 11.854.384 KiJu-GKV-Versicherten sowie 0,2% der 18–44-Jährigen (23.348.399 GKV-Versicherte) eine zahn-hartsubstanzbezogene Diagnose (ICD-10-K02/K03.2) auf. Anhand verfügbarer Literatur zur Kariesprävalenz von KiJu mit PEVS kann von einer mittleren ungewichteten Prävalenz von 51% ausgegangen werden. Daran und der F-Diagnoseprävalenz bei KiJu im Jahr 2017 orientierend, müssten geschätzt 957.952 (Abb. 1) KiJu mit einer PEVS auch eine Zahnkaries aufweisen. Schlussfolgerung Die Kennzahlen mundgesundheitsbezogener, ärztlicher Diagnoseprävalenzen bei Personen der Allgemeinbevölkerung und Altersgenossen mit PEVS sind sehr gering. Offensichtlich werden zahnbezogene Diagnosen von u. a. Haus- und Kinderärzten nur äußerst selten vergeben. Die vorhandene Literatur sowie klinische Erfahrung zeigen, dass diese kassenärztlichen Abrechnungsdaten nicht geeignet sind die Zahngesundheit der Personengruppen realistisch abzubilden. Somit ist für die Vernetzung, die Verbesserung der Versorgungsforschung und Versorgung, z. B. von Subgruppen der Gesellschaft, eine interprofessionelle zahn-/ärztliche, datenschutzkonforme Zentraldatenbank anzustreben.

In Germany, a curriculum in special care dentistry (SCD) is not mandatory. As a result, very little is known about dental students' attitudes toward interacting with people with disability (PwD). The study´s aim was to assess this among dental students at Witten/Herdecke University (UW/H).In the academic years 2016-2017 to 2020-2021, dental students in the 7th semester (UW/H) were asked to complete a self-assessment questionnaire. Prior to its start, the study had obtained a positive vote from the ethics committee of the UW/H (#174/2016).Out of 199 students 166 persons on average 24.5 years old completed the questionnaire. Nearly all participants had already had previous experience with PwD in various domains of life prior to dental school (89.2%). Three-quarters indicated not having reservations about interacting with PwD. The proportion of persons without previous experience in interacting with a PwD was almost twice as high in the group with reservation (16.2%) as it was in the group without reservation (8.7%). Almost all surveyed students (92.8%) affirmed that they would like to learn more on the topic of "Professional Interaction with PwD."Most of the respondents did not have reservations about interacting with PwD. Almost all the participants indicated a high level of interest in topics related to SCD. Also, the authors like to encourage the curriculum committees of dental schools at other universities to integrate courses on SCD, based on the iADH recommendations, into their dental curricula.

To analyse oral health-related diagnostic prevalences in children, adolescents (KiJu), young adults with mental/behavioural disorders (PEVS) and those of the general population in Germany based on claims data.Anonymously provided ZI (Zentralinstitut)-data sets of GKV-insured persons (0-44 years) were i. e. stratified according to gender, age groups, ICD-K00-K14 diagnosis.No reliable oral or dental health-related information was generated from the requested data set. Regardless of the F diagnosis, according to the data set, 1.8% of all 11,854,384 KiJu-GKV-insured persons and 0.2% of 18-44-year-olds (23,348,399 GKV-insured persons) had a diagnosis related to the dental hard tissue (ICD-10 K02/K03.2). Based on available literature on the prevalence of caries in KiJu with PEVS, a mean unweighted prevalence of 51% can be assumed. To evaluate the fluoride release from different fissure sealants sold mainly in non-Western countries via a curve-fitting method, and to investigate the ability of these materials to be recharged with fluoride.Four different fissure sealants which the manufacturers claim to release F- were tested; glass-ionomer cement Argecem (AC), the composite resins Fisskhim (FK), Fissulight (FL), Helioseal F (HF). Helioseal (HS), a composite resin not declared to release F, was used as a negative control. Of each material, 10 disks 8.0 mm in diameter and 1.8 mm thick were produced. For fluoride measurements, a fluoride-ion specific electrode was used. On day 21, the specimens were recharged with fluoride by immersion in toothpaste slurry. Fluoride release was evaluated and statistically compared by fitting an exponential curve through the cumulative release values and statistically comparing curve parameters (t½, AUC and plateau value) among each other as well as before and after recharging.Two materials (AC and HF) released a considerable amount of F- in the first 3 weeks after sample production. Furthermore, the increase of F- release after F- recharge was statistically significant (p0.05) for the materials AC, FL and HF, but not for FK and HS.Fluoride release of Fisskhim and Fissulight was negligible, while Helioseal F and Argecem noticeably released fluoride and thus can enhance the caries preventive effect of fissure sealants. Comparing release curve parameters simplifies statistical procedures.

There is a lack of actual data regarding oral health in children and adolescents with intellectual disabilities.To evaluate the oral health in adolescents with intellectual disabilities participating in the German Special Olympics games 2008.A free voluntary dental examination was offered to the participating athletes. Dental examinations were performed according to WHO criteria by dental clinicians. In addition, information about the athletes' oral hygiene habits was collected.The number of adolescent athletes aged between 12 and 17 years who had their teeth examined was 160. On average they were 15.3 years old. Caries prevalence was 58.1% and the mean DMFT was 2.3. The mean number of fissure sealed teeth was 2.5. About half of the participants showed signs of gum inflammation. The proportion of the adolescents living at home with their parents was 88%. More than 90% of them brushed their teeth by themselves without assistance.Adolescents with intellectual disabilities seem to have benefited from various caries preventive measures which had been introduced during the last two decades in Germany but still have a poorer oral health than the general population. More specific prevention programmes seeking close cooperation with parents, custodians, and caretakers should be developed.
