Retrospective cross-sectional case series.Lesions of the triangular fibrocartilage complex (TFCC) can result in pain during axial load and unstable distal radioulnar joint (DRUJ). Conventional wrist orthoses decrease initial pain sufficiently but also prevent any movement during recovery and do not contribute to the stabilization of the DRUJ.In this retrospective analysis, we tested if the weight-bearing capacity of patients with lesions of the triangular fibrocartilage complex was increased by wearing a brace that stabilizes the distal radioulnar joint.Twenty-three patients had an arthroscopically confirmed TFCC lesion. We compared preoperative dynamic weight-bearing capacity of both hands with and without a commercially available wrist brace (WristWidget). Subgroup analysis was performed for stability of the distal radioulnar joint and etiology of the TFCC lesion. The dynamic ulnar variance was measured in a modified weight bearing test. We used parametric tests for normally distributed values.The weight-bearing capacity of the hand with TFCC lesion was significantly lower than of the control hand (16 verus 36 kg; p0.001). The relative load of the affected hand compared to the unaffected hand increased from 48 % (CI 37-60, SD 27) to 59 % (CI 47-72, SD 29 with a brace. The device had no effect on the control hand. Twelve patients with unstable DRUJ had a lower weight-bearing capacity compared to the eleven with stable joint. The percentage improvement with bracing was higher for those with unstable joints (versus stable) and traumatic lesions (versus degenrative).The use of a wrist brace significantly increases the weight-bearing capacity and therefore the maximum tolerated axial load of patients with a lesion of the TFCC. Patients with traumatic lesion or unstable DRUJ tend to show lower values than with degenerative lesions or stable joints.

Background: Previous studies have discussed the diagnostic value of four dimensional kinematic CT in cases of carpal instabilities. This analysis compares calculated skin doses of 4D CT and conventional cineradiography of the wrist in cases of suspected SLL rupture. Methods: Retrospective calculation and interpolation of skin doses and effective doses for ten consecutive 4D CT examinations and 41 cineradiographies for suspected lesions of the scapholunate ligament. Standardised anterior-posterior and lateral cine sequences using a flat-panel digital subtraction imager and of 4D kinematic CT using a dual-source scanner were acquired and acquisition parameters recorded. We tested if the skin dose of 4D CT is different from cineradiography. Results: Median dose area product (DAP) of cineradiography was 135.34 cGycm2 resulting in a calculated median skin dose of 32.6 mSv (confidence interval 26.86–42.90 mSv) and an estimated effective skin dose of 3.26 µSv. CT dose index (CTDI) for 4D examinations was recorded to be 26.79 mGy and the dose-length product (DLP) was 150 mGy × cm. This resulted in an estimated skin dose of 34 mSv, which is covered by the confidence interval of cineradiography, and an effective skin dose of 3.4 µSv. Conclusions: Skin dose calculations are comparable for 2D cineradiography in two plains and 4D kinematic CT of the wrist. Calculated effective doses are < 0.01 mSv.

Background To investigate the incidence of osseous wrist and hand injuries on whole-body computed tomographies (WBCT) at an urban maximum-care trauma center, to report the number of missed cases in primary radiology reports, and to develop an algorithm for improved detection of these injuries. Methods Retrospective analysis reviewing all WBCT for a period of 8 months for osseous wrist and hand injuries. (1) Reconstruction of hands/wrists in three planes (thickness 1–2 mm) and analysis by a blinded musculoskeletal radiologist. (2) Scanning of primary radiology reports and comparison to the re-evaluation. (3) Calculation of the diagnostic accuracy of WBCT during primary reporting. (4) Search for factors potentially influencing the incidence (trauma mechanism, associated injuries, Glasgow Coma Scale, artifacts). (5) Development of an algorithm to improve the detection rate. Results Five hundred six WBCT were included between 01/2020 and 08/2020. 59 (11.7%) WBCT showed 92 osseous wrist or hand injuries. Distal intra-articular radius fractures occurred most frequently (n = 24, 26.1%); 22 patients (37.3%) showed multiple injuries. The sensitivity of WBCT in the detection of wrist and hand fractures during primary evaluation was low with 4 positive cases identified correctly (6.8%; 95% CI 1.9 to 16.5), while the specificity was 100% (95% CI 99.2 to 100.0). Forty-three cases (72.9%) were detected on additional imaging after clinical reassessment. Twelve injuries remained undetected (20.3%). Motorcycle accidents were more common in positive cases (22.0% vs. 10.1%, p = 0.006). 98% of positive cases showed additional fractures of the upper and/or lower extremities, whereas 37% of the patients without osseous wrist and hand injuries suffered such fractures (p Conclusion Osseous wrist and hand injuries are present in 11.7% on WBCT after polytrauma. 93.2% of injuries were missed primarily, resulting in a very low sensitivity of WBCT during primary reporting. Motorcycle accidents might predispose for these injuries, and they often cause additional fractures of the extremities. Clinical re-evaluation of patients and secondary re-evaluation of WBCT with preparation of dedicated multiplanar reformations are essential in polytrauma cases to detect osseous injuries of wrist and hand reliably. ZusammenfassungVerletzungen durch Bisse sind häufig und stellen zusammen mit den resultierenden Komplikationen etwa 1–2 % aller Vorstellungen in Notfallambulanzen dar. In über 75 % sind die Hände betroffen. Im nordeuropäischen Raum werden Bissverletzungen vorrangig durch Hunde, Katzen und den Menschen verursacht. Aufgrund der vielfältigen und komplexen Kompartimente sowie der geringen Weichteildeckung funktionell relevanter Strukturen führen selbst kleinste und oberflächliche Bissverletzungen an der Hand zu Infektionen. Jede Bissverletzung der Hand kann in der Folge zu fulminanten Infektionen und selten gar tödlichen Verläufen führen. Bis zu 40 % aller Infektionen an der Hand gehen auf Bissverletzungen zurück.Das Erregerspektrum aus der Mundflora des Beißenden ist vielfältig und umfasst aerobe und anaerobe Bakterienstämme. Bissverletzungen stellen sowohl für den Verletzten als auch den behandelnden Arzt eine große Herausforderung dar. Die Rate an Komplikationen steigt nachweislich mit einer verzögerten medizinischen Konsultation, dem Mangel an medizinischer Sorgfalt und einer inadäquaten Wundversorgung. In dieser Übersichtsarbeit diskutieren wir die Arten und Komplikationen von Bissverletzungen, ihr potenzielles Infektionsrisiko, deren Erregerspektrum und Erscheinungsbild sowie deren effektive Therapie.

ZusammenfassungDie Osteomyelitiden im Bereich von Handgelenk und Hand sind weniger häufig als die Knocheninfektionen der langen Röhrenknochen, aber nicht selten. Viele Fragen zur Klassifikation, Pathogenese, Diagnostik, zum Therapiemanagement sowie den erreichbaren Behandlungsergebnissen sind bisher nur wenig ausreichend wissenschaftlich kontrolliert. Die Pathogenese der Erkrankung kann in posttraumatische, postoperative, fortgeleitete und hämatogene Osteomyelitiden, unter Erfassung der patientenindividuellen Risiken, eingeteilt werden. Eine frühe Diagnose und eine adäquate Therapie sind notwendig, um die betroffenen knöchernen Strukturen sowie die Funktion der angrenzenden Gelenke zu erhalten. Ein mehrzeitiges operatives Vorgehen ist dabei häufig notwendig. Zufriedenstellende Raten der Infektionskontrolle und erfolgreiche knöcherne Rekonstruktionen können meist erreicht werden. Komplikationen, verbleibende Defizite und partielle Amputationen, auch bei angemessener Therapie der Osteomyelitiden, sind auf Grund der Schwere der Erkrankung aber nicht immer zu verhindern.

Background We examined the visibility of fractures of hand and forearm in whole-body CT and its influence on delayed diagnosis. This study is based on a prior study on delayed diagnosis of fractures of hand and forearm in patients with suspected polytrauma. Methods Two blinded radiologists examined CT-scans of patients with fractures of hand or forearm that were diagnosed later than 24 h after admission and control cases with unremarkable imaging of those areas. They were provided with clinical information that was documented in the admission report and were asked to examine forearm and hands. After unblinding, the visibility of fractures was determined. We examined if time of admission or slice thickness was a factor for late or missed diagnoses. Results We included 72 known fractures in 36 cases. Of those 65 were visible. Sixteen visible fractures were diagnosed late during hospital stay. Eight more fractures were detected on revision by the radiologists. Both radiologists missed known fractures and found new fractures that were not reported by the other. Missed and late diagnoses of fractures occurred more often around 5 pm and 1 am. Slice thickness was not significantly different between fractures and cases with fractures found within 24 h and those found later. Conclusions The number of late diagnosis or completely missed fractures of the hand and forearm may be reduced by a repeated survey of WBCT with focus on the extremities in patients with suspected polytrauma who are not conscious. ZusammenfassungZiel Ziel der Studie war die Auswertung der Ergebnisse einer spezialisierten interdisziplinären Handrehabilitation in Abhängigkeit vom zeitlichen Abstand zwischen der Verletzung und dem Beginn der Rehabilitation.Patienten und Methoden Mit Hilfe von punktebasierten Fragebögen (Disability of the Arm, Shoulder and Hand Score [DASH-Score], European Quality of Life 5 Dimensions [EQ-5D]) und objektiven Messparametern (TAM = Total Active Motion bei Fingerverletzungen, ROM = Range of Motion bei Handgelenksverletzungen, Grobkraft) wurden die Ergebnisse von 76 Patienten mit Verletzungen von Fingern und Handgelenken oder einem komplexen regionalen Schmerzsyndrom (CRPS) zu Beginn, am Ende und bei der Nachuntersuchung 12–16 Wochen nach Ende der semistationären Rehabilitation analysiert. Die Patienten wurden in Gruppen mit einem frühen (< 120 Tage nach Trauma) bzw. späten Beginn der Rehabilitation aufgeteilt.Ergebnisse Patienten mit frühem Beginn der Rehabilitation hatten bei der Nachuntersuchung signifikant bessere Ergebnisse beim DASH-Score als auch beim Bewegungsausmaß des Handgelenkes. Auch waren signifikant mehr Patienten mit frühem Rehabilitationsbeginn zum Ende der Rehabilitation als auch bei der Nachuntersuchung arbeitsfähig als Patienten mit spätem Beginn der Rehabilitation. Insbesondere Patienten mit Fingerverletzungen und CRPS profitierten von einem frühen Beginn der Rehabilitation.Schlussfolgerung Ein früher Beginn der Rehabilitation nach Finger- und Handverletzungen sowie bei einem CRPS führt zu funktionell besseren Ergebnissen, wobei vor allem Patienten mit CRPS von einem frühen Beginn profitieren.

Background: Osteoarthritis of the proximal interphalangeal (PIP) joint of the finger often leads to global hand-function detriment. Different techniques for the arthrodesis of the proximal interphalangeal joint have been described that all lead to union in a reasonable percentage of patients and period of time. This biomechanical study aims to analyze and compare the primary stability of different techniques of arthrodesis to render postoperative immobilization unnecessary. Methods: Arthrodeses of 40° of composite cylinders were tested with different techniques in four-point bending for stability in extension as well as flexion. Results: In extension, the compression screw and the compression wires showed the highest stability—whereas in flexion, plate fusion was superior. Tension band, cerclage or compression screw fusion showed the best compromise in flexion/extension stability. Conclusions: Fusion techniques that apply compression to the fusion show superior stability. Cerclage, tension band and compression screws might be able to provide enough stability to withstand the forces exerted during unencumbered activities of daily living. Arthrodesis with plates should be limited to patients with special indications and require immobilization during consolidation.

Background Cone beam CT (CBCT) is a widely available technique with possible indications in carpal ligament injuries. The accuracy of CBCT arthrography in diagnosing traumatic tears of the scapholunate ligament has not been reported. Questions/purposes (1) What is the diagnostic accuracy of CBCT and how does it relate to the accuracy of multislice CT arthrography and conventional arthrography in diagnosing scapholunate ligament tears? (2) What is the estimated magnitude of skin radiation doses of each method? Methods This secondary analysis of a previous prospective study included 71 men and women with suspected scapholunate ligament tears and indications for arthroscopy. Preoperative imaging was conventional arthrography and either MSCT arthrography for the first half of patients to be included (n = 36) or flat-panel CBCT arthrography for the remaining patients (n = 35). Index tests identified therapy-relevant SLL tears with dorsal or complete SLL ruptures, and these tears were compared with relevant SLL tears which were determined through arthroscopy as Geissler Stadium III and IV by probing the instable SL joint with a microhook or arthroscope. These injuries were treated by open ligament repair and Kirschner wire fixation. Accuracy values and 95% confidence intervals were calculated. Additional estimates of the radiation skin doses of each CBCT exam and two MSCT protocols were subsequently calculated using dose area products, dose length products, and CT dose indices. Results The diagnostic accuracy was high for all imaging methods. 95% CIs were broadly overlapping and therefore did not indicate differences between the diagnostic groups: Sensitivity of CBCT arthrography was 100% (95% CI 77 to 100), specificity was 95% (95% CI 76 to 99.9), positive predictive value was 93% (95% CI 68 to 99.8), and negative predictive value was 100% (95% CI 83 to 100). For MSCT arthrography, the sensitivity was 92% (95% CI 64 to 99.8), specificity was 96% (95% CI 78 to 99.9), positive predictive value was 92% (95% CI 64 to 99.8), and negative predictive value was 96% (95% CI 78 to 99.9). For conventional arthrography, the sensitivity was comparably high: 96% (95% CI 81 to 99.9). Specificity was (81% [95% CI 67 to 92]); the positive predictive value was 77% (95% CI 59 to 89) and negative predictive value was 97% (95% CI 86 to 99.9). Estimated mean (range) radiation skin doses were reported in a descriptive fashion and were 12.9 mSv (4.5 to 24.9) for conventional arthrography, and 3.2 mSv (2.0 to 4.8) for CBCT arthrography. Estimated radiation skin doses were 0.2 mSv and 12 mSv for MSCT arthrography, depending on the protocol. Conclusion Flat-panel CBCT arthrography can be recommended as an accurate technique to diagnose scapholunate ligament injuries after wrist trauma. Estimated skin doses are low for CBCT arthrography and adapted MSCT arthrography protocols. Level of evidence Level III, diagnostic study.

Background Rhizarthrosis (trapeziometacarpal osteoarthritis) is the second most common site of osteoarthritis in the hand affecting 10–30% of adults over the age of 50. Up to four times as many women as men have rhizarthrosis. Clinical symptoms include functional disability of the thumb, pain, joint swelling, and reduced strength. The first carpometacarpal joint is pivotal in the opposition of the thumb and allows a high degree in flexibility to humans. Current therapies focus mainly on surgical strategies, which should be considered in advanced, therapy-resistant stages to relieve pain and improve function. However, conservative treatment methods are urgently required in presurgical stages. The efficacy of conservative treatment options for rhizarthrosis, which are intended to preserve function, joint integrity and to relieve pain, has not been adequately studied. In the clinical study protocol presented here, we investigate the efficacy of multimodal hand therapy versus therapeutic ultrasound versus combination therapy with both hand therapy and therapeutic ultrasound. Methods This study is a single-center, randomized, controlled, parallel-group pilot trial. One hundred fifty patients with rhizarthrosis and current disease activity will be randomized to one of three conservative interventions over 6 months. Interventions are (1) multimodal hand therapy (2) therapeutic ultrasound, and (3) combination therapy with both hand therapy and ultrasound therapy. The primary outcome measure is the Disabilities of the Arm, Shoulder, and Hand (DASH) questionnaire score after 6 months. Secondary endpoints are changes in pain, quality of life, disability progression, and changes of hand function. Safety will also be assessed. Discussion Clinical data suggest that multimodal hand therapy may improve functionality and reduce pain in rhizarthrosis. Clinical data regarding therapeutic ultrasound are not available. Clinical evidence is lacking. This study is the first clinical study investigating the effects of multimodal hand therapy in direct comparison to therapeutic ultrasound and to a combination therapy with both hand therapy and ultrasound therapy for rhizarthrosis. Darstellung einer nur noch selten durchgefuhrten Operationstechnik zur Hebung und Implantation eines gefasgestielten Beckenkammtransplantates ohne die Notwendigkeit von Kenntnissen zu speziellen mikrochirurgischen Techniken oder Komplikationen freier gefasgestielter Transplantate mit dem Ziel der sicheren Ausheilung von Femurkopfnekrosen. Fruhe Stadien der Knochennekrose bis Stadium II nach ARCO (Association Research Circulation Osseous), ggf. fruhes Stadium III bis zum Nachweis einer Frakturlinie („crescent sign“), jedoch ohne mechanische Insuffizienz. Femurkopfnekrose in spateren Stadien (ARCO III) mit subchondraler Fraktur und Gelenkeinbruch. Patienten mit mangelnder Fahigkeit zur postoperativen Entlastung oder problematischer Gefasversorgung durch stattgehabte Radiatio oder Operation an ipsilateralen Leistenlymphknoten, Gefasanomalien oder schwerer Arteriosklerose. Ausraumen der Femurkopfnekrose und Implantation eines gefasgestielten Beckenkammspanes. Freie Beugung des Huftgelenkes nach 4 Wochen postoperativ. Vermeiden der Ausenrotation fur 3 Monate sowie Entlastung des betroffenen Beines fur 3 bis 6 Monate postoperativ in Abhangigkeit der knochernen Konsolidierung. Vaskularisierte Transplantate zeigen in der Behandlung der Femurkopfnekrose einen deutlichen Vorteil gegenuber avaskularen Transplantaten sowohl radiologisch als auch in der Funktionalitat. Trotz der genannten Verbesserungen findet sich im 5‑Jahres-Follow-up im Stadium II durchschnittlich bei 25 % der Patienten ein weiterer radiologischer Progress. Im Stadium III zeigt sich mit allen revaskularisierenden Methoden eine weitere Zunahme der Nekrose mit fortschreitendem Kollaps und der Notwendigkeit der TEP-Implantation im Verlauf. Bei vergleichbarem Outcome nach freier gefasgestielter Fibula und den wenigen Fallen mit ortsstandigem gefasgestieltem Beckenkammtransplantat muss festgehalten werde, dass die operativen Risiken, neben der anatomisch anspruchsvollen Hebung, trotz biologisch gunstigem Transplantat im Vergleich deutlich hoher liegen. Heutzutage wird vorwiegend das freie gefasgestielte Fibulatransplantat fur die Behandlung der Femurkopfnekrose angewendet.

Bite injuries are common. Along with the resulting complications, they represent approximately 1-2 % of all emergency department visits. In over 75 %, the hands are affected. In Northern Europe, bites and subsequent infections are mainly caused by dogs and cats but also by humans.Up to 40 % of all hand infections are caused to bite injuries. Due to the multiple and complex compartments as well as the low soft tissue coverage of functionally relevant structures, even the smallest and most superficial bite injuries of the hand lead to infections. Any bite injury to the hand may subsequently may result in a fulminant infection and, rarely, even death.The spectrum of pathogens from the oral flora of the biting animal or person is diverse and includes aerobic and anaerobic bacterial strains. Bite injuries represent a major challenge for both the injured person and the attending physician. The rate of complications has been shown to increase with delayed medical consultation, lack of medical care and inadequate wound care. Osteomyelitis of the hand and wrist is uncommon compared to the infections of the long bones but not rare. There is poor evidence on many answers to questions concerning this disease. This includes careful consideration of the prevalence, pathogenesis, microbiology, diagnostic methods and the conservative, medical and surgical treatment. The emergence of the disease can subdivided in post-traumatic, postoperative, spread from contiguous infections and hematogenous. The individual patient-specific risk must be considered. Early diagnosis and correct management are essential to preserve bony structures and the articular function of the hand. A two-step surgical procedure is usually necessary, but uncertainties about the optimal treatment are still existing. Satisfactory control rates of the infection and successful bone reconstructions can be achieved. But complications, a limited range of motion and amputations of finger parts can still not be avoided in all cases.Die Osteomyelitiden im Bereich von Handgelenk und Hand sind weniger häufig als die Knocheninfektionen der langen Röhrenknochen, aber nicht selten. The objective of this study was to evaluate the outcome of a special interdisciplinary hand therapy program depending on the time interval between trauma and rehabilitation.With use of self-assessed scores (Disability of the Arm, Shoulder and Hand Score [DASH-Score], European Quality of Life 5 Dimensions [EQ-5D]) and objective functional parameters (TAM = Total Active Motion for finger injuries, ROM = Range of Motion for wrist injuries, grip strength) the outcome of 76 patients with injuries of the fingers, wrist or a complex regional pain syndrome (CRPS) was analysed at the begin and end of an inpatient rehabilitation and at a follow-up examination after 12 to 16 weeks. The patients were divided into groups with an early (120 days after trauma) or late beginning of their rehabilitation.At the follow-up examination early beginners had a significant better DASH-Score as well as a ROM. At the end of the rehabilitation program and at the time of the follow-up examination significant more patients with an early as patients with a late start of the rehabilitation were back to work. Especially patients with CRPS and finger injuries benefit from an early start of the rehabilitation.Compared to a late start an early start of a rehabilitation program after finger and hand injuries and a CRPS leads to better functional with special benefit for patients with a CRPS.ZIEL: Ziel der Studie war die Auswertung der Ergebnisse einer spezialisierten interdisziplinären Handrehabilitation in Abhängigkeit vom zeitlichen Abstand zwischen der Verletzung und dem Beginn der Rehabilitation.Mit Hilfe von punktebasierten Fragebögen (Disability of the Arm, Shoulder and Hand Score [DASH-Score], European Quality of Life 5 Dimensions [EQ-5D]) und objektiven Messparametern (TAM = Total Active Motion bei Fingerverletzungen, ROM = Range of Motion bei Handgelenksverletzungen, Grobkraft) wurden die Ergebnisse von 76 Patienten mit Verletzungen von Fingern und Handgelenken oder einem komplexen regionalen Schmerzsyndrom (CRPS) zu Beginn, am Ende und bei der Nachuntersuchung 12–16 Wochen nach Ende der semistationären Rehabilitation analysiert. Die Patienten wurden in Gruppen mit einem frühen (120 Tage nach Trauma) bzw. späten Beginn der Rehabilitation aufgeteilt.Patienten mit frühem Beginn der Rehabilitation hatten bei der Nachuntersuchung signifikant bessere Ergebnisse beim DASH-Score als auch beim Bewegungsausmaß des Handgelenkes. Auch waren signifikant mehr Patienten mit frühem Rehabilitationsbeginn zum Ende der Rehabilitation als auch bei der Nachuntersuchung arbeitsfähig als Patienten mit spätem Beginn der Rehabilitation. Insbesondere Patienten mit Fingerverletzungen und CRPS profitierten von einem frühen Beginn der Rehabilitation.Ein früher Beginn der Rehabilitation nach Finger- und Handverletzungen sowie bei einem CRPS führt zu funktionell besseren Ergebnissen, wobei vor allem Patienten mit CRPS von einem frühen Beginn profitieren.

Background Early detection of scapholunate ligament (SLL) tears is essential after minor and major trauma to the wrist. The differentiation between stable and instable injuries determines therapeutic measures which aim to prevent osteoarthritis. Arthroscopy has since been the diagnostic gold standard in suspected SLL tears because non-invasive methods have failed to exclude instable injuries reliably. This prospective study aims to determine the diagnostic accuracy of dynamic, 4D computed tomography (CT) of the wrist for diagnosing instable SLL tears. Methods Single center, prospective trial including 40 patients with suspected SLL tears scheduled for arthroscopy. Diagnostic accuracy of 4D CT will be tested against the reference standard arthroscopy. Radiologists will be blinded to the results of arthroscopy and hand surgeons to radiological reports. A historical cohort of 80 patients which was diagnosed using cineradiography before implementation of 4D CT at the study site will serve as a comparative group. Discussion Static imaging lacks the ability to detect instable SLL tears after wrist trauma. Dynamic methods such as cineradiography and dynamic magnetic resonance imaging (MRI) are complex and require specific technical infrastructure in specialized centers. Modern super-fast dual source CT scanners are gaining popularity and are being installed gradually in hospitals and ambulances. These scanners enable dynamic imaging in a quick and simple manner. Establishment of dynamic 4D CT of the wrist in patients with suspected SLL tears in in- and outpatient settings could improve early detection rates. Reliable identification of instable injuries through 4D CT scans might reduce the number of unnecessary diagnostic arthroscopies in the future. BackgroundThe clinical presentation of complex regional pain syndrome is heterogenous. The Budapest criteria give a list of symptoms and signs for the diagnosis. But the physician will often set the diagnosis clinically from the collection of symptoms and his or her experience. This study examines the accuracy of hand surgeons and pain specialists for diagnosing CRPS using the Budapest criteria as reference.MethodsWe included 124 patients of which 120 were diagnosed CRPS either by a clinician or by Budapest criteria. We analysed epidemiologic data, the number of patients with CRPS and mode of diagnosis and determined the CRPS severity score. Sensitivity, specificity, accuracy, and predictive values were calculated for hand surgeon and pain specialist. The number of CRPS-NOS was determined. We calculated the agreement between hand surgeon and pain specialist for the diagnosis CRPS.ResultsThe Budapest criteria were met in 108 cases with a mean CSS of 10.41 points. Sensitivity of hand surgeons for the diagnosis CRPS was 92 %, specificity 31 % and accuracy 90 %. The pain specialists had a sensitivity of 74 %, specificity of 71 % and accuracy of 74 %. Prevalence of CRPS-NOS was 9 %. In cases where hand surgeon and pain specialist agreed on CRPS, the Budapest criteria were met in 96 %, in all other combinations they were met in around 70 %. The agreement of hand surgeon and pain specialist was fair.ConclusionThe Budapest criteria can support the diagnosis CRPS in clinically ambiguous cases as they were most often met in cases with agreement of both clinicians. Background: Previous studies have discussed the diagnostic value of four dimensional kinematic CT in cases of carpal instabilities. This analysis compares calculated skin doses of 4D CT and conventional cineradiography of the wrist in cases of suspected SLL rupture.Methods: Retrospective calculation and interpolation of skin doses and effective doses for ten consecutive 4D CT examinations and 41 cineradiographies for suspected lesions of the scapholunate ligament. Standardised anterior-posterior and lateral cine sequences using a flat-panel digital subtraction imager and of 4D kinematic CT using a dual-source scanner were acquired and acquisition parameters recorded. We tested if the skin dose of 4D CT is different from cineradiography. Results: Median dose area product (DAP) of cineradiography was 135.34 cGycm 2 resulting in a calculated median skin dose of 32.6 mSv (confidence interval 26.86-42.90 mSv) and an estimated effective skin dose of 3.26 µSv. CT dose index (CTDI) for 4D examinations was recorded to be 26.79 mGy and the dose-length product (DLP) was 150 mGy*cm. This resulted in an estimated skin dose of 34 mSv, which is covered by the confidence interval of cineradiography, and an effective skin dose of 3.4 µSv. Background The aim of this study was to evaluate the potential of whole-body CT for diagnosis of hand and forearm fractures in intubated patients with suspected polytrauma. Methods We performed a retrospective analysis on data collected from two trauma centres in Germany, including demographics, ISS, clinical symptoms, depiction in whole-body CT, and time to diagnosis. Results Out of 426 patients included in the study, 66 (15.5%) suffered a hand or forearm fracture. The total number of fractures was 132, the whole-body CT report mentioned 98 (74.2%). 16 (12,1%) fractures of 12 patients were diagnosed later than 24 h after admission. Late diagnoses of fractures of the hand occurred more often if the hand was not fully included in the CT scan field. The sensitivity of whole-body CT for cases with fractures of hand and/or forearm with full inclusion of the corresponding area in the scan field was 80.2%. Conclusions This study shows that whole-body CT is a valuable diagnostic tool for hand fractures in polytrauma patients. Hands should be evaluated regardless of clinical presentation in intubated patients after suspected polytrauma if they are included in the whole-body CT.

Endoskopie und Arthroskopie haben in der Handchirurgie mittlerweile einen festen Platz sowohl im diagnostischen als auch im therapeutischen Bereich eingenommen. In einem ersten Artikel haben wir 2013 bereits uber die Arthroskopie in der Handchirurgie berichtet und beschaftigen uns nun in einem zweiten Teil mit der Endoskopie im Speziellen. Dabei wird v. a. auf die haufigen endoskopischen Indikationen und Verfahren eingegangen, wobei hier die endoskopischen und endoskopisch assistierten Nervendekompressionen im Vordergrund stehen. Erganzend werden endoskopische Eingriffe im Rahmen seltener Kompressionssyndrome erwahnt.

Background Vascular patency of free vascularised fibula grafts can be postoperatively assessed by island flap which is not always possible or angiography which is invasive. Bone healing is examined based on radiographs with scoring systems. We present data on MRI with contrast agent and a comparison of the Giessler bone healing score by X-ray and MRI and clinical scores in patients with operation on the upper extremity. Methods We reviewed the clinical and radiological outcome of 13 patients with a follow-up of at least 1 year after free vascularised fibula graft of the upper extremity. The examination included the DASH, MSTS, SF-36, Rosén-Lundberg-scores and MRI with contrast agent. We determined the Giessler bone healing score by most recent x-ray and MRI and the Weiland score and its modification for the upper extremity. For statistics, we used the Wilcoxon test and Pearson correlation. Results Vascular patency was directly or indirectly detectable all cases in the MRI with contrast agent. 5 patients did not return to work and 5 patients had ongoing pain medication. Median DASH was 21.7, MSTS of the upper extremity 76.7, of the lower extremity 93.3, the Rosén-Lundberg score for median nerve 2.43, for ulnar nerve 2.32. Giessler score for x-ray and MRI showed no significant difference for proximal or distal junction. Conclusions We could prove with MRI that it was indeed a vascularised graft and its vitality at time of follow-up by assessment of vascular patency, and that MRI can be used to determine the Giessler score similarly to X-ray.

Aims The aim of this study was to investigate whether clinical and radiological outcomes after intramedullary nailing of displaced fractures of the fifth metacarpal neck using a single thick Kirschner wire (K-wire) are noninferior to those of technically more demanding fixation with two thinner dual wires. Patients and Methods This was a multicentre, parallel group, randomized controlled noninferiority trial conducted at 12 tertiary trauma centres in Germany. A total of 290 patients with acute displaced fractures of the fifth metacarpal neck were randomized to either intramedullary single-wire (n = 146) or dual-wire fixation (n = 144). The primary outcome was the Disabilities of the Arm, Shoulder and Hand (DASH) questionnaire six months after surgery, with a third of the minimal clinically important difference (MCID) used as the noninferiority threshold. Secondary outcomes were pain, health-related quality of life (EuroQol five-dimensional questionnaire (EQ-5D)), radiological measures, functional deficits, and complications. Results Overall, 151/290 of patients (52%) completed the six months of follow-up, leaving 83 patients in the single-wire group and 68 patients in the dual-wire group. In the modified intention-to-treat analysis set, mean DASH scores six months after surgery were 3.8 (sd 7.0) and 4.4 (sd 9.4), respectively. With multiple imputation (n = 288), mean DASH scores were estimated at 6.3 (sd 8.7) and 7.0 (sd 10.0). Upper (1 - 2α)) confidence limits consistently remained below the noninferiority margin of 3.0 points in the DASH instrument. While there was a statistically nonsignificant trend towards a higher rate of shortening and rotational malalignment in the single wire group, no statistically significant differences were observed across groups in any secondary outcome measure. Conclusion A single thick K-wire is sufficient for intramedullary fixation of acute displaced subcapital fractures of the fifth metacarpal neck. The less technically demanding single-wire technique produces noninferior clinical and radiological outcomes compared with the dual-wire approach. Background Microvascular problems like increased intraosseous pressure or venous congestion may influence the development of Kienböck’s disease. We examined if wrist position modifies the blood flow in the nutrient vessels. Methods Retrospective analysis of 17 patients with Kienböck’s disease who had a superselective microangiography of the radial, ulnar and interosseous artery in different wrist positions under general anaesthesia. We analysed the data with Fisher’s exact and Wilcoxon-test. Results We found vessels that entered the bone, that ended at the bone edge, and that supplied a vascular plexus. The origins were the anterior interosseous artery in 10 of 17 cases, the radial artery in seven cases, and the ulnar artery in five cases. Movement of the wrist could reduce or stop the blood flow. Type of lunate configuration showed no significant influence on the blood supply in neutral position. Conclusion The radial, ulnar, and anterior interosseous artery contribute to the vascular supply of the lunate bone in different combinations. Wrist movement can reduce blood flow to the lunate bone. These review article analyses the described techniques for fusion of the proximal interphalangeal joint regarding their advantages and disadvantages with respect to the approach, surgical technique resecting the articular surface, angle of fusion, and the postoperative treatment.Diese Übersichtsarbeit analysiert die in der Literatur beschriebenen Verfahren zur Arthrodese des Fingermittelgelenkes hinsichtlich ihrer Vor- und Nachteilen unter Berücksichtigung des Zugangsweges, der Techniken der Gelenkflächenpräparation, der verwendeten Arthrodesewinkel und der Nachbehandlung.

Arthrodesis of the proximal interphalangeal joint is a proven technique for treating of a range of pathological conditions, including osteoarthritis. There are multiple surgical procedures. A biomechanical study was conducted to compare the stability of a compression wire to intraosseous wiring for the arthrodesis. Seventeen formalin-fixed human fingers were randomly assigned into two groups and the bone mineral density was determined. Arthrodesis in 20° flexion was performed using an oblique compression wire (n = 8) or intraosseous wiring (n = 9). The stability of the arthrodesis was tested by applying a tensile bending force until failure. The mean force needed to fail the compression wire arthrodesis and intraosseous wire arthrodesis was not significantly different (76.2 N, SD 31 N and 63.0 N, SD 28 N). There was no correlation between bone density and force to failure. The compression wire was within the approximate range achieved by intraosseous wiring in withstanding substantial force before failure. From a biomechanical point of view, a compression wire is feasible for PIP arthrodesis.

Frakturen des Os scaphoideum sind mit 60 % die haufigsten Frakturen der Handwurzelknochen und stellen ca. 2–3 % aller Frakturen dar. Meist heilen die Frakturen bei adaquater Behandlung vollstandig aus. Bei fehlender Ruhigstellung oder nicht diagnostizierten bzw. ubersehenen Frakturen steigt die Wahrscheinlichkeit einer Pseudoarthrose im weiteren Verlauf an. Bei der initialen Rontgendiagnostik werden ca. 70–80 % der Kahnbeinfrakturen festgestellt. Um eine Fraktur sicher auszuschliesen oder zu bestatigen, sollte, bei entsprechender Klinik und negativem Rontgenbild, eine weiterfuhrende Diagnostik mittels Mehrzeilenspiral-CT (MSCT) oder MRT erfolgen, wobei der MSCT die hohere Bedeutung zukommt. Neben der Frakturbeschreibung gehort die Einteilung der Pseudoarthrosestadien ebenfalls zu den Aufgaben der MSCT. Wegweisend zur Beurteilung der Vitalitat der Fragmente ist die kontrastmittelverstarkte MRT-Untersuchung.

Skaphoidpseudarthrosen nach z. B. proximalen Frakturen oder fehlgeschlagenen Rekonstruktionsversuchen sind mogliche Indikationsbereiche vaskularisierter Knochentransplantate. Hierzu stehen verschiedene vaskularisierte (gestielte und freie) Knochentransplantate zur Verfugung. Gestielte Transplantate aus der unmittelbaren Umgebung des Handgelenkes bieten Vorteile durch geringere Hebemorbiditat und allzeit erhaltene Perfusion ohne Notwendigkeit einer Gefasanastomosierung sowie der damit erwarteten Verbesserung des Einheilungsverhaltens. Ein sehr haufig zur Anwendung kommendes Verfahren stellt das gestielte vaskularisierte Knochentransplantat zur Skaphoidrekonstruktion nach Zaidemberg dar. Von 2008 bis 2016 wurden 49 Patienten mit dem an der 1,2 interkompartimentellen supraretinakularen Arterie gestielten Radiusspan (nach Zaidemberg) operiert. Die Fixierung des Knochentransplantates erfolgte durch Kirschner-Drahte. Eine Konsolidierung konnte in 37 Fallen nachgewiesen werden und 9 Falle zeigten eine erneute Pseudarthrosebildung (3 Patienten erschienen nicht zur Nachuntersuchung). 44 Patienten hatten nach der Klassifikation von Herbert ein Pseudarthrosestadium D2 oder hoher, hiervon zeigten 36 Patienten eine knocherne Konsolidierung (8 nicht konsolidiert). Komplikationen traten in Einzelfallen auf (1x intraoperative Radiusfraktur, 4x passagere Irritationen des R. superficialis N. radialis). Haufig verblieb postoperativ eine Bewegungsminderung des betroffenen Handgelenkes. Die praoperativ angefertigten MRT-Untersuchungen mit Kontrastmittel zur Darstellung der Perfusion der Skaphoidfragmente wurden regelhaft bei der Indikationsstellung berucksichtigt. Je ausgepragter die Veranderungen im MRT, desto haufiger kam es zu Therapieversagen. Das von C. Zaidemberg et al. 1991 beschriebene gestielte vaskularisierte Knochentransplantat aus dem dorsalen distalen Radius ist eine von mehreren Moglichkeiten, um eine durchblutungsgeminderte Skaphoidpseudarthrose u. U. zur Ausheilung bringen zu konnen.

Mit dem Karpaltunnelsyndrom ist 2015 eine weitere Volkskrankheit in die Berufskrankheitenliste unter der BK-Nr. 2113 aufgenommen worden. Aber wann ist ein Karpaltunnelsyndrom auf die berufliche Exposition und wann nicht zuruckzufuhren? Der Beitrag soll aus der Sicht des Handchirurgen Hinweise fur die medizinische Zusammenhangsbegutachtung zur neuen BK 2113 geben unter besonderer Berucksichtigung der wissenschaftlichen Begrundung und der zu diesem Thema veroffentlichten Literatur. Auf die Abgrenzungsproblematik zu auserberuflichen konkurrierenden Faktoren bei der Volkskrankheit Karpaltunnelsyndrom wird verwiesen werden. Das Karpaltunnelsyndrom hat das Potenzial, Berufsgenossenschaften, arztliche Gutachter und Rechtsprechung zu beschaftigen.

Handverletzungen konnen den Teil- bzw. Komplettausfall eines Sportlers bedeuten. Die sichere Ruckkehr nach diesen Verletzungen ist ein wesentlicher Bestandteil der Therapieplanung und Nachbehandlung. Nachfolgend beschaftigen wir uns mit einigen ausgewahlten Verletzungen der Hand im Profisport und gehen auf die Rehabilitation und sichere Wiederaufnahme der sportlichen Aktivitat ein.

Fortschritte in der Grundlagenforschung bezuglich Nahtmaterial, Nahtverfahren und Belastungsgrenzen unterstutzen die Wahl der Behandlungsprotokolle nach Beugesehnenverletzungen. Die Wahl des Nachbehandlungsregimes in Abhangigkeit des Verletzungsmusters sowie der Kenntnis von Einflussfaktoren auf die operativ versorgte Sehne wird dargestellt und das klinischen Outcome nach Beugesehnenverletzung ausgewertet und beurteilt. Die Fragestellung werden anhand aktueller Literatur und Grundlagenstudien untersucht. Verletzungsmuster, operative Versorgung und Compliance des Patienten sowie Forschungsergebnisse bezuglich der Sehnen- und Nahtmaterialbelastung helfen bei der Wahl der Nachbehandlungskonzepte. Passive Nachbehandlungen (z. B. nach Duran-Houser und Kleinert) liefern im Literaturvergleich vorwiegend gute bis befriedigende Ergebnisse. Die fruhe, aktive Bewegungstherapie (z. B. nach Small) zeigt ein besseres Bewegungsausmas der Finger und liefert vermehrt auch exzellente Behandlungsergebnisse klassifiziert nach dem Hand-Functional-Score der American Society for Surgery of the Hand. Neben der spezifischen Therapie der entsprechenden Verletzungszonen ist auch das Erkennen von Fehlerquellen inklusive der korrekten Schienenanpassung essenziell fur das Behandlungsergebnis und von der intensiven Betreuung inklusive der Narben- und Weichteilbehandlung durch fachkundiges Personal abhangig. Beugesehnenverletzungen der Hand, v. a. in der Verletzungszone 2 gelten nach wie vor sowohl operativ als auch in der Rehabilitation als Herausforderung mit unvorhersehbarem Outcome. Weitere Erkenntnisse bezuglich Nahttechnik und -material konnen voraussichtlich in Zukunft den Einsatz fruher, aktiver Bewegungsprotokolle fordern und damit die Ergebnisse verbessern.

Eine haufige Begleitverletzung von Oberarmfrakturen ist die Lasion des N. radialis. Diese kann primar im Rahmen des Traumas oder sekundar als Folge der Frakturbehandlung entstehen. Wie vergleichende Analysen belegen, besteht bezuglich der Therapieentscheidungen bei begleitender Radialisparese noch immer keine Einigkeit. Ausschlaggebend scheint v. a. die hohe Rate an Spontanremissionen zu sein, die die Notwendigkeit einer mikrochirurgischen Revision in Frage stellt. Unabhangig davon, ob die Radialisparese primar oder sekundar aufgetreten ist, persistiert bei ca. einem Viertel der Betroffenen eine dauerhafte Einschrankung, welche die Funktionsfahigkeit der Hand erheblich beeintrachtigt. Zeitgerechte klinische und elektrophysiologische Verlaufskontrollen sowie ggf. bildgebende Diagnostik sind notwendig, um diese Langzeitfolgen zu vermeiden bzw. zu verringern. Eine Stichprobenuntersuchung der Patienten des Unfallkrankenhaus Berlin mit Humerusfraktur aus den Jahren 2012/2013 bestatigte die in der Literatur angegebene Haufigkeit von begleitenden Radialisparesen. Um die zeitkritischen Therapieentscheidungen im abteilungsubergreifenden Konsens zu erleichtern, wurde ein Algorithmus entwickelt. Die im Verlauf notwendige Diagnostik und davon abgeleitete Therapie wird ausgehend von individuellen Behandlungsentscheidungen abgebildet.

Alloarthroplasty of the proximal interphalangeal joint is indicated for patients with osteoarthritis, an inflammatory disease of the joint. According to the current literature, implants made of silicone are not inferior to newer implants and the complications are well known. Therefore, focus should be shifted to the operative approach to improve the results. Eleven silicone arthroplasties in 7 patients with arthritis and 35 silicone arthroplasties in 23 patients with osteoarthritis were reviewed after 15.6 months and after 105.3 months. All implants were inserted using the palmar approach according to Simmen. We compared the pre- and postoperative arc of motion, pain, and strength, and present postoperative complications and satisfaction. Patients with arthritis showed a gain in range of motion of 36° and patients with osteoarthritis of 39°. No implant failure occurred, and one revision was performed,. Average extension lag over all operated joints of patients was 2.8°. No swan-neck deformity or extensor tendon adhesion was observed. There was no significant difference in the postoperative results between patients with osteoarthritis and arthritis or between first and second follow-up. Silicone arthroplasty restores the functionality of proximal interphalangeal joints in patients with osteoarthritis and inflammatory joint disease. Using a palmar approach, the surgeon can reach palmar and dorsal osteophytes. The extensor mechanism stays intact reducing the risk for extensor lag and other complications compared to the dorsal approach. The results do not change significantly between 1 and 8 years postoperatively.

Ulnocarpal impaction syndrome is a common cause of chronic ulnar-sided wrist pain. The distal ulnar shortening osteotomy addresses the often present positive ulnar variance and therefore relieves the excessive load on the ulnocarpal joint. In the present study, the results of a technique that uses an ulnodorsal approach with a compression device and a transverse osteotomy are presented and compared to other techniques.This retrospective study includes 92 wrists with an ulnocarpal impaction syndrome, which were treated with an ulnar shortening osteotomy. The mean duration of postoperative follow-up was 50 months.The mean ulnar variance was 2.17 ± 1.56 mm preoperatively and after the ulnar shortening osteotomy -1.36 ± 1.67 mm (p0.05). Radiological consolidation of the osteotomy could be shown in 91 wrists 6 months postoperatively. Patients rated the preoperative pain level at a VAS 7.9 ± 1.7 which decreased to a of VAS 2.4 ± 2.5 (p0.05). 19 patients complained of mechanical irritation. There was no neurological irritation.It could be shown that the distal ulnar shortening osteotomy by a transverse osteotomy using a compression device and an ulnodorsal approach has a low complication rate. It is comparable to the oblique osteotomy in effectiveness and safety.

Magnetic resonance imaging (MRI) with small surface coils is a well established method for the diagnostic evaluation of finger masses. Until now, histological examination has been required to reliably assess tumor extent and infiltration of surrounding structures. Ultra-high-field MR microscopy (MRM) allows evaluation of anatomical structures and pathologies with submillimeter resolution. This study describes the diagnostic prospects and potential of MRM based on the ex-vivo examination of different finger pathologies. Ten human digits were examined by ex-vivo MRM at 7.1 Tesla (ClinScan, Bruker BioScan) using a T2-weighted turbo spin echo (TSE) sequence. Imaging parameters were: TE 48 ms; TR 8370 ms; slice thickness 700 µm; matrix size 1024 × 1024 pixels; FOV 37 × 37 mm; in-plane resolution 36 × 36 µm/voxel. Afterwards specimens were examined histologically. Histology and MRM were correlated. MRM allowed evaluation of the anatomy of the nail, the tendon insertions, the distal interphalangeal joint, and the neurovascular bundles. Finger abnormalities evaluated by MRM included osteomyelitis and metastatic disease. Subsequent histological examination confirmed MRM findings regarding origin, internal makeup, and extent of the structures visualized. This study demonstrates the potential of MRM for imaging small anatomical structures and pathologies of the human finger. Our ex-vivo findings correlate strongly with histology, suggesting that MRM may gain a central role in assessing anatomical structures and pathology in terms of morphology, extent, and infiltration of surrounding structures. Therefore, with increasing availability, MRM is expected to become an essential tool not only in experimental studies but also for daily routine.

Die Behandlung von Fingerfrakturen ist nach wie vor eine Herausforderung fur den jeweiligen Behandler. Grundsatzlich gilt, dass die fruhzeitige Diagnosestellung und Behandlung von Fingerfrakturen Voraussetzungen fur ein gutes funktionelles Ergebnis sind. Aufgrund der verschiedenen Frakturformen und Frakturlokalisationen sowie der unmittelbaren Nahe zum komplexen Sehnenapparat und zu den Gefas-Nerven-Bundeln gibt es zahlreiche konservative und operative Behandlungsverfahren. Wahrend stabile nichtdislozierte Frakturen eine Domane der konservativen Bruchbehandlung darstellen, sind instabile dislozierte Frakturen der Phalangen operativ zu behandeln 4 8 33 . Bei der Wahl des Osteosyntheseverfahrens sollte nicht nur die Frakturform und -lokalisation, sondern insbesondere auch die funktionelle Anatomie der Finger berucksichtigt werden. Ziel einer jeden Osteosynthese sollte neben der anatomischen Reposition auch eine stabile Retention sein, welche eine fruhzeitige Mobilisation der Finger ermoglicht. Nur so kann in der ergo- und physiotherapeutischen Nachbehandlung, als weiterer zentraler Baustein in der Behandlung von Fingerfrakturen, ein gutes funktionelles Ergebnis erzielt werden 9 . Die Kirschner-Draht-Schrauben und die Plattenosteosynthese sind derzeit die bevorzugten Verfahren. Wahrend die Kirschner-Drahte sehr weichteilschonend eingebracht werden konnen, weisen sie aufgrund unzureichender kortikaler Verankerung haufig eine zu geringe Frakturstabilitat fur eine fruhfunktionelle Nachbehandlung auf. Platten- und Schraubenosteosynthesen dagegen fuhren insbesondere bei Frakturen ohne Knochenverlust zu einer sehr hohen Stabilitat. Allerdings ist die fur die Implantation zusatzlich notwendige Schadigung des vom Trauma selbst schon in Mitleidenschaft gezogenen Weichteilmantels ein nicht unerhebliches Problem, welches das funktionelle Ergebnis, insbesondere bei der Plattenosteosynthese, negativ beeinflussen kann. Es ist bisher nicht geklart, wie viel Stabilitat uberhaupt fur eine fruhfunktionelle Nachbehandlung notwendig ist. Obwohl Fingerfrakturen noch lange im Rontgenbild sichtbar sind, besteht im Allgemeinen schon nach 3 – 4 Wochen, unabhangig vom Behandlungsverfahren, eine ausreichende Ubungsstabilitat. Anhaltspunkt hierfur kann der Druckschmerz im Frakturbereich sein. Ist dieser verschwunden, kann man von einer ausreichenden Frakturstabilitat fur eine funktionelle Nachbehandlung ausgehen. Im Allgemeinen kann bei einfachen Frakturen ein gutes funktionelles Ergebnis erreicht werden. Bei Komplexverletzung dagegen, insbesondere mit Weichteilschadigungen, ist in der Regel mit einem schlechteren funktionellen Ergebnis zu rechnen.
